OBJECTIVETo discuss arthroscopic technique of single Kirschner wire and suture avoiding epiphyseal line fixation for tibial intercondylar eminence fracture and its clinical results.
METHODSFrom May 2008 to December 2012, 21 patients (13 males, 8 females, ranging in age from 6 to 14 years old) with tibial intercondylar eminence fracture were treated arthroscopically with single Kirschner wire and suture avoiding epiphyseal line fixation technique. According to Meyers and McKeever classification, 7 patients were type II, 10 patients were type III, and 4 patients were type IV. Active rehabilitation began at one week after operation. The patients were followed up for 10 to 30 months. X-ray films were taken to evaluate fracture healing at 1, 3, 6 months after operation; range of motion, the anterior drawer test, the lachman test and the Lysholm knee score were used to evaluate clinical effects.
RESULTSAll fractures were healed without displacement at 6 weeks after operation. Anterior drawer test and the lachman test were both negative in all patients at 3 months after operation. Lysholm knee score was 95.5 ±2.5 at 6 months after operation, and postoperative X-ray film did not find epiphyseal line broadening or narrowing.
CONCLUSIONArthroscopic treatment for tibial eminence intercondylar fracture with single Kirschner wire and 8-shaped suture avoiding epiphyseal line fixation technique has many advantages, such as firm fixation, early mobilization, less invasive, less injury of physis and satisfactory effect.

Objective To observe the effect of pulsed electromagnetic fields(PEMFs)of different daily treatment durations on biomechanical properties of femur in ovariectomized rats,so as to find out the optimal daily treatment time. Methods Fifty female Sprague-Dawley rats were randomly divided into five groups:(1)SHAM control(no PEMFs treatment),(2)OVX control(no PEMFs treatment),(3)OVX I(PEMFs treatment at 8Hz fre- quency with 3.8 mT intensity,20 min daily for 30 days),(4)OVX 11(PEMFs treatment at 8 Hz frequency with 3.8 mT intensity,40 min daily for 30 days),and(5)OVXⅢ(PEMFs treatment at 8Hz frequency with 3.8 mT in- tensity,60 rain daily for 30 days). All the rats were subject to bilateral overiectomy except those in the SHAM control group. The biomechanical properties of the femur were assessed after 30 days of PEMFs treatment. Results The values of the parameters of the biomechanical properties obtained with the OVX control group were significantly lower than those of the other 4 groups(P0.05).Conclusion PEMFs of the three different daily treatment durations can maintain the biomechanical properties of the femur in ovariectomized rats.Under certain in- tensity(3.8 mT)and frequency(8 Hz),PEMFs of the three different treatment durations can significantly maintain biomechanical properties of femur in ovariectomized rats approximately to the nomal level, but among the three groups, the difference is not significant. It was shown that exposure to PEMFs for 20 to 60 minutes daily had similar effect of maintaining biomechanical properties of the femur in ovariectomized rats.

To investigate the application of a recently developed metallic oxide semiconductor field effect transistor(MOSFET)detector for use in vivo desimetry. Methods The MOSFET detector was calibrated for X-ray beams of 8 MV and 15 MV,as well as electron beams with energy of 6,8,12 and 18 MeV.The dose linearity of the MOSFET detector was investigated for the doses ranging from 0 up to 50 Gy using 8 MV X-ray beams.Angular effect was evaluated as well in a cylindrical PMMA phantom by changing the beam entrance angle every 15?clockwise.The MOSFET detector was then used for a breast cancer patient in vivo dose measurement, after the treatment plan was verified in a water phantom using a NE-2571 ion chamber,in vivo measurements were performed in the first and last treatment, and once per week during the whole treatment. The measured doses were then compared with planning dose to evaluate the accuracy of each treatment. Results The MOSFET detector represented a good energy response for X-ray beams of 8 MV and 15 MV,and for electron beams with energy of 6 MeV up to 18 MeV.With the 6 V bias,Dose linearity error of the MOSFET detector was within 3.0% up to approximately 50 Gy,which can be significantly reduced to 1% when the detector was calibrated before and after each measdurement. The MOSFET response varied within 1.5% for angles firm 270?to 90?.However,maximum error of 10.0% was recorded comparing MOSFET response between forward and backward direction. In vivo mea surement for a breast cancer patient using 3DCRT showed that, the average dose.deviation between measurement and calculation was 2.8%,and the maximum error was less then 5.0%.Conclusions The new MOSFET detector, with its advantages of being in size, easy use, good energy response and dose linearity, can be used for in vivo dose measurement.

Aim To observe the effect of pyrimidine dithiocarbamate (PDTC) on the variance of disc morphology and the expressions of TNF-α, MMP-9 in the cervical disc in cervical dynamic equilibrium rat models, and to investigate the roles of PDTC in the process of intervertebral disc degeneration and the mechanism involved. Methods Fifty-four SD rats were divided into three groups randomly, then the dynamic equilibrium rat model was established by cutting the nuchal superficial and deep muscle of the rats. The dynamic equilibrium rats with PDTC solution intraperitoneal injection after operations were defined as PDTC group (group A), the models with saline intraperitoneal injection after operations as saline group (group B), the rats of fake operation with saline intraperitoneal injection as blank group (group C), and the animals were sacrificed in batches 10 weeks, 12 weeks, 16 weeks respectively after operation. The C5, C6 vertebrae and C5/6 discs were harvested, and the disc morphology was observed.TNF-α, MMP-9 mRNA expressions were detected by q-PCR and protein expression was observed by Western blot. Results Compared with the saline group, the morphology of disc in PDTC group was destructed slightly and fiber ring arranged orderly.TNF-α, MMP-9 gene and protein expressions had no obvious changes (P>0.05) in blank group (group C) at each time point. The expressions of IL-6, MMP-9 mRNA increased with time in group B, but the amount increased fast firstly, and slow lately, reaching peak in 12 weeks. The expression of TNF-α, MMP-9 protein became steady in group B from 10 weeks compared with other time points(P>0.05).TNF-α, MMP-9 genes and proteins expression decreased obviously in PDTC group (group A) compared with saline group (group B) (P<0.01) at each time point, but higher than blank group C(P<0.01) at each time point. Conclusions TNF-α and MMP-9 are important inflammatory factors involved in rat cervical disc degeneration, PDTC relieves the degeneration of rat cervical disc by reducing the expression of TNF-α and MMP-9 via disturbing the NF-κB signal pathway probably, and PDTC may become potential medicine for disc degeneration.

To investigate the influence of the difference enhancers on the transport mechanism of chlorogenic acid (CGA) across Caco-2 cells model, a RP-HPLC method was adopted to detect the concentrations of CGA. At the concentrations of 20 to 80 microg x mL(-1), the difference of absorption rate constants (K(a)) was not statistically significant. At the concentrations of 40 and 20 microg x mL(-1), the ratios of apparent permeability coefficients (P(app)) of the apical to basolateral and the basolateral to apical were 1.14 and 1.18, respectively. With the effect of enhancers K(a) and P(app) increased, the absorption half-life (T1/2) decreased. CGA passed through the Caco-2 cell membrane mainly by passive transport. It showed that monocarboxylic acid transporter (MCT) could be involved in the across membrane transport process of CGA. Borneol had no effect on the cell membrane transport processes. The order of increasing absorption of CGA caused by the enhancers was sodium lauryl sulphate > sodium taurocholate > carbomer.

Objective:To understand and analyze the levels of individual dose to medical radiation workers from occupational external exposure in Inner Mongolia autonomous region from 2015 to 2019, with a view to provide basic scientific data for workers′ health protection and radiation protection management. Methods:By using National Individual Dose Registry, the individual dose monitoring result and other related data for medical workers were collected and analyzed. Results:The average annual individual doses to medical radiation workers in a time order from 2015 to 2019 were 0.72, 0.87, 0.88, 0.71 and 0.64 mSv, respectively. There were significant differences in the annual effective dose distribution in diagnostic radiology, dental radiology, nuclear medicine, radiotherapy, interventional radiology and other applications ( H=466.56, P<0.05). The average annual effective doses to workers in interventional radiology and nuclear medicine had not shown significant difference ( P>0.05), but significantly higher than those in other occupational categories ( Z=19.95, 9.87, 10.10, 12.76, P<0.05). Conclusions:The average annual effective dose to medical radiation workers is in accordance with the relevant national standards. The dose values for the radiation workers involved in interventional radiology and nuclear medicine are relatively high, especially for interventional radiology workers and radioactive drug operators. The protection measures should be further improved in radiation protection workplace.

Objective To explore the expression and regulatory mechanism of hypoxia inducible factor-1?(HIF-1?)during fracture healing. Methods Mouse models of tibia fracture healing were established, and callus samples were collected 1,3,7,14,21 and 28 days after fracture. The development of callus and new bone formation were evaluated with roentgenology,Micro-CT and tetracycline double labeling method, and the expression of HIF-1?,vascular endothelial growth factor(VEGF),Runx2 and ALP in callus were detected with RT-PCR,Western blotting and immunohistochemistry. The relationship between HIF-1? and fracture healing was analysed. Results The expression of HIF-1? was detected in cells in the fracture sites as well as in evolved osteoblasts, chondrocytes and osteocytes in early callus under hypoxia. The highest expression rate of HIF-1? achieved on the 7th day after fracture, lasted for about 7 days, then decreased gradually, and returned to intact level on the 28th day after fracture. The expression tendency of VEGF resembled that of HIF-1?.Bone formation activity was more active in early callus, and the callus volume peaked on the 14th day after fracture and decreased gradually. The mineralization of callus mainly took place in the late healing period(14th to 28th day after fracture). Conclusion Cells involved in fracture healing are hypoxia-responsive cells, which express HIF-1?.HIF-1? can regulate cell state and function, and can promote angiogenesis so as to play a crucial role in fracture healing.
