1.Progress in diagnosis and treatment of chronic kidney disease-mineral and bone metabolism abnormalities

Chronic kidney disease-mineral and bone disorder (CKD-MBD) refers to a series of clinical symptoms and biochemical and imaging abnormalities caused by minerals and calcium phosphorus metabolic disorder,which is associated with chronic kidney disease (CKD).Since 2006 ,the concept of CKD-MBD was put forward for the first time,doctors are increasingly paying more attention to it.This review introduces the epidemiological characteristics, clinical manifestation, examination methods, diagnostic criteria and the research progress of treatment and prevention of CKD-MBD.
2.Effect of indwelling double-J ureteral stents before flexible ureteroscope lithotrity

Objective To investigate the effect of indwelling double-J ureteral stents before flexible ureteroscope lithotrity in terms of operation time, postoperative hospitalization time and operation effect. Methods All clinical data of flexible ureteroscope lithotrity from 2009 to 2013 were reviewed. Operation time, postoperative hospitalization time and recurrence rate of calculus of patients who received operation directly and patients who received indwelling of double-J ureteral stents 2 weeks before operation were observed and compared. Results Op-eration time, postoperative hospitalization time, and recurrence rate of calculus of patients who received indwelling of double-J ureteral stents 2 weeks before receiving operation were obviously lower than patients who received operation directly. Conclusion Operation time, postoper-ative hospitalization time and recurrence rate of calculus can be decreased by indwelling double-J ureteral stents 2 weeks before operation.
3.Effect of multiple treatments on cystospasm after transurethral resection of prostate

Objective To investigate the effect of multiple treatments on cystospasm after transurethral resection of prostate(TURP). Methods From 2006 to 2013, 663 patients who had received TURP for BPH were civided into 6 groups:2 were treated by kieselgel or lac-toprene urethral catheter,defined as catheter observe group. 2 were treated by warmed or ordinary temperature douche,defined as temperature observe group. 2 were treated by dicaine mixed or normal douche,defined as mixed douche observe group. The incidence rates and duration of cystospasm and visual analogue scores of pain were observed and the diversities were measured by statistics within each pair groups. Results The incidence rates and duration of cystospasm and visual analogue scores of pain of groups treated by kieselgel urethral catheter,warmed douche and dicaine mixed douche were obviously lower than the groups treated by lactoprene urethral catheter,ordinary temperature douche and normal douche. The kieselgel urethral catheter and warmed douche decreasing the irritation on mucous membrane of bladder,the dicaine decreasing the sensibility of bladder nerves may be the mechanism. Conclusion The incidence rates and duration of cystospasm and visual analogue scores of pain can be obviously decreased by treatments of kieselgel urethral catheter,warmed douche and dicaine mixed douche.
5.Dermal scaffold-experimental study on preparing and transplantation of spongiform collagen membrane

Objective: To prepare a collagen rnembrane and to investigate its feasibi1ity as a derma1 scaffold. Metbods:Pig hide collagen was dispersed in 0. 5 mol/L acetic acid, co-precipitated with chordroitin-6-sulfate and lyophilized for 48 h toproduce a highly porous membrane. Cross-linking was in 0. 25% glutaraldehyde for 24 h. The membrane was implanted be-neath the flaps on the dorsal sites of SD rats. After implantation, biopsies were obtained periodically to study the tissue com-patibility, vascularity and stability. Results: The average pore size of the spongiform collagen membrane was 100 pm. Sub-dermal implantation of it showed that the inflammation was minimal, with no evidence of an acute rejective reaction. Conclu-siou: The spongiform collagen membrane prepared is easy to secure and produce, it has satisfied tissue compatibility, abilityof vascularity and optimum dermal scaffold structure.
6.CONSTRUCTION AND GROWTH ABILITY STUDY OF A COMPOSITE SKIN COMPOSED OF KERATINOCYTES AND ACELLULAR DERMAL MATRIX

To investigate the possibility of constructing composite skin, keratinocytes were cultivated in vitro on the epidermal surface of cell free dermis prepared from pig skin.Keratinocytes grown on the dermal matrix were released at selected time points, followed by determining the proliferative capacity with cell number quantity and cell proliferation test. Cells attaching to the dermal matrix after it were seeded for 1 and 2 weeks were observed with histological section HE staining and electron microscopy scanning. Results showed that the number of keratinocytes was markedly increased with culture time. They maintained their proliferative potential after they were seeded on acellular xeno dermal matrix and reached a confluent monolayer or 3 to 6 layers at the 1st and 2nd week after seeding. The data showed that a living composite skin combined with keratinocytes and acellular dermal matrix could be successfully prepared in vitro.
7.EXPERIMENTAL STUDY OF TRANSPLANTATION AND THE FATE OF COMPOSITE SKIN COMPRISING MIXED KERATINOCYTES SEEDED ON ACELLULAR DERMAL MATRIX

To investigate the fate of composite skin comprising mixed keratinocytes seeded on acellular dermal matrix (ADM) after its transplantation to the wound. Newborn BALB/c and human keratinocytes were mixed in various ratios, seeded on the surface of ADM, and cocultured. The composite skin substitute were then grafted onto the full thickness skin wounds in BALB/c mice. The fate of human keratinocytes was observed. The results showed that the composite skin substitutes could close the full thickness wounds in BALB/c mice. Human keratinocytes were mainly located in the upper layer of the epidermis, and were gradually replaced by BALB/c keratinocytes. This indicated that the mixed culture of keratinocytes of two different species on ADM could close full thickness wounds, having the advantages such as saving the donor skin and shortening the culture time in vitro .
8.RECONSTRUCTION OF A COMPOSITE SKIN SUBSTITUTE WITH COLLAGEN SPONGE AS DERMAL SCAFFOLD

To reconstruct a composite skin substitute composed of keratinocytes and collagen sponge as dermal scaffold. Collagen was extracted from fresh porcine skin. After being mixed with chondroitin sulfate, it was frozen and dried under vacuum to form a spongy membrane. Keratinocytes were separated from the foreskin with the routine method, and they seeded on the collagen membrane. The keratocytes which were carried by the collagen sponge were cultured, and the growth and proliferation were observed. The result showed that keratinocytes could grow and proliferate to form a confluent layer after 2~3 weeks. It indicated the the collagen sponge membrane showed no toxicity to human epidermal cells, and it could be used as a dermal scaffold in the construction of a composite skin substitute.
9.BIOACTIVITY AND TRANSPLANTATION OF EGF GENE TRANSFECTED KERATINOCYTE

To investigate the expression of epidermal growth factor (EGF) of EGF gene transfected keratinocytes in vivo and in vitro after grafting. EGF levels in the supernatant of the culture media of EGF gene transfected keratinocytes cultured for different lengths of time and different passages were determined with ELISA method. Then, the gene transfected keratinocytes were seeded on the surface of acellular dermal matrix, After culture, the composite skin substitutes were grafted onto the full thickness wounds in nude mice. Specimens were harvested at intervals after grafting and stained for EGF with immunohistochemistry. The results showed that keratinocytes transfected with EGF gene secreted EGF, which was detected in the supernatant of the culture, for 5 passages. Immunohistochemical staining method showed that EGF was expressed in the newly generated epidermis 1~3 weeks after grafting of the composite skin substitute. The data showed that gene transfected keratinocytes could express EGF stably in vivo and in vitro , which would be of benefit to the construction of the tissue engineering skin.
10.Treatment of chronic heart failure of Xin-Shen yang deficiency, interior retention of water-fluid syndrome by external application of Zhuangshenling recipe combined with western medicine: a clinical study.

OBJECTIVETo observe the effect of external application of Zhuangshenling Recipe (ZR) combined with Western medicine (WM) on the heart function of chronic heart failure (CHF) patients of Xin-Shen yang deficiency, interior retention of water-fluid syndrome (XSYDIRWFS).
METHODSTotally 140 CHF patients of XSYDIRWFS were randomly assigned to two groups, the treatment group and the control group, 70 in each group. All patients received WM therapy. Those in the treatment group were applied with ZR at Xinshu (BL15) and Shenshu (BL23), while those in the control group were applied with placebos at Xinshu (BL15) and Shenshu (BL23). The therapeutic course for all was 12 weeks. The integrals of TCM syndrome, grading of cardiac function, brain natriuretic polypeptide (BNP), and 6 min walking distance were observed before and after treatment.
RESULTSAfter twelve weeks of treatment, the effective rate of improved grading of cardiac function, the total effective rate of TCM syndrome efficacy, and the BNP level were obviously better in the treatment group (P < 0.05). There was no statistical difference in 6 min walking distance between the two groups (P > 0.05).
CONCLUSIONExternal application of ZR combined with WM could improve the heart function of CHF patients of XSYDIRWFS.
