We report a case of retinal detachment in morning glory syndrome(MGS). We think that identification of the retinal break, removal of the traction force by vitrectomy, the use of long-acting gas as endotamponade, all contributed to the successful treatment of the disease. The pathogenic mechanisms of the disease and the function of B-scan to the diagnosis are included.

Objective To investigate the clinical effect of using ambroxol hydrochloride and dexamethasone by nasal spray inhalation after endoscopic sinus surgery in treating chronic sinusitis.Methods One hundred and forty patients with chronic sinusitis receiving endoscopic sinus surgery in our hospital from January 2014 to December 2014 were selected as the research subjects.After surgery,the control group was given dexamethasone nasal spray inhalation,while the observation group was given ambroxol hydrochloride and dexamethasone by nasal spray inhalation.The curative efficacy,the score of visual analogue scale(VAS) was adopted to evaluate the subjective symptoms severity and the nasal endoscopy and CT examination results were evaluated by the Lund-Kennedy score.The serum levels of IL-5 and IL-12 were detected in the two groups.Results All cases were successful completed surgery and no severe adverse reactions appeared after surgery.The observation group had a total effective rate of 97.1 ％,which was signif icantly higher than 88.6 ％ in the control group,the difference was statistically significant(P＜0.05).In the aspects of sinusitis related symptoms after operation,compared with the control group,the VAS scores of nasal discharge,nasal obstruction,cheek pain,headache and hyposmia in the observation group were significantly decreased,the difference was statistically significant(P＜0.05).In the postoperative re-examination process,the score of assessment of nasal endoscopic score and paranasal sinuses CT score in the observation group were significantly decreased,the difference was statistically significant(P＜0.05).In the serological indicator determination,serum levels of IL-5 and IL-12 after treatment in the observation group were significantly lower than those in the control group,the difference was statistically significant(P＜0.05).Conclusion Using ambroxol hydrochloride combined with dexamethasone by nasal spray inhalation after endoscopic sinus surgery has good efficacy in treating chronic sinusitis,can decrease serum levels of IL-5 and IL-12.

Objective To discuss the diagnostic and therapeutic value of duodenoscopy for chronic pancreatitis. Methods A total of 67 patients with chronic pancreatitis were treated with duodenoscopy in our hospital from January 2000 to December 2006. Among the patients, 19 patients who had duodenal papilla stenosis without obvious dilation of the pancreatic duct were treated with EST; 48 patients who had dilation of the pancreatic duct underwent EST combined with EPS. ERBD was performed on 21 patients who had stenosis of the inferior segment of the common bile duct, among which 12 received insertions of two stents into the biliary pancreatic duct. In 27 patients with stones in the pancreatic duct, the stenotic segment of the pancreatic duct was dilated after EPS, and then the stones were removed using basket or balloon. Afterwards, ERPD was performed if necessary. These patients were followed up with ERCP every 3 months, stents were used if necessary. Results Duodenoscopy was completed in 62 of the 67 patients with a success rate of 92.5% (62/67). Abdominal pain disappeared or markedly relieved in 60 cases after the treatment showing an effective rate of 96.8% (60/62). The operation was ineffective in 2 patient, who had no relief of their abdominal pain. The failure of stent insertion was due to the multiple stenosis of the pancreatic duct. Abdominal pain was markedly relieved in 21 patients who received ERBD and 38 patients who underwent ERPD. During a 39.5-month follow-up (5-70 months), no recurrence of the symptom was found. In the 27 patients with stones in the pancreatic duct, the stones were removed in 19 patients. After the treatment, 15 patients developed transient hyperamylasemia, 5 had acute pancreatitis, 3 had hemorrhage. No perforation, conversion to an open surgery, or death occurred in this series. Conclusions Duodenoscopy, which is safe, effective, and minimally invasive, is the first choice for chronic pancreatitis. The treatment outcomes can be improved by combining duodenoscopy with other therapies.

Objective To study the clinical and pathologic features,and endoscopic diagnosis as well as therapy and prognosis of the primary gastrointrestinal lymphoma(PGIL), and to improve its diagnosis and therapy efficiency. Methods The clinical data in 29 cases of PGIL were retrospectively reviewed. Results In these cases, the tumor was mainly located at stomach (n=21), colon(n=6) and small intestine(n=2). The type of pathology was mucosa associated lymphoid tissue lymphoma(n=8), diffuse large B-cell lymphoma(n=18), T-cell lymphoma(n=3). The commom manifestation include abdominal pain, anaemia, abdominal distention, melena or hemafecia, loss of weigh etc. The tumor appeared infilitrative in 11 cases, ulcerative in 10 cases, protruding in 6 cases, 9 cases were diagnosed by firstly routine biopsy endoscopiccally. 10 cases were treated by operation and chemotherapy, 19 cases were treated by chemotherapy. Up to now, 6 cases survival in operation group, 13 cases survival in chemotherapy group, the difference was not statistically significant . Conclusions PGIL was mainly located at stomach, which was not specific clinical manifestation; The type of pathology was mainly diffuse large B-cell lymphoma; the diagnositic rate of routine endoscopic biopsy was low. chemotherapy was the first treatment of PGIL.

Objective To assess the diagnostic value and safety of push enteroscopy in intestinal diseases. Methods From March.1998 to December 2006,345 patients were examined by push enteroscopy.Results The enteroscope bypassed the Treize ligament in 318 cases, The average examination spend 30 minutes. 144 patients were diagnosed intestinal diseases, the detection rate was 41.74%. 28 patients underwent endoscopic haemostasis treatment, 13 patients received endoscopic polypectomy. No complication of perforate, hemorrhage, abdominal pain occurred. Conclusions Push enteroscopy is an efective and safe method for diagnosing intestinal diseases, and could undertook corresponding therapy.

Compared with GBZ 49 2002 and GBZ 49 2007 diagnosis with GBZ 49 2014 had （ vs ， vs ， P ）， （ vs ， a higher rate of ONID 57.9% 66.0% 44.8% 66.0% both <0.01 and had a higher rate of mild ONID 47.3% 54.6% vs ， P ） - - 36.0% 54.6% both <0.01 . The diagnostic rate for ONID using GBZ 492014 was higher than those using GBZ 49 2002 and - （ P ）Conclusion - GBZ 49 2007 in each age groups all <0.01 . GBZ 49 2014 improved the diagnostic rate of ONID compared - - with GBZ 49 2002 and GBZ 49 2007. The reason is related to the inclusion of 4 000 Hz hearing threshold with a weight of 0.1 - as the diagnostic hearing threshold and the use of a new age and gender correction method in GBZ 49 2014.

Objective To determine the effects of prostaglandin E_2 alone and the combination with omeprazole for poisoning related acute mucosal lesion. Methods Forty-two healthy SD rats were randomly divided into 7 groups: control group (control), methamidophos poisoning model group (intox), 10 mg/kg omeprazole group (ome1), 50 mg/kg omeprazole group (ome2), 10 μg/kg prostaglandin E_2 group (PG1), 50 μg/kg prostaglandin E_2 group (PG2), 10 μg/kg prostaglandin E_2 combined with 10 mg/ kg omeprazole group (union). The gastric ulcer index was counted and the content of serum cholinesterase, endothelin, and tumor necrosis factor were measured, and dependability was analyzed. Results There was no obvious difference between the prostaglandin group and the poisoning model group in gastric ulcer index, endothelin, and tumor necrosis factor. The omeprazole alone and the combination treatment could improve the acute gastric mucosa lesion induced by methamidophos poisoning. Conclusion Prostaglandin E_2 alone can not obviously improve the acute gastric mucosa lesion by methamidophos poisoning. The combination with omeprazole may be a promising treatment.

Lipid droplets are the main storage organelles for intracellular neutral lipids. Many recent studies have found that lipid droplets are closely related to hepatitis C virus (HCV). Lipid droplets play important roles in the multiple processes of HCV life cycle, such as infection, replication, assembly, and secretion. In this review, we summarize the recent research progress in the roles of lipid droplets in HCV life cycle.

Objective To study the neurotoxicity of chronic aluminum overload and the protective effects of nimodipine in rats.Methods The brain damage models of rats were established via intragastric administration of aluminum gluconate(element aluminum 400 mg/kg)once a day,5 d/week for 12 weeks.The step-down test and programmed Morris water maze test were used to evaluate the changes of learning and memory functions of rats.Pathomorphological changes of hippocampi of rats were observed.The activities of SOD,ChAT,AchE,MAO-B and the contents of MDA of brain tissue in rats were also measured.Nimodipine(80 mg/kg)were intragastrically administered 4 h after aluminum administration every time for 12 weeks.Results Chronic aluminum administration induced the impairment of avoidance learning and memory ability and spatial oriental ability.Consistent with the behavioral changes,neuronal death in the hippocampi,decreased activities of SOD and ChAT,increased content of MDA,and increased activity of MAO-B and AchE were detected in the aluminum-overload mice.The administration of nimodipine could significantly protect rats from the brain damage,and behavioral and biochemical changes above caused by aluminum overload were in a dose-dependent manner.Conclusion These results suggest that changes of cellular calcium overload and oxide stress and MAO-B activities are involved in pathophysiological mechanisms of brain injury induced by chronic aluminum overload.Nimodipine has a protective effect on neurotoxicity of chronic aluminum overload.
