Objective To probe into a doctor-seeking medical management model for China's urban hypertension patients under medical insurance, and enhance the level and quality of medical insurance management. Methods Hypertension patients were surveyed with a structured questionnaire, and the staff at community health centers and medical insurance management centers received qualitative interview. Results The hypertension patients are found to see doctors mostly at community health centers and tertiary hospitals; complaints among hypertension patients for existing medical insurance vary with their areas, mostly targeting on complicated reimbursement procedure, high threshold of medical insurance payment and low level of reimbursement. Recommendation Experiment with the single-disease payment for hypertension, with the total prepayment in per capita and per disease; greater efforts in hypertension control in communities, with more favorable medical insurance policies for communities;elevated collaboration with medical institutions at all levels, and building and standardizing the management information network for hypertension patients.

BACKGROUND:The strength of al-ceramic crowns is affected by many factors. At present there are many studies concerning the neck edge shape, cutting thickness, binder and convergence angle, but little has been reported on the effects of different occlusions on al-ceramic crowns. OBJECTIVE:To explore the maxilary central incisor al-ceramic crowns in different occlusal states by finite element analysis. METHODS: Three-dimensional finite element models of the maxilary central incisor al-ceramic crowns were established. Empress II and In-Ceram Zirconia were selected. The normal overbite position and deep overbite position were applied to force a load of 230 N, with the long axis of the tooth at a 45° angle. Distribution of inner stress and edge stress was analyzed at different occlusal states. RESULTS AND CONCLUSION:(1) The stress distribution of al-ceramic crowns under different occlusion relationship was different. The stress peaks of cementum, alveolar bone and periodontium in the deep overbite were lower than those in the normal overbite. The stress peaks of al-ceramic crowns and binder were higher than in the deep overbite than the normal overbite. The stress peak of the dentin in the Empress II group was higher in the deep overbite than in the normal overbite. The stress peak of the dentin in the In-Ceram Zirconia group was lower in the deep overbite than in the normal overbite. Different al-ceramic crowns had no influence on the stress distribution, but in the deep overbite, the stress was mainly concentrated in the occlusal contact area, and in the normal overbite, the stress was mainly concentrated in the occlusal contact area and at the labial cervical margin. (2) The stress distribution of different al-ceramic crowns under the same occlusal state was different. In the normal overbite, different al-ceramic crowns had no evident influence on the stress peak, and in the deep overbite, there was a certain effect of different al-ceramic crowns on the stress peak, but there was no significant difference. The stress peaks of blinder and dentin were obviously affected by al-ceramic crowns. The stress peaks of blinder and dentin in Empress II group were higher than those in the In-Ceram Zirconia group.

Objective To investigate the correlation between vertical infection of hepatitis B virus(HBV) and Hofbauer cell in placenta. Methods Placental structural change, the correlation between HBV infection and Hofbauer cell in 14 vertical transmission cases(HBsAg and HBV-DNA seropositive in both mothers and their neonates),62 non-vertical transmission cases(HBsAg and HBV-DNA seropositive in mothers, but HBsAg and HBV-DNA seronegative in their neonates) and 10 controls(HBsAg and HBV-DNA seronegative in both mothers and their neonates) were determined by inmumohistochemical techniques and photomicroscopy. Results 1.necrosis,edema,villi capillary sclerosis, intravillifibrosis, fibrinoial sendimentation, morphokarrioleukocyet, lymphacyte, granulocyte were observed in placentas both vertical transmission cases and non-vertical transmission cases.2.Placental HBsAg postive rate in vertical transmission cases was 100%,but 58% in non-vertical transmission cases. There was significant difference in two groups(P=(0.013)).The positive sign was observed in trophoblast cell,Hofbauer cell and endothelial cell.3.Total Hobauer cell and ones combined with HBV in placentas of vertical transmission cases obviously increased(P

Apoptosis play an important role in the process of secondary spinal cord injury(SCI). Bcl-2 is a multigene family, which have fifteen members. Bcl-2 and Bcl-xL can inhibit apoptosis, but Bax, Bad, Bak, and Bcl-xS can promote apoptosis. Bcl-2 and Bax is the most representative gene in Bcl-2 gene family, Bcl-2/Bax determine cells whether goes to die. In this paper, we reviewed the latest developments in Bcl-2/Bax adjustment for SCI.

Objective To preliminary screen serum microRNA which was related to class Ⅳ and class Ⅳ+Ⅴ lupus nephritis (LN)by using microRNA array technology, and explore the role of serum microRNA in renal pathological classification of lupus nephritis. Methods Eight serum samples of lupus nephritis patients from biological specimen library in our hospital were enrolled. Experimental group was composed of 4 class Ⅳ cases and 4 class Ⅳ+Ⅴ cases, control group was composed of 4 healthy persons. MicroRNA array was used to screen the differentially expressing microRNA.Results There were part of same miRNA in class Ⅳand classⅣ+Ⅴ LN,among which expression of 22 miRNA were significantly different,29 miRNA of classⅣ was unique,3 miRNA of class Ⅳ+Ⅴ was unique. Conclusion Serum miRNA has a potential to serve as a biomarker to identify class Ⅳ and classⅣ+Ⅴlupus nephritis.

Objective To explore the relationship between cough variant asthma (CVA) and mycoplasma pneumoniae (MP) infection. Methods Fifty children with CVA were chosen as the experimental group at random, and 50 children with acute upper respiratory infection, who went to the hospital in the same time and with similar age, were chosen as control group. The MP-IgM of children in both groups were tested by the granule agglutinating method. Results Significant difference (? 2=9.013 P

To survey the serum uric acid (SUA) levels and associated risk factors of hyperuricemia among youth and middle-aged residents in Guangzhou.A total of 1420 subjects, aged from 20 to 60 years, receiving health check-up at our hospital in 2010 were enrolled. The total prevalence of hyperuricemia was 22.04%,32.01% in males and 14.07% in females. The average SUA was (388 ±78) μmol/L in males and (288 ± 63) μ mol/L in females. The prevalence of hyperuricemia in males was 30.11% before 40 years of age and 33.81% between 40 and 60 years of age.The average level of SUA in males was significantly higher than that of females.logistic regression analysis showed that BUN,body mass index (BMI) and hypertriglyceridemia were the independent risk factors of disease while HDL-C and gender (females) the protective factors.

Objective To analyze the clinical characteristics of macrophage activation syndrome (MAS). Methods Clinical data was analyzed in 9 children who had been diagnosed as MAS in our hospital from Jan, 2003 to Aug, 2006. Results Seven children were boys, 2 children were girls, aged 5 months to 12 years. Clinical manifestations included long-term fever, hemophagocytic cell in bone marrow, anemia, arthritis, enlargement of lymph nodes, enlargement of liver and spleen, liver dysfunction, abnormal fat metabolism. Two cases had acute respiratory, distress syndrome(ARDS), 2 cases were complicated with multi- pie organ failure(MOF), two patients died. Glucocorticoid combined with immunosuppressive therapy were effective, HP(Plasma Exchange)was applied in one severe case and was shown to he effective. Conclusion MAS is a serious complication of JIA, especially in systemic-onset juvenile idiopathic arthritis. It is very im- portant to recognize and treat MAS earlier.
