Objective Investigate and intervene the psychological health state of clinical trainees in the hospital of Yuxi City, in order to explore the method to improve the psychological health and clinical quality of teaching. Methods From 2009 to 2013, clinical trainees were investigated by SCL-90. Intervention for interns included holding psychological health lectures and making psychological counseling for the serious interns. At the same time, the teachers were trained to carry out two-way intervention between the teachers and clinic interns and to improve clinical interns' psychological health. Results Before the intervention, among 216 interns,34 (15.74%) showed positive. Except fear factor, other factors were significantly different than national norms ( <0.05). After the intervention, among 149 interns, 20 ( 13.42%) showed positive. Obsessive-compulsive symptom, depression, anxiety, hostility, spirit factors and the mean scores were all lower than before ( <0.05),similar to the national norm. Conclusion Clinical trainees have prominent psychological problems. Teaching hospitals should pay more attention to psychological health of them. Intervention is necessary and effective in improving psychological health and quality of teaching.

Objective:To determine the expression of BMP4 in hepatocellular carcinoma (HCC) and to study the role of BMP4 in inducing epithelial-mesenchymal transition (EMT) to analyze the effect of BMP4 on the migration and invasion of HCC cells. Methods: The expression of BMP4 in HCC specimens was examined by immunohistochemistry staining, and the correlations were analyzed between the expression of BMP4 and clinicopathological data. The BMP4 expression plasmid was transfected into HepG2 cells to induce exogenous overexpression of BMP4 protein. The changes of HepG2 cell morphology were detected after BMP4 transfection by using a microscope; the changes of the expression of BMP4, EMT-related protein (E-cadherin, Vimentin) in HepG2 cells were detected by Western blot after transfection of BMP4;the wound healing assay in vitro was used to detect the effects of BMP4 gene transfection on the ability of migration of HepG2 cells;the invasion assay was used to determine the role of transfection of BMP4 on the invasive potential of HepG2 cells. Results: Immunohistochemistry staining method displayed that BMP4 expression was positively associated with age, histological differentiation, stage, and poor prognosis. After BMP4 overexpression, the morphology of HepG2 cells showed significant changes from a paving stone structure with cell-cell adhesion to a fibroblastic shape, which showed typical EMT change; Western blot exhibited that the expression of E-cadherin was downregulated and the Vimentin expression was upregulated in HepG2 cells;the wound healing and invasion assay showed that the migration and invasion potentials of HepG2 cells were significantly enhanced. Conclusion: BMP4, which displayed a high expression in HCC specimens, was closely associated with clinicopathologic data, and BMP4 may promote migration and invasion of HCC cells by inducing epithelial-mesenchymal transition.

Academic Journal of Second Military Medical University 2016;37(12):1555-1558
Objective To investigate the expression of sodium-coupled neutral amino acid transporter 1 (SNAT1) in esophageal squamous cell carcinoma (ESCC) tissues, and to evaluate its prognostic value in patients with ESCC. Methods The tissue microarray including 183 ESCC tissues and their corresponding adjacent tissues was subjected to immunohistochemistry detection and Western blotting analysis of SNAT1 expression, and their correlation with the clinicopatho logical parameters and overall survival time was analyzed. Results SNAT1 was highly expressed in ESCC tissues, with the positive rate being 45. 4% (83/183); we also noticed that t was not expressed in normal squamous epithelium tissues. Our analysis results indicated that overexpression of SNAT1 was significantly correlated with tumor size (P=0.023), lymph node metastasis (P=0.007) and tumor stage (P=0.003). The Kaplan-Meier analysis showed that patients with overexpression of SNAT1 had significantly worse outcome than those with negative SNAT1 expression (P=0.001). Cox regression analysis revealed that over expression of SNAT1 was an independent factor for prognostic of patients with ESCC (P=0.004). Conclusion The expression of SNAT1 is abnormally increased in ESCC tissue, indicating that SNAT1 expression may serve as an important biomarker for prognosis evaluation in patients with ESCC.

Objective To compare the diagnostic value of renal ultrasound scan (RUS) and 99Tcmdimercaptosuccinic acid (DMSA) renal scintigraphy in children with acute pyelonephritis (APN). Methods In all, 165 children with initial clinical diagnosis of APN, aged from 1.5 months to 11 yrs ( median 20 months), were included in the study, all of which were examined with RUS and DMSA renal scientigraphy. The diagnosis with DMSA renal scientigraphy results was taken as the standard reference to evaluate the diagnostic sensitivity and specificity of RUS. Results Of 99 out of all 330 kidneys that were found abnormal on DMSA renal scientigraphy, 31 were abnormal on RUS. Of the rest normal kidneys on DMSA scans renal scientigraphy, 4 were abnormal on RUS. Thus diagnostic sensitivity of RUS for APN was 31.3%(31/99) and specificity was 98.3% (227/231). Conclusions Although RUS provides with high diagnostic specificity for children with APN, its low sensitivity may underestimate the clinical evaluation of APN.More often than not, 99Tcm-DMSA renal scientigraphy is a clinical necesscity for the definite RUS diagnosis.

OBJECTIVEA survey on cancer patients at the community level in Shanghai was performed, in order to study the factors affecting the quality of life (QOL) on them.
METHODSTo evaluate the QOL of cancer patients, self-administered Functional Assessment of Cancer Therapy-General (FACT-G) questionnaire and personal interview methods were used. Data analysis was performed by SPSS software (SPSS,Chicago, IL), including one-way ANOVA and linear regression methods.
RESULTSThere were 7580 valid questionnaires retrieved. Many factors were certificated to have significant relationship with QOL of cancer patients, such as family incomes, education, occupation, awareness of disease, physical exercise, cost of health care and pain (P < 0.01). Some factors however, only affecting certain aspects of the QOL, of which age, marriage state, type of tumor and stage were also noticed. KPS had strong Linear correlation with every aspect of FACT-G (P < 0.000).
CONCLUSIONPatients having a poor KPS, metastasis, low educational level, divorced or lost of spouse, pain, poor income, little exercise and old age would tend to have a poor QOL outcome.
