Objective To investigate the diagnostic points of multiple sclerosis and multiple metastatic encephaloma.Methods Choose a case of multiple sclerosis,which had been misdiagnosed as multiple metastatic encephaloma and operated on,to analyse its reasons of misdiagnosis.Results Reasons for the misdiagnosis:The neglect of the importance of the disease history.The scant knowledge of the early represen- tations of the atypical MS(demyelinating pseudotumor).The lack of the knowledge of the non-demyelinating lesions of the MS.The lack of attention to the different representations of the MRI scan between the acute MS and the multiple metastatic encephaloma.The excessive importance is attached to the imageological report. The neglect of the routine examination of cerebrospinal fluid.Conclusion When multiple metastatic en- cephaloma develops,the differential diagnosis of MS should be made.The further examinations should be done to those cases which are hard to tell the difference with metastatic encephaloma by clinical and imageo- logical representations.Try to avoid undergoing surgery or radiotherapy hastily.
3.Relationship of kawasaki disease and human parvovirus B19 infection

【Objective】To explore the role of cell cycle regulators in nasopharyngeal carcinoma (NPC) relapse.【Method】To assay p53,MDM2,p21ras and p21WAF1 proteins by LsAB immunohistochemical technique in 69 cases of primary and relapsing NPC tissues.【Results】As compared with primary NPC,the expression rate of p53 or MDM2 protein in relapsing NPC was similar (78% to 80%,84% to 83%),and the expression rate of p21ras or p21WAF1 protein in relapsing NPC was obviously descended (73% to 93%,52% to 84% );the high-expression rate of p53 protein in relapsing NPC was similar (42% to 51%),the high-expression rate of MDM2 protein in relapsing NPC was obviously risen (57% to 32%),and the high-expression rate of p21ras or p21WAF1 protein in relapsing NPC was obviously descended (16% to 65%,17% to 46%).Among of them,the significant rise of MDM2 protein expression level in relapsing NPC mainly occurred in the patients of group 2 which relapsing-interval was shorter than 34 months,P<0.05;the significant descent of p21ras or p21WAF1 protein expression level in relapsing NPC occurred in the patients of group 2 and group 1 which relapsing-interval was equal to or longer than 34 months,P<0.02,respectively.【Conclusions】The overexpression of p53 and MDM2 proteins and the low or negative expression of p21WAF1 protein after clinical cure might still play an important role in NPC relapse,the obvious rise of MDM2 protein level and the obvious descent of p21WAF1 protein level might further accelarate the process of NPC relapse.

7.Clinical thinking ability construction of geriatric medicine professional degree graduate students

Professional postgraduates in geriatric medicine are one of the most important re-courses of geriatric specialists. However, training model for geriatric postgraduates is still unclear in China. In general, geriatric medicine has no independent training plan, and is included in internal medicine training. Such model cannot represent the feature of geriatric medicine. The present paper aimed to discuss the ability construction for geriatric postgraduates based on the geriatric medicine specialist training experience both at home and abroad, including diagnosis and treatment of multi-system diseases, ability of geriatric assessment, management of geriatric syndromes, consultation for geriatric rehabilitation and ability for continuity health service in order to cultivate the compound tal-ents suitable for the aging society requirement for geriatric medicine in China.
8.Clinical Comparative Study on Massage Therapy and Cisapride in Treating Functional Dyspepsia

To observe the clinical efficacy of massage therapy and Cisapride in the treatment of functional dyspepsia (FD).Methods:Eighty subjects were randomized into two groups:treatment group in which 40 cases were treated by massage therapy and control group in which 40 cases were treated by Cisapride,with a course of 4 weeks;meanwhile,another 40 healthy people were taken as normal group.Abdominal fullness,acid regurgitation,diminished appetite and anorexia,nausea and vomiting and health survey were observed;symptom scores were recorded.Results:These two treatment methods were effective for FD.Conclusion:Mental disorder is one cause of FD;massage therapy is quite effective for it.
10.Pre-radiation neutrophil to lymphocyte ratio, platelet to lymphocyte ratio, prognostic nutritional index, and CA19-9 as prognostic factors in pancreatic cancer patients treat-ed with radiotherapy

Objective:To investigate the prognostic value of neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), and prognostic nutritional index (PNI) before radiation in pancreatic cancer patients underwent radiation therapy. Methods:Clinical data of 61 patients with pathologically confirmed pancreatic cancer were retrospectirely analyzed. Query index values for each patient were clinically tested through electronic medical records. Overall survival (OS) data were collected through electronic medical records or telephone follow-up. Survival curves were compared using Kaplan-Meier methodology and log-rank test. Cox regression methodolo-gy was used for univariate analysis. Results:Using log-rank test and univariate regression analysis, we found that patients with lower NLR, PLR, CA19-9, increased PNI, and lower TNM-staging lived longer than those with increased NLR, PLR, CA19-9, lower PNI, and high-er TNM-staging, and that the difference was statistically significant. Through multivariate analysis, we found that NLR (P=0.029, OR 2.344, 95%CI:1.090-5.041) and PNI (P=0.026, OR 0.477, 95%CI:0.248-0.917) were independent predictors of OS in patients with pan-creatic cancer. Conclusion:NLR, PLR, and PNI are simple, effective, and reliable indicators for evaluating the prognosis of pancreatic cancer.
