1.Therapeutic effects of treatment of posterior malleolar fractures with medial-extension type through posterioromedial and posteriorlateral incision.

OBJECTIVETo explore clinical outcomes of posterior malleolar fractures with medial-extension type through posterioromedial and posteriorlateral incision.
METHODSFrom January 2008 to January 2011,25 patients with posterior malleolar fractures with medial-extension type were treated by hollow lag screw. Among them, 15 patients were treated through posteromedial incision,including 9 males and 6 females,aged from 21 to 67 years old with an average of 48.1 +/- 1.3; there were 5 cases with type A, 6 cases with type B and 4 cases with type C,according to Denis-Weber classification. Ten patients were treated by through posterior-lateral incision,including 6 males and 4 females, aged from 23 to 64 years old with an average of 46.9 +/- 1.5; there were 3 cases with type A, 5 cases with type B and 2 cases with type C,according to Denis-Weber classification. Operation time, blood loss, length of incision, times of X-ray exposure and complications of two groups were recorded and compared, Baird-Jackson effective evaluation were applied for evaluate clinical outcomes.
RESULTSAll patients were followed up from 12 to 49 months with an average of 20.6 months. There were significant differences in operation time, blood loss, times of X-ray exposure and complications between two group (P < 0.05). While there was no obvious meaning in clinical outcomes between two groups (P > 0.05).
CONCLUSIONTreating posterior malleolar fractures with medial-extension type through posteromedial approach can expose and fix fracture under direct vision, has advantages of shorter operation time, less X-ray exposure and blood loss, is a good choice of surgical approach.

2.Expression of chemokines and chemokine receptors in organ-graft rejection:recent progress

Acute and chronic graft rejection are the major factors leading to graft non-function.There is an active expression of chemokines early after transplantation.They recruit T cells and antigen presenting cells selectively to the graft, leading to inflammatory reaction and finally to graft non-function.Accordingly,monitoring the expression status of chemokines and their receptors regularly may help to the diagnose rejection.To determine one or more chemokines or their receptors as the new targets for anti-rejection therapy will be of great clinical significance.This review focuses on the research progression in the above areas.
3.Application of internal fixation combined with external fixator for unstable pelvic fracture.

OBJECTIVETo invesitigate the clinical effects of percutaneous closed reduction and cannulated screw internal fixation combined with external fixation in the treatment of unstable pelvic fractures.
METHODSFrom April 2006 to May 2009,29 patients with pelvic fractures of rotatory instability were treated with closed reduction and screw internal fixation combined with external fixation. There were 19 males and 10 females with an average age of 31 years old (ranged from 19 to 53 years). Based on the Tile classification,17 cases were type CI and 12 case were type C2. Tornetta standard and Majeed score were used to evaluated the clinical effect after operation.
RESULTSAll patients were followed up from 10 to 24 months with an average of 16 months. There were no nerve injuries and other organ injuries, only one case of infection was found and it was cured with wound dressing. Time of fracture union was from 14 to 18 weeks with the mean of 16.2 weeks. No loosening, slippage or breakage of the screw were found. According to the Tornetta standards,14 cases obtained excellent results, 10 good, 4 fair and 1 poor. The Majeed score was 87.2 ± 11.3, 16 cases got excellent results, 9 good and 4 fair.
CONCLUSIONClosed reduction and screw internal fixation combined with external fixation is an effective way to treat unstable pelvic fractures with Tile type C1 and Tile type C2.

4.Comparison of Contents of Deoxyschizandrin and Schisantherrin A in Fructus Sch isandrae Chinensis

Objective To establish a HPLC method for the determination of deoxys chizandrin and schisantherrin A in Fructus Schisandrae Chinensis. Methods The as say was conducted on a YWG-C18 column with methonal-water (77 ∶23) as mobile phase. The flow rate was 1.0 mL/min and detection wavelength was 230 nm. Results Satisfactory linearities of deoxyschizandrin and schisantherrin A were shown in the range of 0.125~1.25 ?g and 0.08~0.8 ?g,and the recovery was 100.1 %an d 100.0 %(RSD being 0.15 %and 0.28 %。Schisandra sphenanthcra),and 99.7 %an d 101.2 %(RSD being 0.32 %and 0.67 %。Schisandra chinensis) (n=5). Conclusion The HPLC method was simple,curate,highly sensitive and reproducible. It may b e used for the quantitative determination of deoxyschizandrin and schisantherrin A in Fructus Schisandrae Chinensis.

7.Spatial characteristics of non-iodized salt at household level based on geographic information system in Chongqing

Objective To analyze the spatial distribution characteristics of non-iodized salt at household level based on geographic information system (GIS) in Chongqing.Methods The database of non-iodized salt at county level from 2001 to 2010 was established in Chongqing.By using GIS technology,the spatial distribution and spatial autocorrelation were analyzed by ArcGIS 9.3 software.Results The rate of non-iodized salt was fluctuated between 2.35%-5.78% during 2001-2006 and the rate was reduced to less than 2.00% after 2007.The result of spatial autocorrelation analysis on non-iodized salt from 2001 to 2006 indicated that Moran's Ⅰindex was 0.145578,0.078801,0.108033,0.091957,0.127749,0.214302,respectively(Z value was 3.066275,1.977321,2.541619,2.309972,2.900446,3.874203,respectively,all P < 0.05).The spatial distribution of non-iodized salt had marked spatial cluster through Chongqing region from 2001 to 2006.The result of local spatial autocorrelation analysis from 2001 to 2006 indicated that Fengdu and Fuling were two high-risk areas(all P < 0.05).Dianjiang,Yubei,Jiangbei,Wulong and Banan were also confirmed as high-risk areas in 2001,2005 and 2006(all P < 0.05).The results also indicated that the distribution of non-iodized salt in the seven high-risk areas was positively correlated.The result of spatial autocorrelation analysis on non-iodized salt from 2007 to 2010 indicated that Moran's Ⅰ index was 0.018361,0.016186,0.040769,-0.059691,respectively (Z value was 1.093310,0.787361,1.071811,-0.583820,respectively,all P > 0.05).The spatial distribution of non-iodized salt was at random on the whole from 2007 to 2010.However,there were four local high-risk areas.The distribution in Fengdu and Dianjiang was positively correlated,while that in Jiangjin and Shizhu was negatively correlated.Conclusions The distribution of non-iodized salt at households level in Chongqing is changed from spatial distribution before 2006 to random distribution after 2007,but there are high value areas,which should be taken as the focus of monitoring.

9.Delayed union or nonunion of the ulna after intramedullary nailing for pediatric forearm fractures.

OBJECTIVETo analyze the causes of delayed union or nonunion of the ulna after intramedullary nailing in pediatric forearm fractures.
METHODSFrom February 2005 to February 2010,5 patients with forearm fractures who were treated with titanium elastic nailing (TEN) were identified to fulfill the criteria of having developed a delayed union or nonunion of the ulna. The causes of delayed union or nonunion were investigated according to mechanism of injury, fracture location, treatments methods and postoperative management. All patients were male and the age was 3 to 14 years old with an average of 9.4 years. All fractures were located on the mid-third part of forearm. Two cases had a re-fracture. Among them, 3 cases caused by high-energy injury and 2 cases by falling down. Open reduction were performed in 4 cases while the other one was treated with closed reduction. Four patients were immobilized in an above-elbow cast, postoperatively.
RESULTSAll patients were followed up from 7 to 19 months with an average of 11.4 months. There were 4 delayed union and 1 nonunion. Three patients healed after the removal of the nail and avoidance of weight-bearing. Two patients healed by replacing another fixation. No patients had soft-tissue irritation or nail-entry-site infections.. The clinical effect was evaluated according to Daruwalla and Price scores with 3 excellent and 2 good of the results.
CONCLUSIONSUsing titanium elastic nailing for the treatment of pediatric both-bone forearm fractures is a good method. However,strict indication selection should be followed to avoid delayed union or nonunion.

10.Epidemiological investigation on confirmed cases of schistosomiasis in Hu-bei Province

Objective To grasp the distribution and epidemiology of confirmed cases of schistosomiasis in Hubei Province, so as to provide the evidence for promoting the prevention and control work. Methods The confirmed cases of schistosomiasis in Hubei Province from 2010 to 2014 were epidemiologically investigated,and the prevalence characteristics and main influenc?ing factors were analyzed. Results A total of 10 102 confirmed cases from 2010 to 2014 were surveyed. There were 1 062 local infected patients,accounting for 10.51%and including 354 repeated infections and 17 newly infected. There were 290 foreign?infected patients,accounting for 2.87%,with 206 repeated infection cases and 84 newly infected. There were 8 750 historical patients,including 2 229 patients who leaked the former schistosomiasis investigations,accounting for 22.06%;570 patients missed treatment,accounting for 5.64%;3 640 patients were treated with non?standard therapy,accounting for 36.03%;2 311 patients were treated with poor medication efficacy,accounting for 22.88%. The multivariate non?conditional Logistic regres?sion,targeting at confirmed cases in 2014,showed that,for the leaking investigations,the potential risk factors included the age,educational level,and latrine renovation(b>0,OR>1),the protective factors were the times of previous treatment,cat?tle feeding in villager team,and Oncomelania hupensis snails in surroundings(b<0,OR<1);for the treatment?missing,the age,educational level,snails in the surroundings of residence were risk factors(b<0,OR<1);for the substandard treat?ment,the risk factors included the occupation and snails in the surroundings of residence(b>0,OR>1),and the educational level and snails in the own field were protective factors(b<0,OR<1). Conclusion The epidemiological investigation on the confirmed cases of schistosomiasis could grasp the epidemic factors so as to improve the management and carry out the scientific control.
