1.Nonaneurysmal perimesencephalic subarachnoid hemorrhage: clinical and imaging features

ObjectiveTo investigate the clinical and imaging features of nonaneurysmal perimesencephalic subarachnoid hemorrhage (NAPSAH).MethodsThe patients with NAPSAH were retrospectively enrolled, and their clinical and imaging features were collected and analyzed.Results In 489 patients with subarachnoid hemorrhage treated over the same period, a total of 23 (4.7%) with NAPSAH were diagnosed with cranial CT and CT angiography.Subarachnoid blood centered in the prepontine cistern in 4 patients (17.39%), interpeduncular cistern in 8 (34.78%), ring pool in 7 (30.43%), and quadrigeminal cistern in 4 (17.39%).Two patients were transferred to other hospital for treatment, and the remaining 21 patients had a good outcome without complications and neurological deficits.Among them, the modified Rankin scale scores in 17 patient were 0, and 4 were 1.Their cardinal symptoms were forgetfulness, irritability, anxiety, and other subjective symptoms.They were improved after receiving psychological counseling and anti-anxiety treatment.ConclusionsThe incidence of NAPSAH is low.The clinical symptoms in patients with NAPSAH are mild, the outcomes are better without obvious complications.
2.Purification of adipose tissues prior to primary culture of stem cells

BACKGROUND: Adipose tissue-derived stem cells receive a high attention in tissue engineering research. Adipose tissue-derived stem cells lack of specific surface marker, there is no effective purified method. Purified adipose is a simple method to elevate purify of stem cells. OBJECTIVE:To analyze how to purify adipose tissues before primary culture of adipose tissue-derived stem cells. DESIGN, TIME AND SETTING: The controlled animal experiment was performed at the Shanghai Animal Center of Experimental Medicine of Shanghai Sixth People’s Hospital between December 2007 and March 2008. MATERIALS: Four-week old Sprague Dawley rats were used for obtaining adipose tissues from the inguinal groove. METHODS: Adipose tissues from rat inguinal groove were dissected to educe superficial blood vessel and blood vessel branches. Both blood vessel inside and elliptic nodal tissues surrounding blood vessels were excised. MAIN OUTCOME MEASURES: Stained elliptic nodal tissues stained by Hematoxylin-Eosin were observed with a microscope to make sure what kind of tissues they are. The purified adipose tissues and unpurified adipose tissues were stained by Hematoxylin-Eosin. The differences in their tissue construction were observed using the microscope. RESULTS: Elliptic nodal tissues stained by Hematoxylin-Eosin were proved to be lymphatic tissues. The tissue construction of purified adipose tissues was pure, and the cellular component was simple. Conversely, the tissue construction of unpurified adipose tissues was complicated, and cells were various with complicated components. CONCLUSION: The component of adipose tissues used to primary cultured adipose tissue-derived stem cells is complicated. As resection of superficial blood vessel, skin and muscle tissues, blood vessel inside tissues and lymphatic tissues should also be excised.
3.Holmium:YAG laser resection combined with intravesical chemotherapy for superficial bladder tumor (report of 30 cases)

Objective To evaluate Ho:YAG laser in the treatment of superficial bladder tumors. Methods 30 patients with superficial bladder tumors underwent endoscopic holmium laser resection with postoperative intravesical instillation therapy.There were 25 male patients and 5 famale with an average age of 54.The growth has been single in 15 and multiple in 15.The diameter of the tumor was 0.2~3.5 cm with the pathology grade G 1~G 2 and clinical stage T 1~T 2. Results The procedure has been successful in all and cold cup biopsies of the tumor base and incisal margin were negative.The mean resection time was 18 minutes,the mean volume of bleeding was less than 15 ml and no intraoperative bladder perforation or secondary hemorrhage occurred.All patients have been followed up for 3 to 14 months with a mean of 7.5 months.One patient (3%) had out-of-field recurrence. Conclusions Ho:YAG laser resection of bladder tumor (HoLRBt) provides an useful clinical therapeutic method that has the advantages of precise resection,simple technique,as well as no serious intraoperative or delayed complications.
4.Primary fibroepithelial polyp of ureter (report of 7 cases)

Objective To highlight diagnosis and treatment of primary fibroepithelial polyp of ureter. Methods 7 cases of primary fibroepithelial polyp of ureter treated from May of 1990 to July of 2002 were analyzed retrospectively.Among them ,18~56 age of year,and right side in 5 cases,left in 2 cases.The foci are in upper,middle and lower part 2,3,2 cases respectively.The diagnosis was all ascertained on pathological studies. Results Hematuria and flank pain are the usual presentation.Preoperative image will reveal the lesion but the diagnosis of fibroepithelial polyp cannot be made by radiographic means alone.Surgical intervention was undertaken in all 7 cases.The initial case has undergone unnecessary nephroureterectomy as misdiagnosis of carcinoma of ureter.Surgical exploration and polypectomy was performed in 4 cases and polypectomy with holimlaser under ureteroscopy in 2 cases.The therapeutic results were satisfactory without relapse in 3 months~11 years follow up. Conclusions The diagnosis may be established with intravenous urogram and retrograde pyelography and surgical exploration may be performed when necessary.With the advent of ureteroscopy, a new minimally invasive approach to fibroepithelial polyp of ureter was introduced.
5.Clinical significance of CD4+ CD25+ CD127 low regulatory T cells expression in the peripheral blood of patients with acute cerebral infarction

Objective To investigate the expression of CD4 +CD25 +CD127 low regulatory T cells in peripheral blood mononuclear cells( PBMNCs) in order to explore the influence on the immune status and disease progression in the different period of acute ischemic stroke. Methods 60 patients with acute cerebral infarction were selected as the stroke group,and divided them into the 24~48 h group (n=16), 3~7 d group (n=22) and 8~14 d group (n=22);22 healthy human were set as the control group. To analyze the percentage of CD4 +CD25 +CD127low regu-latory T cells in the peripheral blood of acute cerebral infarction patients and healthy human with flow cytometry. Results The percentages of CD4 +CD25 +CD127 low and CD4 +CD25 high regulatory T cells in the peripheral blood of the stroke group were significantly decreased at 24 ~48 h,3 ~7 d ( P <0.01 ) when compared with the control group, and then tended to return toward the control value at 8~14 d (P>0.05);the percentage of CD4 + regula-tory T cells in the peripheral blood of the stroke group was significantly higher than control group ( P <0.05 ) . Conclusion Imbalance of regulatory T cells is very likely to play an important role in the immunological injury of acute ischemic stroke. Regulatory T cells may be involved in the pathogenesis of acute ischemic stroke,and early detection may provide a basis for treatment.
6.Envelope Extraction of Heart Sounds Based on Hilbert-Huang Transform

Objective Based on the analysis of time domain of heart sound with envelop to extract the envelope character of heart sounds.Methods The envelope extraction of heart sounds based on Hilbert-Huang Transform was given.Firstly,the original heart sounds signal was preprocessed by Huang Transform.Secondly,the envelope of heart sounds was got with Hilbert Transform.Results The first heart sound and the second heart sound were extracted,and all kinds of characters in time domain of heart sound were acquired more accurately.Conclusion The envelope of heart sound is extracted correctly.The foundation for further analysis of heart sounds is established.
7.Cartilaginous tumors of the cricoid: a case report.

Review a case of cricoid cartilaginous tumour in our hospital in December 2013 retrospectively, which received laryngofissure surgery and was followed up for 1 year. The case is still alive without recurrence. The incidence of laryngeal cartilaginous tumour is low, most are cases of cricoid cartilaginous tumour. Try to preserve laryngeal function in surgery as much as possible.

8.Testing method of human body's current threshold for perception based on EEG analysis.

Electric and electronic products are required to pass through the certification on electrical safety performance before entering into the market in order to reduce electrical shock and electrical fire so as to protect the safety of people and property. The leakage current is the most important factor in testing the electrical safety performance and the test theory is based on the perception current effect and threshold. The traditional method testing the current threshold for perception only depends on the sensing of the human body and is affected by psychological factors. Some authors filter the effect of subjective sensation by using physiological and psychological statistical algorithm in recent years and the reliability and consistency of the experiment data are improved. We established an experiment system of testing the human hody's current threshold for perception based on EEG feature analysis, and obtained 967 groups of data. We used wavelet packet analysis to detect a wave from EEG, and used FFT to do spectral analysis on alpha wave before and after the current flew through the human body. The study has shown that about 97.72% alpha wave energy changes significantly when electrical stimulation occurs. It is well proved that when the EEG feature identification is applied to test the human body current threshold for perception, and meanwhile alpha wave energy change and human body sensing are used together to confirm if the current flowing through the human body reaches the perception threshold, the measurement of the human body current threshold for perception could be carried out objectively and accurately.

9.Pathogens and Drug Resistance in Intensive Care Unit Patients

OBJECTIVE To investigate the bacterial distribution and the extent of drug resistance in ICU patients,and offer the first-hand information to the clinical preventive and therapeutic countermeasures. METHODS The antimicrobial susceptibility tests to 28 commonly used antibiotics were performed using the ATB Expression of Bio-Merieux with K-B method.The ESBLs were detected by the disk diffusion tests and the confirmatory tests,and the MRSA,MRCNS,and VRE were also tested at the sametime. RESULTS Totally 264 strains were isolated from the 201 positive samples,among them 192 strains were Gram-negative bacteria,43 strains were Gram-positive ones,and 29 strains were fungi.The percentage of these three groups were 72.7%,16.3% and 11.0%,respectively.The main strains of the Gram-negative bacteria were PAE,ABA,KPN,ECO and SMA,and of the Gram-positive bacteria were EC,SAU and CNS.The major strain of fungi was C.albicans.The pathogens tested showed high drug resistance.The Gram-negative bacteria showed tendency of sensitivity to IPM,AZT,CAZ,FEP,SFC,AMK and CIP,and the Gram-positive bacteria to VAN,SXT,RIF and NIF.For KPN and ECO,the percentage of strains producing ESBLs were 64.7% and 64.3%.And the percentage of MRSA,MRCNS,VRE were 80%,66.7% and 22.2%,respectively. CONCLUSIONS It was showed that the major pathogens infected the ICU patients are Gram-negative bacteria,and the pathogens show the high drug resistance.Doctors should pay more attention to analyze the bacterial resistance profile in order to decrease the incidence of drug resistance and use the antibiotics properly.
10.Clinical efficacy and ultrasonographic evaluation of anti-thyroid drugs in the treatment of primary hyperthyroidism

Objective To investigate the clinical efficacy and ultrasonographic changes treated with anti-thyroid drugs ( ATD ) in patients with primary hyperthyroidism(PHT).Methods 83 cases of PHT patients admitted to the department of ultrasound from February 2013 to August 2015 in Zhuji People's Hospital of Zhejiang Province were selected, the patients were divided into two groups according to the results of ultrasonography, echo nonuniform group 41 cases and echo uniform group 42 cases.ATD therapy was administered to the both groups (were treated with methimazole).The thyroid related indexes and hemodynamics of thyroid right superior diagnosed by color doppler ultrasound, and the curative efficacies of the two groups were analyzed.Results After treatment, the systemic vascular resistance index(RI) of echo uniform group was (0.62 ±0.17), and the echo nonuniform group was (0.65 ±0.18), there was no significant difference between the two groups.The echo uniform group of thyroid artery diameter on the right lobe(D), the peak systolic velocity(Vmax), minimum diastolic blood flow velocity(Vmin), thyroid volume(V) and blood flow volume(Q) were significantly lower than the echo nonuniform group, the difference was statistically significant (P <0.05).The total effective rate of the echo nonuniform group (48.78%) was significantly lower than that in the echo uniform group (85.71%), the difference was statistically significant(P <0.05).Conclusion ATD has a good effect in treating PHT, and can significantly improve the thyroid hemodynamics in patients, it has a positive effect on the regulation of thyroid-related parameters in patients, the clinical treatment is effective, and the effect is better especially in patients with uniform echo.
