1.Study on pharmacokinetic of vancomycin in normal and endophthalmitis eyes in rabbit

Background Vancomycin has been increasingly recommended for the management of endophthalmitis,but few research report has been published about the pharmacokinetics of intravitreal vancomycin up to now.It is necessary to have an exact method to measure the concentration of vancomyein in animal eyes after intravitreal injection.Objective This study was to observe and compare the phamacokinetical process of vancomycin in serum,vitreous and aqueous humor between normal and infected rabbit eyes.Methods Seventy-two healthy adult rabbits were randomly divided into normal group and infected group and 36 rabbits for each.The animal models of endophthalmitis were established by intravitreal inoculation of 2000 CFU/ml staphylococcus aureus in the right eyes of rabbits in the infected group.Once endophthalmitis developed,0.1 ml vancomycin ( 10 g/L) solution was injected into the vitreous of every rabbit.The peripheral blood,vitreous and aqueous humor samples were respectively collected in 4 rabbits for each group at 0.5,2,4,6,12,24,48,72 and 84 hours after injection for detection of vancomycin concentration by high performance liquid chromatography(HPLC-UV).3p97 software was used to create fit parameters of pharmacokinetics.This experiment followed the Regulations for the Administration of Affair Concerning Experimental Animals by State Science and Technology Commission (Version 1988).Results The accuracy of HPLC fitted the detecting request of biological specimen.The concentration-time data of vancomycin in normal rabbit aqueous humor and vitreous was subject to two-compartment model.The pharmacokinetic parameters were separately as following:Cmax was 50.16 mg/L and 751.42 mg/L,t1/2was 51.04 hours and 53.21 hours.The concentration-time data of vancomycin in infected rabbit aqueous humor and vitreous was subject to one-compartment model.The pharmacokinetic parameters were separately as following:Cmaxwas 24.94 mg/L and 687.66 mg/L,t1/2was 11.42 hours and 12.91 hours.The concentration of vancomycin in serum was much lower and almost undectable.The concentration of vancomycin in vitreous was gradually reduced as the prolong of time after injection in both normal group and infected group,but a obvious decline after increased level was scen in aqueous humor.Compared with normal group,the concentrations of vancomycin in both vitreous and aqueous humor were reduced at various time points(P<0.05,P<0.01 ).Conclusions HPLC is simple,highly sensitive and specific for the pharmacokinetic analysis of vancomycon.These results indicate that pharmacokinetic parameters of vancomycin alter in pathological condition,which is helpful for us to establish the better treatment guidelines for endophthalmitis.
2.Role of short haipin RNA targeting vascular endothelial growth factor-C on biological characteristics of human breast cancer cell MCF-7.

OBJECTIVETo study the effect of short haipin RNA (shRNA) on expression of vascular endothelial growth factor C (VEGF-C) and proliferation and invasion behavior of human breast cancer cell MCF-7.
METHODSThe recombinant vector (pSIREN-VEGF-C) was transfected into the human breast cancer cell MCF-7 by liposome and the positive transfected cell clones were screened with puromycin. Expression of VEGF-C in MCF-7 cells after gene transfer was detected by real-time quantitative PCR and Western blot assay, respectively. Proliferation and invasion ability of transfected cells were analyzed by MTT and Transwell filter.
RESULTSThe expressions of VEGF-C mRNA and protein were decreased markedly compared with the control group after the transfection and the inhibitive ratio was 95% and 100% respectively (P<0.05). The proliferation of MCF-7 cells transfected by pSIREN-VEGF-C, measured with MTT assays, was significantly decended (P<0.05). The invasion ability of passing through the Transwell filter of MCF-7 cells transfected by pSIREN-VEGF-C were declined evidently (P<0.05).
CONCLUSIONThe recombinant vector (pSIREN-VEGF-C) have been proved not only to be effective and specific for down-regulation of VEGF-C, but also can inhibit the proliferation and invasion of MCF-7 cells significantly.

4.Investigation of iodine nutritional status of children in Xiaodeng island of Xiamen Fujian provence in 2010

Objective To find out the iodine nutritional status of children in Xiamen island, and to provide the scientific basis for iodine supplimentation. Methods On March 2010, thyroids of all children aged 6to 12, from primary school on the Xiaodeng island of Xiamen were examined by palpation, urinary iodine, iodine content of salt athome and IQ level were tested, and were collected 20 households, iodine content of drinking water was tested randomly. Results IQ testing and thyroid palpation were carried out among a total of 156 children, the goiter rate of children was 1.28% (2/156), the mean IQ was 110; 154 urine samples were taken, the median urinary iodine was 219.1 μg/L; a total of 153 salt samples were tested, and the qualified rate of iodized salt was 87.58%(134/153), and the mean iodine content in the tap water was 4.52 μg/L Conclusions Iodine nutritional status of the island residents is better, and there are no such problems as excessive iodine.

7.Effect of diabetes liaisons on nursing in the department of neurology

Objective To investigate the influence of diabetes liaisons on the practicing skills of nurses and life quality of patients in neurology department.Methods Diabetes liaisons were established in 2009 in neurology department.40 nurses' practicing skills of diabetes nursing were compared 3 months before with 3 months after the establishment,and 120 diabetes patients' knowledge 3 months before and 112 diabetes patients' knowledge 3 months after the establishment were compared.Results The correct rates of 40 nurses' practicing skills of diabetes nursing in blood sugar monitoring,insulin pen injection and hypoglycemic emergency measures were 70%,75% and 60% 3 months before the establishment of diabetes liaisons and 100%,100%,and 100% 3 months after the establishment of diabetes liaisons.And the differences were statistically significant (x2 =69.83,72.57,60.28,respectively;P <0.01).The rates of diabetes patients' knowledge in the field of reasonable diets,drug knowledge,regular exercises,skin care and self management were respectively 76.79%,64.07%,59.82%,58.04% and 82.14% 3 months after and 29.17%,20.83%,33.33%,20.00% and33.33% 3 months before the establishment.And differences were statistically significant (x2 =44.68,46.28,13.33,27.73,47.64,respectively; P < 0.01).Conclusions Diabetes liaisons in the department of neurology can improve nurse' s practicing skills and both nurse' s and patient' s knowledge of diabetes,so as to improve nurse' s nursing quality as well as patient' s life quality.
8.Preparation of enteric nanoparticles of Schisandra total lignanoids and preliminary study on its pharmacokinetics.

To study the preparation method of Schisandra total lignanoids enteric (SLE) nanoparticles and evaluate its pharmacokinetics in rats, SLE nanoparticles were prepared by modified emulsion solvent diffusion method. The properties of SLE nanoparticles were evaluated of morphology, mean diameter and entrapment efficiency. An HPLC method was employed to determine the concentration of deoxyschisandrin (QS) and schisantherin A (SA) in plasma, which were used as an index of Schisandra total lignanoids, and the bioavailability of the nanoparticles was compared with the reference group by oral administration using SD rats. The nanoparticles observed by transmission electronmicroscopy were round, and the mean particle sizes of SLE were (36.7 +/- 4.4) nm. Entrapment efficiency of QS and SA were (97.5 +/- 0.7)% and (91.3 +/- 0.8)%, respectively. Its pharmacokinetic process calculated with 3p97 software was fitted to a one-compartment model. The pharmacokinetic parameters showed sustained-release property. Compared with reference formulation, the AUCs of SLE nanoparticles were 2.3 and 5.8 times separately. These results suggested that the incorporation into Eudragit S100 of Schisandra total lignanoids can improve the bioavailability.

9.Effects of inhaled nitric oxide on rabbits with meconium aspiration pneumonia.

OBJECTIVETo evaluate effects of inhaled nitric oxide (iNO) on the expression of lung neutrophil adhesion molecule CD(11b) in experimental meconium aspiration pneumonia treated with conventional mechanical ventilation under room air or 100% O(2).
METHODSRabbits were randomly allocated to 10 groups (n = 60), 6 of each group. Control or meconium aspiration pneumonia model groups were inhaled with room air or 100% O(2). Six treatment groups were treated with continuous NO inhalation at the doses of 6 x 10(-6), 10 x 10(-6) and 20 x 10(-6), respectively for 12 hours under room air or 100% O(2). The ratio of wet/dry (W/D) lung weight, alveolar septal width (ASW), myeloperoxidase (MPO) activity and lung injury score were measured. The expression of CD(11b) in neutrophils of the bronchoalveolar lavage fluid (BALF) was detected with flow cytometry.
RESULTSAfter 12 hours ventilation, the oxygenation was maintained better in treatment groups under different O(2) concentrations than that in model groups. Inflammatory evidence was found in lungs from all the model groups and treatment groups, which was characterized by serious inflammatory cell infiltration in alveolar space and hyaline membrane formation. The lung inflammation was decreased in all groups with nitric oxide inhalation. The ratio of W/D lung weight and ASW among different groups had no significant difference. MPO activities were significantly decreased in groups treated with 10 x 10(-6) and 20 x 10(-6) iNO compared with the model groups [with the concentration of 21% O(2), (1.8 +/- 0.2) U/g vs (4.4 +/- 0.5) U/g and (2.0 +/- 0.1) U/g vs (4.4 +/- 0.5) U/g;with the concentration of 100% O(2), (1.7 +/- 0.4) U/g vs (2.8 +/- 0.5) U/g and (1.4 +/- 0.3) U/g vs (2.8 +/- 0.5) U/g, P < 0.05, respectively]. MPO activities in the 20 x 10(-6) iNO group under 100% O(2) were significantly reduced compared with those under 21%O(2) [(1.4 +/- 0.3) U/g vs (2.0 +/- 0.1) U/g, P < 0.05]. Nitric oxide inhalation with the doses of 10 x 10(-6) and 20 x 10(-6) significantly decreased the expression of CD(11b) (MFI) in neutrophils of the BALF compared with the expressions in model groups without NO treatment (with 21% O(2), 121 +/- 20 vs 392 +/- 204 and 112 +/- 30 vs 392 +/- 204; with 100% O(2), 113 +/- 24 vs 293 +/- 65 and 102 +/- 14 vs 293 +/- 65, P < 0.05, respectively). Under the same iNO dose (10 x 10(-6) or 20 x 10(-6)) no statistic difference was found between groups of different inspired oxygen concentrations (21% and 100%).
CONCLUSIONSInhaled nitric oxide with the doses of 10 x 10(-6) to 20 x 10(-6) could significantly down-regulate the CD(11b) expression in neutrophil of the BALF and reduce the neutrophil sequestration and MPO activity in rabbit lungs, which may decrease the lung inflammation process in meconium aspiration pneumonia.

10.Jugular bulb oxygen saturation during desflurane and isoflurane anesthesia in pa tients undergoing brain tumor surgery

Objective: To investigate jugular bulb venous oxyg en partial pressure(PjO2), hemoglobin saturation (SjO2) and the arterial t o jugular bulb venous oxygen content difference(AjDO2) during anesthesia with desflurane and isoflurane in patients with brain tumor. Methods: Fifty-six patients with brain tumor were randomized into desflur ane or isoflurane for maintaining anesthesia. PjO2, SjO2 and AjDO2 in pati ents were measured during normoventilation, hyperventilation and hypoventilation . Results: During normoventilation, SjO2 and PjO2 in desflu rane group was significantly higer than those in isoflurane group(P<0.05 or P<0.01), and AjDO2 in desflurane group was significantly lower than that in isoflurane group(P<0.05).Except that PjO2 in desflurane group was si gnificantly higer than that in isoflurane group during hyperventilation (P< 0.01), there were no differences in SjO2, PjO2 or AjDO2 between the 2 g roups during hyperventilation or hypoventilation. While anesthesia with desflura ne and isoflurane, there was a positive correlation between PaCO2 and SjO2. Conclusion: At the same anesthetic effect concentration, desflur ane can significantly increase SjO2 and PjO2 in comparison to isoflurane un der normoventilation, suggesting that desflurane may have stronger effect of rel axing cerebral vessel than isoflurane.
