1.Effect of different compatibility of zhizi dahuang decoction on pharmacokinetics of naringenin and hesperetin.

An HPLC-UV method was developed for the determination of total naringenin and total hesperetin in rat plasma after oral administration of Citrus aurantium Immaturus extracts and Zhizi Dahuang decoction. Plasma samples were pretreated with liquid-liquid extraction procedure and acid hydrolysis method was used for converting conjugated naringenin and hesperetin to their respective free forms. Plasma samples were separated on a C18 column (4.6 mm x 150 mm, 5 microm), using 0.1% phosphoric acid and methanol as mobile phase at a flow rate of 1.0 mL x min(-1) with gradient elution. DAS 2.0 software was applied to calculate the pharmacokinetic parameters while the SPSS 16.0 software was used for statistical analysis. Significant differences were observed, the C(max) AUC(0-t) of total naringenin in ZS group was 73.5% and 65.9% higher than those in ZZDHD group, respectively; the C(max), AUC(0-t) of total hesperetin in ZS group was 63.5% and 119.1% higher than those in ZZDHD group, respectively. There is a obvious decrease in C(max) and AUC(0-t) of total naringenin and total hesperetin after compatibility and their pharmacokinetic characteristics changed greatly due to the combination of other herbs. The established method was rapid, sensitive, selective and accurate, and it could be applied in the determination of total naringenin and total hesperetin in rat plasma.

2. Effects of regulation mode of cabin gas on arterial blood gas of mariners during a prolonged voyage in a nuclear submarine

Objective To study the effect of different regulation modes of the air in cabin on the arterial blood gas of mariners during a prolonged voyage in a nuclear submarine. Methods During three long distance voyages of the same nuclear submarine, the same air-regeneration device was adopted. According to the different doses and percentages of the oxygen-lithium hydroxide absorbers, the mariners of three voyages were divided by sequence of voyages into control group (C), observation group 1 (A), and observation group 2 (B). The change in arterial blood PaO2 and PaCO2 of mariners in the three groups were detected on the third day of surface navigation (T1), 10th day (T2), 12nd day (T3), 13th day (T4) of submarine navigation, and the day before returning (T5). Result There was no difference between the three groups in T1 and T2 (P>0.05). There was a significant difference in PaO2 and PaCO2 between the two observation groups and the control group on T3, T4 and T5 (P<0.05). Conclusions Different amount and percentage of the oxygenlithium hydroxide showed an influence on PaO2 and PaCO2 of the mariners. Optimal amount and percentage of the oxygen-lithium hydroxide is very important in keeping normal physiological status of mariners in a nuclear submarine.

Objective To establish the standard for the quality control of Shanlamei Granule.Method Chimonanthus nitens Oliver.was identified quantitativ ely by thin -layer chromatography(TLC),quercetin and kaempferol in Shanlamei Granule were determined by RP -HPLC.Results The TLC Spots were highly clear without the interference of negative control and were reproductive.The c alibration curve for quercetin was l inear(r =0.9999)in the range of 1.61~25.72?g /mL and the mean recovery was 98.89%,RSD being 1.21%(n=5);for Kaempferol ,the calibration cu rve was linear(r =0.9999)in the range of 2.16~34.60?g /mL and the mean recovery was 97.75%,RSD being 1.53%(n=5).Conclusion This method is accurate,simple,sensitive and reliable,and it can be used for the quality control of Shanlamei Granule.

8.Intermediary effect of depression between the Housebound and cognition in community elderly

Objective To investigate the intermediary effect of depression between housebound and cognition in community-dwelling older adults and to provide intervention for the improvement of the life quality.Methods A total of 720 senior adults were selected to fill out demographic questionnaire,including Rasch-Derived Center for Epidemiological Studies Depression Scale (CES-D-R),housebound scale and the minimental state examination (MMES).Results The depression score of the elderly in community was (3.95±4.12),housebound score was (4.26±1.23),cognition score was (21.52±4.04) and the incidence rate of the cognitive disorder was 37.1%.Cognition was significantly negatively related to housebound and depression (r=-0.409,P<0.01;r=-0.485,P<0.01).Housebound was significantly positively related to depression (r=0.555,P<0.01).The community elderly age,sex,marital status,degree of education,self-assessment of health,living space,depression and the housebound could be used to predict the cognitive function,and all of them could explain the variance of cognitive function of 50.8%(P<0.05).Housebound had a direct effect of-0.02 on cognitive function(P>0.05).The indirect effect of housebound on cognitive function through depressive mood was-0.45(P<0.05).Conclusions The elderly housebound has an intermediary effect on the cognitive function through the depression.
9.Influence of induction chemotherapy on target volume and dosimetry of intensity modulated radiotherapy for locally advanced nasopharyngeal carcinoma

As induction chemotherapy goes on,target volume,dose distribution in the surrounding organs at risk (OARs),and target dose conformity all change.Therefore,the question is how to develop reasonable radiotherapy plans in clinical practice.Induction chemotherapy followed by radiotherapy is commonly used around the world,but it is recommended to delineate the target volume based on the gross tumor volume before induction chemotherapy and not to reduce the dose.This point of view lacks the basis of evidence-based medicine.The experts and scholars in China clarify the advantages of radiotherapy plans after induction chemotherapy from the aspects of reducing the target volume,reducing the volume of high-dose region in the target volume,increasing the uniform dose coverage in target volume,reducing dose to OARs,and increasing dose conformity.However,at present,there are no objective data on its long-term efficacy and benefit.Besides,no consensus has been reached on how to delineate the target volume and determine the dose distribution after induction chemotherapy,and further studies are needed.
10.Value of FDG PET-CT in outcome assessment and prognostic evaluation for recurrent nasopharyngeal carcinoma

Objective To investigate the value of FDG PET-CT in the outcome assessment and prognostic evaluation for recurrent nasopharyngeal carcinoma (rNPC).Methods From January 2008 to December 2013,92 rNPC patients were treated in our center,who were histologically or radiologically diagnosed and re-staged according to the 2008 clinical staging system for nasopharyngeal carcinoma in China.The numbers of patients in stage Ⅰ,stage Ⅱ,stage Ⅲ,and stage Ⅳ were 8,11,39,and 34,respectively.According to the recurrent T stage (rT),the numbers of patients in rT1,rT2,rT3,and rT4 were 10,11,38,and 33,respectively.Twenty-eight patients had recurrence in the neck lymph nodes.All patients underwent pretreatment FDG PET-CT for the whole body or head/neck,and treated by radiotherapy with or without chemotherapy.The relationship of maximum standard uptake value (SUVmax) and clinical factors with clinical outcomes was analyzed.The Kaplan-Meier method was used to calculate overall survival (OS),disease-free survival (DFS),local recurrence-free survival (LRFS),regional recurrence-free survival (RRFS),and distant metastasis-free survival (DMFS).The log-rank test was used for survival difference analysis and univariate prognostic analysis.The Cox model was used for multivariate prognostic analysis.Results The 3-year OS,DFS,LRFS,RRFS,and DMFS were 33.6%,32.1%,32.8%,31.8%,and 33.7%,respectively.The median SUVmax was 8.35 (2.7-21.5).The SUVmax of 7.0 was taken as the optimal cut-off value for all patients.Patients with SUVmax ≤7.0 had a significantly higher 3-year OS rate than those with SUVm ax >7.0 (42.0% vs.28.3%,P=0.019).The univariate analysis revealed that patient age,SUV and rN were significantly associated with OS (P=0.023,0.019,and 0.002).The multivariate analysis showed that SUVmax and rN were significant influencing factors for OS,DFS,and DMFS (HR=1.68,P=0.045 and HR=2.23,P=0.003;HR=1.67,P=0.042 and HR=2.39,P=0.001;HR=1.77,P=0.025 and HR=2.40,P=0.001).Conclusions SUVmax may be one of the useful prognostic factors for OS,DFS,and DMFS in rNPC patients.
