1.Expert consensus on evaluation index system construction for new traditional Chinese medicine(TCM) from TCM clinical practice in medical institutions.

Medical institutions, with their clinical practice foundation and abundant human use experience data, have become important carriers for the inheritance and innovation of traditional Chinese medicine(TCM) and the "cradles" of the preparation of new TCM. To effectively promote the transformation of new TCM originating from the TCM clinical practice in medical institutions and establish an effective evaluation index system for the transformation of new TCM conforming to the characteristics of TCM, consensus experts adopted the literature research, questionnaire survey, Delphi method, etc. By focusing on the policy and technical evaluation of new TCM originating from the TCM clinical practice in medical institutions, a comprehensive evaluation from the dimensions of drug safety, efficacy, feasibility, and characteristic advantages was conducted, thus forming a comprehensive evaluation system with four primary indicators and 37 secondary indicators. The expert consensus reached aims to encourage medical institutions at all levels to continuously improve the high-quality research and development and transformation of new TCM originating from the TCM clinical practice in medical institutions and targeted at clinical needs, so as to provide a decision-making basis for the preparation, selection, cultivation, and transformation of new TCM for medical institutions, improve the development efficiency of new TCM, and precisely respond to the public medication needs.

Objective To explore the correlation of CPNE3 expression with long-term prognosis of patients with gastric cancer(GC)and the possible mechanism.Methods We retrospectively collected the data of 104 GC patients undergoing radical surgery in our hospital from February,2013 to October,2017.TCGA database and immunohistochemistry were used to analyze CPNE3 expression level in GC tissues and its effects on tumor progression and long-term prognosis of the patients.GO analysis was performed to predict the biological role of CPNE3 in GC.We also conducted cell experiments with MGC803 cells and observed the effects of CPNE3 knockdown,CPNE3 overexpression and LY294002(a PI3K/AKT inhibitor)treatment on cell apoptosis and cellular expressions of apoptotic proteins using flow cytometry and Western blotting.Results TCGA analysis and immunohistochemistry both showed high expressions of CPNE3 in GC(P<0.05).The patients with high CPNE3 expressions had a reduced 5-year survival(P<0.01),and a high CPNE3 expression,CEA level≥5 μg/L,CA19-9 level≥37 kU/L,T3-T4 stage,and N2-N3 stage were all independent risk factors for a lowered 5-year survival rate after surgery.The sensitivity and specificity of CPNE3 for predicting 5-year mortality was 79.59%and 74.55%,respectively(P<0.05).GO analysis predicted that CPNE3 negatively regulated GC cell apoptosis.In MGC803 cells,CPNE3 knockdown significantly increased cell apoptosis,enhanced Bax and Cleaved Caspase-3 expressions and decreased Bcl-2 expression,while CPNE3 overexpression produced the opposite results(P<0.05).The cellular expressions of p-PI3K and p-AKT were significantly decreased following CPNE3 knockdown and increased following CPNE3 overexpression(P<0.05).Treatment with LY294002 obviously attenuated the inhibitory effect of CPNE3 overexpression on apoptosis of MGC803 cells(P<0.05).Conclusion CPNE3 is highly expressed in GC tissues and affects the long-term prognosis of the patients possibly by inhibiting GC cell apoptosis through activation of PI3K/AKT signaling.

5.First case of group X meningococcal disease in Hunan Province:case re-port and literature review

The group X meningococcal disease is rare in China and developed countries.No related cases have been reported in Human Province.The disease progresses rapidly and leads to critical severity.Serious complica-tions may occur,if not treated actively.Group X Neisseria meningitidis(Nm X)vaccine has not yet obtained per-mission at present.This paper collects data on the symptoms,signs,auxiliary examinations,and treatment process of the first patient with severe group X meningococcal disease in Hunan Province,reviews relevant literatures,so as to improve clinicians'understanding on group X meningococcal disease,conduct early identification,diagnosis and treatment of the disease.

Objective To study and analyze the theory, policy framework, and core content of physical activity policies and physical activity guidelines. Methods Using a policy research and content analysis approach and the theory of the six components of World Health Organization (WHO) health service system, we specifically analyze the theory, framework, and core content of WHO Global Action Plan on Physical Activity and WHO Physical Activity Guidelines. Results The Global Plan of Action for Physical Activity 2018-2030 (Action Plan) is an international policy document on physical activity issued by WHO that incorporates physical activity within the context of the seven principles of human rights, the life span, evidence-based practice, proportional universality, policy coherence and integration of health into all policies, participation and empowerment, and multisectoral partnerships into health services and social development. The Action Plan consists of four strategic objectives and 20 policy actions, covering six areas of WHO health service system, and the integration of physical activity policies into health services is of great importance in promoting the achievement of the United Nations Sustainable Development Goal 3 of universal health coverage. As a technical document for the implementation of the Action Plan, 2020 WHO Guidelines on Physical Activity and Sedentary Behavior (Guidelines) adopted the PI/ECO approach to analyze the physical activity needs of various groups of people, and provide guidelines to increase physical activity and reduce sedentary behavior for children and adolescents, adults, older adults, pregnant and postpartum women, people with chronic diseases and people with disabilities. The guidelines cover duration, frequency, and intensity of physical activity, types of physical activity, critical and important health outcomes of physical activity, and health risk prevention and related considerations. The Guidelines implement the relevant guiding principles of the Action Plan and aim to improve overall population participation in physical activity at the micro level and improve critical and important health outcomes for the overall population. Conclusion As a health and development strategy, the Action Plan promotes the integration of physical activity into the health delivery system to facilitate the achievement of the United Nations 2030 Sustainable Development Goal 3 of universal health coverage.The four strategic objectives and 20 policy actions of the Action Plan can be integrated into these six areas based on the six components of WHO Health Service Delivery System: leadership and governance, financing, human resources, service delivery, medical technology, and health information. As a technical document to implement the Action Plan, the Guidelines are based on the PI/ECO approach framework and provide guidance on increasing physical activity and reducing sedentary behavior for children and adolescents, adults, older adults, pregnant and postpartum women, chronic patients, and persons with disabilities. The core content addresses the target populations, duration, frequency, and intensity of physical activity, types of physical activity, critical and important health outcomes of physical activity, and health risk prevention and related considerations.

Objective:To observe the effect of Qiyu Sanlong decoction (QYSL) on the expressions of key molecules in signal axis of mammalian rapamycin target protein (mTOR)/yeast Atg6 homologous (Beclin1)/ microtubule-associated protein1 light chain3 (LC3) in A549 cells. Method:With A549 cells as the research object, the effect of QYSL medicated serum on cell viability of A549 cells were detected by cell counting kit-8 (CCK-8) method. The effect of QYSL decoction on A549 cell apoptosis, autophagosome formation and the expression of autophagy markers were detected by Terminal-deoxynucleoitidyl transferase mediated nick end labeling (TUNEL) method, transmission electron microscope (TEM), Real-time polymerase chain reaction (Real-time PCR) and Western blot. Objective:To investigate the molecular mechanism of Qiyu Sanlong prescription (QYSL) in inhibiting the "addiction" of lung cancer A549 cells to miRNA21. Method:The human lung cancer A549 cells were routinely passaged and divided into the blank group, blank serum group, QYSL-containing serum group, and siRNA group. The prepared QYSL-containing serum was used for intervention, with the optimal concentration and action time determined in previous studies. The protein and mRNA expression levels of miRNA21 and related molecules in its target phosphatase and tensin homolog deleted on chromosome 10 (PTEN)/phosphatidylinositol 3-kinase (PI3K) signaling pathway were detected by real-time polymerase chain reaction (Real-time PCR) and Western blot assay. 9. Expression of MicroRNA-7-5p in human colorectal cancer and its influence on proliferation and apoptosis of colon cancer cell CaCo2

Objective MicroRNAs are differentially expressed in colorectal cancer tumor tissues and adjacent normal tissues, which play an important role in the development of colorectal cancer. This study aims to investigate the expression of microRNA-7-5p in colorectal cancer patients and its influence on the proliferation and apoptosis of colon cancer cell CaCo2. Methods The high-throughput microarray was used to screen differentially expressed microRNAs, and real-time quantitative PCR (RT-PCR) was used to detect the expression of microRNA-7-5p in 10 cases of colorectal cancer patients and corresponding adjacent normal tissues. Western blot was performed to detect the expression of intestinal trefoil factor (TFF3) in different tissues and CaCo2 cells after transfection with microRNA-7-5p. The expression of TFF3 in different tissues and CaCo2 cells transfected with microRNA-7-5p was detected. TUNEL combined with flow cytometry was used to detect the apoptosis of CaCo2 cells after transfection. The CCK-8 assay was used to detect the proliferation of CaCo2 cells after transfection. Results The relative expression of MicroRNA-7-5p in colorectal cancer tissue was 0.409 ± 0.095, which was significantly lower than that of normal tissue adjacent to cancer (1.000 ± 0.014), and the difference was statistically significant (P <0.01). The relative expression of TFF3 in colorectal cancer tissues was significantly higher than that in normal adjacent tissues (P <0.01). The relative expression of TFF3 in CaCo2 cells decreased in the overexpressed microRNA-7-5p of the control group (0.729 ± 0.041). The proliferation ability (0.930 ± 0.007) was significantly lower in the blank control group (0.990 ± 0.005) (P <0.01), and it could increase the proportion of early apoptotic cells (50.700 ± 0.989) and late apoptotic cells (40.525 ± 0.515). Conclusion MicroRNA-7-5p is lowly expressed in colorectal cancer and is associated with the occurrence and development of colorectal cancer. Up-regulated microRNA-7-5p inhibits the proliferation of colon cancer cell CaCo2 and promotes its apoptosis.
10.Effects of Guizhi Fuling Capsules and Its Main Components on Intestinal Flora of Primary Dysmenorrhea Model Rats

OBJECTIVE:To investigate the effects of Guizhi ful ing capsules and its principal components (paeoniflorin, paeonol and amygdalin )on the intestinal flora of primary dysmenorrhea model rats. METHODS :Female SD rats were randomly divided into normal group ,model group ,capsule group(Guizhi fuling capsule ,1 000 mg/kg),paeoniflorin group (15.0 mg/kg), paeonol group (10.3 mg/kg)and amygdalin group (12.1 mg/kg),with 6 rats in each group. Except for normal group ,other groups were given estradiol benzoate subcutaneously on the back of rats and oxytocin intraperitoneally to induce primary dysmenorrhea model. From the 4th day after subcutaneous injection of estradiol benzoate ,normal group was given constant volume of normal saline intragastrically ;model group was given constant volume of 0.5%CMC-Na solution intragastrically ;administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 7 days. The writhing times and the contents of and MDA in uterus tissue of rats were determined ,and then com the contents of short-chain fatty acids (SCFAs)such as acetic acid,propionic acid ,butyric acid in colonic contents were detected by GC method. Using diver sity index as index , Rep-PCR and Eric-PCR were used to evaluate the d iversity of intestinal flora in feces of rats. RESULTS :Compared with normal group,the writhing times of rats were increased significantly in model group ;the contents of NO and MDA in uterus were increased significantly ,while the contents of acetic acid ,propionic acid and butyric acid in colonic contents and total content of SCFAs were decreased significantly (P<0.05 or P<0.01);the number of DNA electrophoresis bands of intestinal flora was significantly reduced ,the brightness of most bands was significantly reduced ,and the diversity indexes (by Rep-PCR and Eric-PCR method ,hereinafter)1 h after administration were significantly reduced (P<0.05 or P<0.01). Compared with model group,writhing times of rats were decreased significantly in capsule group ,paeoniflorin group and paeonol group ;the contents of NO in uterus of rats in capsule group and paeoniflorin group as well as the contents of MDA in capsule group ,paeoniflorin group and paeonol group were decreased significantly (P<0.05 or P<0.01);the propionic acid content and total content of SCFAs in colon of rats in capsule group ,the contents of acetic acid ,propionic acid and butyric acid ,total content of SCFAs in paeoniflorin group as well as the contents of propionic acid and butyric acid ,total content of SCFAs in paeonol group were increased significantly;the content of isovaleric acid was decreased significantly in paeoniflorin group (P<0.05 or P<0.01);DNA electrophoresis bands and its brightness of intestinal flora changed to different extents in administration groups ,and the diversity indexes of intestinal flora 1 h after administration were increased significantly in capsule group and paeoniflorin group ,while those indexes were decreased significantly in paeonol group and amygdalin group (P<0.05 or P<0.01). CONCLUSIONS :Guizhi fuling capsules can significantly reduce writhing times and the contents of NO and MDA in uterus of primary dysmenorrhea model rats. At the same time ,the capsules also can regulate SCFAs content in colonic contents and intestinal flora diversity of rats. The above effects may be related to paeoniflorin and paeonol in the capsules.
