1.Peroxisome proliferator-activated receptor-gamma in the regulation of inflammatory reaction in rats with myocardial hypertrophy

BACKGROUND: Peroxisome proliferator-activated receptor-gamma(PPAR-γ) can restrain the inflammatory reaction of hypertrophic myocardium through restraining the expression of interleukin-6, cyclooxygenase, endothelin-1, nitricoxide synthase, matrix metalIoproteinase-9, gelatinase and adhesion molecule, etc.OBJECTIVE: To observe the influence of rosiglitazone sodium(the ligand for PPAR-γ) on inflammatory factors in rats with myocardial hypertrophy in the course of myocardial hypertrophy resulting from pressure load.DESIGN: Randomized controlled trial based on animals.SETTING: Department of Cardiovascular Surgery, Xinqiao Affiliated Hospital, the Third Military Medical University of Chinese PLA.MATERIALS: Fifty purebred male SD rats of S.P.F. Grade, whose body mass was (220±22) g.METHODS: The experiment was completed in the Institute of Battle Surgical Research, the Third Military Medical University of Chinese PLA from August 2004 to October 2005. Fifty rats were randomly divided into 5groups: control group, sham operation-normal saline group, sham operationrosiglitazone group, myocardial hypertrophy-normal saline group and myocardial hypertrophy-rosiglitazone group, 10 rats per group. The rat model of myocardial hypertrophy induced by pressure overload was established with the method of coarctation of abdominal aorta. Rosiglitazone group: At the postoperative 4th week, the rats were injected intraperitoneally with the Normal saline group: At the postoperative 4th week, the rats were injected intraperitoneally with normal saline[1 mL/(kg.d)] for 1 week. At the postoperative 5th week, the indexes of myocardial hypertrophy and hemodynamics were determined. The contents of tumor necrosis factor-α, platelet activating factor and myeloperoxidase in the left ventricle muscle were determined with radioimmunosorbent technique. The expression of PPAR-γ mRNA was detected with RT-polymerase chain reaction. The activity of nuclear factor-κB was detected with EMSA.MAIN OUTCOME MEASURES: The indexes of hemodynamics, cardiac ventricle reconstitution and cardiac muscle in the rat models.RESULTS: Except 1 rat in the control group died of the external injury induced by biting after 3 weeks, 49 of 50 rats entered the result analysis.①After the coarctation of aorta, the contents of tumor necrosis factor-α, platelet activating factor and myeloperoxidase of hypertrophic myocardium in the myocardial hypertrophy-rosiglitazone group were lower significantly than those in the myocardial hypertrophy-normal saline group(P < 0.01-0.05), but they were still higher than those in the control group(P<0.01).②The expressions of PPAR-γ mRNA of myocardial tissue in both the myocardial hypertrophy-rosiglitazone and myocardial hypertrophy-normal saline groups were higher obviously than those in the control group(P<0.01), and those in the myocardial hypertrophy-rosiglitazone group were higher than those in the myocardial hypertrophy-normal saline group(P<0.01).③The activity of nuclear factor-κB combined with DNA in cardiac muscle cell in both the myocardial hypertrophy-normal saline and myocardial hypertrophy-rosiglitazone groups were higher obviously than those in the control group (P<0.01), and those in the myocardial hypertrophy-rosiglitazone group were lower obviously than those in the myocardial hypertrophy-normal saline group(P<0.01).CONCLUSION: The increasing of pressure load induces myocardial hy pertrophy. The activation of nuclear factor-κB in the tissue of hypertrophic myocardium is strengthened obviously. The expressions of tumor necrosis factor-α, platelet activating factor and myeloperoxidase in hypertrophic myocardium increase. This inflammatory reaction, which is strengthened obviously, can be restrained by rosiglitazone sodium that is the synthetical lig and for PPAR-γ.
2.Regulation of peroxisome proliferator activated receptors-? on inflammatory reaction in left ventricular hypertrophic rats

Objective To investigate the effects of rosiglitazone, an synthetic ligand of peroxisome proliferator activated receptor-?(PPAR-?) on the expression of proinflammatory factors in rats of myocardial hypertrophy. Methods The rat model of hypertrophy were established by coarctation of abdominal aorta in male SD rats, and the rats with myocardial hypertrophy were treated with rosiglitazone or physiological saline (n=10 for each therapy). Some rats underwent sham operation and were given rosiglitazone or physiological saline (n=10 for each therapy). Rosiglitazone at the dose of 4 mg?kg -1?d -1 or saline of 1 ml?kg -1?d -1 was given intraperitoneally for 1 week after postoperative 4 weeks. Another 9 rats served as control. Hemodynamics, ventricular hypertrophic index, TNF-?, PAF, MPD, PPAR-? and NF-?B were detected in all rats at postoperative 5 week. Results Significant hypertrophy was found in the left ventricle in rats undergoing the coarctation of abdominal aorta. Results Evaluation methods of traditional Chinese medicine EMR system should be the establishment form 3 part:establishing evaluation index system and evaluation standard, establishing qualitative and quantitative evaluation, and establishing evaluation scores system. Conclusion Traditional Chinese medicine EMR system application evaluation method provides a scientific basis for designing traditional Chinese medicine EMR system evaluation index system.

To explore the molecular mechanisms underlying the immunomodulatory effects of hypertonic sodium chloride and hetastarch 40 injection (HSH) resuscition on the development of lung inflammation in a hemorrhagic shock model, Sprague Dawley rats were subjected to hemorrhagic shock and intratracheal LPS,the so called two hit rodent model.The animals resuscitated by transfusion of either Ringer's lactate(RL) or a 6ml/kg volume of HSH were sacrificed at 1,2,4 hour after resuscitation,respectively. Surface expression of adhesion molecule CD11b on circulating polymorphonuclear (PMN) was measured by flow cytometry. Bronchoalveolar lavage fluid(BALF) neutrophil counts were determined.Lung tissue myeloperoxidase(MPO) activity reflecting total lung neutrophil infiltration was assessed by modified Bradly′ method. OBJECTIVETo explore the alterations of after depolarization and triggered activity in myocardiac muscle during progression of mouse aortic stenosis.
METHODSAfter the establishment of mouse aortic stenosis model, the heart was collected and intracellular potential was recorded using standard glass microelectrode. After the recording, the action potential (AP), early after depolarization (EAD), delayed after depolarization (DAD) in papillary muscles were analysed.
RESULTS(1) Compared with that of time-matched control mice, there were no changes of the action potoutial duration at 90% repolarization(APD90) in model mice at 2 and 5 weeks, whereas the APD90 was progressively prolonged at 9 and 13 weeks. (2) During the 30 minites of the experiment, spontaneous EADs and DADs were frequently present in medel mice at 9 and 13 weeks, were not in control and model mice at 2 and 5 weeks. (3) Compared with those of control group, EADs and DADs triggered by hypokalaemia or isoproterenol were increased significantly. The incidence rate of triggering EADs and DADs was progressively increased over time.
CONCLUSIONEAD, DAD and trigger activities increase gradually during the process of aortic stenosis, and it suggests that the myocardium electrical-instability is markedly increased in the insult hearts.

6.The dynamic alterations of electrocardiogram during progression of mouse cardiac hypertrophy.

OBJECTIVETo find the characteristic parameters from electrocardiogram (ECG) which is most related to pathological progress, surface ECG was performed in mice at 2, 5, 9 and 13 week post surgery.
METHODSElectrocardiogram recordings: The mice were anaesthetized with tribromoethanol (250 mg/kg, i.p.). Lead II surface ECG were acquired by using the Biopac System MP150 at a rate of 5 kHz.
RESULTS(1) No arrhythmia was observed in Sham and 2-week Band mice. Spontaneous arrhythmias were observed in ECG recordings, with an incidence of 15%, 28% and 63% in 5-, 9- and 13-week Band mice, respectively. Tachyarrhythmias, such as frequent premature ventricular extrasystole, bigeminy, trigeminy, and paroxysmal tachycardia were found. (2) Analysis of ECG recordings revealed a significant prolongation of QT and QTc intervals. Compared with age-matched Sham mice, the increment in QT and QTc intervals was 20.4%, 32.7%, 49.7%, 61.0% and 27.1%, 32.1%, 43.9%, 59.1% at 2, 5, 9 and 13 week, respectively (P < 0.01). (3) The characteristic change in electrocardiogram was on the J wave. The amplitude of J wave was upward in Sham mice, and it was significantly flattened or inverted in Band mice. (4) Except that RR interval at 2 week showed a slight decrease, there was no significant change in PR interval and RR interval in Band mice.
CONCLUSIONIn the present study, we found that the increase incidence of spontaneous arrhythmias, prologation of QT interval and changes in the amplitude of the J wave in the surface ECG during progression of mouse cardiac hypertrophy and failure, and the progressive pattern indicated that a gradual aggravation of the ventricular repolarization delay in this mouse model.

Methods A retrospective study, including 2903 normal nulliparous women who had delivered between July 1,2002, and December 31,2004, was conducted. Then the research development of competencies for physical therapist in China was represented.
10.Tumors originated from the inferior nasal turbinates: clinical features in 34 patients.

OBJECTIVE:
The purpose of this study was to analyzed the histopathologic spectrum and clinical features of the tumors originated from the inferior nasal turbinates.
METHOD:
Clinical data of 34 patients with tumours of the inferior nasal turbinates, treated from Jan. 1998 to Dec. 2012, were retrospectively reviewed. Of the 34 patients, 18 male and 16 female, aged from 25 years to 76 years (mean: 45.6yr).
RESULT:
(1) Pathology : Of the 34 patients, 23 (67.6%) were benign and 11 (32.4%) were malignant. Hemangiomas were the most frequent benign tumour accounting for 18/23 (78.3%) in benign tumour and for 52.9% in all the tumors originated from inferior nasal turbinates. Other benign tumours included inverted papilloma, squamous papillomas and extrapleural solitary neurofibroma tumor. The most common malignant tumour was non-Hodgkin's lymphoma accounting for 6/11 (54.5%) in malignant tumour and for 17.6% in all the tumors. Other malignant tumour included squamous cell carcinoma, melanoma and undifferentiated carcinoma. (2) Distribution: Most tumors (31/34) arose from unilateral turbinates. Hemangiomas is the commonest tumor originated from anterior part of the turbinate in 14 cases of 18 (77.8%). NHL and melanoma involved through turbinates. 3. Symtoms: Nasal obstruction was the most common symtom (25/34, 73.5%). Epistaxis and bloody nasal discharge were the chief complaint in hemangiomas.
CONCLUSION
1. The most common benign and malignant tumors of the inferior turbinate were hemangioma and NHL. The majority of the hemangiomas (14/18) arose from the anterior part of the inferior turbinate. NHL or melanoma often presented progressive, unilateral or bilateral diffuse enlargement of the inferior turbinate with poor vascular contractile reactivity to the ephedrine.
