1.A report of twice outbreak epidemic of acute tonsillitis and acute pharyngitis

Objective:To discuss the pathogenic factors,prevention and treatment of fulminant epidemic of acute tonsillitis and acute pharyngitis.Method:We investigated and analysed the cases of twice fulminant epidemic of acute tonsillitis and acute pharyngitis.Result:In the first time,91 patients among 518 persons were attacted,the sick rate was 17.6%.In the second time,39 among 295 persons were attacted,the sick rate was 13.2%.The main symptoms were pharyngodynia and fever.Bacterial culture of throat swab of 20 cases of the first time and 17 cases of the second time were carried out.The positive rate of β-hemolytic streptococcus was 82.5% and 85.3% respectively.Conclusion:Environment and individual health condition are important factors in the fulminant epidemic of acute tonsillitis and acute pharyngitis.

Angiotensin-converting-enzyme inhibitors(ACEI) are generally used in the treatment of cardiovascular system diseases.They are found by epidemiological studies to have an anti-tumor activity and are shown to play various roles in suppressing carcinogenesis and tumor growth,preventing angiogenesis,inhibiting matrix degradation and reducing cancer cachexia.
3.Strategic exploration on AIDS prevention and control among men who have sex with men in small-medium sized cities

Objective To probe into the characteristics of men who have sex with men living in small-medium sized cities and provide scientific foundation for the formulation of strategy for AIDS prevention and control.Methods A qualitative survey was carried out by convenience sampling for thirty one selected MSM.Results Interviewees had a large age gap,widely different occupations and various degrees of education;they depended mainly on internet and friends' contacts to find their sex partners;their knowledge about HIV/STDs was poor and they were indifferent to the risk of AIDs,but were faced with enormous psychological pressure.Multiple sex partners,unprotected anal intercourse were their main high risk behaviors,and they hoped to have access to free condoms,free HIV detection and hot line consultation.Conclusion To implement prevention in an effective manner,close attention should be paid to the role of key persons within the MSM group,to increasing knowledge of the target population,to protecting their mental health and to creating a friendly social environment for their existence and effective use of internet.
4.Pathogenic Bacterium Distribution and Antibiotic Resistance Analysis of Urinary Tract Infection

OBJECTIVE To analyze pathogenic bacterium distribution and antibiotic resistance of our hospital,and provide scientific basis for clinical rational using of antibiotics.METHODS The patients′ clean catch(midstream)(urine) was collected from Jan 2004 to Dec 2005 and cultivated.Antibiotic sensitivity test and adopted by Kirby-Bauer method.RESULTS The pathogenic bacteria mainly consisted of Gram-negatives,among which Escherichia coli was the most frequent,the others in turn were Pseudomonas aeruginosa,Enterobacter cloacae and(Proteus) mirabilis;Enterococcus were the most common among Gram-positives;fungal infection obviously(increased).The bacteria showed different antibiotic resistance rate and multi-drug resistance.CONCLUSIONS It′s very important for making the clinical use of antibiotic more reasonable and controlling drug resistant strains(transmission).
5.Investigation of relevant genes to the pathogenesis and metastasis of osteosarcoma with SBC gene microarray

[Objective]To investigation the relavant genes to the pathogenesis and metastasis of osteosarcoma,and to discuss the significance of gene expression in the pathogenesis and metastasis of osteosarcoma. [Method]Two well-known ATCC osteosarcoma cell lines and an osteoblastic cell line were collected.After the total RNA was extracted,the finally synthesized biotinylated cRNAs were hybridized to SBC arrays.After the design and synthethesis of the primers,four of the differentially expressed genes were then chosen at random to further confirm the array results using the SYBR Green real-time RT-PCR method in freshly collected osteosarcoma specimens and the cell lines.The data analysis depended on the ABI Prism 7300 system.[Result]By an entrance limit of ≥2.0 or ≤0.5,189 up-regulated and 84 dowm-regulated genes which were considerable differentially expressed in the two osteosarcoma cell lines compared with the osteoblastic cell line hfob1.19 were finally detected.Many of the genes were first reported to be related to the pathogenesis of osteosarcoma.The array results were further confirmed by the real-time RT-PCR.[Conclusion]Many genes are involved in the pathogenesis and metastasis of osteosarcoma.It is helpful to apply gene array to find the laws of gene expression and to study the gene-gene interactional relationships in this tumor.
6.Selectivity treatment of patellar fractures with Cable-Pin system

[Objective]To study the effects of Cable-Pin system used as a treatment alternative for patellar fractures.[Method]Thirty-five cases of patellar fracture were operated with the Cable-Pin system from October 2005 to May 2007.Their diagnosis were 19 transverse close ractures and 16 comminuted of patella.There were 21 males and 14 females.Their ages ranged from 21 to 75 years old.After open reduction,the author used a drill bit vertically through the patella in a superior direction,drilled the threaded portion of the pin across the fracture site,inserted a second Cable Pin through the second hole in a similar manner.A horizontally directed drill hole anterior to the pins was created in the superior pole of the patella.The two cables were crossed in a fighre eight over the an-terior surface of the patella.The Crimper Tensioning Handle was,the cable carefully,the handles of the crimper were squeezed until the crimp was fully crimped.[Result]The 35 patients were followed up for 12 to 18 months(mean 13.2 m).Fractures healed in all the cases without such complications as loosening of screw and cable loop,loss of reduction or irritation to soft tissue.Restoration of the knee joint motion was excellent in 32 cases and good in 3 cases according to Bostman score system.[Conclusion]Cable-Pin system as a new option for the treatment of patella fracture,can result in reliable fixation and low rate of complications.
7.Effect and treatment of Mason type-Ⅲ radial head fractures with type-T plate

[Objective]To analyze the result of mini-type T plate for treatment of Mason type-III radial head fracture.[Method]Seven cases of Mason type-Ⅲ radial head fractures,6 male and 1 female,including 2 cases with radial neck fractures,were treated by open reduction and mini-type T plate fixation associated with Kirscher wires.If necessary,bone graft was placed between radial head and neck.[Result]The patients were followed up for an average of 20.3 months(ranged from 14 to 39 months).All fractured radial heads were heal and no case appeared ischemic necrosis.According to the Brobrg and Morrey functional elbow index,the averaged score was 87 points and the outcome was excellent in 2 patients,good in 4,the excellent and good rate was 85.7%.[Conclusion]Treatment of Mason type-Ⅲ radial head fractures by open reduction,mini type T plate fixation assocated with Kerscher's wire and autogenous bone graft showed a high union rate of fracture and satisfactory joint function but no ischemic necrosis with the advantages of reservation of radial head and decreasing the rate of re-section and prosthesis replacement.Further observation should be taken to get more clinical cases number.
8.Distribution and Drug Resistance Changes of Bacteria Isolated from Blood Specimens

OBJECTIVE To investigate the distribution and the drug resistance changes of the pathogenic bacteria isolated from the blood culture specimens collected during the period of 2006-2008.METHODS Blood culture of patients in our hospital was performed by BacT/Alert 240 and the isolated bacteria were identified by API and Microscan and tested for drug resistance against antimicrobial agent by K-B method.A retrospective analysis was made to the blood culture results during the period of 2006-2008 with WHONET 5.4 software.RESULTS Gram-negative rods were the predominant bacteria which caused sepsicemia.The isolated rates of Escherichia coli took the first place during the period of 2006-2008.Klebsiella pneumoniae,Pseudomonas aeruginosa and Staphylococcus aureus(SAU) were also the most important pathogens which caused blood infection.The infection rate of coagulase negative staphylococcus(CNS) and P.aeruginosa had increasing tendency.Imipenem and meropenem were the most effective antibiotics(100%) to E.coli and K.pneumoniae.Then amikacin and cefoxitin also had high susceptibility to E.coli and K.pneumoniae.Furthermore,drug susceptibility in 2008 was higher than that of two years before.All antibacterials had low drug susceptibility to P.aeruginosa,and its drug resistance rate rised obviously.The drug resistance situation of Acinetobacter baumannii was serious.Except imipenem and meropenem had relatively higher susceptibility(20-69.2%),the susceptibility to other antibacterials was lower than 41.7%.Vancomycin,teicoplanin and quinupristin/dalfopristin were the most effective antibiotics to(SAU).CONCLUSIONS The species and drug resistance of the bacteria isolated from blood specimens have changed.More attention should be paid to the detection and surveillance of bacterial resistance in blood culture to promote the rational use of antibiotics.
9.Autologous bone-marrow stem-cell transplantation for nonunion of tibia fracture

Objective To investigate the effect of the percutaneous autologous bone marrow stem cell transplantation for the treatment of nonunion of tibial fracture. Methods From 2007 to 2011,the data of 11 patients with tibial nonunion who received autologous bone marrow stem cell transplantation was analyzed retrospectively. Taking bone marrow,examine,isolation,cultivation and expansion marrow mesenchymal stem cells( MSCs) ,and then marrow needle was inserted into the site of the nonunion under the X-ray,the MSCs were injected into the site of nonunion. Compression bandage was applied after operation. X-rays following-up were reviewed. Results All 11 patients were followed up from 4~27 months,with average of 13 months,X-ray showed:fracture healed well,no adverse events happen. Conclusion Satisfactory out-come can be obtained by percutaneous autologous bone marrow stem cell transplantation in treatment of tibial nonunion. Clinical application value is high,especially to patients who had suffered from severe skin and soft tissue injury with keloids healed.
10.Simultaneous detection of paraquat and creatinine in urine by capillary electrophoresis

Objective To establish a method for detecting the concentration of paraquat (PQ) and creatinine(Cr) in urine simultaneously by capillary electrophoresis.Methods Experimental methodological study.8 acute PQ poisoned patients who were treated in the First People's Hospital of Lianyungang from January 2011 to June 2012 were collected.2 were male,and 6 were female.The separation were carried out using a 25 mmol/L pH1.97 glycine-HCl buffer(containing 40 mmol/L NaCl) in a fused-sillica capillary tube of 47 cm ×75μm I.D.by capillary zone electrophoresis.Urine had been injected by pressure for 4 s after samples were centrifuged and diluted for 10 times with H2O.The detection were monitored by a diode-array detector at 200 nm while samples were separated at a voltage of 20 kV.A systemic methodological evaluation of this method was carried out (The linear range,detection limit,repeatability test,recovery test and interference test).And the method was used to detect the concentration of PQ and Cr in PQ poisoned patients' urine.Results The peaks of PQ and Cr appeared within 5 min.The linear ranges of PQ and Cr were 2-1000,10-5000 μmol/L,respectively,with the correlation coefficients of 0.9997 and 0.9999 (P <0.01).The detection limits were 1.0 μmol/L for PQ and 5.0 μmol/L for Cr.The mean within-day(n =10) CVs of peak area for PQ and Cr were 2.84% and 1.72%,while the mean inter-day(n =10) CVs of peak area were 3.62% and 3.06%.The average recovery rate of PQ and Cr were 88.6% and 90.2% respectively.Diquat(DQ) didn't interfere with the assay.The range of PQ/Cr(μmol/mmol) for 8 cases was 8.9-2215.Conclusions A method was established successfully for the rapid determination of PQ and Cr in urine by capillary electrophoresis.The CE method devised here for direct measurement of urinary PQ and Cr from PQ poisoned patients is simple,fast,automatic and with good repeatability.It is an ideal method for rapid detection of urinary PQ in PQ poisoned patients.
