2.Recurrent bleeding tendency in a school-aged boy.

The study reports a boy with alpha1-antitrypsin Pittsburgh mutation. The boy was admitted into the hospital because of recurrent joint hematoma. The laboratory examinations revealed that prothrombin time and activated partial thromboplastin time were prolonged and cannot be corrected by 1:1 fresh plasma. The inhibitor of factor VIII, anticardiolipin antibody and lupus anticoagulant were all negative. Platelet aggregation test indicated the existence of the inhibitor of thrombin. Alpha1-antitrypsin Pittsburgh mutation was confirmed by genomic sequencing. The clinical manifestations, diagnosis and treatment of this disorder are discussed in this paper.

3.Viable myocardium evaluation by two dimensional speckle tracking imaging combined with adenosine stress echocardiography.

OBJECTIVERegional left ventricular (LV) function could be detected by measuring peak-systolic strain by speckle tracking imaging (STI). We evaluated the value of STI combined with adenosine stress echocardiography on assessing myocardial viability in patients with myocardial infarction (MI).
METHODSTwo dimensional echocardiography was performed at rest and after adenosine stress echocardiography (infused at 140 µg×kg(-1)×min(-1) over a period of 6 min) in 39 stable patients with previous MI. Peak-systolic (Speak-sys) circumferential strain, radial strain and longitudinal strain were assessed by STI. Radionuclide myocardial perfusion/metabolic imaging served as the "gold standard" to detection of myocardial viability.
RESULTS(1) There were 215 viable and 153 non-viable regions among 368 abnormal motion segments out of 624 segments in 39 MI patients according to radionuclide imaging results. (2) Speak-sys was similar between viable and nonviable myocardium at rest (all P > 0.05). After adenosine infusion, radial Speak-sys [(37.98 ± 5.45)% vs. (30.22 ± 5.47)%], longitudinal Speak-sys [(-23.71 ± 4.53)% vs. (-17.52 ± 4.34)%] increased significantly (P < 0.05)in viable segments compared to baseline levels and were significantly higher than in nonviable segments radial Speak-sys [(37.98 ± 5.45)% vs. (30.12 ± 5.37)%] and longitudinal Speak-sys [(-23.71 ± 4.53)% vs. (-16.95 ± 4.62)%] (P < 0.05), while remained unchanged in nonviable segments before and after adenosine infusion. Circumferential Speak-sys was similar before and after adenosine infusion in both viable and nonviable segments (all P > 0.05). (3) Delta radial strain change > 9.8% has a sensitivity of 82.3% and a specificity of 81.1% whereas a delta change of longitudinal strain > 16.5% has a sensitivity of 83.5% and a specificity of 92.3% for detecting viable segments.
CONCLUSIONSSpeckle tracking imaging combined with adenosine stress echocardiography could serve as a new and reliable method of assessing myocardial viability.

4.Study on the relationship between endocrine hormone and prognosis in critically ill patients.

OBJECTIVETo investigate the profile and significance of changes in thyroid function and cortisol in critically ill patients.
METHODSChemiluminescent microparticle immunoassay (CMIA) was used to measure the serum thyroid hormone, thyroid stimulating hormone and cortisol levels in 51 patients at the first day and in the 45 patients still alive at the third day after hospitalized in ICU. In the 45 patients, 17 cases were complicated with multiple organ dysfunction syndrome (MODS) and 28 cases were not. The age, blood glucose levels and APACHE II scores were also recorded at the first day.
RESULTSIn the 45 patients, serum FT(3) decreased in 41 patients (91%), cortisol increased in 33 patients (73%), especially in the first 24 hours. Compared with non-MODS group, serum FT(3), FT(4) and cortisol levels changed significantly in MODS group. Serum FT(4) reduced apparently in 6 dead patients. Serum FT(3) level was negatively correlated with APACHE II, and cortisol level was positively correlated with APACHE II.
CONCLUSIONSThyroid hormone and cortisol were apparently abnormal and correlated with APACHE II score, especially in patients with MODS. Serum FT(4) might serve as an index to judge the prognosis of the patients.

6.Expressions of CD49f splicing isoforms in different cell lines

Objective To compare the expression of CD 49f splicing isoforms in different human stem cells and colon cancer cell lines , and construct lentiviral vectors overexpressing specific isoform .Methods The expression of CD49f isoforms in different cells was detected by RT-PCR and real-time PCR.The lentiviral vectors overexpressing CD49f isoforms were constructed by molecular cloning method .The overexpression of CD49f in colon cancer cell line HT29 was confirmed by FCM, Western blot and real-time PCR, the invasive ability was examined by transwell assay.Results CD49f splicing isoform B was highly expressed in H 9 human embryonic stem cell line , while iso-form A expressed in epithelial cells .Both isoform A and B were expressed in mesenchymal cells .CD49f isoforms A and B were also expressed in colorectal cancer cell lines , while HT29 and HCT116 showed higher expression of iso-form A than isoform B , HCT8 and LoVo showed higher expression of isoform B than isoform A .Overexpression of CD49f isoform B greatly increased the invasive ability of HT 29 cells, while isoform A showed no effect .Conclusions The expressions of CD49f splicing isoforms A and B in different types of cells are significantly different , which sug-gests that CD49f isoforms play different biological functions in cells .
7.Comparative studies on isolation and purification of Chinese medicine decoction.

8.Research on Chinese medicine syndrome distribution of asymptomatic HIV infection patients.

OBJECTIVETo explore Chinese medicine syndrome distribution laws of asymptomatic HIV infection patients.
METHODSUsing Chi-square test, Chinese medicine syndrome distribution laws were compared and analyzed in 1 156 asymptomatic HIV infection patients from March 2009 to October 2011 from four aspects, i.e., age, possible infection time, disease duration, and different routes of infection.
RESULTSQi deficiency syndrome (QDS) and internal dampness-heat accumulation syndrome (IDHAS) were dominant in all syndrome types. Along with aging, QDS showed a growing tendency, while IDHAS showed obvious declining tendency. There was no obvious change in other syndrome types. There was statistical difference in the distribution of each syndrome type among each age period (P < 0.01). Within 15 years, along with the increase of infection time, QDS showed a growing tendency, while IDHAS ratio showed an obvious declining tendency. No obvious laws were found in other syndrome types. There was statistical difference in the distribution of each syndrome type (P < 0.01). Along with the prolongation of disease duration, the case number of each syndrome showed a decreasing trend, but QDS and IDHAS still accounted for higher ratios in each stage. There was statistical difference in the distribution of each syndrome type (P < 0.01). As for infection routes, QDS was predominant in paid blood donation, blood transfusion infection, intravenous drugs. IDHAS was predominant in sexual transmit. No obvious laws were found in other syndrome types. There was statistical difference in the distribution of each syndrome type (P < 0.01).
CONCLUSIONSDIS, IDHAS, and no confirmable syndrome typing were dominant in asymptomatic HIV infection patients. Deficiency and dampness were important pathological factors for them.

9.Comparison of three methods for the gene analysis of fetal cells from maternal peripheral blood.

BACKGROUNDAlthough great advances in techniques for noninvasive prenatal diagnosis using fetal cells from maternal peripheral blood have achieved, current technology does not meet the demands required for clinical use. In this study, we aimed to establish reliable methods for the gene analysis of fetal cells from maternal peripheral blood.
METHODSPrimed extension preamplification (PEP)-polymerase chain reaction (PCR), multiple primed in situ labeling (PRINS), and nested PCR were individually applied to detect the sex determining region Y (SRY) gene in single fetal cells collected from maternal peripheral blood.
RESULTSThe sensitivity and specificity of the detection of the SRY gene by PEP-PCR were 97.39% (149/153) and 99.17% (119/120), respectively. The sensitivity and specificity of PRINS were 97.56% (40/41) and 100% (35/35), respectively. The sensitivity and specificity of nested-PCR were 80.00% (24/30) and 87.50% (14/16), respectively.
CONCLUSIONSPEP-PCR and PRINS are reliable techniques for the gene analysis of single fetal cells from maternal peripheral blood because of their high sensitivity and specificity. PEP-PCR and PRINS can be used as standard methods of noninvasive prenatal diagnosis using single fetal cells from maternal peripheral blood.

10.Preparation and in Vitro Anti-Laryngeal Cancer Evaluation of Protopanaxadiol-loaded Hollow Gold Nanoparticles

Protopanaxadiol (PPD) has inhibitory effects on many tumors and receives much attention. However,it has poor water solubility and low utilization, which limits its clinical application. Considering these issues,in this study,we used hollow gold nanoparticles as transport carriers of PPD and synthesized PPD hollow gold nanoparticles (HAuNs). We conducted a number of experiments to investigate the in vitro anti-laryngeal cancer Hep-2 effect of a PPD HAuNs carrier. High performance liquid chromatography(HPLC) was used to detect the sustained release effect of PPD HAuNs. MTT assay was used to detect the inhibitory effect of PPD HAuNs on the proliferation of Hep-2 cells. Effect of PPD HAuNs on Hep-2 cell apoptosis was investigated by flow cytometry. The results of in vitro release showed that PPD HAuNs had sustained release effect. Compared with blank control group,HAuNs group and PPD group,the survival rate of Hep-2 cells in HAuNs-PEG-PPD group decreased more significantly and the apoptosis rate increased more significantly (p<0.01). PPD HAuNs could significantly enhance anti-laryngeal cancer effect of PPD in vitro and promote the apoptosis of tumor cells. It promotes tumor cell apoptosis, and is expected to be a new PPD drug delivery system, further promoting the application of PPD in clinical anti-laryngeal cancer.
