2.Analysis of dental plaque by using cellular neural network-based image segmentation.

OBJECTIVETo establish and evaluate a new method for measurement of dental plaque by using cellular neural network-based image segmentation.
METHODSA total of 195 subjects were selected from community population. After dental plaque staining, oral digital picture of anterior teeth area was taken by an Olympus digital camera (C-7070 Wide Zoom). At the same time, the Turesky dental plaque indices of anterior teeth were evaluated. The image analysis was conducted by cellular neural network-based image segmentation.
RESULTSThe image cutting errors between two operators were very small. The Kappa value is 0.935. Pearson's correlation coefficient is 0.988 (P < 0.001). There was high correlative consistency between traditional dental plaque index and plaque percentage obtained by using image analysis. Pearson's correlation coefficient was 0.853 (P < 0.001).
CONCLUSIONSCellular neural network-based image segmentation is a new method feasible for evaluating dental plaque.

3.Molecular Epidemiological Characteristics of Staphylococcus aureus Strains Associated with Food Poisoning in Xi’an from 2013 to 2017

Objective To understand the molecular epidemiological characteristics of staphylococcus aureus(S.aureus),which was isolated from food poisoning in Xian between 2013 and 2017. Methods A total of 26 S.aureusisolates from 8 outbreaks were analyzed by using pulsed field gel electrophoresis(PFGE), multilocus sequence typing(MLST) and spa typing. polymerase chain reaction(PCR) analysis was used to analyze the staphylococcal enterotoxin genes sea to sei. Results 26 strains of S.aureus were divided into 7 types of A-G by PFGE; They were divided into 3 types by MSLT, ST6 accounted for 65.38% which was the main type; They were Divided into 4 types by spa typing, t701 accounted for 65.38 % which is the dominant type. The sea accounted for 76.92% (20/26) in the enterotoxin gene testing; sed-seg accounted for 23.08% (6/26); other enterotoxin genes were not detected. Conclusion The predominant type of S.aureus isolated from food poisoning in Xi'an was ST6-t701-seaprofile from 2013 to 2017. The first detection of the new enterotoxin gene seg, PFGE, MLST and spa typing were combined to fully understand the molecular epidemic characteristics of strains.
4.Clinical study of chronic pain after breast cancer surgery

Objective To study the epidemiological characteristics of chronic pain following breast cancer surgery and analyze the related factors.Methods Nine hundred and twelve patients following breast cancer surgery were enrolled in the Department of Breast Surgery,Affiliated Hospital,Academy of Military Medical Sciences.The Douleur Neuropathique-4 (DN-4) questionnaire was used to identify neuropathic pain,and the related factors were statistically analyzed.Results The follow-up was completed in 821 patients,including 263 (32%) patients with chronic pain.DN-4 score of 47 (17.9%)patients was over 4 points.There was significant difference between the painful group and the non-painful group in methods of surgery and axillary lymph node dissection (P < 0.05).Conclusion Findings suggest that chronic pain after breast cancer surgery is a significant problem clinically.Proper surgery and psychological buildup are of clinical value in preventing post-surgery chronic pain.
5.Treatment of an infant with severe neonatal hypoxic-ischemic encephalopathy sequelae with transplantation of human neural stem cells into cerebral ventricle.

OBJECTIVESevere newborn hypoxic-ischemic encephalopathy (HIE) has a very high rate of disability and no effective treatment is available. The present study aimed to preliminarily evaluate the effects of human neural stem cell transplantation in treatment of severe neonatal HIE.
METHODSThe patient was a 75-day old male infant with sequelae of severe HIE who had highly delayed development of intelligence and movement and myotonia. MRI showed multiple cerebromalacia and encephalatrophy. Cells obtained from the forebrain of an 11-week old fetus were cultured and amplified for 15 days. And then the human fetal neural stem cells were injected into cerebral ventricle of this infant.
RESULTSTwenty eight days after transplantation, remarkable improvement occurred not only in his myotonia but also in his intelligence and movement, which became similar to those of the normal infants of the same age. Positron emission tomography (PET) showed significantly increased radioactivity at temporal and occipital lobes which suggested that the cellular metabolism had increased greatly.
CONCLUSIONThe short-term effect of NSCs transplantation on the infant with severe HIE sequelae was significant. PET suggested that the implanted NSCs survived. Many more studies are needed to evaluate long-term effects of NSC transplantation in treatment of HIE.

6.Endovascular stent-grafts for acute and chronic type B aortic dissection: comparison of clinical outcomes.

BACKGROUNDEndovascular stent-graft treatment has emerged as an alternative for patients with type B aortic dissection (AD), either at acute or chronic phase, in selected patients. This study aimed to investigate the results of endovascular stent-graft repair for acute and chronic type B AD.
METHODSFrom May 2002 to July 2007, 67 patients with type B AD were treated by endovascular stent-graft placement. There were 32 patients in the acute phase (AAD group) and 35 patients in the chronic phase (CAD group). The patients were followed up from 1 to 65 months (average, 17 +/- 16 months). The immediate and follow-up clinical outcomes were documented and compared between the 2 groups.
RESULTSPlacement of endovascular stent-grafts across the primary entry tears was technically successful in all 67 patients. Compared with patients in the CAD group, those in the AAD group had higher percentages of pleural effusion (15.6% vs 0, P = 0.02) and visceral/leg ischemia (21.9% vs 2.9%, P = 0.02). Procedure related complications, including endoleak and post-implantation syndrome occurred more frequently in AAD group than in CAD group (21.9% vs 2.9% and 31.3% vs 8.6%, respectively; P = 0.02 and P = 0.02). Kaplan-Meier analysis showed no significant difference in survival rate at 4 years between the 2 groups (86.4% vs 92.3%, P = 0.42 by Log-rank test). But the 4-year event-free survival rate was higher in patients with chronic dissection than in patients with acute dissection (96.2% vs 73.9%; P = 0.02 by Log-rank test).
CONCLUSIONSEndovascular repair with stent-graft was safe and effective for the treatment of both acute and chronic type B AD. However, both immediate and long term major complications occurred more frequently in patients with acute dissection than in those with chronic dissection.

Objective To investigate the expression of cyclin dependent kinases 5(CDK5)in the temporal lobes of the epilepsy patients and to explore the possible roles of CDK5 in the pathogenesis of refractory epilepsy.Methods The brain tissues of intractable epilepsy(IE)were studied by fluorescence quantative polymerase chain reaction(FQ-PCR)for CDK5 mRNA,while immunohistochemistry and Western blot were used to study the protein expression.Nonepileptogenic control brain tissues were used for comparison.Results FQ-PCR analysis showed that the expression of CDK5 mRNA in epilepsy patients was significant higher than those in the control group.And immunohistochemistry showed that the protein mainly existed in the neuron and glial.At the 35000 relative molecular mass,Western blot could been seen that there is a limpid strap.The optical density of CDK5 in IE(temporal lobe 1.4293?0.1839,hippocampus 2.0733?0.4738)was significantly higher than that in the control(temporal lobe 0.9680?0.4147, hippocampus 1.4030?0.6160,P
8.Analysis of the vacuum sealing drainage technique combined with sural neurovascular pedicle fascio-cutaneous flap to repair deep wounds in the foot near the ankle joint with exposed bone and tendons.

OBJECTIVETo evaluate the practical method of vacuum sealing drainage (VSD) technique combined with sural neurovascular pedicle fasciocutaneous flap to repair deep wounds in the foot near the ankle joint with exposed bone and tendons.
METHODSFrom January 2006 to January 2009, 79 patients with deep wounds in the foot near the ankle joint with exposed bone and tendons were treated by VSD technique combined with sural neurovascular pedicle fasciocutaneous flap including 58 males and 21 females with an average age of 34 years old ranging from 7 to 59 years. There were 17 cases in low 1/3 part of leg and achilles tendon, 28 in lateral malleolus and lateral dorsum of foot, 21 in medial malleolus and medial dorsum of foot, 13 in heel and pelma. Firstly the wounds were debrided and cultivated by using VSD technique, then the soft tissue defections were repaired with sural neurovascular pedicle fasciocutaneous flap.
RESULTSThe area of flap was from 6 cm x 5 cm to 18 cm x 15 cm; All patients stayed in hospital for 14 to 30 days, 18 days in average. Living flaps of all patients were followed-up from 6 months to 3 years, the flaps of 2 patients were mostly necrotic, 3 were necrotic, 5 cases appeared obstacle of venous back streaming. The others survived with no infections.
CONCLUSIONThe wound would become fresh and clean as soon as possible with VSD. The sural neurovascular pedicle fasciocutaneous flap could provide a good covering for the exposed wound. Therefore the wound healed faster with friction resistance and fine appearance. The time of hospitalization were greatly shortened after combined application.

As the most homologic homologue of silent information regulator 2 of yeast, Sirt1 gene is extensively expressed in mature tissues, and is rich in early embryo and reproductive cells. It is involved in the regulation of gene transcription, energy metabolism and cell aging. It promotes fat mobilization in adipocytes and glucose production in liver and regulates insulin secretion in islet beta cell. Furthermore, Sirt1 gene is an essential endogenous apoptosis inhibitor. In future, it may be used as new drug targets or applied in other disease management modalities.

10. Up-regulated expression of Beclin-1 in missed abortion and its clinical significance

Objective Reports are rarely seen on the role of autophagy in missed abortion. This study aimed to explore the association of missed abortion with autophagy by detecting the expressions of the autophagy-related factors Beclin-1 and LC3 in the villus and decidua of normal pregnancy and missed abortion women so as to provide some theoretical evidence for the early prevention of missed abortion.Methods We included in the experimental group 30 missed abortion patients treated in the Department of Obstetrics and Gynecology, the Fourth Affiliated Hospital of Jiangsu University, from January to March 2017 and enrolled another 30 normal pregnancy women as controls. We determined the expression level of Beclin-1 mRNA in the villus and decidua by quantitative fluorescence PCR and those of Beclin-1 and LC3II/LC3I proteins by Western blot. We also positioned Beclin-1 in the villus and decidua tissues of the two groups of women by immunohistochemistry.Results Compared with the normal pregnancy women, the patients with missed abortion showed significantly up-regulated expression of Beclin-1 mRNA in the villus (1.00±0.28 vs 2.17±0.87, P<0.05) and decidua (1.75±0.41 vs 4.74±0.93, P<0.05) and those of Beclin-1 and LC3II/LC3I proteins (P<0.05). Immunohistochemistry revealed markedly increased expression of the Beclin-1 protein in the villus and decidua of the missed abortion patients (P<0.05).Conclusion Beclin-1 mRNA, Beclin-1 protein and LC3II/LC3I are all up-regulated in the villus and decidua of missed abortion patients, suggesting the involvement of autophagy in the occurrence of missed abortion, which may provide a new theoretical basis for studying the pathogenesis of missed abortion.
