Objective To investigate the role of PPAR-γ in the gene expression of T-bet/GATA-3 in Jurkat T cells, and to explore the mechanisms underling this sensitizing effect of the change of TH cell subpopulation group. Methods Jurkat T cells were stimulated with PPAR-γ agonist pioglitazone.TH cell related cytokine IFN-γ and IL-10 was detected by ELISA,and the expression of transcription factors(T-bet and GATA-3)mRNA was detected by RT-PCR.To prove the PPAR-γ-dependent effect.the PPAR-γ-specific antagonist GW9662 was used. Results Stimulated with agonist PPAR-γpioglitazone.the concentration of IFN-γ and IL-10 and the expression of transcription factor T-bet and GATA-3 mRNA were both significantlY decreased in Jurkat T cells obviously, and these actions were dependent on the time and the concentrations of pioglitazone.Added with antagonist GW9662 at the same time, such inhibitory actions of IFN-γ and T-bet expression were recovered.but not IL-10 and GATA-3.Conclusion Pioglitazone can inhibite T cells proliferation and their secretion of cytokines.Pioglitazone can inhibit TH1 cells from secreting cytokines, and it is a PPAR-γ-dependent effect related to T-bet.The inhibition on TH2 is not a PPAR-γ-dependent effect and it is GATA-3 related.

ObjectiveTo introduce clinical experience of the modified lateral crural reverse island skin flap with two sets of blood supply system for aged ankle & foot soft tissue defect.MethodsOn the anatomy base that cutaneous artery branches of lateral crural reverse island skin flap and perforating branches of sural neurocutaneous vascular flap were originated from peroneal artery, we designed the modified lateral crural reverse island skin flap located in posterolateral cruris treated for aged ankle & foot soft tissue defect. The modified flap had two sets of blood supply system which were from lateral crural flap and sural neurocutaneous flap.ResultsAll 11 flaps survived. The skin grafting of donor sites healed well. Followed up 3-9 months, six cases were satisfactory. Five cases with extensor defect were regained by tendon transplantation after 3 months of the operations. Conclusion The modified lateral crural reverse island skin flap with two sets of blood supply system is a good method for aged ankle & foot soft tissue defect.

Objective To investigate the diagnostic value of double balloon endoscopy (DBE) in the elderly. Methods Clinical manifestations and endoscopic findings of 42 elderly patients (aged 60-80 years) and 73 young and middle-aged patients (aged 12-59 years) with small bowel lesions were obtained and compared. Factors influencing the diagnostic outcome of DBE in patients with small bowel bleeding were identified, and the optimal check time after the latest bleeding was determined. Results The procedures of 85.7% (36/42) in the elderly and 79.5%(58/73) in young and middle aged were completed (P>0.05).There was no significant difference in the procedure time between the two age groups. No severe complications were observed in the elderly group. The overall positive rate by double balloon enteroscopy examination were 71.4 % (30/42) and 63.0 % (46/73),respectively in the two groups (P> 0.05). Ulcer and tumor lesions were the most common findings, and diverticula and angiodysplasia were the second common findings. Longer duration of bleeding and higher number of bleeding episodes were found in the elderly with positive DBE findings than those with negative findings. Positive diagnostic rate was significantly higher when DBE was performed within 7 days than that after 7 days (90% vs. 40%). Conclusions DBE is a safe, reliable diagnostic modality, especially in the elderly patients with small bowel bleeding in which ulcer and tumor lesions are the most common identifications.DBE is of greater benefit in patients with more bleeding episodes over a long period, and should be performed within 7 days after the last bleeding.

OBJECTIVE:
To investigate the expression of hyperpolarization-activated cyclic nucleotide-gated cation channel 4 (HCN4) and connexin43 (Cx43) in the sinoatrial node of electric shock death.
METHODS:
As experimental group, 34 cases of electric shock death who had definite current mark evidence were selected from pathology department of Xuzhou Medical College from 2010 to 2013. As the control group, 20 cases of fatal severe craniocerebral injury in traffic accidents were chosen. The expressions of HCN4 and Cx43 in the sinoatrial node were observed by immunohistochemical technology.
RESULTS:
HCN4 positive cells expressed in the cell membrane and cytoplasm of the sinoatrial node. Cx43 positive cells expressed in the cell membrane and cytoplasm of T cells and myocardial cells. The expression of HCN4 was significantly higher than that of the control group (P < 0.05) and the expression of Cx43 was significantly lower than that of the control group (P < 0.05).
CONCLUSION
The changes of HCN4 and Cx43 expressions in the sinoatrial node illustrate electric shock death might be related to the abnormalities of cardiac electrophysiology and conduction.

Chinese Medical Journal 2010;123(17):2320-2323
BACKGROUNDRepair of anterior mitral leaflet (AML) prolapse is still a technical challenge for cardiac surgeons. It is an important issue to find a way to repair the AML prolapse with a reliable and reproducible technique.
METHODSBetween January 2002 and June 2009, the operation of chordal transfer based on the "edge-to-edge" technique was performed in 21 patients with serious mitral valve regurgitation because of prolapse of the anterior leaflet. After the operation, echocardiography was performed in each patient before discharge and at the time of follow-up.
RESULTSAll patients survived the operation. One patient required mitral valve replacement because of anterior leaflet perforation 3 days after the operation. The other patients were free from reoperation. At the time of follow-up, all these patients were in New York Heart Association (NYHA) functional class I. In all these patients, pre-discharge and follow-up echocardiography showed neither stenosis nor significant regurgitation of the mitral valve: the cross-sectional area of the mitral valve was 3.3 - 4.8 cm(2) (mean (3.78 ± 0.52) cm(2)), the mean regurgitation area was (0.45 ± 0.22) cm(2). At the same time, both dimension of left atrium and left ventricle reduced significantly (left atrium diameter: pre-operation (48.26 ± 11.12) mm, post-operation (37.57 ± 9.56) mm, P < 0.05; the end-diastolic diameter of the left ventricle: pre-operation (61.43 ± 8.24) mm, post-operation (42.35 ± 10.79) mm, P < 0.01).
CONCLUSION"Edge-to-edge" chordal transfer technique is a simple, reliable, and reproducible technique that can provide good results for repair of anterior leaflet prolapse of mitral valve.

China Journal of Chinese Materia Medica 2008;33(8):878-883
OBJECTIVETo study the growth and propagation of Pinella ternata in cultivation.
METHODForty six populations of P. ternata originated from Sichuan, Shanxi province and chongqing city were collected and cultivated in experimental field under the same cultivation condition.
RESULT AND CONCLUSIONReproduction tubers at the weight of 0.51-2.00 g (diameter 0.9 - 1.5 cm) had higher increments of tubers and general weight. The P. ternata populations originated from Peng-an county, Shehong county, Zhongjiang county, Lezhi county, Lu county grew faster, on the other hand, the populations originated from Lezhi county, Guangyuan county, Peng-an county, Jianyang county had stronger ability of propagation. The sprouting rate of above 0.30 g (diameter 0.7 cm) tubers was about 90%. There was no significant difference in the growth between tubers and bulbils that have the same weight. The two factors that affected the yield increment were weight range and population, and the former was the main one. Significantly positive correlations were found between weight of reproduction tuber and weight of harvested tubers, number of bulbils. However, significantly negative correlations were found between weight of reproduction tuber and weight increments of tubers, increments proportion of tubers and general weight, and a negative correlations was found between weight of reproduction tuber and general weight increments. Curve-estimated models were conjectured about the weight of reproduction tuber and growth or propagation parameters such as the diameter, the sprouting rate, increments of tubers and general weight, increments proportion of tubers and general weight and the number of bulbils. Some proposals to improve the cultivation of P. ternate were suggested.

Chinese Journal of Neuromedicine 2010;09(8):816-819
Objective To comprehend the anatomical image data related to the keyhole microsurgery operation approaches in patients with saddle lesions. Methods Fifteen healthy volunteers and 10 patients with intracranial aneurysm were selected in the experiment and 16-slice spiral CT, CT angiography (CTA) and 3D-reconstruction were performed on them. The anatomitical distance and angle were measured through the ways from supraorbital keyhole approach, frontolateral keyhole approach, pterional keyhole approach, and subtemporal keyhole approach to the anterior/posterior clinoid process, the anterior communicating artery and the bifurcation of internal carotid. Results Of the distances between keyhole centre and anterior/posterior clinoid process, the shortest one was the way from subtemporal keyhole approach (4.98±0.54 em; 5.35±0.65 cm) and the longest one was the way from supraorbital keyhole approach (6.44±0.36 cm; 37.68±1.06 cm). Of the angles between the skull sagittal plane and the operation plane from keyhole centre to anterior/posterior clinoid process, the way from subtemporal keyhole approach was better than the way from frontolateral keyhole approach. Of the angles between skull transverse plane and the operation plane from keyhole centre to anterior/posterior clinoid process, that of subtemporal keyhole approach was better than pterional keyhole approach. Distances between keyhole centre and bifurcation of intemal carotid ordered as follows: the way from frontolateral keyhole approach, that from supraorbital keyhole approach, that from subtemporal keyhole approach and that from pterional keyhole approach. Of the angles between cranial sagittal plane and the operation plane from keyhole centre to the anterior communicating artery or the bifurcation of ipsilateral internal carotid, the way of frontolateral keyhole approach was better than that of supraorbital keyhole approach. Conclusion CT and CTA 3D-reconstruction, by demonstrating the distances and angles of each approach, play a significant role in choosing the right keyhole operation approach in different saddle lesions.
