Objective: To evaluate the outcome and postoperative reaction of dural substitute (Neuropatch) applying in the treatment of Chiari I malformation(CMI) associated with syringomyelia(SM). Methods:Forty patients of CMI associated with SM were operated in our department from Jul. 2002 to Jul. 2004. All patients underwent posterior cranial fossa decompression and duraplasty. They were divided into two groups, 20 patients being repaired with Neuropatch (Neuropatch group), and the others with autologous fascia lata (fascia group). There were 6 males and 14 females in Neuropatch group and 10 males and 10 females in fascia group. The operations were performed under general anesthesia via suboccipital approach and the extent of posterior cranial fossa decompression ranged from 20 cm2 (5 cm×4 cm) to 35 cm2 (5 cm×7 cm). The removal of posterior arch of atlas depended on the extent of tonsillar herniation, and the dura was opened in Y shape. The Neuropatch was cut into triangular shape, and the same sized autologous fascia lata was used in fascia group. The patches were sutured tightly to the dura matter in each group. The incision was closed layer by layer and drainage was used, if necessary. Antibiotics and hormone were routinely used. The duration of operation, postoperative fever were evaluated, the outcome of the operation was evaluated by Tator scale, and the data were analyzed with statistic software SPSS 10.0. Results: There were12 patients (60%) who suffered from postoperative fever in the Neuropatch group, and 9 patients (45%) in the fascia group(χ2=0.902,P=0.342). Seventeen patients in each group were improved postoperatively. The duration of operation, postoperative fever and antibiotics used were compared between the two groups. No significant difference was found, but the duration of postoperative fever and the time of hormone used were different. There were no postoperative infections that occurred after the follow up for 1 to 2 years, except for one patient in fascia group who developed infective granuloma and recovered later by treatment. Conclusion: Neuropatch is a useful dural substitute for the repair of dural defects in the treatment of CMI associated with syringomyelia.

Objective To investigate the effects of early activities combined with evodia hot salt pack applying to the navel in prophylaxis against abdominal distension after surgery for gynecological malignancies. Methods 318 patients suffered from gynecological malignancies were randomly divided into three groups, activity group (n =102),vitamin B1 group ( n =110) and evodia group ( n =106 ).The activity group began activities within 24 hours after surgery; the vitamin B1 group,in addition to the traditional nursing, received injection of vitamin B1 through tsu-san-li meridians, each side with 50mg; the evodia group, besides the activities, was managed with evodia mixed with hot salt pack applied to the navel. The incidence of abdominal distension, the average time of bowel sound recovery, the first time of flatus passing and defecation of the three groups were compared. Results The incidence of abdominal distension in the evodia group ( 16.9% ) was significantly lower than that in the activity group (55.9%) and the vitamin B1 group (67.3%) ( x2 =70.50,P < 0.01 ).The average time of bowel sound recovery, the first time flatus passing and defecation in the evodia group [ (30.02 ±4.37)h,(42.16 ± 16.03 )h,(72.62 ± 20.79 )h] was significantly shorter than those in the activity group [ (40.57 ±5.27)h,(74.25 ± 12.14)h,(135.33 ±40.18) h] and the vitamin B1 group [ (44.23 ± 12.16)h,(77.25±16.32)h,(139.71 ±38.25)h] (F=8.88,4.37,5.49; P<0.01).Conclusions Early activities after surgery combined with evodia hot salt pack applying to the navel could effectively facilitate the recovery of gastrointestinal function for patients suffered from gynecological malignancies and prevent abdominal distension.

Objective To study the diagnosis and treatment of primary adenosquamous carcinoma (ASC) of liver. Methods The clinical and histopathological data of 2 patients with ASC were analyzed retrospectively. The cases were confirmed by surgery and histopathological examination. Results The tumors were located in the left liver in these 2 patients. Both patients presented with abdominal pain and they underwent surgical resection. On gross pathological examination, the tumors were irregular in shape with unclear boundaries.Microscopically, the tumor cells were arranged in a nesting pattern with a tubular structure, which was an adenosquamous carcinoma structure. One patient survived 4 months after surgery and the other patient died of intrahepatic tumour recurrence 7 months after surgery. Conclusions The diagnosis of primary hepatic ASC relied on histopathological examination.Surgical resection was safe and feasible.

Baicalin extremely fine powder was made by using ball-mill and the effect of micronization on the micromeritics properties of baicalin was studied and analyzed. The microstructures of baicalin ordinary and extremely fine powder were compared by scanning electron microscope, differential scanning calorimeter and X-ray diffraction and the powder characteristic of them was investigated. The hygroscopicity was studied. The effect of micronization on the dissolution of baicalin was investigated. The results showed that the chemical constituents of baicalin were not changed after micronization with better compressibility. It was confirmed that micronization technology had a certain application value in promoting the insoluble component of baicalin absorption with higher dissolution.

OBJECTIVETo explore the correlation between hand and face-mouth, so as to provide nerve reflex basis for the theory "Hegu (LI 4) regulates face and mouth".
METHODSSeven hundred and sixty-three participants who met the inclusive criteria were divided into different age groups. The skin around participants' thenar eminence was gently scraped to be observed whether there was an involuntary movement around the face or mouth, which was palmomental reflex. The results of palmomental reflex were recorded.
RESULTSThe total occurrence rate of palmomental reflex was 46.26%. For those who were 0 to 1 years old, the palmomental reflex was all positive; for those who were 21 to 36 years old, the positive rate was 20.45%, which was the lowest; for those who were 65 to 85 years old, more than half of them were positive. The majority of those who were 0 to 2 years old were bilateral positive palmomental reflex, while the majority of those who were 65 to 85 years old were unilateral positive palmomental reflex.
CONCLUSIONThere is a certain connection between hand and face-mouth. The occurrence rate of palmomental reflex changes from high to low over age increasing, and then changes from low to high with the aging, presenting a "high-low-high" U-shaped curve, which is possible related to the growth and recession of nervous system.

OBJECTIVETo compare the dissolution characteristics of colloidal silica and porous silica as the solid dispersion carrier, with baicalin as the model drug.
METHODThe baicalin solid dispersion was prepared by the solvent method, with colloidal silica and porous silica as the carriers. In the in vitro dissolution experiment, the solid dispersion was identified by scanning electron microscopy, differential scanning and X-ray diffraction.
RESULTThe solid dispersion carriers prepared with both colloidal silica and porous silica could achieve the purpose of rapid release. Along with the increase in the proportion of the carriers, the dissolution rate is accelerated to more than 80% within 60 min. Baicalin existed in the solid dispersion carriers in the non-crystalline form.
CONCLUSIONThe release behaviors of the baicalin solid dispersion prepared with two types of carrier were different. Among the two solid dispersion carriers, porous silica dissolved slowly than colloidal silica within 60 min, and they showed similar dissolutions after 60 min.
