1.Efficacy of ambroxol combined with imipenem on severe neonatal pneumonia

Objective To investigate the efficacy of ambroxol combined with imipenem on severe neonatal pneumonia .Methods 62 newborns with severe pneumonia were randomly divided into the two groups ,31 cases in the control group and 31 cases in the treatment group .Two groups of children were given warm ,conventional oxygen ,anti-biotics,and correction of the acid-base balance,and so on.The control group received intravenous imipenem on this basis,the treatment group was given ambroxol on the basis of the control group .The time of body temperature returned to normal,pulmonary rales disappeared time,the times of daily suction and treatment time were observed .Results The total effective rate of treatment group(93.5%) was significantly higher than the control group (83.9%)(P<0.05).The treatment time,time of temperature returned to normal ,pulmonary rales disappeared time and the times of daily suction of treatment group were significantly shorter than those of the control group ( t=8.12,7.99,7.63,7.21, P<0.05).Conclusion Ambroxol combined with imipenem can significantly shorten the duration of treatment ,time of body temperature returned to normal ,pulmonary rales disappeared time and the times of daily suctioning in patients with neonatal pneumonia ,ambroxol can enhance treatment effect of imipenem ,they have synergistic effect in the treat-ment of neonatal pneumonia ,,and have no significant adverse reactions ,which worthy of clinical use .
2.The management of facial nerve tumors involving the internal auditory canal.

OBJECTIVE:
10 summarize tne clinical features of the facial nerve tumors involving the internal auditory canal and promote the management of facial nerve tumor.
METHOD:
We retrospectively reviewed the clinical manifestations, the experiences of diagnosis and treatment of the facial nerve tumor involving the internal auditory canal. All these 5 cases were enrolled during January 2013 to Apr 2015.
RESULT:
Among the 5 cases, 3 cases were facial neurilemmoma and the others were facial neurofibroma. The main symptoms of facial nerve tumors involving the internal auditory canal most commonly were facial paralysis companied with hearing loss. All the patients accepted the surgical treatment with various approaches, 3 cases of translabyrinthine approach, 1 case of middle fossa approach, and 1 case of combination of translabyrinthine and transotic approach. Total tumor resection were achieved in all 5 cases. Facial-hypoglossal nerve anastomosis was performed in one case, another case was undergone great auricular nerve graft.
CONCLUSION
Surgical intervention for patients with facial neuroma involving internal auditory canal should be considered when facial weakness has deteriorated to grade 4. The management should be based on the patient's hearing, facial nerve function, tumor size and invasive extension to select the appropriate surgical procedures.

3.Diagnostic Value of MR Urography in Ureteral Obstruction in Children

Objective To study the diagnostic value of magnetic resonance urography (MRU) in the urinary tract obstruction(UTO) in children.Methods 65 children with UTO underwent intravenous pyelography(IVP),B-ultrasound(BUS) and MR urography(MRU) ,MRU using hearvily T_2-weighted fat suppression turbo echo sequence.MRU features were compared with that of operation.The accurate rate in diagnosis of UTO with MRU was analysed comparatively with that of IVP and BUS.Results The lesions diagnosed with MRU included stricture of ureteropelvic junction in 36 children,and duplication of renal pelvis in 3 children (diagnostic error in 1),primary megaloureter in 9,narrowing of cystic inlet of ureter in 4,cysts of cystic inlet of ureter in 3,solitary kidney in 2,nephrolith in 5,nervous bladder in 2 and congenital renal dysplasia in 1.The accuracy was 98.5% in defining the UTO with MRU(64/65),which was superior to that with IVP and BUS(P<0.05).Conclusion MRU is of great value in the diagnosis of upper urinary tract obstruction in children.

Objective To evaluate the correlation between spiral CT ( SCT ) features and expression of cyclooxygenase-2 , nm23 in esophageal carcinoma.Methods Spiral CT scans were preformed in 44 patients with esophageal carcinoma before operation, and the results were compared with that of operation and pathology. Immunohistochemical streptavidin peroxidase conjugate method was used to analyze the expression of Cox-2 and nm23 in esophageal carcinoma. Statistical analysis was performed with SPSS 12.0 software. Objective To explore the changes of MR diffusion imaging (DWI) appearance in newborn rats with hypoxic‐ischemic brain damage(HIBD) ,and its relationship with the changes of Triphenyl tetrazolium chloride (TTC) staining .Methods Using liga‐tion of the left carotid artery method to establish three different degrees of HIBD animal models ;DWI was performed at each time point(6-12 h ,12-24 h ,3 d ,7 d);Fresh brain tissue taken from another model groups of newborn rats in 12 h ,24 h ,3 d ,7 d were staining in TTC ,then we observed its relationship with DWI .Results The lesion location of three model groups mainly distributed in the left side of cortex and subcortical region ,with prolonged hypoxia time ,hippocampus ,lateral side white matter ,thalamus were also have varying degrees of involvement .The right side of the cortex and subcortical in some cases involved .TTC staining showed posi‐tive results in 3 d ,its loss stained area were consistent with DWI abnormal signal area .Conclusion DWI can be evaluation of HIBD lesions early .The early lesions of HIBD mainly distributed in the left side of cortex and subcortical region .
6.Determination of osthole and imperatorin in Compound Shajiziyou Suppository by RP-HPLC

AIM:To develop a HPLC method of determining osthole and imperatorin in Compound Shajiziyou Suppository(oil of Semen Hippophae,Fructus Cnidii,Radix Sophorae flavescentis,etc.). METHODS: Phenomnex(luna) C_(18) column was used at(25 ?C).The mobile phase consisted of methanol-water(65(∶)35).The flow rate was 1.0 mL/min.The detection wavelength was set at 310 nm. RESULTS: The linear ranges of osthole and imperatorin were 0.051-0.816 ?g(r=0.999 6) and 0.026-0.416 ?g(r=0.999 8),respectively,The average recoveries were 98,4% with RSD of 1.4% and 97.6% with RSD of 2.0% respectively. CONCLUSION: This method is simple,accurate,reproducible and can be used for the determination of osthole and imperatorin in Compound Shajiziyou Suppository.

Objective To observe the impacts of the transfection of antisense DNA methyltransferase Ⅰ(DNMT1) gene fragment into SMMC 7721 cell on the changes of cell sensitivities to tumor necrosis factor related apoptosis inducing ligand(TRAIL). Methods The eukaryon expression vector pCl neo was transfected into SMMC 7721 cells by liposomes and the transfection was identified by PCR. Survival cell rate was measured by trypan blue exclusion, apoptotic rate was determined by in situ TdT dUTP terminal nick end labeling(TUNEL) method. Results PCR detection indicated that the eukaryon expression vector pCl neo was successfully transfected into SMMC 7721 cells. Aim: To search for novel modulators of glycogen metabolism through structural modifications of natural pentacyclic triterpenes. Methods: A series of N-heterocyclic derivatives were synthesized by fusing indole, qui-noxaline and pyrazine rings with A-ring of oleanolic and ursolic acids. The compounds were biologically evaluated for their inhibitory activity against rabbit muscle glycogen phosphorylase. Results and Conclusion: Twelve heter-ocyclic triterpene derivatives were synthesized and their structures were confirmed by IR, ~1H NMR, ~(13)C NMR and MS. Except for compound 12, all of the compounds exhibited glycogen phosphorylase inhibitory activity with IC_(50) values in the range of 14-252 μmol/L Among this series of compounds, compound 15 showed the best potency with IC_(50), of 14 μmol/L

Objective To understand the clinical features of hepatitis B virus(HBV)/hepatitis C virus(HCV)coinfected patients with different virological profiles.Methods The clinical data of 186 patients with HBV/HCV coinfection from May 1999 to May 2010 in the Third Affiliated Hospital of Sun Yat-Sen University were analyzed retrospectively.The demographic data,epidemiological data,laboratory results and pathological index were analyzed.The statistical analysis was done using t test and chi square test.Results A total of 186 patients were divided into 4 groups:66(35.5%)in HBV DNA(-)/HCV RNA(-)group,8(4.3%)in HBV DNA(+)/HCV RNA(+)group,68(36.6%)in HBV DNA(+)/HCV RNA(-)group and 44(23.7%)in HBV DNA(-)/HCV RNA(+) group.The gender composition,complication incidence,transmission among drug users,alanine aminotransferase(ALT)level,total bilirubin(TBil)level,prothrombin activity(PTA)and hapatitis B e antigen(HBeAg)negative rate were all significantly different among four groups(F or x2=11.578,8.451,11.738,2.669,5.102,4.254 and 18.413,respectively;all P<0.05).In groups of HCV RNA(-)and HCV RNA(+),the proportions of patients infected through drug abuse were 49.3%and 23.1%,respectively(x2=9.987,P:0.002)and blood transfusion transmission were 29.9%and 46.2%,respectively(x2=4.412,P=0.036).When HBV DNA was negative,the median ALT levels in HCV RNA(-)and HCV RNA(+)patients were 177 U/L and 62 U/L,respectively(t=2.200,P<0.05),median TBil levels were 133 μmol/L and 20μmol/L,respectively (t=3.608,P<0.05)and PTA were 70.6%±27.7%and 83.3%±27.8%,respectively(t=-1.982,P<0.05).The HBeAg negative rate was not affected by HCV RNA levels(t=0.707,P>0.05).The HBeAg negative rate in HBV DNA(-)patients was 85.5%,which was higher than that in HBV DNA(+)patients(59.2%)(x2=16.393,P<0.05).Conclusions HBV DNA(+/-)/HCV RNA(-)profile were major components in HBV/HCV confection.HBV DNA level is related to disease progression and prognosis,but not relate to disease severity.Liver function damage and disease severity are aggravated with HCV RNA level decreases.HBV DNA level is related to HBeAg negative rate,while HCV RNA level is not related to HBeAg seroconversion rate.

The authors introduced,against the backdrop of the new round of accreditation,organization and practice of the hospital.In accordance with the five management elements of planning,organization,leadership,coordination and control,and the level management theory,the hospital divided,based on a top-down design and step-by-step implementation,the process into four stages of mobilization and deployment,study and training,self-assessment and rectification,supervised self-assessment and constant improvement.These efforts aim at exploring the key points and methodology of hospital accreditation,proposing such key points as the combination of the accreditation with building a long-term mechanism,that of theory with practice,that leadership with full staff involvement,that of top-down design with step-by-step implementation,that of training and rectification,that of self-assessment and supervision,and that of system management with implementation of provisions.This way the hospital accreditation may upgrade the hospital as a whole.
