OBJECTIVETo compare therapeutic effects between patella replacement and patella osteotomy in total knee arthroplasty.

METHODSFrom April 2004 to April 2011, 52 patients (54 knees) were enrolled in the clinical trail of total knee arthroplasty, who received patella replacement (24 knees, including 13 males and 11 females,ranging in age from 53 to 78 years old or patella osteotomy (30 knees,including 16 males and 12 females,ranging in age from 55 to 79 years old. The average follow-up period was 56 months,ranging from 20 to 80 months. The American HSS Score for knee, the Feller score for patella, range of motion (ROM) for knee, patient satisfaction and complications related to the patella were used to evaluate therapeutic effects.

RESULTSIn the patella replacement group,the preoperative and final follow-up HSS scores of patients were 38.4 +/- 8.2 and 91.2 +/- 8.6 respectively; Feller scores were 13.6 +/- 6.2 and 25.2 +/- 4.2; scores of anterior knee pain were 3.9 +/- 3.2 and 11.2 +/- 3.7; ROM were (78 +/- 26) degrees and(108 +/- 18) degrees. In the patella osteotomy group,the preoperative and final follow-up HSS scores of patients were 39.5 +/- 8.4 and 91.0 +/- 8.5 respectively;Feller scores were 13.4 +/- 6.5 and 25.6 +/- 4.0; scores of anterior knee pain were 3.7 +/- 3.1 and 11.3 +/- 3.6; ROM were (76 +/- 27) degrees and (110 +/- 19) degrees. In the patella replacement group,patient's satisfaction was 91%, and complication related to the patella was 16.7%; in the patella osteotomy group, patient's satisfaction was 89%, and complications related to the patella was 10.0%. There were no statistically significant differeneces in final follow-up HSS scores, Feller scores, scores of anterior knee pain and ROM between the two groups. However,there was no significant difference of patient's satisfaction between them. There was statistically significant differenece of patella-related complications between the two groups, and the complication rate in the patella replacement group was higher than that in the patella osteotomy group.

CONCLUSIONTotal knee arthroplasty with patella replacement or patella osteotomy dramatically relieves pain and improves the knee function. Patella-related complications are associated with its treatment methods, but post-operative anterior knee pain and patient's satisfaction are not related to treatment methods of the patella.

Objective: To analyze the diagnosis, treatment and prognosis of primary gastric diffuse large B-cell lymphoma (PGDLBCL). Methods: Retrospective analysis was conducted in 69 cases of PGDLBCL between January 2007 and December 2009.The data of clinical presentations, diagnosis, treatment and prognosis were collected, and the association of clinicopathological features with treatment outcomes and prognosis was analyzed. Results: PGDLBCL comprised about 10.44% (69/661) of all lymphomas, including 29 males and 40 females; the median age was 54 years old (range: 21-78). The major clinical presentations of PGDLBCL were abdominal pain, abdominal distension and melena. ALL patients were treated with chemotherapy-based comprehensive treatment. In this cohort, the complete remission rate was 37.7% (26/69), the partial remission rate was 46.4% (32/69), and the overall response rate was 81.4% (58/69). The univariate analysis revealed that the patients with increased level of serum lactate dehydrogenase (LDH) had poor response (P<0.05). The one-, two- and three-year survival rates were 89.2%, 84.2% and 81.3%%, respectively. The univariate analysis revealed that the age, international prognostic index (IPI) and serum LDH had significant effects on the prognosis; COX regression multivariate analysis showed that only serum LDH had significant effect on the prognosis. Conclusion: PGDLBCL is more common in female and treated mainly with chemotherapy-based comprehensive treatment. The prevention and treatment of gastrointestinal bleeding and perforation should be emphasized. Serum LDH was an important prognostic factor for PGDLBCL. Copyright© 2011 by TUMOR.

The dried cane of Kadsura interior (Kadsurae Caulis) is recorded in Chinese Pharmacopoeia 2010 as Dian Jixueteng for the treatment of rheumatism,irregular menstruation,and deficiency of Qi and blood.In this paper,morphological characteristics,chemical constituents,and pharmacological activities in the cane of K.interior were summarized.Moreover,some suggestions about application and quality control of Kadsurae Caulis were proposed in order to provide a theoretical basis for further scientific use.

To investigate the role of distribution and phylogeny of phenylethanoid glycoside in medicinal plants of Gesneriaceae, five phenylpropanoid glycosides, acteoside, paraboside B, isonuomioside A, paraboside II, and paraboside III were quantitatively determined in 12 species of Gesneriaceae by HPLC. The existence and content of these compounds were analyzed. The results showed that phenylethanoid glycosides were found in the most of those plants, but the kind of phenylethanoid glycosides varied in different species. Acteoside distribute in most of this plant group, paraboside B, isonuomioside A, paraboside II, and paraboside III were rare in those plants. The results of this study support morphological viewpoint that Trib. Trichosporeae is more developmental than Trib. Didymocarpeae.

OBJECTIVE:To provide reference for the safe and rational drug use for children. METHODS:Information manage-ment system was used to investigate the use of essential medicines system variety in stock in 2015 and analyze the medication infor-mation for children in the drug instructionsin our hospital in 2015. RESULTS:Only 201 kinds of medicines belonged to children’s medicines in all the 685 kinds of medicines in our hospital. And 89 kinds (44.28%) of medicines belonged to essential medicine system among the 201 kinds of children’s medicines,in which,78 (87.60%) showed complete medication information for chil-dren;112 kinds(55.72%)of medicines belonged to non-essential medicine system,in which,38(33.93%)showed complete medi-cation information for children. The proportions of showing complete medication information for children in the essential medicines and in its chemicals,biological products,injections and oral preparations were higher than non-essential medicines,the differences were statistically significant(P<0.05). Only 41 kinds of medicines belonged to child-specific medicines among the 201 children’s medicines;62 showed complete medication information for children in the 73 kinds of essential medicines among the non-child-spe-cific medicines;only 13 showed complete medication information for children in the 87 kinds of non-essential medicines,the pro-portion of showing complete medication information for children in essential medicines among the non-child-specific medicines was higher than non-essential medicines,the difference was statistically significant(P<0.05). CONCLUSIONS:The use proportion of essential medicine system variety for children’s medicines is high in our hospital;but there are lacking of child-specific medicines and the medication information for children is insufficient. However,compared with non-essential medicines for children,the essen-tial medicines show better medication information for children in aspects of types,dosage form distribution and non-child-specific medicines,and it is suitable for children.

Objective To establish a stable primary culture of rat cerebellar granule neurons in vitro for further study the toxic effects of chronic arsenic exposure on cerebellar cells.Methods Cerebellar cortices were taken from brain of Wistar rat 5-7 day old after born under stereoscopic microscope.Single cell suspension was acquired after digestion and washing with trypsin (0.25％) and DNase Ⅰ solution,respectively.Granule cells were purified from other cells by differential velocity adherence method for two times.Rat cerebellar granule neurons were seeded in culture plate pre-coated with poly-L-lysine.Neurons growth,development and synaptic connections were observed daily.The neurons were identified by neuron specific enolase (NSE) immunofluorescence technique.Results The neurons were affixed to the culture plate in 24 hours,in reticular arrangement observed under contrast microscope.Granule cells gradually turned round from oval and outlines became clearer in 2-3 days.In 4-6 days,there were a wide range of synaptic connections among the neurons and a mature nerve cell network formed.A large quantity of cerebellar granule neurons was seen by NSE identification.Few bigger cells such as purkinjes cells and glial cell outlines were also seen in the same visual field.Conclusions This is a successful primary culture method for acquirement of rat cerebellar granule neurons.The method can provide experimental basis for future studies the toxic effects of chronic arsenic exposure on cerebellar cells.

Objective To evaluate the clinical significance of video-electroencephalogram(VEEG) in the diagnosis of paroxysmal events in children.Methods The VEEG and routine electroencephalogram(EEG) were used to inspect 262 cases of seizures respectively collected of outpatient clinic and admission department from Apr.2006 to Dec.2007,and VEEG monitoring results were analyzed and compared with primary diagnoses retrospectively.According to the characteristics of EEG and clinical observation to decide whether it be epilepsy or not and compare the difference between the results of EEG and VEEG to evaluate the application value of the 2 examination methods in children with NES.SPSS 13.0 software was used to analyze the data.Results Of all 262 cases,113 cases of clinical outbreak were recorded,meanwhile the diagnose of 4 patients of epileptic seizures(ES) group were changed to non-epileptic seizures(NES) and 3 patients of NES were rediagnosed to ES,69 cases couldn′t be confirmed according to VEEG or EEG and there was no outbreak recorded in the other 149 cases.In the period onset,79 children′s characteristics of EEG were captured which included 76 children in ES group,and 3 children in NES group.Thirty-six children′s characteristics of EEG were not captured which included 4 children in ES group and 30 children in NES group.The differences of the ratio of total detect and abnormity between EEG and VEEG were significant,improving the diagniosis rate and the control rate of symptom.VEEG had a clinical significance in differential diagnosis of ES and NES.Conclusions Compared with EEG,VEEG will be more helpful in diagnosing and differentiating seizures diseases,improving the diagnosis rate and the control rate of symptom.VEEG has a clinical in diffe-rential diagnosis for ES and NES.

The technique of double-layered plate was developed for screening the library of mutant endoglucanase III from Trichoderma reesei generated by the method of directed evolution.The enzyme activity was determined according to the velocity of the formation of halos on the plates.Several mutants with higher activity than the wild type at low temperature or alkaline pH were obtained by using this strategy under different screening conditions.Further results of spectrophotometric determination of the activities of these mutants were consistent with the results of plate screening.The establishment of such strategy will broaden the applications of the directed evolution methods for improving the existing proteins to obtain useful enzymes with new properties for industrial applications.
