1.CT guided the preventation and treatment of hemorrhage after renal cancer cryoablation

Objective To explore the methods of the hemorrhage prevention and the measures of hemostasis by CT guided after renal cancer cryoablation.Methods Retrospective analysis of 8 patients of renal cell carcinoma patienthave been done,all the patients were underwent cryoablation.The basic model included 10 min frozen,3 min thawed and 1 cycle were repeated.Before the cryoprobe puncture,a CT scan was taken to observe the blood supply of the tumor and the relationship between the border of the tumor and renal cortex,renal pelvis.Besides,the path of reducing the injury of normal renal cortex and renal pelvis was chosen,and the decreased number of probes was adopted by taking conformal cryoablation to reduce the damage to the renal capsule,in theensurance of the treatment efficacy.Intraoperative CT scan was performed discontinuously,to observe the covering status of the range of ice ball on the lesion and to monitor the perirenal bleeding.Postoperative ECG and blood pressure changes was observed 24 h after operation.Besides,CT scan or blood routine examination is necessary to monitor whether active bleeding happened.Results According enhanced CT image positioning puncture path,the path of probes were selected in the shortest path between the tumor margins and the renal cortical which avoid the renal pelvis.Probes were CT-guided conformal puncture into the lesion successfully for 8 patients,according to freezing basic mode to complete cryoablation.Timely intraoperative CT scan can clearly observe that a low-density ice ball wrapped lesion completely without involving the renal pelvis and normal tissue surrounding the kidney.There was no bleeding in the procedure of cryoablation.After the probe was pulled,CT scan showed:3 of the 8 patients didn't show any sign of henorrhage,in which 1 patient had heavily bleeding,about 150 ml due to the biopsy after cryoablation.The bleeding was stopped by interventional embolization.A little hemorrhage was found in two patients,about 10 ml outside renal capsule.Medium hemorrhage took place in two patients,30 nl in one patient which underwent several cycles of thawing and 60 ml in another.The patients of mild and medium hemorrhage was treated with intravenous injection of Reptilase and strict bed rest.There was a great hemorrhage in one patient,about 200 ml.The bleeding was stopped by interventional embolization.A small and moderate bleeding,it necessary to execute intravenous infusion 2 IU reptilase and strict bed rest.There was no significant active bleeding from the CT scan after 24 h operation and no change in the blood routine examination of blood hemoglobin and red blood cell count.Conclusions Before the puncture,it is necessary to perform an enhanced CT scan to understand the position,which can determine the path of the puncture.It can reduce the damage of renal cortex,renal capsule and surrounding tissue and prevent and reduce the risk of bleeding.For larger amounts of intraoperative and postoperative bleeding subjects,the embolization should be given immediately,as for the small or moderate amount of bleeding,the conservative treatment may be given firstly.
2.Mechanism of IL-35 inhibition of inflammatory response and T cell response in alleviate of allergic rhinitis

Objective:To investigate the effect of IL-35 on inflammatory response and T cell response in allergic rhinitis.Methods: 37 patients(observation group) with allergic rhinitis and 35 healthy volunteers(control group) after allergen detection of allergic rhinitisin in our hospital from Jan 2012 to Jan 2016 were selected as study subjects.The peripheral blood of observation group and control group were collected,and the serum levels of IL-35 were detected by ELISA.The animal model of allergic rhinitis in mice was established,the peripheral blood of mice was collected,and the serum level of IL-35 and IgE were detected by ELISA.The eosinophils that infiltrated in nasal mucosa were detected after tissue biopsy in mice.The mouse spleen cells were isolated and the ovalbumin antigen was added in the culture medium,IL-35 was or was not added into the culture medium,the ovalbumin specific T cell responses was detected.The cytokines IL-2,IL-4,IL-5,IL-10,IL-13,IL-17,IL-23,IL-27 and TNF-α in culture supernatant of ovalbumin specific T cells were detected by ELISA.The expression of IL-2,IL-4,IL-5,IL-10,IL-13,IL-17,IL-23,IL-27 and TNF-α in ovalbumin specific T cells were detected by Real-time PCR.The activation of JNK,Erk1/2 and p38 signal pathway in ovalbumin specific T cells were detected by Western blot.Results: The serum level of IL-35 in observation group was significantly lower than control group(P<0.05).The results showed that the number of eosinophils which infiltrated in AR mice nasal mucosa was significantly higher than normal mice(P<0.05),while the serum level of IL-35 in AR mice was significantly lower than normal mice(P<0.05).Ovalbumin specific T cell reactivity assay showed that IL-35 could significantly inhibit the T cell response.ELISA and Real-time PCR results showed that IL-35 could significantly down regulate the expression of IL-4,IL-5,IL-13,IL-17,IL-23 and TNF-α,and up regulate the expression of IL-2,IL-10 and IL-27.The Western blot results showed that IL-35 can inhibit the activation of JNK,Erk1/2 and p38 signal pathway of ovalbumin specific T in cells.Conclusion: IL-35 can regulate the expression of inflammatory cytokines in inflammatory response and inhibit T cell response,thus reducing allergic rhinitis,the mechanism may be through regulation of JNK,Erk1/2 and p38 signal pathway activation.
3.The clinical characters of tear film instability in diabetes patients

Background The current epidemiology study had shown the prevalence of age and sex adjusted dry eye was higher in patients with diabetes than population without diabetes.Further researches demonstrated that the tear film disturbance is common after the phacoemulsification or photocoagulation in the eyes of diabetic patients.Objective The aim of this study was to characterize the clinical features of tear film instability in diabetes patients.Methods One hundred and sixty-two patients with tear-film abnormality referred to Tongren Eye Center from January 1,2010 to September 1,2010 underwent questionnaire about diabetes and other diseases,BUT,Schirmer test.Tear film instability was diagnosed as abnormality of either Schirmer test or BUT and showed as M ( Q25,Q75 ).The right eyes of 162 dry eye patients meeting with the including criteria were enrolled.The patients were assigned to two groups according to with ( 80 patients) or without ( 82 patients) diabetes mellitum.DEQ questionnaire were scored.The percentage of cases with meibomain gland abnormal score > 1 was calculated.Mann-Whitney U analysis and Chisquare analysis were used to compare the difference between the two groups.Results The Schirmer test in diabetic group was 8 ( qualities:5,9 )mm and was longer than 6 ( qualities:5,7 ) mm in non-diabetic patients ( U =2452,P =0.00).The result of BUT test was 3 ( qualities:2,4 ) seconds in diabetic patients and was shorter than 4 (qualities:3,5) seconds in non-diabetic patients( U=2104,P<0.01 ).The DEQ score of diabetic patients was 15 ( qualities:1 0,19,which was less than21 ( qualities:19,23.25 ) in non-diabetic patients.51.2 % ( 41/80 ) diabetic participants and 32.9% (27/82) nondiabetic participants appeared meibography ( grade larger than 1 ) (x2 =16.07,P=0.00).The percentages of dry eyes were 51.2% (41/80) and 93.9% (77/82) respectively in diabetes and nondiabetes groups(x2 =37.24,P<0.01 ).No significant correlation was found between the diabetes course and DEQ score or meibography( r =0.16,P =0.16 ; r =0.10,P =0.36 ).Conclusions Diabetes patients with tear film instability have longer Schirmer test results,shorter BUT,more severe meibomain glands damage and lower DEQ scores.The dry eye symptom is lack in the diabetic patients though appearing the tear film and meibomain glands damage.Therefore,more attention should be given to ocular surface health in diabetes patients.

6.Pharmaphylogeny vs. pharmacophylogenomics: molecular phylogeny, evolution and drug discovery.

With the surge of high-throughput sequencing technology, it is becoming popular to perform the phylogenetic study based on genomic data. A bundle of new terms is emerging, such as phylogenomics, pharmacophylogenomics and phylotranscriptomics, which are somewhat overlapping with pharmaphylogeny. Phylogenomics is the crossing of evolutionary biology and genomics, in which genome data are utilized for evolutionary reconstructions. Pharmaphylogeny, advocated by Prof. Pei-gen Xiao since 1980s, focuses on the phylogenetic relationship of medicinal plants and is thus nurtured by molecular phylogeny, chemotaxonomy and bioactivity studies. Phylogenomics can be integrated into the flow chart of drug discovery and development, and extend the field of pharmaphylogeny at the omic level, thus the concept of pharmacophylogenomics could be redefined. This review gives a brief analysis of the association and the distinguished feature of the pharmaphylogeny related terms, in the context of plant-based drug discovery and sustainable utilization of pharmaceutical resource.
7.Expression characteristics of chemotherapeutic related genes in non-small cell lung cancer

Objective To analyze the expression characteristics of chemotherapeutic related genes (ERCC1 ,BRCA1 ,APE1 , RRM1 ,TUBB3 and TS) in non-small cell lung cancer(NSCLC) and their associations with the pathological types to provide the ex-perimental evidences for clinical individual chemotherapy .Methods The immunohistochemical SP method was used to determine the expression of ERCC1 ,BRCA1 ,APE1 ,RRM1 ,TUBB3 and TS genes in 733 patients with NSCLC .Chi-square test was performed to analyze the gene expression characteristics among different pathological types .Spearman relationship was conducted to evaluate the associations among the six genes and different pathological types .Results The expression of BRCA1 ,TUBB3 and TS had sta-tistical differences among three pathological types (P<0 .01);and these three genes showed a relationship with the pathological type (r=0 .107 ,-0 .229 and 0 .168 ,respectively ,P<0 .01) .ERCC1 vs .BRCA1 ,ERCC1 vs .APE1 and APE1 vs .RRM1 showed the pos-itive correlation(r=0 .214 ,0 .316 and 0 .222 ,respectively ,P<0 .01) .Conclusion The chemotherapeutic related gene expressions of NSCLC are related with the clinical pathological types .And the expressions of multiple markers are associated with each other .The combined detection of the chemotherapeutic related markers and the pathological types would be helpful to the decision making of individual chemotherapy in clinic .
8.HPLC Fingerprint of the Ingredients in Simiao Junyi Ointment

Objective:To establish an analytical method for HPLC fingerprint chromatography of Simiao Junyi ointment to provide basis for the quality control standard. Methods:The separation conditions were established to obtain the HPLC fingerprint chromatog-raphy of the main ingredients in Simiao Junyi ointment. The conditions were as follows:the chromatographic column was Ultimate C18-ODS(250 mm ×4.6 mm,5 μm), the mobile phase was acetonitrile-0.1% phosphate solution, the flow rate was 1.0 ml·min-1, the detection wavelength was 280 nm, and the column temperature was 35℃. The common peaks in the chromatography were analyzed for their belongings. Results:Gradient elution was performed under the above optimal separation conditions, the constituents in Simiao Ju-nyi ointment were separated from each other perfectly, and the optimal fingerprint chromatography was obtained. Though the methodolo-gy examination, the indicators such as precision, stability and repeatability of the method were all promising, and the fingerprint chro-matography could be seen clearly and was easy to be analyzed. The relationships between Simiao Junyi ointment and the common peaks of four medicinal materials in the fingerprint chromatography were preliminary determined, which provided important basis for the quali-ty control of Simiao Junyi ointment. Conclusion:The HPLC fingerprint chromatography of Simiao Junyi ointment can be used as an a-nalysis method for the quality control of Simiao Junyi ointment, which provides reference for the quality control standard for the finished product.
9.Comparative Studies on Antioxidant Activities of Green Tea and Four Tree-type Non-Camellia Teas

This article was aimed to compare the antioxidant activities of Green tea and four tree-type non-Camellia teas. The antioxidant activities of the water extract of Green tea, Eagle tea, Large-leaved Kuding tea, Shiya tea and Qingqianliu tea were evaluated with four different in vitro assays including 1,1-diphenyl-2-picryl-hydrazyl (DPPH), ferric-reducing antioxidant power (FRAP), ferrous ion-chelating (FIC) and β-carotene-linoleate bleaching assay. In addition, the polyphenol contents of different teas were estimated by the Folin-Ciocalteu assay. The results showed that Shiya tea had strong antioxidant activity; its FIC and FRAP abilities are better compared with Green tea. The Eagle tea and Large-leaved Kuding tea have better antioxidant activity. The FRAP activity of Large-leaved Kuding tea was better than green tea. The DPPH of Eagle tea and Large-leaved Kuding tea were slightly lower than Green tea, but higher than the butylated hydroxytoluene (BHT). The antioxidant ability of Qingqianliu tea was the lowest. It was concluded that Shiya tea, Eagle tea, Large-leaved Kuding tea had relatively high development value which re-quires in-depth research and promotion.
10.Efficacy of percutaneous chemoablation of malignant metastatic lymph nodes

Objective To investigate the technique,safety and efficacy of percutaneous chemoablation of single and multiple confluent metastatic lymphatic nodes (LNs).Methods Forty six patients of malignant metastatic LNs were treated with percutaneous chemoablation with CT guidance,and they were divided into two groups,including 30 patients of single isolated metastatic LN (group A) and 16 patients of multiple and/or confluent metastatic LNs (group B).A mixture of 5 ml absolute ethyl alcohol,2 ml iodized oil and 10 mg epirubicin was injected into the lymph node(s).The average tumor diameter was (2.0 ± 0.5) cm in group A and (3.8 ± 0.6) cm in group B.The intratumoral drug coverage rate and the occurrence of reflux were recorded.Enhanced CT scans were performed to observe the therapeutic effects at the intervals of 3,6 and 12 months.Results The coverage rate was 100% in group A,which was significantly higher than group B (70% to 80%).The occurrence of reflux was 70% for group A versus 40% for group B.For group A,a complete response (CR) patients of 14,21,28 and a partial response (PR) patients of 16,9,2 were obtained at 3,6 and 12 months after therapy respectively.For group B,a CR patient of 0,0,0 and PR patients of 3,8,9 were observed at 3,6 and 12 months after therapy respectively.Standardized rate by direct method was performed and the CR rate (74.4%) for group A was higher than that for group B (0) (P =0.0001).The tumor FDG uptake was lower compared with the preoperative images.No serious complications were observed.A few patients had mild pain during the procedure,which was relieved after 2.0 to 3.0 ml of 2% lidocaine injection adjacent to the tumor.No needle tract seeding or infection were observed.There was no effect on patients' overall survival.Conclusions CT-guided chemoablation is an effective,low-cost and minimally invasive procedure,which is better for treating single metastatic LN than multiple and/or confluent oues.
