2.Clinical and biological significance of clonal macrophage detection in hemophagocytic syndrome.

By using the method of clonal analysis the evidence to prove that Hemophagocytic syndrome (HPS) is reactive or malignant was investigated to probe into the pathogenesis of HPS and its relations with clinical prognosis. The macrophages abnormally proliferated in bone marrow were isolated. Electrophoresis analysis was made after DNA extraction, enzyme restriction of human ardrogen receptor (HUMARA) genetic locus, and PCR amplification. In the 9 specimens, clonal proliferation was found in 2 cases and nonclonal proliferation in 7. Among the 7 cases of nonclonal proliferation, 3 were voluntarily discharged without clinical outcome, 2 cases fully recovered after 2-3 week treatment of large dose gamma globulin intravenous drip and hormone therapy, 1 case died at the 43th day after the hormone and anti-infection therapy, and one case was found to have granular leukoblast in peripheral blood after 3 weeks and diagnosed as having M2a after bone puncture. For the two patients with clonal proliferation, one obtained remission after chemotherapy and the other was died after 32 days without chemotherapy. It was concluded that there do exist clonal or malignant proliferation in HPS, so not every case is reactive.

Objective: To investigate effects of Fang Ji Huang Qi Capsules on the level of blood uric acid and blood pressure in the patient of primary hypertension of type of stagnation of phlegm, Methods: 61 patients were divided into the treatment group (34 cases) treated with Fang Ji Huang Qi Capsules plus Norvasc and the control group (27 cases) treated with Norvasc. They were treated for 6 weeks and blood uric acid level, dynamic blood pressure and TCM clinical symptoms were observed. 4.Expression of TGF-? in Villus and Deciduas after Medical Abortion

Objective To investigate the expression of transforming growth factor-?(TGF-?) in the villus and deciduas of different outcomes after administration with mifepristone and misoprostol in the termination of early pregnancy. Methods From May 2004 to March 2005,50 women who were comfirmed early pregnancy and requested termination of pregnancy in the outpatient department were recruited.Patients with medical abortion were divided into two groups according to the final outcomes: group of complete abortion(n=15) and group of incomplete abortion((n=)15).Another 20 patients with surgical abortion were served as control.The villus and deciduas were collected at the third day of medical abortion,whereas the same was done at the day of surgical abortion.The expression of TGF-? in villus and deciduas of the three groups were measured by immunohistochemical method. Protein tyrosine phosphatase (PTP) 1B is a potential target for the treatment of diabetes and obesity. We have previously identified the benzoyl sulfathiazole derivative II as a non-competitive PTP1B inhibitor with in vivo insulin sensitizing effects. Preliminary SAR study on this compound series has been carried out herein, and thirteen new compounds have been designed and synthesized. Among them, compound 10 exhibited potent inhibition against human recombinant PTP1B with the IC50 value of 3.97 micromol x L(-1), and is comparable to that of compound II.

8.Making the scale for the compliance of patients with chronic obstructive pulmonary disease during home oxygen therapy

Objective To make the scale for the compliance of patients with chronic obstructive pulmonary disease (COPD) during home oxygen therapy.Methods By literature searching,semi-structured interview,expert consultation,as well as pre test,the tentative scale was formed at first.Then the investigation was conducted among 324 patients with COPD selected.Finally the scale was formed by testing the reliability and validity.Results The scale was made up of two dimensions with 16 items in total.It showed that Cronbach α coefficient was 0.984,split-half reliability was 0.976,and content validity index was 0.98.The exploratory factor analysis combined with AMOS 7.0 confirmatory factor analysis was used for construct validity of the home oxygen therapy compliance scale.Conclusions The self-management behavior scale for patients with COPD shows good reliability and validity,which can be used to evaluate the compliance during home oxygen therapy and provide reference tool for COPD patients' family intervention.
9.Clinical and Experimental Study on Autoimplantation Technique in the Treatment of Verruca Plana

Objective To apply autoimplantation technique in the treatment of verruca plana, and to investigate the mechanisms of it. Methods The changes of T cell subsets and IL-2 level in the peripheral blood of the patients with verruca plana, in conjunction with histopathological and immunohistochemical features of verruca plana tissue at reactive stage were studied by APAAP and radioimmunoassay, respectively. The tissue specimens were studied by H.E stain and immunohistochemical techniques. Results Significantly decreased CD4+ cell numbers, CD4+/CD8+ ratio and IL-2 level were found before treatment, compared with those in healthy controls (P 0.05). Lymphocytic infiltration, epidermal edema and dissolution were shown in the histopathological study. There were CD45RO+, CD6+, CD4+ and CD8+ cells in the lymphocytic infiltration, but no CD20+ and Mac387- cells. Langerhans cells with positive S-100 protein were observed in basal layer of the epidermis and reaction area. Conclusions T cell-mediated immune responses could be improved in autoimplantation, the warts are rejected mainly by of CD8+ subset cytotoxicity.
10.Pulmonary sequestration: case report and review of the literature

Objective To highlight the characteristics of pulmonary sequestration (PS) with combination of practical experience and review of literature. Methods One patient with PS confirmed by biopsy was described and the relevant literatures of 279 cases were reviewed. Results PS can be divided into two types: intralobar type and extralobar type. The percentage of intralobar type is about 78.5%, intralobar type is 13.3%, other unreported is 8.2%. Clinically, PS was often combined by repeated pulmonary infection. The mis-diagnose rate is 72.8%. Etrograde arteriography or enhanced CT scan could find the abnormal artery, from which we can make correct diagnosis. The diagnose rate is 100%. Conclusion For patients with above clinical and X-ray symptoms, PS should be considered. Enhanced CT or retropgrade arteriography examination should be done as soon as possible. Timely diagnosis and surgery treatment are important for curing this kind of disease.
