1.THE EFFECT OF DIFFERENT CONCENTRATIONS OF HYPERTONIC SALINE FOR CERE-BRAL EDEMA ON BLOOD BIOCHEMICAL AND OSMOTIC PRESSURE

Objective To explore the effect of different concentrations of hypertonic saline for cerebral ede -ma on blood biochemical and osmotic pressure .Methods 60 cases were randomly divided into the 3% HS group, 7.5%HS group and the mannitol group .The patients were randomly assigned to infusion of 3% HS, 7.5% HS or 20%mannitol (all 2 mL/kg).Blood biochemical and osmotic pressure on patients were measured before and after the treatment among groups .Results Serum Na +, Cl -and osmotic pressure were significantly higher at 2 hours and 22 hours of reperfusion than at baseline (p<0.05) in the 7.5% HS group and the mannitol group .The difference were significant on serum Na +, Cl-and osmotic pressure at 2 hours and 22 hours of reperfusion in the 7.5% HS group and the mannitol group than in the 3% HS group(p<0.05).Conclusion 3% hypertonic saline had less effects on blood biochemical and osmotic pressure than other groups for patients of cerebral edema .It is worthy of popularization and application .
2.Research on Prognosis and Severity Evaluation of Serum Procalcitonin to Sepsis

Objective To study the serum procalcitonin (PCT)in patients with sepsis clinical application value,for the hospi-tal,infection control,to provide a scientific basis for reasonable Shaanxi General of antibiotics.Methods Retrospective anal-ysis of Shaanxi General hospital of CARF in 2011 May~2013 May 201 cases of patients with ICU,According to the severi-ty of infection were divided into 4 groups(SIRS)63 cases,(Sepsis)40 cases,(Severe sepsis)70 cases,(Septic shock)19 ca-ses,Dynamic analysis of ICU monitoring in 1,3,5,7,10,14,17 days each in the blood in patients with PCT,hs-CRP and WBC levels and blood culture,and PCT on the sensitivity,sepsis diagnostic specificity,positive predictive value,and negative predictive value,diagnostic accordance rate.Results PCT levels of SIRS and Sepsis (1.92 ng/ml)(10.8 ng/ml)group,the Severe Sepsis (24.0 ng/ml),and Septic shock compared (34.0 ng/ml)group,the difference was statistically significant (q1=13.8,q2=15.6,q3=17.9,P<0.05).Four groups of hs-CRP and WBC count,and there was no statistically significant difference (F=5.10,P>0.05),PCT levels and the severity of sepsis in positive correlation (r=0.781).The positive rate of blood culture in 26.6%,and PCT the on diagnosis of sepsis sensitivity,specificity,positive predictive value (PP),negative predictive value (NPV)were 87.2%,85.4%,78.9% and 89.3%,respectively,for sepsis diagnosis coincidence rate 85%, the increase of PCT level(≥ 1 ng/ml)was anindependent index 28 natural deposit rate.Conclusion Compared with PCT hs-CRP and WBC could better evaluate the severity of sepsis,and dynamic j udgment of sepsis disease prognosis was more sensitive.Joint detection with blood culture could improve the of accuracy differential diagnosis of sepsis,and to guide clinical rational application of antibiotics.The change of concentration of PCT daily can help predict mortality risk in patients with sepsis in ICU of the hospital,reduce complications and reduce mortality.
3.Clinical Distribution and Drug-resistance Analysis of 1 587 Escherichia coli

Objective To analyze the β-lactamases-producing Escherichia coli which were isolated from hospital specimens from Jun 2012 to Dec 2014,and provide a more accurate evidence to guide drug-selecting for antibacterial use.Methods An-alyzed 1 587 E.coli from patient sample selected between Jun 2012~Dec 2014 using the microbial analysis system manufac-tured by ZHUHAI DL BIOTECH CO.LTD,A phenotypic test was also conducted to test ESBLs.Results 1 587 stains of E-.coli were isolated,which counted for 23.9% of the specimens;901 stains (56.8%)out of total 1 587 stains appeared to be ESBLs-producing E.coli,and others were non-ESBLs-producing E.coli.The 1 587 Escherichia coli mainly came from De-partment of Urology (408,weight 25.7%),Department of Endocrinology (271,weight 17.1%),Department of Respiration (249,weight 15.7%).Rine specimen contributed 609 stains (47.3%),411 E.coli stains (31.9%)were discovered in spu-tum samples,and 83 stains (6.5%)showed in blood samples.The resistance to imipenem were discovered in the analysis. The rates of resistance to penicillins,cephalosporins were over 90.0%,the rates of resistance to Levofloxacin,ciprofloxacin were above 70.0%,and resistance rates to amoxicillin-clavulanic acid,Ticarcillin-clavulanic acid,piperacillin/tazobactam, amikacin were lower than 7.7%.Conclusion The drug-resistance of the E.coli were increasing over the past three years. There was a significant portion of MDR and PDR present in the trend.Thus,the reinforcement of the drug-resistance survey and testing have a far-reaching meaning to promote rational drug selecting.
4.The clinical significance of ultrasonography in the diagnosis of simple polyhydramnios

Objective To investigate the clinical significance of ultrasonography in the diagnosis of simple polyhydramnios . Methods 186 singleton pregnancies cases of inpatient and outpatient diagnosed with simple polyhydramnios were divided into three groups with a two-dimensional semi-quantitative method ,separate amniotic fluid index (AFI)> 20cm (A group) ,separate maxi-mum amnionic fluid volume(AFV)>8 cm(B group) ,AFI≥20 cm and AFV≥8 cm(C group) ,and the changes were dynamically monitored .Results Occurrence rates of persistent polyhydramnios and fetal malformations in C group was higher than in the other two groups ,and the difference showed no statistical significance(P<0 .05);No correlation was found between recurrent polyhydr-amnios and fetal malformation among these three groups(P>0 .05);Among the 84 .4% of simple polyhydramnios ,no fetal malfor-mation was found .Conclusion Ultrasound diagnosis of simple polyhydramnios shows clinical value in assessment of fetal malfor-mation ,fetal outcome ,perinatal morbidity and prognosis ,and is good for prenatal counseling and treatment .
5.Study on oral absorption enhancers of astragalus polysaccharides.

Astragalus polysaccharides was lounded to 4-(2-aminoethylphenol), followed by labeling the APS-Tyr with fluorescein-5-isothiocyanate (FITC) at the secondary amino group. The absorption enhancement effects of low molecular weight chitosan and protamine on astragalus polysaccharides were evaluated via Caco-2 cell culture model. The results show that the fluorecent labeling compound has good stability and high sensitivity. On the other hand low molecular weight chitosan and protamine also can promoted absorption of the astragalus polysaccharides without any cytotoxity, and the absorption increase was more significant with increasing the amount of low molecular weight chitosan and protamine. At the same time, the low molecular weight chitosan has slightly better effect. The transepithelial electric resistance (TEER) of Caco-2 cells show that absorption enhancers could improve its membrane transport permeability by opening tight junctions between cells and increasing the cell membrane fluidity.

To study the mechanisms of total flavonoid from Glycyrrhizae Radix et Rhizoma (TFGR) and its ingredient isoliquiritigenin (ISL) on their regulation of M2 phenotype polarization of macrophages. IL-4 (60 μg x L(-1)) induced RAW264.7 cells for 6 h to establish the M2 macrophage model. TFGR and ISL restrained breast cancer cells migration with the aid of M2 macrophages in vitro. TFGR and ISL inhibited gene and protein expression of Arg-1, up-regulated gene of HO-1 and protein expression of iNOS, enhanced the expression of microRNA 155 and its target gene SHIP1, meanwhile down-regulated.the phosphorylation of STAT3 and STAT6. So TFGR and ISL were the bioactive fraction and ingredient in Glycyrrhizae Radix et Rhizoma to reverse M2 phenotype macrophages polarization. TFGR and ISL inhibited the promotion of M2 macrophages to breast cancer cells migration in vitro, STAT signal pathways and miR155 were partly involved.

7.Research advances of photosensitizer in photodynamic therapy of glioblastoma

Glioblastoma is a malignant tumor in central nervous system, which has strong invasion, poor prognosis and short survival time. At present, the main treatment strategy of glioblastoma is surgical excision, supplemented by radiotherapy and chemotherapy. However, due to incomplete resection and high recurrence rate, it is urgent to find novel therapeutic method for glioblastoma. Photodynamic therapy, as a promising non-surgical treatment, provides a new strategy for postoperative adjuvant therapy of glioblastoma. This review summarizes the mechanism and clinical application of photodynamic therapy mediated by various photosensitizers in glioblastoma, in order to provide help for the treatment of glioblastoma.
9.Effect of Shuwel Decoction on Enteric Nervous System-Interstitial Cells of Cajal-Smooth Muscle Network Structure Injury in Deep Muscle Nerve Plexus of Functional Dyspepsia Rats.

OBJECTIVETo observe morphological changes of enteric nervous system (ENS)-interstitial cells of Cajal (ICC)-smooth muscle cell (SMC) structure injury in deep muscle nerve plexus offunctional dyspepsia (FD) rats, and the repair of Shuwei Decoction (SD) on it, and to explore its effecton FD.
METHODSTotally 72 rats were randomly divided into the control group, the model group, the lowdose SD group, the medium dose SD group, and the high dose SD group, the Mosapride group, 12 ineach group. Rats in the low dose SD group, the medium dose SD group, and the high dose SD group were intragastrically fed with SD at 0.767, 1.534, 3.068 g/mL, respectively. Rats in the Mosapride group were intragastrically fed with Mosapride (1.37 mg/kg). FD rat model with Gan depression Pi deficiency syndrome (GDPDS) was established using complex pathogenic factors. Corresponding liquors were respectively administered to rats in corresponding groups from the 3rd day after modeling. Distilled water(10 mL/kg) was administered to rats in the control group and the model group, once per day for 14 successive days. Rats were sacrificed and small intestine tissues collected for observing ENS-ICC-SMC structure injury using immunofluorescence double labeling, laser scanning confocal microscope, and transmission electron microscope at day 15. Repair of SD on it was also observed.
RESULTSENS-ICC SMC structure was incomplete, with obvious injury in mutual link of ICC, ICC, SMC, and connecting structure. ENS-ICC-SMC structure was more complete in high, medium, and low dose SD groups, with close link of ICC and SMO. Their connecting structures were in good conditions.
CONCLUSIONSD could keep the integrity of ENS-ICC-SMC structure by promoting regeneration and morphology of ICC, thereby, improving gastrointestinal movement disorder and showing therapeutic effect on FD.

10.Epidemiological investigation and related risk factors analysis of allergic rhinitis in primary and middle school students in Changsha.

OBJECTIVE:
To analyze the epidemiological characteristics and related risk factors of allergic rhinitis in primary and middle school students in Changsha so that to provide the scientific basis for the disease control and intervention.
METHOD:
From June 2011 to April 2012, we randomly chose primary and middle school students aged from 10 to 17 years old in Changsha as our research objects. Through stratified sampling and cluster sampling, we carried out epidemiological investigation using questionnaire investigation, physical examination and skin prick test. After statistical analysis of the data,we explored the related risk factors.
RESULT:
(1) 7,023 copies of questionnaire were sent out and 6,407 copies of valid questionnaire were received in the stratified sampling survey. The prevalence of complaining about having got allergic rhinitis is 44.6% while the real rates is 20.5%; (2) 814 research objects were investigated in cluster sampling survey. The result of the incidence of allergic rhinitis is 26.3%, and the prevalence of allergic rhinitis is 17.2%. Analysis of Visual Analog Scale Questionnaire showed significant influence of allergic rhinitis on students character, memory, sleep quality, etc.
CONCLUSION
The prevalence of allergic rhinitis in primary and middle school students aged at 10-17 years old in Changsha is 17%-20%. The occurrence and development of allergic rhinitis is related to environment, genetic background and living habits. Allergic rhinitis influenced the students' character, memory and sleep quality significantly.
