2.Effect of polyclonal antibodies against ciliary neurotrophic factor on botulinum toxin-induced axonai sprouting

Objective To study how polyclonal antibodies against ciliary neurotrophic factor (CNTF) reduce botulinum toxin-in- duced axonal sprouting and affect paralytic muscle.Design Experimental study.Participants 20 New Zealand rabbits.Methods In 10 rabbits randomly selected,left superior rectus were as control (Group 1),right superior rectus were injected with botulinum toxin A (BTXA) 2.5U (Group 3).At 14th day,bilateral superior recti were taken out under general anaesthesia.In the other 10 rabbits,left su- perior recti were injected equivalent physical saline (NS) as control (Group 2);right superior recti were injected with BTXA 2.5U,after 4 days,injected with 50?l polyclonal antibodies against CNTF at same site (Group 4).And at 14th day,bilateral superior recti were taken out for electron microscopy,dyed with acetylcholinesterase,argentums (Ag) to show nerve axonal sprout,and accounted and mea- sured with Leica microsystems.Main Outcome Measures The mean number of sprouts and the mean total length of sprouts,and the uhrastructure change of muscle by electron microscopy.Results In Group 1,the mean number of sprouts and the mean total length of sprouts were 5.75% and 10.53?m respectively;Group 2 were 6.11% and 11.16?m;Group 3 were 84.04% and 170.71?m;and Group 4 were 54.77% and 68.12?m.The differences were statistically significant (all P=0.000).Electron microscopy showed that after admin- istration BTXA alone (Group 3),muscle atrophied obviously,nerve myelin sheath increased,while the structure of nerve and muscle re- main invariable.The Group 4 showed that local myofilament disrupted and dissolved,degenerative myocytes necrotized and disintegrat- ed into fragments,which led to partial unreversible destroy.Conclusion Polyclonal anti-CNTF can reduce BTXA-induced axonal sprout- ing,lead to partial unreversible destroy of muscle,which may prolong the time of paralytic muscle resuming.It suggests that polyclonal anti-CNTF could prolong the duration of muscle paralyses induced by botulinum toxin.

Objective To explore the effect of progesterone on the rate of neuronal apoptosis and nitric oxide(NO) level in the cortex and hippocampus tissue of newborn rats with hypoxic-ischemic encephalopathy(HIE).Methods Thirty 7-day-old neonatal rats were randomly divided into 3 groups:sham-operated group,hypoxic-ischemic(HI) group and pretreatment group.Rats in HI group and pretreatment group were subjected to left common carotid artery ligation,then were exposed to 80 mL/L oxygen and 920 mL/L nitrogen gas in 37 ℃ closed container for up to 2.5 h to establish HIE model.Progesterone was injected intraperitoneally into rats in the pretreatment group respectively for 30 minutes before hypoxia,and solution was injected into the sham-operated group and HI group.All rats were killed at 24 h after operation.The neuron apoptosis was identified and analyzed by flow cytometry.Nitrate/nitrite was assayed to represent nitric oxide content of brain tissues.Results The ratio of neuronal apoptosis and NO contents in cortex and hippocampus tissue in HI group [(10.09?0.36)%,(12.32?0.28)%,(51.36?9.71) ?mol/L,(52.34?4.26) ?mol/L] were significantly higher than those in sham-operated group [(2.49?0.23)%,(2.58?0.26)%,(18.16?6.24) ?mol/L,(19.28?3.58) ?mol/L)](P_a
4.Investigation and analysis of the nursing staff education and work status in primary hospital

Objective To deeply understand the nursing staff education and work status in primary hospital,provide reference for training,health care reform of grassroots in China. Methods A total of 520 nursing in five level two-grade of primary hospital were selected. A questionnaire survey was conducted on the title,education,treatment,job satisfaction,nursing training,editing. Results Primary hospital nursing staff starting education:starting education undergraduate and junior titles accounted for (36.23±9.42)%, intermediate title for (12.43±2.68)%, (3.57±2.38)%, senior titles between group differences statistically significant, P<0.05. Through continuing education after junior titles nursing bachelor degree or above accounted for (84.78±34.13)%, intermediate title to reach (60.73±17.94)%, senior titles to reach 100.00%,the differences were significant, P<0.05; (63.77±19.20)%of junior titles through continuing nursing education degrees, intermediate titles accounted for (72.32±20.62)%, and senior titles accounted for (96.43±37.50)%. Nursing staff salary: junior titles accounted for (1 082 ±258) yuan,intermediate and senior titles advanced to (1 633±426) yuan and (2 864±587) yuan,the differences were significant, P<0.05. Editing: junior titles accounted for (18.84±3.27)%, intermediate title for (56.50±6.58)%, senior titles for (85.71±17.63)%, the differences were significant, P<0.05. Turnover rate:junior titles accounted for (25.36±7.54)%, intermediate title for (-6.50±2.14)%, senior titles for (-3.57±1.53)%, the differences were significant, P<0.05. Job satisfaction survey:junior titles accounted for (26.09±6.32)%, intermediate title for (37.85±5.16)%, senior titles for (57.14±8.49)%,the differences were significant, P<0.05. Conclusions Primary hospital nursing staff starting education level is generally not high, after re-education to achieve higher degree. Job satisfaction is not high, staffing, erosion is serious. Off in the form of health care, nursing education level, the gener-al improvement in quality of care at the same time,improve the economic benefits of the nursing for the development of career of our country basic care not allow to ignore.
5.The practice and thinking of curriculum construction on patient safety in undergraduate medical education

To strengthen the medical students in patient safety education is the first step to effectively prevent medical accidents.But at present patient safety education has not been given enough attention by domestic mnedical colleges.Research group consulted literature at home and abroad related to patient safety courses, according to the Medical colleges patient safety curriculum guide proposed by The World Health Organization (WHO) Alliance for Patient Safety, opened the patient safety course in undergraduate medical education with the goal to improve the medical students to maintain patient safety consciousness, ability and methods and implemented scene simulation teaching and formative assessment.We achieved good teaching effect and appraisal by students.
6.Spontaneous Remission of Congenital Leukemia-One Case Report

Objective To discuss the clinical features and possible mechanisms of congenrtal leuleemia spontaneous remission. Methods By means of literature review, 17 cases with spontaneous remission of congenital leukemia were retrospectively analyzed. Results Among the 17 patients, 8 were persistent remission(47%), 9 had late relapses(53%). Neither initial white cell count nor bone marrow involvement was associated with late relapses. 5 cases(29%) had abnormal karyotype and gene rearrangment,11q23 translocation indicated poor prognosis. Conclusions Conservative treatment can be given unless there was evidence of progressive leukemia. Because of occasional late relapses, long term follow-up was recommended. The detection of karyotype and gene rearrangment was a powerful prognostic tool available.
7. Progress of the Mechanism of Acupuncture to Promote Repair of Facial Nerve Injury

Facial nerve injury results in facial muscle movement disorders that interferes with patient's life. Acupuncture is an effective therapy for facial nerve injury, but its mechanism has not been clarified. The evidence supports that acupuncture can accelerate the repair of facial nerve injury by adjusting the electric wave of patient's facial muscle, improving facial blood circulation, and promoting the expression of cell and protein molecule.

10.Drug Resistance Surveillance of Gram-negative Bacteria in Intensive Care Unit

OBJECTIVE To investigate the drug resistance of Gram-negative bacteria in our hospital′s intensive care unit(ICU).It′s important to apply rationally antibiotics in clinic.METHODS We recruited all samples from infected patients of ICU during from Jan 2005 to Feb 2006.Totally 245 strains were identified to be Gram-negative bacteria.Kirby-Bauer method was performed to test extended spectrum ?-lactamases.RESULTS The first 3 of 245 strains were Pseudomonas aeruginosa(22.4%),Escherichia coli(20.8%) and Klebsiella pneumoniae(16.3%).Antibiogram analysis shew that CTX-M was the major type.CONCLUSIONS Imipenem keeps the highest sensitivity and antibacterial activity for Enterobacteriaceae and nonfermenters.Cefoperazone/sulbactam,cefepime,ceftazidime and amikacin keep the higher antibacterial activity.
