Objective To study the imaging features regarding perineural spread of tumor along the trigeminal nerve in malignant head and neck tumors, investigate its usefulness in improving diagnostic accuracy and palnning for clinical treatment. Methods Images in 9 patients with clinical or radiological findings suggestive of perineural spread along trigeminal nerve were retrospectively studied. Results Among the 9 patients, 6 were adenoid cystic carcinoma of the hard palate (n=3), maxillary sinus (n=1), parotid gland (n=1) and buccal space (n=1), respectively. Two were nasopharyngeal carcinoma and 1 was squamous carcinoma of the maxillary sinus. Enlargement and fat effacement of greater palatine foramen and pterygopalatine fossa distant from primary diseases were seen in all the 3 patients with adenoid cystic carcinoma of the hard palate and 1 with squamous carcinoma of maxillary sinus, furthermore, enlargement of foramen ovale and invasion of meckal cave was seen in 1 case. Erosion of infraorbital foramen and enlargement of the pterygopalatine fossa was seen in 1 patient with adenoid cystic carcinoma of maxillary sinus. Enlargement of pterygopalsatine fossa and foramen rotundum and invasion of the Meckal cave were seen in 1 patient with adenoid cystic carcinoma of buccal space. Perineural spread along auriculotemporal nerve and intracranial invasion through foramen ovale were seen in 1 patient with adenoid cystic carcinoma of parotid gland. Enhancement of mandibular nerve was seen in 2 patients with nasopharyngeal carcinoma. Conclusion Perineural tumor spread along trigeminal nerve can be seen in malignant head and neck tumors, and knowledge of the anatomy of trigeminal nerve and its surrounding structures is important for correct diagnosis.

BACKGROUND:Primary vascular endothelial cels are mostly harvested through aorta endothelial cel cultures and micro-artery endothelial cel cultures using enzyme digestion and tissue adhesion methods, and the quality and purity of harvested cels cannot meet the need for current scientific research.
OBJECTIVE:To investigate an improved extraction of primary vascular endothelial cels and the relevant identification method.
METHODS: A segment of rabbit aorta was cut to culture vascular endothelial cels using the improved extraction method in group A or using adhesion method in group B. In the group A, the vascular intima was striped out with microsurgical instruments, and digested enzymaticaly to acquire single primary cels folowed by culture in endothelial cel culture medium. In the group B, the whole vascular intima was adhered to the culture dish that was incubated in a 5%CO2, 37℃ incubator for 1 hour. Cel pelets in the two groups were culturedin vitro. Cel morphology was observed using a microscope; immunohistochemical staining was used to detect CD31, VIII factor and Vimentin protein for identification of vascular endothelial cels.
RESULTS AND CONCLUSION:The purity and number of vascular endothelial cels extracted by the improved method were higher than those by the adhesion method. Immunohistochemical findings showed positive expression of CD31 and VIII factor, but negative expression of Vimentin. These findings indicate that the improved extraction method can obtain more vascular endothelial cels with higher purity, which is of strong operability and practicality.

Purpose To evaluate 1H proton magnetic resonance spectroscopy (1H-MRS) in detecting traumatic brain injury (TBI) and its metabolic changes during the initial two weeks after trauma using rabbit modes. Materials and Methods Fifteen Chinese rabbits were randomly divided into sham control group (n=5) and TBI group (n=10), 1H-MRS was performed 1 hour, 6 hours, 24 hours and 2 days, 7 days, 14 days after trauma, the concentration of N-acetylaspartate (NAA), creatine (Cr), choline-compound (Cho), NAA/Cr and Cho/Cr in each group was evaluated. At 6 hours, 24 hours and 168 hours after injury, 1 rabbit was slaughtered after abdominal anesthesia at each time point, the sample was ifxed with left heart catheterization perfusion after death, brain tissue was obtained, blocked, dehydrated into frozen section, HE staining was used for observation. Results Compared with the control group, NAA/Cr ratio in the trauma region decreased by 29%1 hour after trauma, with maximal reduction of 40% (at 24 hours) before increasing slightly, the NAA/Cr ratio returned to control level gradually until 168 hours later. The Cho/Cr ratio decreased by 16% 1 hour after trauma, with maximal reduction of 30%(at 6 hours) and increased gradually until finally su rpassed control level (168 hours) for 20% and then became stable. NAA/Cr ratio and Cho/Cr ratio showed statistically significant changes between each observing time point (P<0.05). Conclusion 1H-MRS can be used for dynamic detection of cerebral metabolism without injury, and is superior to MRI in detecting early abnormality of the brain, with NAA/Cr as the most sensitive parameter. Detecting the changes of NAA/Cr, Cho/Cr ratios in the trauma region can be used as a guide for assessment of the clinical treatment effectiveness of TBI.

Objective To investigate the pathogenesis of diagnosis and treatment for delayed gastric emptying(DGE) after pancreaticoduodenectomy(PD).Methods The clinical data of 226 patients admitted between 1983 and 2001 undergoing PD were retrospectively analysed. Results In 226 PD cases DGE was observed in 62 patients(27.4%). Patients with DGE had longer duration of nasogastric tube drainage(21 days vs. 5.6 days). All patients were treated by non-operative methods. The average recovery time was 21 days. Conclusions It is emphasized that DGE has been associated with several factors, including preoperative status of diabetes, bowel obstruction, and jaundice, and postoperative pancreatic fistula, intra-abdominal abscess. Octreotide administration had no relation with the occurrence of DGE.

Objective To investigate the CT features of the invasion of sublingual space by malignant oropharyngeal tumors in order to provide more accurate information for clinical treatment. Methods Fifty-eight cases of pathologically proven malignant oropharyngeal tumors were collected and retrospectively analyzed. Results Among all the cases, invasion of sublingual space by malignant oropharyngeal tumors could be seen in 14 cases, of which, 7 cases got access to sublingual space through tongue base, 3 cases through parapharyngeal space, 2 cases through pterygomandibular raphe, 2 cases through uncertain routes. Invasion of sublingual space manifested on CT scan as obliteration of fat plane in sublingual space and involvement of the sublingual vessels in the space. Conclusion Malignant oropharyngeal tumors can invade the adjacent sublingual space via tongue base, pterygomandibular raphe, and parapharyngeal space. The invasion of sublingual space by malignant oropharyngeal tumors manifests in CT as effacement of sublingual fat plane and envelopment of hyoid artery.
