2.Value of FDG PET-CT in outcome assessment and prognostic evaluation for recurrent nasopharyngeal carcinoma

Objective To investigate the value of FDG PET-CT in the outcome assessment and prognostic evaluation for recurrent nasopharyngeal carcinoma (rNPC).Methods From January 2008 to December 2013,92 rNPC patients were treated in our center,who were histologically or radiologically diagnosed and re-staged according to the 2008 clinical staging system for nasopharyngeal carcinoma in China.The numbers of patients in stage Ⅰ,stage Ⅱ,stage Ⅲ,and stage Ⅳ were 8,11,39,and 34,respectively.According to the recurrent T stage (rT),the numbers of patients in rT1,rT2,rT3,and rT4 were 10,11,38,and 33,respectively.Twenty-eight patients had recurrence in the neck lymph nodes.All patients underwent pretreatment FDG PET-CT for the whole body or head/neck,and treated by radiotherapy with or without chemotherapy.The relationship of maximum standard uptake value (SUVmax) and clinical factors with clinical outcomes was analyzed.The Kaplan-Meier method was used to calculate overall survival (OS),disease-free survival (DFS),local recurrence-free survival (LRFS),regional recurrence-free survival (RRFS),and distant metastasis-free survival (DMFS).The log-rank test was used for survival difference analysis and univariate prognostic analysis.The Cox model was used for multivariate prognostic analysis.Results The 3-year OS,DFS,LRFS,RRFS,and DMFS were 33.6%,32.1%,32.8%,31.8%,and 33.7%,respectively.The median SUVmax was 8.35 (2.7-21.5).The SUVmax of 7.0 was taken as the optimal cut-off value for all patients.Patients with SUVmax ≤7.0 had a significantly higher 3-year OS rate than those with SUVm ax >7.0 (42.0% vs.28.3%,P=0.019).The univariate analysis revealed that patient age,SUV and rN were significantly associated with OS (P=0.023,0.019,and 0.002).The multivariate analysis showed that SUVmax and rN were significant influencing factors for OS,DFS,and DMFS (HR=1.68,P=0.045 and HR=2.23,P=0.003;HR=1.67,P=0.042 and HR=2.39,P=0.001;HR=1.77,P=0.025 and HR=2.40,P=0.001).Conclusions SUVmax may be one of the useful prognostic factors for OS,DFS,and DMFS in rNPC patients.
3.Clinical analysis of 35 cases of craniocerebral injury patients with intracranial infection *

Objective To investigate the characteristics of pathogenic bacteria/the drug resistance/the correlated risk fators/the prophylaxis control strategy of the severe craniocerebral injury patients combined with intracranial infection .Methods The clinical data of 35 craniocerebral injury patients with intracranial infection were retrospectively analyzed .Results 35 patients′cerebrospinal fluid were separated and 54 pathogenic bacterium had been cultured ,including G+ bacterium(61 .11% ) ,the G - bacteria(33 .33% ) , fungi(5 .56% ) .The pathogenic bacteria showed a higher resistance .The single factor analysis found that the wound itself exists in-fection factors ,the postoperative drainage of incision ,liquorrhoea ,with other basic diseases ,surgery lasted for a long time (>4 h) reoperative ,surgery is placed foreign body is severe craniocerebral trauma combined with intracranial infection were the main rele-vant factors .The total effective rate was 62 .86% ,and the mortality was 11 .43% by the positive therapy .Conclusion G+ bacteria were the main pathogenic bacterium in the severe craniocerebral injury patients combined with intracranial infection .The iatrogenic factors leaded to the increase of the proportion of intracranial infection and the resistance increased year after year .The clinical in-travenous antibiotics combined intrathecal injections were beneficial to control intracranial infection ,shorten the course of treatment and enhance the curative effect .
4.Influence of induction chemotherapy on target volume and dosimetry of intensity modulated radiotherapy for locally advanced nasopharyngeal carcinoma

As induction chemotherapy goes on,target volume,dose distribution in the surrounding organs at risk (OARs),and target dose conformity all change.Therefore,the question is how to develop reasonable radiotherapy plans in clinical practice.Induction chemotherapy followed by radiotherapy is commonly used around the world,but it is recommended to delineate the target volume based on the gross tumor volume before induction chemotherapy and not to reduce the dose.This point of view lacks the basis of evidence-based medicine.The experts and scholars in China clarify the advantages of radiotherapy plans after induction chemotherapy from the aspects of reducing the target volume,reducing the volume of high-dose region in the target volume,increasing the uniform dose coverage in target volume,reducing dose to OARs,and increasing dose conformity.However,at present,there are no objective data on its long-term efficacy and benefit.Besides,no consensus has been reached on how to delineate the target volume and determine the dose distribution after induction chemotherapy,and further studies are needed.
5.Application of 18F-FDG PET-CT in nasopharyngeal carcinoma

18F-fluorodeoxyglucose positron emission tomography-computed tomography (18F-FDG PET-CT) is the integration of functional imaging and anatomic information,which is found to be particularly valuable in TNM staging,tumor volume delineating,post-treatment assessment,identification of recurrent and residual nasopharyngeal carcinoma (NPC).The combination of 18F-FDG PET-CT with other image technologies,different tracer agents,and specific molecular biomarkers can improve the application value of 18F-FDG PET-CT in NPC.
6.The relationship between the reexpression of nestin protein and the expression of bFGF protein in cerebral ischemia

Objective To study the relationship between the reexpression of nestin protein and the expression of basic fibroblast growth factor (bFGF) protein in cerebral ischemia. Methods By using rat permanent cerebral ischemia as a model, immunohistochemical staining was used to observe the expression of nestin protein and the bFGF protein 1,3,7,14 and 28 days after cerebral ischemia. Results The number of nestin cells around the ischemic area increased 1 to 3 days after cerebral ischemia,but it decreased 7 days after cerebral ischemia. However, the number of bFGF cells around the ischemic area increased obviously 14 days after cerebral ischemia, but it decreased 28 days after cerebral ischemia. Conclusion Reexpression of nestin protein around ischemic cerebral tissues does not need the nourishment and susteinance of bFGF protein; nestin cells around ischemic foci are originated from astrocytes which replay the course of fetation.
7.Research on multiplication of the H9N2 subtype avian influenza virus in large-scale microcarrier-based MDCK cell culture system

To explore the regularity for the multiplication of avian influenza virus subtype H9N2 in large-scale microcarrier-based MDCK cell culture system, and to determine the optimal proliferation conditions. H9N2 subtype of avian influenza virus was inoculated into the MDCK cell growing on 24 well plate, and the HA titers of virus at different time were detected in the conditions of different infectious doses,different concentrations of TPCK- trypsin and different pH. The optimal conditions were determined. Then the H9N2 subtype avian influenza virus was grown in microcarrier-based MDCK cell in 250mL and 5L roller bottles. It was demonstrated that high viruse yield with a hemagglutination unit of 9 log2(1:512) could be obtained under the optimal conditions of multiplication . The result indicated the H9N2 subtype avian influenza virus could be produced in microcarrier-based MDCK cell in a large-scale culture system with a high virus yield and demonstrates the feasibility of the development of mammalian cell-based in influenza vaccine in microcarrier culture systems.

The biotechnology of glycolipid fermented b y a bacillus coagulans was studied and the fermentation pro cess in 10L bioreactor was conducted.Suitable medium contained 6% bean oil as ca rbon source,3 5g/L NaNO 3 as nitrogen source,0 75g/L yeast extract and some i no rganic salts.The fermentation temperature of 30℃,initial pH of 8 5,strring rev olution of 150~240r/min and fermentation period of 96h proved to be optimal.The yield of glycolipid in 10L bioreactor was 7 073g/L.
9.Analysis of Therapeutic Effect of Zoledronic Acid Combined with Chemotherapy in Patients with Cancer and Bone Metastases

Objective To evaluate therapeutic effect of zoledronic acid combined with chemotherapy in patients with cancer and bone metastases. Methods 60 patients with cancer and skeletal me-tastases were devided randomly into tow groups, 30 patients received zoledronic acid combined with chemotherapy, 30 patients only received chemotherapy. Cheotherapeutic program of tow groups were same. Results Response rate of bone pain relief was 83. 3% in zoledronic acid combined with chemotherapeutic group and 56.7% in chemotherapy alone group. Response rate of skeletal metastases was 53. 3% in zoledronic acid combined with chemotherapy group and 20. 0% in chemotherapy alone group. Response rate of movement capacity improvement was 73. 3% in zoledronic acid combined with chemotherapeutic group and 30. 0% in chemotherapy alone group. 10. The Curative Effect of Shugan Granule in the Treatment of Post-stroke Depression and Effect on BDNF and Inflammatory Factors

OBJECTIVE: To investigate the curative effect of Shugan granule in the treatment of post-stroke depression (PSD) and effect on brain devived neurotrophic factor(BDNF) and inflammatory factors. METHODS: Eighty Patients with PSD were randomly divided into control group and study group, 80 cases in each group. Besides routine treatment, control group received citalopram hydrobromide 20 mg•d-1. Study group received Shugan granule 1 bags•time-1, twice a day based on control group.8 weeks for a course of treatment. HAMD scores, BI index, BDNF and inflammatory factors levels were compared. RESULTS: The total effective rate in study group was significantly higher than that in control group[71 cases,88.75% vs 61 cases, 76.25%, χ2=4.329, P<0.05]. Repeated measurement data analysis of variance showed, the overall scores of HAMD in study group were significantly lower than those in control group [Fgroup=36.541, P<0.05]. The HAMD scores of the two groups decreased with the extension of treatment time [Ftime=668.574, P<0.05].The reduction extent of HAMD score in study group was significantly higher than that in control group[Fgroup×time=18.495, P<0.05]. BI indexes and BDNF levels in study group were significantly higher than those in control group [Fgroup=48.147, 41.561, P<0.05]. BI indexes and BDNF levels of two groups increased with the extension of treatment time [Ftime=316.586, 595.864, P<0.05]. The increasing extent of BI indexes and BDNF levels in study group was significantly higher than that in control group [Fgroup×time=22.849, 10.515, P<0.05]. The overall levels of TNF-α, IL-6 and hs-CRP in study group were significantly lower than those in control group [Fgroup=117.849, 85.168, 66.763, P<0.05]. TNF-α, IL-6 and hs-CRP levels of two groups decreased with the extension of treatment time [Ftime=864.584, 697.717, 1 037.167, P<0.05]. The reduction extent of TNF-α, IL-6 and hs-CRP levels in study group was significantly higher than that in control group[Fgroup×time=31.271, 26.174, 17.585, P<0.05]. There was no significant difference in adverse reactions between the two groups [9 cases, 11.25% vs 15 cases, 18.75%, χ2=1.765, P>0.05]. CONCLUSION: On the basis of conventional western medicine treatment, Shugan granules can improve the degree of depression and reduce the level of inflammatory factors in patients with PSD.
