1.Change of Level of Blood Ghrelin in Children with Anorexia and Its Relationship with Insulin Sensitivity

Objective To observe the change of the level of blood ghrelin in the children with anorexia and its relationship with insulin sensitivity.Methods Height,weight,body mass index(BMI)of 40 children in anorexia group,20 healthy children in control group were measured;the level of blood grelin,insulin and blood glucose were determined;insulin sensitivity index(ISI) was calculated,then the results in both groups were analyzed.Results Levels of height,weight and BMI in anorexia group were significantly lower than those of control group.The ghrelin and ISI levels in anorexia group were significantly higher than those in control group,but the insulin and glucose levels were significantly lower in anorexia group than those in control group,the differences were significant.Ghrelin level showed negative correlation with insulin and blood glucose concentration in both groups and positive correlation with ISI level.Conclusions The decrease of appetite in children with anorexia may not necessarily result from under-secretion of ghrelin.The increase of insulin sensitivity in children with anorexia may be a response to high level of ghrelin secretion.
2.Comparison of microcalorimetric fingerprint profiles of Lonicerae japonicae Flos and Lonicerae Flos.

To compare the microcalorimetric fingerprint profiles of Lonicerae japonicae Flos (Lj.F) and Lonicerae Flos (L.F), microcalormietry was applied to find the heat change regularity of Bacillus shigae (B. shigae) metabolism affected by Lj.F and L.F (we choose Lonicera macranthoides Hand.-Mazz in this paper) with different concentrations. The thermogenic curves and thermodynamics parameters were investigated as evaluation index, and then the date of experiment was studied by similarity analysis. All the results indicated that the Lj.F and Lonicera macranthoides Hand.-Mazz (L.m.H-M) significantly impacted the microbial growth and had good similarity in its inhibitory activities. The combination approach of chemical analysis with bioassay was developed and employed to ensure the safety and efficacy of Chinese herbal medicines.

3.Mechanism of ER stress involved in ATO inhibited HL-60 proliferation

Aim To explore the pharmacological mechanism of arsenic trioxide (ATO) on endoplasmic reticulum(ER) stress and provide indirect pharmacological indicators for the therapeutic drugs monitoring of acute promyelocytic leukemia.Methods The ATO toxicology database was utilized to associate gene data and establish protein-protein interaction networks.MCODE algorithm was used to analyze clustering of network topology relationship in order to find the critical functional gene group.The HL-60 cells were used as the model in this study.The cytotoxicity of ATO to HL-60 was evaluated by MTT assay.Real-time PCR assay was involved in detecting the expression of stress-related genes in ER stress.Western blot was used to detect the expression of stress-related proteins in ER stress.ER-specific staining was conducted by ER-Tracker Red.Cell differentiation and apoptosis was tested by flow cytometry.Results 206 ATO-related genes had 3 794 pairs of relationships,and ER stress-related module was outstanding among the top 4 gene cluster.ATO inhibited HL-60 cell proliferation in a dose-dependent manner;4 h after administration ATO could induce ER stress.The expressions of GRP78 and GRP94 were significant and the gene expression level changed greatly over time.The expression level of GRP78,GRP94 and Nrf2 was induced by 1 ~4 μmol · L-1 ATO,and the expression of CHOP was significant by 8 μmol · L-1 ATO.ATO could induce the phosphorylation level of eIF2α protein at 1 ~2 μmol · L-1 as well as a large expression of CHOP protein markedly at 8 μmol · L-1.As shown in ER-specific staining,ATO at 4,8 μmol · L-1 induced ER swelling.Flow cytometry showed that ATO could induce cell differentiation at 1 μmol · L-1,and the differentiation group was obvious at 2 μmol · L-1.So as to the concentration of 4 and 8 μmol · L-1,but the apoptotic group emerged at 8 μmol · L-1.Conclusion ATO can inhibit the proliferation of HL-60 and in this process,ER stress is involved,which shows a concentration-dependent phaseswitching effect.
4.The level and clinical significance of interleukin-17 in primary Sj(o)gren's syndrome with interstitial lung disease

Objective To investigate the level of IL-17 and its clinical significance in patients with primary Sj(o)gren's syndrome (pSS) complicated with interstitial lung disease (ILD).Methods IL-17 levels were detected by enzyme linked immunosorbent assay (ELISA) in 53 untreated patients with pSS (25patients with only lacriminal and/or salivary gland involvement,28 with only interstitial lung disease involvement) and 15 healthy controls.The related clinical and laboratory data were recorded.ANOVA,LSDtest and Pearson test were used for statistical analysis.Results There were significant differences between the 3 groups(F=22.504,P=0.000).The mean concentration of sera IL-I7 in the patients with ILD was significantly higher than in patients with only lacriminal or salivary gland involvement (P<0.05).There was no difference in the levels of sera IL-17 between the patients with only lacriminal and/or salivary gland involvement and healthy controls (P>0.05).Conclusion Our results show that patients with pSS concomitant with ILD have high serum IL-17 levels,which highlight the role of IL-17 in Sj(o)gren's syndrome interstitial lung injury.
5.Comparison of paravertebral soft tissue tension changes in patients with chronic lower back pain treated by sliver needle and traditional Chinese medicine fumigation.

OBJECTIVETo evaluate clinical significance of waist soft tissue tension detection in treating chronic nonspecific low back pain.
METHODSFrom August 2011 to March 2012,60 patients with chronic nonspecific low back pain were divided into two groups (sliver needle group and TCM fumigation group) according to propotion of 1:1. In sliver needle group, there were 17 males and 13 females aged from 28 to 55 years old with an average age of (45.70 +/- 4.15), treated with sliver needle; In TCM fumigation group,there were 19 males and 11 females aged from 27 to 55 years old with an average age of (43.03 +/- 5.86), treated with TCM fumigation. Changes of force-displacement distance (FDD), specific absorption rate (S) of two groups were observed before treatment, 1 week and 3 months after treatment respectively, VAS scoring and Roland-Morris disability questionnaire (RMDQ) were used to access clinical effects.
RESULTS(1) VAS score of silver needle group was 4.77 +/- 0.78, 1.99 +/- 1.08 and 2.55 +/- 0.94, respectively before treatment, at 1 week and 3 months after treatment,while VAS score in TCM fumigation group were 4.43 +/- 0.61, 2.48 +/- 0.71 and 3.05 +/- 0.86, respectively. VAS score of two groups after treatment were sigificant decrease than that of before treatment (P < 0.05). There was no sigificant differences between two groups before treatment, but sliver needle group performed well in analgesia than TCM fumigation group, and had obvious differences (P < 0.05). RMDQ score of silver needle group was 13.63 +/- 1.96, 5.87 +/- 2.33 and 6.53 +/- 2.89, respectively before treatment, at 1 week and 3 months after treatment, while RMDQ score in TCM fumigation group were 13.40 +/- 2.01, 6.90 +/- 2.31, 9.23 +/- 2.87, respectively. There was no significant differences between two groups before treatment and 1 week after treatment (P > 0.05), and had obvious differences between two groups at 3 months after treatment (P < 0.01). Both groups could obvious improve dysfunction caused by chronic low back pain, and curative effect of sliver needle groups was more endurable. (2) Following-up at 3 months after treatment, FDD of multifidus, erector spinae of effected side and multifidus of healthy in sliver needle group were obvious increased (P < 0.05); In TCM fumigation group, FDD of multifidus and erector spinae on both side were increased at 1 week after treatment (P < 0.05), but had no significant meaning at 3 months after treatment on health side (P>0.05). There was no significant meaning before treatment (P > 0.05), FDD of multifidus, erector spinae of effected side in sliver needle group were obvious increased at 1 week after treatment (P < 0.05); but no obvious meaning on health side. FDD of both side in sliver needle group were obvious increased at 3 months after treatment. (3) There was correlation among differences of FDD in multifidus and erector spinae, VAS score and differences of RMDQ, and Spearman correlation coefficient R was 0.517, 0.811, 0.746 and 0.625; There was correlation between items of soft tissue tension and sympotoms, function and life quality. Conclusion:Soft tissue tension detection can effectively manifest degree of pain and dysfunction of low back, and improve objectivity of therapeutic evaluation for chronic low back pain.

6.Application of multispectral animal living imaging technology in evaluating osteoarthritis model.

OBJECTIVETo observe application value of multispectral animal living imaging technology in rats model of osteoarthritis.
METHODSFifteen male SD rats weighed (180 +/- 20) g (3 months old) were received intra-articular injection of iodoacetic acid for establishing osteoarthritis. Articular cavity of left knee of rats were injected into 50 microl iodoacetic acid. The same volume of sterile saline was injected into right knee articular cavity as control. X-ray living imaging and bone mineral density were observed at 2 and 4 weeks after establishment of model. After 4 weeks,rats were sacrificed and their bilateral joints were collected and determined histologically based on Collins classification and Kellgren-Lawrence classification.
RESULTSOsteoarthritis model was successfully established, compared with control group, model group showed typical manifestation of osteoarthritis, including irregular cartilage surface,osteophyte formation,joint deformity and cartilage defect,and combined with significant decrease of bone density (P < 0.01), while the decrease was not obvious in proximal tibia (P < 0.05). After 2 weeks, knee joints in model group was classified as Collins grade 1 and Kellgren-Lawrence grade 2,then classified as Collins grade 4 and Kellgren-Lawrence grade 3 after 4 weeks,control group showed smooth articular surface,normal joint space and intact cartilage surface, knee joints was classified as Collins and Kellgren-Lawrence grade 0, and bone density of distal femur and proximal tibia were normal.
CONCLUSIONMultispectral animal living imaging technology could be used in dynamic observation of living imaging and detection of bone density in the animal model of osteoarthritis, and it is significant for evaluation of osteoarthritis model, and its realted tesearch.

Objective To develop and evaluate three types of biodegradable intravascular stent (BIS)with biodegradable materials of poly-L-acid(PLLA)or Poly](1-lactide-co-glycol ide)(PLLGA). 8.Clinical analysis of microsurgical anterolateral thigh free flap for head and neck region reconstruction.

OBJECTIVE:
To summarize the experience of improving survival rate of microsurgical free flap transfers for head and neck reconstruction.
METHOD:
One hundred and seventy-nine consecutive microsurgical free flap transfers performed in patients with head and neck region defects after surgical procedure due to carcinoma were reviewed. The microsurgical free flap survival rate and postoperative complication were evaluated.
RESULT:
The overall success rate of flap was 98.9%. The overall complication rate was 7.8%. The flap crisis rate was postoperative flap crisis incidence rate was 4.4%. The success rate of immediate surgical exploration within 6 hours successfully rescued the flap in were 6 six cases, and the other else two cases cut died flap and repaired the region defect in the head and neck with pectoralis major myocutaneous flap.
CONCLUSION
Key factors in improving the succeess rate are to guarantee vascular anastomosis patency, to achieve a good haemostasis, and to promptly closely observe discover blood vessel flap crisis after operation in the first 6 hours and immediate surgical exploration.

9.Case-control study on midterm follow-up effects of the extended release of posterior clearance in total knee arthroplasty.

OBJECTIVETo evaluate the midterm follow-up results of extended release of posterior clearance in total knee arthroplasty.
METHODSA total of 120 patients with knee osteoarthritis were equally randomly assigned to the experimental group and control group, and underwent unilateral TKA from March 2010 to March 2012. In experimental group, there were 21 males and 39 females with an average age of (62.2±10.9) years old. In the control group, there were 25 males and 35 females with an average age of (64.9±11.4) years old. All the patients were performed using the anterior knee approach. During operation, after osteotomy of the tibia and the femoral condyle, extended release of the posterior knee clearance were taken in experimental group, while only the clearance of osteophyte in the posterior condyle were performed in the control group. The KSS scores including knee functional score and knee clinical score,as well as the range of motion (ROM) of patients, were compared between the two groups at midterm follow-up.
RESULTSTotally 49 patients in the experimental group and 54 patients in the control group were followed up, and the median follow-up time was 46 months. The knee functional score of patients in the experimental group was 91.3±3.4, which was better than 86.4±3.9 of patients in the control group; initiative ROM of flexion of patients in the experimental group was (133.2±5.9)°, which was better than (126.9±7.4)° of patients in the control group. There were no significant difference of knee clinical score between 86.9±4.6 of patients in the experimental group and 85.7±5.1 of patients in the control group, and the initiative ROM of extension between (0.5±1.1)° and (0.3±1.2)°.
CONCLUSIONExtended release of the posterior knee clearance contributes to the knee function and initiative flexion ROM during a midterm follow-up and patients benefit.

10.Effect of Ningzhi capsule on blood lipid spectrum in type 2 diabetes mellitus patients complicated with hyperlipidemia.

OBJECTIVETo observe the effect of Ningzhi Capsule (NZC) on blood lipid spectrum in type 2 diabetes mellitus patients complicated with hyperlipemia (DM-HL).
METHODSAdopting randomized, parallel and controlled trail method, a total of 70 DM-HL patients of qi-yin deficiency and phlegm-blood stagnant syndrome type were randomized into two groups. The original medication for lowering blood sugar and blood pressure was unchanged, the trial group received oral administration of NZC 5 tablets, 3 times a day, while the control group received Lipanthgl or Simvastatin depending on their different constituents of blood lipids. After 6 months of treatment, sixty subjects completed the trail while two patients dropped out due to side effect and 8 patients lost follow-up (4 in each group). Levels of blood lipids, blood routine, liver and kidney function and symptoms in patients were detected and compared.
RESULTSAfter treatment, levels of total cholesterol (TC), triglyceride (TG), low-density lipoprotein (LDL-C), apoprotein B, and lipoprotein a (LPa) lowered, while levels of high-density lipoprotein (HDL-C) and apoprotein A raised in the trial group as compared with those before treatment (P < 0.05, P < 0.01), but showed no difference between the two groups after treatment except HDL level (P > 0.05). Scores of symptoms were also lowered significantly in the trial group (P < 0.01). In the observation period, no abnormal findings in blood and urine routine examination as well as in liver and renal function were found.
CONCLUSIONNZC could lower the blood lipid spectrum and improve the TCM symptoms in DM-HL patients without any adverse reaction.
