1.Analysis of risk factors and treatment of postoperative delirium in patients with benign prostatic hyperplasia

Objective To investigate the risk factors and treatment of postoperative delirium in patients with benign prostatic hyperplasia(BPH). Methods For 256 postoperrative BPH patients in our department,the clinical parameters including age,hypoxemia,preoperative infection,postoperative pain,and introvert were analyzed,and the treatment were summarized. Results The incidence of postoperative delirium was higher in patients aged≥75 years than<75 years[17.4%(19/109)vs.5.4%(8/147),X2=8.307,P=0.0043,higher in patients with preoperative infection (pulmonary or urinary)versus without the infection[15.0%(17/113)vs.7.0%(10/143),X2=4.337,P=0.037],higher in patients with hypoxia versus without hipoxia[16.5%(16/97).vs.6.9%(111159),X2=4.885,P=0.027]and higher in patients with post-operative pain versus without the pain(16.9%(10/59)vs.1.2%(1/85),X2=10.146,P=0.0014].Hematocrit and hemoglobin in patients with postoperative delirium were lower after operation versus pre-operation[(0.29±0.04)vs.(0.42±0.04),t=12.2314,P<0.001;(102.39±6.58)g/L vs.(114.58±8.36)g/L,t=5.9537,P<0.001].All delirium patients were recovered after sedation treatment,improving the quality of sleep,controlling the infection and correcting electrolyte turbulence. Conclusions Advanced age,hypoxemia,preoperative infection,postoperative pain are important risk factors for delirium.
2.Study of quality of life in patients with stroke at the acute stage and the suitability of SF-36

Objective To assess the quality of life(QOL)in patients with stroke at the acute stage,and to determine whether all the dimensions of the Medical Outcomes Study 36-Item Short-Form Health Survey(SF-36)are applicable to the stroke patients.Methods Fifty stroke patients at the acute stage were studied.The SF-36 was employed to assess the quality of life of the patients,and a self-designed questionnaire was used to record the general information of them.The data obtained from the patients was compared with the Chinese norm reported in literature. We also calculated the percentage of subjects who achieved the lowest possible score(score of 0,floor effect)and the percentage of subjects who achieved the highest possible score(score of 100,ceiling effect).Results The QOL of the acute stroke patients,as reflected by scores with all the dimensions of the SF-36 scale,was significantly reduced when compared with the norm data(P
3.Video-assisted thoracoscopic mitral valve replacement

Objective To summarize the experience of mitral valve replacement (MVR) using video-assisted thoracoscope. Methods 28 cases of MVR were performed through three right anterior ports under thoracoscopy using femorofemoral extracorporeal circulation. The aorta was cross-clamped and the myocardium was protected by coronary perfusion with cold cardioplegic. 33 patients underwent routine open approach MVR as control group. Results There was no death and no morbidity directly related to this approach. Comparing with control group, time of extracorporeal circulation was 82~146 (96?38)min vs. 80~132(92?31)min, cross-clamped 37~76(47?18)min vs. 34~72(45?13)min and ventilation time were 8.6~14.8(10.2?3.1)h vs. 8.3~15.9(11.3?3.4)h . 5.MRI characteristics and pathological comparison of cesarean scar pregnancy during the first-trimester

Objective To study the MRI findings and pathologic characteristics of cesarean scar pregnancy during the first-trimes-ter.Methods Clinical data,MRI findings and pathologic manifestations of 23 CSP patients confirmed by surgery and pathology were analyzed retrospectively.Results On pathologic specimens by the microscope,villi and decidua tissue were found in the myometrium of all 23 cases (100%),and smooth muscle tissue in the uterus scar was incomplete in 12 cases (52.2%).The incision scars were found in the uterine lower antetheca in 21 cases,which showed low signal on T1 WI and T2 WI.A majority of the gestational sac pro-truded into intrauterine in 12 cases.The gestational sac located in the scar of lower uterine antetheca in 10 cases and in the myometri-um around the scar in 1 case.The gestational sac of 22 cases were quasi-round with the size between 1.8 cm×1.2 cm and 6.4 cm× 2.7 cm.The gestational sacs showed homogeneous low signal on T1 WI and high signal on T2 WI in 8 cases,heterogeneous high signal on T2 WI and STIR sequence in 1 5 cases,a little short T1 signal of bleeding in 9 cases,and heterogeneous high signal on DWI sequence in 3 cases.The gestational sac borders were well-defined in 18 cases and blurred in 5 cases.Among 5 cases performed by enhanced MRI,there were 3 cases which gestational sac walls were ring-shaped enhanced.Conclusion MRI findings of CSP had certain char-acteristics and could reflect the pathological characteristics of CSP.MRI had important reference value in early diagnosis of CSP and the choice of treatment.

Transforming growth factor-?_1 (TGF-?_) mRNA or protein expression in renal cortex of diabetic rats was assessed by real-time quantitative RT-PCR with SYBR Green,immunohistochemistry or Western blot.After melatonin treatment,the expressions of TGF-?_1 mRNA and protein were decreased,suggesting that beneficial effect of melatonin may result from its antioxidative property and inhibiting TGF-?_1 expressions.
9.Placental expression of lipoprotein lipase and endothelial lipase in women with severe preeclampsia

Objective To investigate the effect of abnormal lipid metabolism in the pathogenesis of preeclampsia.Methods Thirty women with severe preeclampsia who delivered by cesarean section in the International Peace Maternity & Child Health Hospital from November 1,2011 to June 30,2012,were included as the study group.Thirty normal pregnant women delivered during the same period by cesarean section were included as the control group.The mRNA levels of lipoprotein lipase (LPL) and endothelial lipase (EL) in the placentas were quantified by real-time polymerase chain reaction.The expression of LPL/EL protein was detected by Western-blot assay and the distribution on the placenta was determined by immunohistochemical staining.The maternal serum level of lipid markers was detected by automatic biochemical analyzer.Difference between the two groups was compared with t-test or rank-sum test.Results (1) The triglyceride (TG) concentration [(3.35±0.80) mmol/L vs (2.75± 0.92) mmol/L,t =3.082,P=0.003] and atherosclerosis index (AI) (2.76±0.46 vs 2.47±0.34,t=3.066,P=0.003) increased in the study group compared with the control group,while the concentration of high-density lipoprotein-cholestrol (HDL-c) decreased[(1.64 ± 0.34) mmol/L vs (1.85 ± 0.22) mmol/L,t=-3.157,P =0.003].(2) The median level of LPL mRNA expression in placenta of the study group was higher than that of the control [1.26 (0.46-1.58) vs 0.52 (0.34-0.84),Z=-2.587,P=0.010],whereas the expression of EL mRNA was lower [0.65 (0.48-0.76) vs 1.32 (0.94-1.62),Z=-4.154,P=0.000].(3) Immunohistochemical analysis showed that the placenta EL and LPL located in cytoplasm of syncytiotrophoblast cells at the materno-placental interface and of endothelial cells at the feto-placental interface.(4) Compared with the control group,the median level of LPL protein expression in placenta inthe study group increased [0.68(0.46-0.83) vs 0.31(0.15-0.55),Z=3.335,P=0.001],and the expression of EL protein decreased [0.13 (0.11 0.16) vs 0.70(0.56 0.81),Z=-5.711,P=0.000].The expression of LPL protein was negatively correlated with that of EL protein (r=-0.501,P=0.000).Conclusions Abnormal lipid metabolism contributes to the pathogenesis of severe preeclampsia.
10.Totally thoracoscopic surgery for isolated atrial fibrillation

ObjectiveThis paper reported our experience with thoracoscopic management of isolated atrial fibrillation to define the efficacy and safety of this approach.MethodsThirtytwo patients ( 17 mem,15 women) with isolated atrial fibrillation underwent thoracoscopic surgery.All procedures were finished under 3 port incisions on left posterior chest.Among them 18 cases are paroxysmal and 8 persistent.ResultsThere was no operative death or major perioperative complications.One case was converted to limited thoracotomy because of bleeding.Operation time was 87 - 238 min.Paroxysmal atrial fibrillation occurred in 9 cases in hospital and all the cases were sinus rhythm after discharge.Followup 4 to 20 months,One persistent case was converted paroxysmal.ConclusionPatients with isolated atrial fibrillation can benefited by Videoassisted thoracoscopic left posterior approach with better exposure of left atrial and resection of the left atrial appendage,with decreased operative trauma and better results.
