1.The Significance of Detecting Serum HE4 Levels in the Diagnosis of Lung Cancer

Objective To discuss the serum value of human epididymis protein 4(HE4) in the diagnosis of lung can-cer and to analyse the serum levels of HE4 in different pathological types and TNM staging of lung cancer patients. Meth-ods Forty-seven patients with lung cancer and thirty-one healthy controls were selected to join this study. According to various pathological types and TNM staging, the selected lung cancer patients were divided into different subgroups under the two categories. The serum HE4 levels were compared between subgroups. ROC curves of serum HE4 level and serum CEA level were drawn for the diagnosis of lung cancer with the pathological diagnosis as the golden standard. Results There was significantly higher level of serum HE4 in lung cancer group[(253.47±170.03) pmol/L] than that of healthy group [(84.09±51.03) pmol/L](t=5.365). There were no significant differences in serum levels of HE4 between different pathological subgroups of lung cancer patients [non-small cell carcinoma group (241.34±161.81) pmol/L vs small cell carcinoma group (293.5±198.76) pmol/L, t=0.847;squamous cell carcinoma group (304.29±287.61) pmol/L, adenocarcinoma group (224.39± 122.15) pmol/L and small cell carcinoma group F=0.969;and different TNM staging subgroups [ (stageⅠ~Ⅲgroup (255.27± 183.04) pmol/L vs stageⅣgroup (288.16±216.49) pmol/L, t=0.528]. Compared with ROC curves of serum HE4 and serum CEA,the area under the curve (AUC) of serum HE4 (0.902) was larger than that of serum CEA(0.765),( P>0.001). When the serum level of HE4 was 149.145 pmol/L, the sensitivity and specificity in the diagnosis of lung cancer were 72.3% and 90.3%. When the serum level of CEA was 4.685μg/L, the sensitivity and specificity in the diagnosis of lung cancer were 57.4%and 83.9%. Conclusion The serum level of HE4 is a sensitive and specific tumor marker in lung cancer. There are no significant differences in the serum levels of HE4 between different pathological types and different TNM staging in lung cancer patients. The detection of serum levels of HE4 are useful for the diagnosis of lung cancer.
2.Dosimetric verification of a commercial three-dimension treatment planning system

Objective Objective To verify the accuracy of dose calculation and the precision of beam modeling in a Pinnacle three-dimension treatment planning system(TPS).Methods Based on the photon beam model data of a Varian 600C Linac,output factor verification and 12 tests were carried out ac- cording to the recommendation of Netherlands Commission on Radiation Dosimetry(NCS).The results of ion chamber measurement and TPS calculation at selected points were compared.Deviation and confidence limit were used to evaluate dose calculation accuracy.Results The Pinnacle TPS had a high precision of dose calculation when compared with the measurement data.Deviation of calculated output factors was<1% on all opening patrols,and was<3% on wedged portal except one with the size of 15 cm?40 cm and angled 60?.The results of 12 tests were in accordance with the criteria of NCS recommendation,with maximum av- erage deviation<1%.The overall accuracy is better than those of other TPS reported in the literature. Conclusions Dose verification is a prerequisite before a three-dimension treatment planning system is used in the clinical practice.Pinnacle treatment planning system installed in our institute is reasonably acceptable and satisfactory.
3.Analysis on related factors of anxious condition about patients in Wenchuan Earthquake

Objective To approach the related factor analysis of anxious condition about patients in Wen Chuan earthquake.Methotis 96 patients of Wen Chuan earthquake were inquired through Self-Rating Anxiety Scale(SAS).Results The average standard SAS score of this group is(44.17 4±9.86),which is higher than the country norm of(33.80±5.90),and it is a statistically significant diffefence.The factors which jnfluence anxious condition of patients of Wen Chuan earthquake condition include sex,age,eduction level,social support system,iniury condition,and the extent of the disaster.Conclusions The anxious condition of patients of Wen Chuan earthquake is hisher and related to multiple factors;and the nursing staff should provide specific nursing to them.
4.Genetic diversity analysis of Andrographis paniculata in China based on SRAP and SNP.

In order to reveal genetic diversity of domestic Andrographis paniculata and its impact on quality, genetic backgrounds of 103 samples from 7 provinces in China were analyzed using SRAP marker and SNP marker. Genetic structures of the A. paniculata populations were estimated with Powermarker V 3.25 and Mega 6.0 software, and polymorphic SNPs were identified with CodonCode Aligner software. The results showed that the genetic distances of domestic A. paniculata germplasm ranged from 0. 01 to 0.09, and no polymorphic SNPs were discovered in coding sequence fragments of ent-copalyl diphosphate synthase. A. paniculata germplasm from various regions in China had poor genetic diversity. This phenomenon was closely related to strict self-fertilization and earlier introduction from the same origin. Therefore, genetic background had little impact on variable qualities of A. paniculata in domestic market. Mutation breeding, polyploid breeding and molecular breeding were proposed as promising strategies in germplasm innovation.

6.Study on the multilocus sequence typing of carbapenem resistant Klebsiella pneumonia

Objective To investigate the molecular epidemiology of car-bapenem resistant Klebsiella pneumonia( CRKP) from four teaching hos-pitals in Nanchang by multilocus sequence typing ( MLST ) .Methods Twenty-nine imipenem and/or meropenem resistant Klebsiella pneumoni-a isolates were totally collected from four teaching hospitals in Nanchang from Jan 2011 to June 2012.The polymerase chain reaction( PCR) am-plifications of drug resistant genes were performed.The molecular genetic of CRKPs were analyzed by MLST.The population structure of CRKPs was evaluated by eBURST with the results of MLST.Results Twenty-nine CRKPs were successfully divided into 16 sequence types ( STs ) by MLST.The main types were ST11 and ST15 (5/29 each); followed by ST395 ( 3/29 ); ST334, ST273, ST147 ( 2/29 each ); ST1128, ST42, ST1031,ST764,ST629 and ST37(1/29 each).It is worth nothing that 4 new STs have been confirmed by the PubMlst database and named ST1369,ST1411,ST1412,ST1413.Among 2 ST groups, each ST group respectively included 2 STs ( ST395 and ST1369, ST147 and ST273 ) , the other 12 STs were single type, which ST11 ,ST15 and ST395 belongs to the CC258 clonal complex. Conclusion Gene polymorphism was existed on CRKPs of this region and the main types of 29 experimental strains were ST11 and ST15, and ST15 has been briefly local disseminated.
7.Research of rapid mass spectrum identification of Acinetobacter baumannii

Objective To establish the rapid identification of Acineto-bacter baumannii by detecting specific volatile metabolites of Acinetobacter baumannii through surface desorption atmospheric pressure chemical ioni-zation mass spectrometry ( SDAPCI-MS).Methods Ten clinical isola-ted Acinetobacter baumannii were resuscitated and vaccinated to blood agar plates at 37 ℃incubation and culturing.SDAPCI-MS was adopted to detect volatile metabolites of Acinetobacter baumannii by different cul-turing time.Results Compared with blank culture medium , the pecu-liar volatile metabolites were detected ,the characteristic peaks are mass -to-charge ratio 60 and 118.Basically the same volatile metabolites of Acinetobacter baumannii were generated by different culturing time.Con-clusion The rapid identification of Acinetobacter baumannii through peculiar volatile metabolites is hopefully applied by SDAPCI -MS.
8.Study of the treatment of comminuted fractures of proximal humerus with open reduction and internal fixation.

OBJECTIVETo study the effect of the treatment for comminuted fractures of proximal humerus with open reduction and internal fixation, and the influence of the different internal fixation.
METHODSFrom January 2003 to June 2007, 423 cases of comminuted fractures of proximal humerus were treated with open reduction and internal fixation. Among them, 139 patients included 51 males and 88 females were treated with the Kirschner needle with an average age of 55.8 years old ranging from 35 to 72 years, and the average course was 7 days (from 3 to 20 days); 103 patients included 48 males and 55 females were treated with the cloverleaf plate with an average age of 56.7 years old (from 22 to 76 years), and the average course was 8.5 days (from 3 to 23 days); 181 patients included 85 males and 96 females were treated with the locking plate with an average age of 57.1 years old (from 29 to 77 years), and the average course was 7.9 days (from 3 to 21 days). The pain, daily activities, orbit, and the strength of the shoulder were evaluated with the Constant's scale.
RESULTSThe wound of all the cases was primary healing. All patients were followed-up for over 12 months, the sings and symptoms of all the patients were improved very well. There was significant difference between, before and after operation on Constant's scoring. While there was not significant difference among the different internal fixation.
CONCLUSIONOpen reduction and internal fixation is effective for comminuted fractures of proximal humerus. There is no significant difference on therapeutic efficacy in different internal fixation.

9.Immunoassay for cadmium detection and quantification.

OBJECTIVETo detect cadmium in environmental and food samples by graphite furnace atomic absorption spectroscopy (GFAAS) and inductively coupled plasma atomic emission spectroscopy (ICPAES).
METHODSAn indirect competitive enzyme-linked immunosorbent assay (IC-ELISA) was developed based on a cadmium-specific monoclonal antibody. IC-ELISA for cadmium in environmental and food samples was evaluated.
RESULTSIC-ELISA showed an IC50 of 45.6 microg/L with a detection limit of 1.95 microg/L for cadmium, and showed a mean recovery ranging 97.67%-107.08%. The coefficient of variations for intra- and interassay was 3.41%-6.61% and 4.70%-9.21%, respectively. The correlation coefficient between IC-ELISA and GFAAS was 0.998.
CONCLUSIONIC-ELISA can detect and quantify cadmium residue in environmental or food samples.
