1.Variation of contrast sensitivity after femtosecond laser in situ keratomileusis in changes environments

Abstract?AlM: To evaluate the difference of contrast sensitivity ( CS) in photopic and scotopic environments in eyes with myopia and myopic astigmatism operated with femtosecond laser in situ keratomileusis ( femto-LASlK ) and laser in situ keratomileusis ( LASlK) .?METHODS: ln a prospective study 160 myopia and myopic astigmatism patients' eyes were involved, which accepted femto- LASlK or LASlK in our hospital from January 2010 to February 2012. The myopia degree was -1. 50 ~ -10. 00D, the astigmatism degree ≤-6. 0D. Eighty eyes were treated with femto-LASlK in group A, and 80 eyes were treated with LASlK in group B, All patients in the treatment group completed the final 6mo of follow-up. The uncorrected visual acuity ( UCVA) and the best spectacle-corrected visual acuity ( BSCVA) , objective and manifest refractions, results of slit-lamp examination, the side effects, intraocular pressure, corneal topography, CS in photopic and scotopic environments were noted.? RESULTS: All of operations on 160 cases were successful without severe complication after 6 mo follow-up. CS of femto-LASlK group ( group A) at each spatial frequency environment were higher than that of LASlK group (group B). ln group A, after 1mo the photopic CS, after 3mo of scotopic CS recovered to the preoperative level, 6mo after surgery improved than before the operation. ln group B, after 3mo photopic CS to the preoperative level, scotopic CS at 6mo after operation was still not recovered to the preoperative level.?CONCLUSlON:Femto-LASlK for correction of myopia and myopic astigmatism, in improving the postoperative contrast sensitivity under shade environment has more advantages than LASlK.
3.Effects of wing-like memory alloy intrasegmental fixator on healing of lumbar spondylolysis in dogs

[Objective]To investigate the influence of wing-like memory alloy intrasegmental fixator(WMAIF)on healing of lumbar spondylolysis.[Method]Eighteen healthy hybrid adult dogs were used in the experiment,regardless of gender,the L_6 isthmic portion were injured by operation randomly and equally divided into three groups.Group Ⅰ were fixed with WMAIF,groupⅡ were fixed with steel wire,group Ⅲ were treated with nothing.Every group was examined through CT at 2,4,6,8 and 10 weeks.[Result]In group Ⅰ,the fracture achieved healing at 6 weeks after operation.In group Ⅱ,the fracture achieved healing at 10 weeks after operation,while no repair of fracture was found in group Ⅲ.[Conclusion]Lumber spondylolysis fixation with WMAIF has stable and continued axial stress,WMAIF is beneficial to fracture healing,and is a new and reliable intrasegmental instrument for direct repair of lumbar spondylolysis.

5.Analysis of serum AGEs and sRAGE levels in newly diagnosed patients with type 2 diabetes

Objective To understand the serum levels of advanced glycation end products (AGEs),soluble receptor for advanced glycation end products (sRAGE) in newly diagnosed patients with type 2 diabetes.Methods 60 patients of newly diagnosed type 2 diabetes were chosen from 2010 September to 2011 December in Department of Endocrinology Changshou Chinese medicine hospital in Jiangsu Province,and 30 healthy people were screened as control group.Various clinical and biochemical parameters were detected,using enzyme-linked immunosorbent assay for the detection of serum AGEs and sRAGE levels.Results The levels of serum AGEs in newly diagnosed type 2 diabetic patients and control group were (1379.2 ± 431.8) and (1154.5 ±326.4) pg/ml,and there were significant differences between two groups (P<0.05).The levels of serum sRAGE in newly diagnosed type 2 diabetic patients and control group were (14.6± 9.3)and (19.5 ± 8.9)ng/ml,and there were also significant differences between two groups (P<0.05).The level of serum AGEs in newly diagnosed type 2 diabetic patients was positive correlation with glycosylated hemoglobin (HbA1c),and the level of serum sRAGE was associated with low density lipoprotein cholesterol (LDL-C) negative correlation.Conclusion The level of serum AGEs increase in newly diagnosed type 2 diabetic patients,and the level of serum sRAGE decrease in newly diagnosed type 2 diabetic patients.
6.Simultaneous surgery in patients suffering from tumor combined with coronary artery disease

Objective:To investigate the possibility and feasibility of the simultaneous coronary artery bypass grafting (CABG) with tumor excision in patients suffering from tumor combined with coronary artery disease(CAD). Methods:From August 2000 to July 2006, ten patients who were suspected of tumor (four patients suffered from urinary system tumor, two digestive system tumor, and four pulmonary tumor) with coronary artery disease were successfully treated by simultaneous surgeries. Surgical incisions were chose by the different tumor position. All carried on the CABG before tumor excision except one case. The two cases had cardiopulmonary bypass CABG, the rest eight had off-pump CABG. Follow-up was conducted after operation. Results:The combined operations were satisfied. There was no in-hospital mortality, no postoperative hemorrhage, no myocardial ischemia or infarction, with few complications. Postoperative in-hospital stay was 10-34 days, average 22.4 days. All discharged in cure. One patient lost the follow-up. Nine patients were followed up from six months to seven years three months. Two patients died from metastasis, and the others survived with satisfied follow-up without cardiovascular events. Conclusion:The simultaneous coronary artery bypass grafting with tumor excision for patients who suffer from either CAD with benign or malignant tumor is safe and feasible with satisfied short-and long-term survival.
7.Drug Resistant Genes in ICU Isolates of Pseudomonas aeruginosa and Cluster Analysis of Strains

OBJECTIVE To investigate the existence of genes for beta-lactam antibiotic resistance and for aminoglycosides modification enzymes(AMEs) in Pseudomonas aeruginosa(PAE) isolates from ICU patients and analyze the homology among strains.METHODS ?-Lactamase genes including TEM,SHV,OXA-10,PER,VEB,GES,CARB,IMP,VIM,SPM,GIM,DHA,FOX,MOX and oprD2,were detected by PCR amplication in 21 PAE isolates.The genes for AMES including aac(3)-Ⅰ,aac(3)-Ⅱ,aac(6′)-Ⅰ,aac(6′)-Ⅱ,ant(3″)-Ⅰ and ant(2″)-Ⅰwere determined by PCR amplification as well.RESULTS Among 21 isolates 21(100%),2(9.5%),1(4.8%),2(9.5%)and 4(19.0%) were positive for TEM,SHV,GES,CARB and VIM genes,respectively.The deletion of oprD2 gene was found in 14 out of 21 strains.Other ?-lactamase genes were absent in all isolates.As for AME genes,aac(3)-Ⅱ,aac(6″)-Ⅰ,aac(6)-Ⅱ,ant(3″)-Ⅰ,ant(2″)-Ⅰ and aac(3)-Ⅰgenes were present in 19.0%,23.8%,9.5%,4.8%,and 19.0% of 21 isolates,However,aac(3)-Ⅰ gene was no position in any isolates.CONCLUSIONS P.aeruginosa carries various beta-lactamase and AME genes in ICU patients.Genetic cluster analysis suggested that clonal propagation result in nosocomial infection of PAE.
8.Reverse hybridization applied in detection on human papillomavirus infection of twenty-three subfamilies

Objective To establish a method of reverse hybridization to detect five subfamilies of low risk Human Papillomaviruses(HPV6,11,42,43 and 44)and eighteen subfamilies of high risk HPV (HPV16,18,31,33,35,39,45,51,52,53,56,58,59,66,68,73,83 and MM4)in one reaction.Methods Special probes for twenty-three HPV subfamilies were fixed on nylon membrane bars,biotin labeled general primers mediated polymerase chain reaction(GP-PCR)were applied in HPV DNA amplification.PCR amplified DNA fragments were reversely hybridized with special probes that were fixed on the membranes. All samples(136)detected by reverse hybridization method were paralleled with the methods of Hybridization Capture Ⅱ(HC-Ⅱ)and sequencing.Results Positive rate of the 136 samples detected by reverse hybridization was 41.9%,while HC-Ⅱ 42.6% and sequencing 40.4%.Reverse hybridization detection indicated coherence with the other two methods(Kappa 0.8644 and 0.9089,respectively).While sequencing was lab standard for DNA test,the sensitivity was 96.36%,specificity was 95.06%,accuracy was 95.59%.Conclusions Method of reverse hybridization is adaptable to 23 kinds of HPV subfamilies, which can confirm the exactly subfamilies of HPV infection.This method is adaptable in clinical detection of HPV,with high sensitivity,high specificity,simply and convenient operation and the results are easily to be read.
9.Study on the effect of astragalus on serum AGEs, sRAGE levels with newly diagnosed type 2 diabetic patients

Objective To understand the effect of astragalus on serum levels of advanced glycation end products (AGEs),soluble receptor for advanced glycation end products (sRAGE) in newly diagnosed patients with type 2 diabetes.Methods 60 patients of newly diagnosed type 2 diabetes were randomly divided into control group (28 patients) and treatment group (28 patients),the control group was given control of blood glucose,blood pressure,lipids and other necessary treatment,and the treatment group was additionally treated with astmgalus granule based on the control group.The course for both treatments is 12 weeks.Various clinical and biochemical parameters were tested before and after treatment in both groups with enzyme-linked immunosorbent assay for the detection of serum AGEs,sRAGE levels.Results ① Glycosylated hemoglobin (HbA1c) in the control group after treatment (7.28± 0.83)% was significantly decreased than before treatment (9.57±1.14) %,the difference was statistically significant (t=5.75,P<0.05); glycerin three greases (TG) in the control group after the treatment (1.45± 0.39)mmol/L decreased significantly compared with those before treatment (1.92± 1.01)mmol/L,the difference was statistically significant(t=2.79,P<0.05); ② HbA1c (7.16± 0.88) % in the treatment group after treatment was significantly decreased than before treatment (9.29± 1.62)%,the difference was statistically significant (t=6.08,P<0.05) ; TG (1.53 ± 0.41) mmol/L in the treatment group after treatment was significantly decreased compared with those before treatment (2.11 ± 0.79)mmol/L,the difference was statistically significant (t = 2.40,P < 0.05) ; the levels of serum sRAGE (20.38 ± 8.01) ng/ml in the treatment group after treatment increased obviously compared with those before treatment (15.10 ± 9.22)ng/ml,the difference was statistically significant (t=-2.29,P<0.05); ③ The level of serum sRAGE (20.38 ± 8.01) ng/ml in the treatment group after treatment increased obviously than the levels of serum sRAGE (15.13 ± 9.27)ng/ml in the control group after treatment,the difference was statistically significant (t=-1.17,P<0.05).Conclusion Astragali can increase serum sRAGE expression in newly diagnosed type 2 diabetic patients.
10.The effects of astragalus combined with metformin on insulin resistance and plasma amylin level in newly diagnosed type 2 diabetic patients

Objective To understand the effect of astragalus on insulin resistance and plasma amylin levels in newly diagnosed type 2 diabetic patients.Methods 88 patients of newly diagnosed type 2 diabetes were randomly divided into three groups:Group A (lifestyle intervention group) contained 30 patients,Group B (metformin treatment group) also contained 30 Patients,and Group C (astragalus and metformin treatment group) contained 28 Patients.Patients in group A were intervened with the control of diet,blood pressure and lipids level; patients in group B were additionally treated with metformin on the basis of group A; patients in group C were additionally treated with metformin and astragalus on the basis of group A.The course for both treatments were 8 weeks.Various clinical and biochemical parameters were detected before and after the treatment in all three groups and enzyme-linked immunosorbent assay was adopted for the detection of plasma amylin.Results Fasting blood glucose levels were significantly decreased after treataent in the three groups (t=-2.696、-4.029、-3.995,P<0.05) ; insulin resistance index reduced in group B and group C (t=-2.599、-3.813,P<0.05),the difference between group C and group B was statistically significant (t=-2.334,P<0.05) ;treatments of group B and group C could improve the beta cell function index (t=2.303、2.384,P<0.05),and they also could increase the plasma amylin level (t=2.341、3.045,P<0.05).Conclusion Astragalus and metformin can improve insulin resistance index and increase plasma amylin levels in newly diagnosed type 2 diabetic patients.
