1.The expression and significance of Krebs von den lungen-6, pulmonary surfactant protein-A, D, interleukin-6 in patients with connective tissue diseases interstitial pulmonary disease

Objective To determine the levels and significance of Krebs von den lungen-6(KL-6), pulmonary surfactant protein A (SP-A), SP-D and interleukin (IL)-6 in patients with connective tissue disease interstitial lung disease (CTD-ILD). Methods The serum KL-6, SP-A, SP-D and IL-6 in all subjects were detected and the imaging and pulmonary function were recorded t test, χ2 test, non-parametric test, ANOVA and correlation analysis were used for data analysis. Results ① The levels of serum KL-6, SP-A, SP-D, IL-6 in the CTD-ILD group [551.4 (428.2, 883.5) U/ml, 938.4(435.2, 2324.7) pg/ml, 90.7 (80.7, 100.3) ng/ml and 30.4 (22.9, 41.7) pg/ml; P all<0.05] was significantly higher than that in the CTD group [192.9 (139.2, 266.2) U/ml; 458.0 (372.6, 529.0) pg/ml; 80.0 (71.2, 98.3) ng/ml; 18.6 (4.9, 31.0) pg/ml, Z=-5.383, -3.76, -2.123,-3.903, P all <0.05]; and higher than healthy controls (n=30) [183.2(141.9, 216.6) U/ml; 229.0(162.0, 248.0) pg/ml;50.8(26.1, 96.4) ng/ml;7.1(3.7, 8.7) pg/ml, Z=-5.801,-8.13, 2.272, 3.266;P all<0.05].②The levels of KL-6 in pulmonary HRCT for active ILD group was significantly higher than the non-active ILD group [998.5 (640.3, 1293.3) U/ml vs 565.0(434.0, 799.5) U/ml, Z=2.182, P=0.023], there was no statistical difference in the levels of SP-A, SP-D, IL-6 between the 2 groups. ③ Spearman correlation analysis showed that KL-6 was negatively correlated with forced vital capacity (FVC%);SP-D, IL-6 and diffusing capacity of carbon monoxide (DLCO %). ④ Logistic multiple regression analysis showed that KL-6 [OR=1.017, P=0.002, 95%CI (1.006, 1.028)], SP-A [OR=1.023, P=0.009, 95%CI (1.006, 1.041)], SP-D [OR=1.175, P=0.009, 95%CI (1.075, 1.264)], IL-6[OR=1.213, P=0.001, 95%CI(1.088, 1.354)] were the risk factors for ILD. Conclusion Serum KL-6, SP-A, SP-D and IL-6 are significantly increased and correlate with CTD-ILD. KL-6 is related to the pulmonary inflammatory disease and vital capacity, while SP-D and IL-6 are related to diffusion function.
3.Establishment and analysis of the nomogram model of lean tissue decline in Stage 4-5 patients with chronic kidney disease without dialysis

Objective:To establish a nomogram prediction model for lean tissue decline in non-dialysis patients with Stage 4-5 chronic kidney disease(CKD),in order to provide reference for early clinical identification and screening of related risk factors.Methods:A total of 247 patients with CKD stage 4-5 who were treated in Wuhan Fifth Hospital from July 2018 to December 2021 were selected and divided into normal LTI group and low LTI group according to lean body tissue index(LTI).Clinical data were collected.Logistic regression were used to analyze the risk factors of lean tissue decline.A nomogram prediction model was constructed by R software package(R3.5.3),and calibration curve and ROC curve were drawn to evaluate the model differentiation and accuracy.Results:Among 247 CKD stage 4-5 patients,148 cases(59.92%)were in normal LTI group and 99 cases(40.08%)were in low LTI group.Logistic regression analysis showed age(OR=1.441,P=0.012),diabetic kidney disease(OR=2.153,P=0.029),body mass index(OR=1.639,P=0.023),calf circumference(OR=3.247,P=0.011)and carbon dioxide(OR=3.573,P=0.004)entered the regression equation.Based on the above five risk factors,a nomogram prediction model was established and verified.The correction curve all approached the ideal curve,and the slope of the calibration curve was close to 1(x2=3.549,P=0.637).The AUC of ROC curve was 0.792(95%CI:0.714~0.897).Conclusions:The nomogram based on age,diabetic kidney disease,body mass index,calf circumference and carbon dioxide showed good differentiation and accuracy in predicting lean tissue decline in CKD stage 4-5 non-dialysis patients.

5.Treatment of acne vulgaris with auricular acupoint pricking-bloodletting plus auricular point sticking therapy: a randomized controlled study

Objective: To observe the clinical efficacy of auricular point pricking-bloodletting plus auricular point sticking therapy for acne vulgaris. Methods: A total of 66 patients with acne vulgaris were randomized into an observation group and a control group by the random number table, with 33 cases in each group. The observation group was treated with auricular point pricking-bloodletting plus auricular point sticking therapy, and the control group was treated only with auricular point sticking therapy. The treatments of both groups were performed twice a week, 4 weeks as a course of treatment, for 3 courses in total. The scores of skin lesions and dermatology life quality index (DLQI) scores were recorded before and after treatment to assess the clinical efficacy. Results: During the trial, there were 3 cases of drop-out both in the observation group and the control group. After 3 courses of treatment, the total effective rate of the observation group was 96.7%, while that of the control group was 76.7%. The difference between the two groups was statistically significant (P<0.05). The intra-group comparison showed that the scores of skin lesion and DLQI were both decreased with the increase of treatment times, that was, the scores were lower than those at the previous time point (allP<0.05). After 1, 2, and 3 courses of treatment, the scores of skin lesion and DLQI of both groups were statistically different from those of the same group before treatment (allP<0.05). At every time point during the treatment, the scores of skin lesion and DLQI of the observation group were lower than those of the control group, and the differences between the two groups were statistically significant (all P<0.05). Conclusion: Auricular point pricking-bloodletting plus auricular point sticking has a better curative effect than auricular point sticking therapy alone in the treatment of acne vulgaris, and has a time-effect correlation.
6.Clinical trials of antiphlogistic agent series in treating chronic nonbacterial prostatitis.

OBJECTIVETo investigate the curative effect of antiphlogistic agent series on treating chronic nonbacterial prostatitis (CNP).
METHODSOne hundred and sixty patients were randomized into 4 groups for an 8-week clinical observation: group A (oral antiphlogistic medicinal granules only), group B (oral antiphlogistic medicinal granules + retention enema), group C (oral antiphlogistic medicinal granules + rectal), and group D (antiphlogistic medicinal granules + rectally + hip bath). Single blind trials were employed.
RESULTSThe curative rates of the 4 groups were 37.5%, 57.5%, 52.5% and 82.5% respectively, while the total efficacy rates were 42.5%, 82.5%, 77.5% and 92.5% respectively. Compared with groups A, B and C, the curative rate of group D was significantly higher (P < 0.05). The difference in efficacy rates was slight between groups B and D (P < 0.05), but significant between groups A and C (P < 0.05).
CONCLUSIONSCombined treatment therapy can improve the effect of CNP treatment and clear away heat and toxic material. The antiphlogistic agent series, with the effect of motivating blood circulation and removing blood stasis, turned out to be an effective traditional Chinese medicine in treating CNP.

8.Study on the pathophysiologic basis of classification of 'spleen' deficiency in chronic gastritis.

BACKGROUNDMost of the studies on traditional Chinese medicine (TCM) 'spleen' deficiency syndrome in the recent 30 years were conducted only on the basis of single functional index, neglecting the study on the pathophysiologic internal relationship between spleen deficiency syndrome and gastric diseases in modern medicine. But it was at the subcellular molecular biological level that we explored the pathophysiologic basis of classification of spleen deficiency in chronic gastritis by detecting the bioactive substances in gastric mucosa nuclei and mitochondria.
METHODSBy means of optical microscope, scanning electron microscope (SEM), transmission electron microscopy (TEM) and histochemical staining, we conducted histopathological, subcellular ultrastructural analysis and nuclei and mitochondrial ultrastructural analysis of gastric mucosa of 188 spleen deficiency patients and of 42 voluntary blood donors. At the same time, bioactive substances were measured by means of X-ray energy dispersive analysis system (EDAX) image analysis system, radioimmunoassay method and chemiluminescence method.
RESULTSThe content of cAMP, superoxide dismutase (SOD), Zn and Cu in gastric mucosa, and the content of Zn and Cu in mitochondria decreased progressively in order of groups: healthy control (HC), spleen Qi deficiency without organic lesion (F-SQD), spleen Yang deficiency without organic lesion (F-SyangD), disease without symptoms group, spleen Qi deficiency with organic lesion (G-SQD), spleen Yang deficiency with organic lesion (G-SyangD), spleen Yin deficiency (SyinD) and spleen deficiency with Qi stagnation (SDQS), chronic spleen deficiency gastritis (CSG) and chronic atrophic gastritis (CAG); decreased in order of HC, intestinal metaplasia (IM)Ia, IMIb, IMIIa and IMIIb, P < 0.05. The content of DNA, Zn and Cu in nuclei progressively increased in order mentioned above, P < 0.05.
CONCLUSIONSThe quantitative changes of gastric mucosal cAMP, SOD, Zn, Cu, of mitochondrial Zn, Cu and of nuclear DNA, Zn and Cu are not only the substance base on which the lesion of gastric mucosa tissue structure occurs, but also the substance base on which spleen deficiency is classified. G-SQD and G-SyangD were more likely to be found in low-grade or middle-grade CSG and CAG, while SyinD and SDQS in middle-grade or high-grade CSG, CAG and IMIIb.

10.Effects of electroacupuncture on CRH, IL-2 and beta-EP contents in the hypothalamus in the rat of adjuvant arthritis.

OBJECTIVETo explore Mechanisms of electroacupuncture in anti-inflammation and immunoregulation, and to compare differences among "Dazhui" (GV 14), "Mingmen "(GV 4) and non-acupoint in these actions.
METHODSAdjuvant arthritis rat model was made. The rats were divided into 5 groups: normal group, model group, "Dazhui" (GV 14) electroacupuncture group, "Mingmen" (GV 4) electroacupuncture group and non-acupoint electroacupuncture group. Effects of electroacupuncture on local joint inflammation and corticotropin-releasing hormone (CRH), beta-endorphin (betaEP) and interleukin-2 (IL-2) contents in the hypothalamus were observed and differences of the actions among different points were compared.
RESULTSCRH content in the hypothalamus in the "Dazhui" (GV 14) electroacupuncture group decreased as compared with the model group (P < 0.05); there was no significant change in beta-EP and IL-2 contents of the hypothalamus in the 3 electroacupuncture groups as compared with the model group (P > 0.05); there was positive correlation between CRH and IL-2 contents, and between IL-2 and beta-EP contents in the hypothalamus. The swelling rate of the toe in the "Dazhui" (GV 14) group and the "Mingmen" (GV 4) group was lower than that in the non-acupoint group (P < 0 05).
CONCLUSIONElectroacupuncture exerts anti-inflammation and immunoregulation possibly via regulation of CRH, IL-2 and beta-EP each other, and the anti-inflammation action of "Dazhui" (GV 14) and "Mingmen" (GV 4) is stronger than that of non-acupoint.
