1.Comparison of clinical efficacy of intra-articular ropivacaine mixed with dexmedetomidine or morphine for postoperative analgesia after arthroscopic knee surgery

Objective To compare the efficacy of intra?articular dexmedetomidine or morphine mixed with ropivacaine for postoperative analgesia after arthroscopic knee surgery?Methods Sixty ASA physical status I or II patients scheduled for elective arthroscopic knee surgery were randomly assigned into 3 equal groups using a random number table?The group R received 0?25% ropivacaine, and the group RD received 1 μg/kg dexmedetomidine and 0?25% ropivacaine,and the group RM received 2 mg morphine and 0?25% ropivacaine intraarticularly in a total volume of 20 ml?Visual analogue scales( VAS) scores when the patients actively flexed the operated knee to 90° were recorded at 2, 4, 8, 12, 18 and 24 h after surgery?The analgesic duration and consumption of fentanyl at 24 h after surgery were recorded?The complications such as bradycardia,hypotension, nausea,vomit,cutaneous pruritus and uroschesis were followed up?Results At 8 and 12 h after surgery,VAS scores were significantly lower in group RD ( ( 3?23 ± 0?45 ) points, ( 3?18 ± 0?47 ) points ) and RM ( ( 3?20 ±0?46) points,(3?13±0?45) points) than in group R((4?01±0?74) points,(3?93±0?71) points),and at 18 and 24 h after surgery,they were significantly higher in group R((3?85±0?62) points,(3?72±0?57) points) and RD((3?83±0?57) points,(3?71±0?55) points) than in group RM((3?07±0?43) points,(3?02±0?41) points),and there was significant difference(F between groups=124?65,P=0?021,F inner grouP=11?65,P=0?004,F across groups=67?65,P=0?024)?The analgesic duration was significantly different in three groups (F=45?78,P<0?001),and in group RD((668?3±133?4) min) and RM((1 412?8±275?67) min) was significantly longer than in group R(402?3±81?5) min,P<0?05),and the group RM was longer than the group RD( P<0?05)?The consumption of fentanyl at 24 h after surgery was significantly different in three groups ( F=34?47,P<0?001),and it was significantly lower in group RD((32?6±7?3) μg) and RM((12?8±3?7) μg) than in group R((151?3±28?5) μg,P<0?05),and the group RM was lower than the group RD(P<0?05)?No significant side effects were found?Conclusion Intra?articular dexmedetomidine or morphine all can improve the efficacy of ropivacaine for postoperative analgesia after arthroscopic knee surgery,while morphine is superior to dexmedetomidine.
2.STUDDZS OF SCREENING MICROORGANISMS DEGRADING LINEAR SODIUM ALKYLBENZENESULFONATE (LAS) AND ITS DEGRADINGCHARACTERISTICS

A bacterium which can effectively degrade LAS (Linear Sodium Alkylbenzenesulfonate) was isolated from washing powder manufacturing effluent and was preliminarily identified as Corynebacterium jeikeium GZ6. The bacterium can degrade LAS up to till 700 mg/L, and the optimum pH, temperature and concentration of LAS are 7.0, 30℃ and 400 mg/L, respectively. The biodegradation rate can reach 98.7% after 24 hours'cultivation in the suitable conditions. Experiments also showed that some heavy metal ions such as Hg2+ , Co2+ , Cd2+ can differently inhibit the degradation of LAS.
3.Quality control of Maca (Lepidium meyenii).

To control the quality of Maca, the quality standard was established in this study. According to the methods recorded in the Appendix of Chinese Pharmacopoeia (2010 Edition), the water, extract, total ash, acid insoluble substance, and heavy metals inspections in Lepidium meyenii were carried out. N-benzyl-9Z, 12Z-octadecadienamide in L. meyenii was identified by TLC, and it was determined by HPLC. The results showed that the N-benzyl-9Z, 12Z-octadecadienamide identification of TLC was a strong mark and specificity. In content determination experiment, the linearity of N-benzyl-9Z, 12Z-octadecadienamide was in the range of 0.01-2 microg (r = 0.9998), and the average recovery (n=9) was 99.27% (RSD 2.0%). The methods were simple, accurate, with good reproducibility. It is suitable for quality control L. meyenii.

4.Study on the use of model life tables methodology in birth defect's life expectancy estimation:the case of Down's syndrome

5.Comparison of 99Tcm-DMSA renal cortical scintigraphy and ultrasonography in diagnosing acute pyelonephritis in children

Objective To compare the diagnostic value of renal ultrasound scan (RUS) and 99Tcmdimercaptosuccinic acid (DMSA) renal scintigraphy in children with acute pyelonephritis (APN). Methods In all, 165 children with initial clinical diagnosis of APN, aged from 1.5 months to 11 yrs ( median 20 months), were included in the study, all of which were examined with RUS and DMSA renal scientigraphy. The diagnosis with DMSA renal scientigraphy results was taken as the standard reference to evaluate the diagnostic sensitivity and specificity of RUS. Results Of 99 out of all 330 kidneys that were found abnormal on DMSA renal scientigraphy, 31 were abnormal on RUS. Of the rest normal kidneys on DMSA scans renal scientigraphy, 4 were abnormal on RUS. Thus diagnostic sensitivity of RUS for APN was 31.3%(31/99) and specificity was 98.3% (227/231). Conclusions Although RUS provides with high diagnostic specificity for children with APN, its low sensitivity may underestimate the clinical evaluation of APN.More often than not, 99Tcm-DMSA renal scientigraphy is a clinical necesscity for the definite RUS diagnosis.
6.Possible effects of chemokine-like factor-like MARVEL transmembrane domain-containing family on antiphospholipid syndrome

Antiphospholipid syndrome (APS) is a systemic autoimmune disease defined by thrombotic or obstetrical events and persistent antiphospholipid antibodies (aPLs). Chemokine-like factor-like MARVEL transmembrane domain-containing family (CMTM) is widely expressed in the immune system and may closely related to APS. This review aimed to systematically summarize the possible effects of CMTM on APS. Publications were collected from PubMed and Web of Science databases up to August 2020. CKLF, CKLFSF, CMTM, antiphospholipid syndrome, immune cells, and immune molecules were used as search criteria. Immune cells, including neutrophil, dendritic cells (DCs), T-cells, B-cells, and inflammatory cytokines, play an important role in the development of APS. Chemokine-like factor 1 (CKLF1) has a chemotactic effect on many cells and can affect the expression of inflammatory cytokines and adhesion molecules through the nuclear factor- kB (NF- kB) pathway or mitogen-activated protein kinase (MARK) pathway. CKLF1 can participate in the maturation of DCs, T lymphocyte activation, and the activation of neutrophils through the MAPK pathway. CMTM1 may act on Annexin A2 by regulating Ca 2+ signaling. CMTM2 and CMTM6 are up-regulated in neutrophils of APS patients. Some CMTM family members influence the activation and accumulation of platelets. CMTM3 and CMTM7 are binding partners of B-cell linker protein (BLNK), thereby linking B cell receptor (BCR) and activating BLNK-mediated signal transduction in B cells. Moreover, CMTM3 and CMTM7 can act on DCs and B-1a cell development, respectively. CMTM may have potential effects on the development of APS by acting on immune cells and immune molecules. Thus, CMTM may act as a novel prognostic factor or immunomodulatory treatment option of APS.
7.Maintenance treatment of multiple myeloma-review.

Chemotherapy with traditional standard dose or autologous stem cell transplantation (ASCT) after chemotherapy with high dose have some remission effect for multiple myeloma, but relapse still exists. The maintenance treatment would prolonged the remission stage. These treatments included the use of alkylating agent, interferon, corticosteroids, thalidomide and so on. Every maintenance treatment has some advantages and some side effects. The truly effective maintenance treatment would not only minimize the harm of the disease, but also would prolong the overall survival, progression-free survival and the event-free survival. This article summarizes the current progress in the maintenance treatments for multiple myeloma.

8.Reconstitution dynamics of plasmacytoid and myeloid dendritic cell precursors after allogeneic HLA mismatched/ haploidentical hematopoietic stem cell transplantation.

OBJECTIVETo explore the characteristics and significance of reconstitution of peripheral blood dendritic cells (DCs) after HLA haploidentical hematopoietic stem cell transplantation (HSCT).
METHODSTwenty-one leukemia patients received HLA-haploidentical HSCT. Twelve healthy donors were as normal control. Peripheral blood DC subsets were analyzed by flow cytometry, and correlation between DC reconstitution and acute graft versus host disease (aGVHD) were measured.
RESULTSCompared with healthy subjects, leukemia patients had a significantly decreased proportion and absolute number of DC, myeloid DC (MDC) and MDC1 (0.13% vs 0.76%, 7.89 cells/microl vs 42.66 cells/microl, 0.05% vs 0.65%, 1.52 cells/microl vs 35.56 cells/microl, 0.03% vs 0.44%, 1.33 cells/microl vs 26.70 cells/microl, respectively (P < 0.05). MDC2 and plasmacytoid DC (PDC) were not statistically different between the two groups. The ratio of MDC/PDC was significantly reduced (0.05 vs 4.21) (P = 0.001). After transplantation, the absolute numbers of MDC1, MDC2 and PDC reached to pretransplant levels at day + 14(1.45, 0.32 and 1.60 cells/microl, respectively) (P > 0.05), but remained lower than those of healthy controls up to day + 100. The ratio of MDC/PDC had significantly different between that on + 14 d and pretransplant (1.40 vs 0.50) (P = 0. 024) and reached to healthy controls levels at day +30 (2.00 vs 4.21) (P = 0.602). Patients with higher PDC, DC levels and lower MDC/PDC ratio had lower cumulative incidences of grade II - NV aGVHD as compared with those with lower PDC, DC level and higher MDC/PDC ratio at day + 14(P = 0.006, 0.013 and 0.037, respectively).
CONCLUSIONIn HLA haploidentical HSCT recipients, DC reconstitution is a slow process, possibly contributing to the cumulative incidences of grade II - IV aGVHD.

9.PSO/ACO algorithm-based risk assessment of human neural tube defects in Heshun County, China.

OBJECTIVETo develop a new technique for assessing the risk of birth defects, which are a major cause of infant mortality and disability in many parts of the world.
METHODSThe region of interest in this study was Heshun County, the county in China with the highest rate of neural tube defects (NTDs). A hybrid particle swarm optimization/ant colony optimization (PSO/ACO) algorithm was used to quantify the probability of NTDs occurring at villages with no births. The hybrid PSO/ACO algorithm is a form of artificial intelligence adapted for hierarchical classification. It is a powerful technique for modeling complex problems involving impacts of causes.
RESULTSThe algorithm was easy to apply, with the accuracy of the results being 69.5%±7.02% at the 95% confidence level.
CONCLUSIONThe proposed method is simple to apply, has acceptable fault tolerance, and greatly enhances the accuracy of calculations.

10.Investigation of plasmacytoid dendritic cell reconstitution and its relationship with cGVHD and relapse after haploidentical hematopoietic stem cell transplantation.

To investigate the characteristics and significance of reconstitution of peripheral blood plasmacytoid dendritic cell (PDC) precursors after allogeneic human leukocyte antigen mismatched/haploidentical hematopoietic stem cell transplantation, and its relationship with chronic graft versus host disease (cGVHD) and relapse, 19 patients with leukemia were enrolled for this study. Peripheral blood dendritic cell (DC) subsets of patients and healthy controls were detected by flow cytometry, and the correlations between reconstitution of DC and cGVHD, relapse were analyzed. The results showed that compared with healthy subjects, patients with leukemia had a significantly decreased proportion and absolute number of myeloid dendritic cell (MDC), MDC1, DC and the ratio of MDC/PDC (P<0.05). There were not statistically different in MDC2 and PDC between patients and healthy subjects. After transplant, all the proportion of WBC and absolute numbers of DC reached to healthy controls levels at 9 months (P>0.05), besides the proportion of PDC which reached to healthy controls levels at 1 year (P=0.494). Compared with levels before relapse, the proportions of MDC1, MDC, DC and the ratio of MDC/PDC were lower, but proportions of MDC2 and PDC were slightly higher after relapse. Patients with a 'high' PDC recovery profile had an improved cumulative incidence of cGVHD in contrast to patients with a 'low' PDC recovery profile on day 120 after transplantation (P=0.007). It is concluded that compared with healthy subjects, de novo leukemia patients have a significantly decreased proportion and absolute number of DC and the ratio of MDC/PDC before haploidentical hematopoietic stem cell transplantation; while ratio of MDC/PDC can be normalized with relative rapidity, the proportions of all DC subsets reached to normal levels on the whole at 9 months after transplantation, and also recovery level of DCs is correlated with occurrence of cGVHD and relapse.
