1.Applying value of 13C-urea breath test in the diagnosis of Helicobacter pylori infection in gastric remnant

Objective To investigate the diagnostic accuracy of 13C urea breath test (13C-UBT)in the diagnosis of Helicobacter pylori (Hp) infection in patients with gastric remnant.Methods A total of 94 gastric remnant patients (gastric remnant group) and 100 non-gastric remnant patients (control group) received 13C-UBT.According to the course of the disease,the gastric remnant group were divided into group 1 (29 cases,≤5 years),group 2 (33 cases,6 to 10 years) and group 3 (32cases,>10 years).Before surgery,the diagnosis of Hp infection was according to 13C-UBT and gastric mucosa tissue staining.After surgery,the diagnosis was according to gastric mucosa tissue staining.The applying value of 13C-UBT in gastric remnant patients was evaluated.The consistency of results between 13C-UBT and gastric mucosa tissue staining were evaluated by the Kappa test.Results In gastric remnant group,the Hp infection rate after surgery (37.2%) was obviously lower than that before operation (79.8%;x2 =35.058,P<0.01) and that of control group (77.0%).After surgery,the Hp infection rate of patients with Billroth Ⅱ gastrectomy (24.4%) was lower than that of patients with Billroth Ⅰ gastrectomy (47.2% ; x2 =5.133,P=0.023).Before surgery,the Hp infection rate of gastric remnant subgroups were similar (P>0.05).With the extension of the postoperative course,the Hp infection rate gradually decreased in gastric remnant subgroups (group 1:58.6%,group 2:36.4%,group 3:18.8%; x2=7.839,P=0.021).Setting the cut-off point at 2.0‰ in the gastric remnant group and 3.5‰ in the control group,the accuracy rate of diagnosis was the highest (92.6% and 96.0%).There was good consistency between the results of 13C-UBT and gastric mucosa tissue staining (Kappa =0.84,P<0.01).Conclusion With suitable operation and standard,13C-UBT may be a easy,safe and effective assay applied in the gastric remnant patients.
2.The value of double contrast-enhanced unltrasonography in macroscopic classification of rectal carcinoma

Objective To investigate the clinical value of double contrast-enhanced ultrasonography (DCUS) in the macroscopic classification of rectal carcinoma. Methods Seventy-one patients with rectal carcinoma were examined by ultrasound after intrarectal infusion and intravenous bolus injection of SonoVue preoperatively. And three types were divided according to the shape of the tumor:protruding type, ulcerative type and invasive type. After surgery, DCUS and perfusion contrast-enhanced ultrasonography (PCUS) were compared with final pathologic results respectively, and the relationship between general tumor type and TN staging were analyzed.Results The accuracy of DCUS [88.7%(63/71) ] in macroscopic classification of rectal carcinoma was significantly higher than that of PCUS [73.2%(52/71) ](χ2=5.534,P<0.05). For the invasion depth into the intestinal wall, the protruding type was mainly in T1, T2 and T3 stages while ulcerative and invasive type in T2, T3 and T4 stages (χ2=12.322, P<0.05 ). For the stage of lymph node, the protruding type was mainly in N0 and N1 while ulcerative and invasive type in N1 and N2(χ2=6.733,P < 0.05 ). Conclusions DCUS, with its high accuracy, is a new valuable method for macroscopic classification of rectal carcinoma.In the aspect of intestinal wall invasion and the lymph node, protruding type is less severe than ulcerative type and invasive type.

Objective To explore the relationship between social norms and smoking intention, among male smokers in China. Methods A total 3 621 (with 95.8% of effective response rate) male smokers were selected and investigated with questionnaires from August to November in 2013.Univariate logistic regressions and multivariate logistic regressions were conducted to find the associations between social norms, smoking intention and quitting attempts. Results Smokers perceived low pro-quitting social norms from their family members, friends and other people around.With a 1 point increase in pro-quitting social norms score, smokers′ willingness to quit in the next 6 months increased by 69% (
5.Effect of heat-sensitive point moxibustion on BMD, S-AKP, U-Ca/Cr in patients with primary osteoporosis.

OBJECTIVETo compare the curative effects of primary osteoporosis treated with heat-sensitive point moxibustion and Gaitianli (Oyster Shell and Calcium Carbonate Chewable) tablets for oral administration and explore the treatment mechanism.
METHODSSixty cases of primary osteoporosis were randomly divided into a heat-sensitive point moxibustion group (moxibustion group) and a Gaitianli tablets group (medication group), 30 cases in each group. In the moxibustion group, the heat sensitized points were searched around Zusanli (ST 36), Pishu (BL 20), Shenshu (BL 23) and Mingmen (GV 4) and treated by heat-sensitive point moxibustion; in medication group, Gaitianli tablets were taken by oral administration, 3 pills for once and 3 times a day. The curative effects, bone mineral density (BMD), alkaline phosphatase (S-AKP) and urinary calcium to creatinine ratio (U-Ca/Cr) in both groups were observed before and after treatment.
RESULTSThe total effective rate was 86.7% (26/30) in moxibustion group, superior to that of 76.7% (23/30) in medication group (P < 0.05). After treatment, the BMD of lumbar vertebrae (L2-L4) mean was improved (P < 0.05), and the S-AKP and U-Ca/Cr were reduced (all P < 0.05); in medi cation group, the indexes above were no obvious changes (all P > 0.05).
CONCLUSIONThe therapeutic effect of primary osteoporosis treated with heat-sensitive point moxibustion is superior to that with Gaitianli tablets for oral administration. The mechanism is restraining bone resorption, increasing bone strength, keeping balance of bone metabolism, in order to increase bone mineral density and improve the clinical symptoms.

6.Effect of serotonin depletion on seizures learning-memory in pilocarpine-induced epileptic rats.

OBJECTIVETo investigate the relationship between serotonin (5-HT) and epilepsy and the mechanism of learning-memory in pilocarpine (PILO)-induced epileptic rats after 5,7-dihydroxytryptamine (5,7-DHT) microinjection in median raphe nucleus.
METHODSAdult S D rats were randomly divided into 3 groups: PILO group, PILO+ 5,7-DHT group, vehicle control group; PILO group was divided into two groups by status epilepticus (SE): PILO + SE group and PILO - SE group. The rats' seizures and cortex electroencephalography (EEG) were observed by video EEG. The rats' spatial learning-memory was evaluated by Morris water maze. Finally, serotonergic neuron in raphe nuclei was observed by immunohistochemistry.
RESULTSAfter treatment of 5,7-DHT (PILO + 5,7-DHT group), the success rate, the mortality and the frequency of chronic spontaneous seizures in pilocarpine-induced epilepsy model were all improved. Compared with the control group, the number of serotonergic neuron in raphe nuclei was decrease in PILO + SE group (P < 0.05). Moreover, it's extremely decrease in PILO + 5,7-DHT group (P < 0.01). Compared with control group, the mean escape latency was prolonged, the times of crossing target was decreased and the retention time in target zone was shortened in PILO + SE group (P < 0.05), but there was no significant difference between PILO + SE group and PILO + 5,7-DHT group.
CONCLUSIONDepletion of serotonin may facility the rats' epileptic seizures, but we could not interpret which may cause epileptic rats' cognitive deficit.

7.Difference of diagnosis by CT angiography and DSA for patients with vertebral artery stenosis and efficacy observation of stenting

Objective To observe the significance of neck CT angiography (CTA) in detecting the vertebral artery stenosis,and to study the feasibility and safety of stenting and angioplasty in patients with symptomatic vertebral artery stenosis.Methods Patients with ischemic stroke in the posterior circulation,admitted to our hospital from May 2007 to April 2011,were chosen in our study; all the patients were performed neck CTA and brain MRA; If there might be vertebral artery stenosis in the CTA,DSA was chosen.Patients diagnosed as having vertebral artery stenosis by DSA were performed stenting and angioplasty after being individual assessment and informed consent.Results Thirty-eight patients diagnosed as having ischemia in the posterior circulation and vertebral artery stenosis were received stenting and angioplasty.There were 38 vertebral artery stenoses (above 50%) detected by neck CTA in 29 patients,among which,30 stenoses were in the V1 part of vertebral artery and 8 in the V4 part; while these 29 patients detected by DSA only showed 4 stenoses in the V4 part and 28 in the V1 part (above 50%).And 9 patients with VI part of vertebral artery stenosis (above 50%) couldn't be found by neck CTA but diagnosed by DSA finally.These 38 patients treated with 41 stents,which were all successful (achievement ratio 100%).The mean ratio of stenosis before stenting was (65.0±11.2)% and the mean ratio after stenting was (11.6±8.9)%,with significant difference.One patient experienced lacunar infarction and 1 severe stroke with lock in syndrome during the 6-25 months of follow-up; and 1 patient experienced subarachnoid hemorrhage after the procedure.Conclusion The neck CTA is useful in the screening of vertebral artery stenosis.Stenting and angioplasty are the safe and effective treatments for patients with symptomatic vertebral artery stenosis.
8.The role of cyclooxygenase on angiogenesis and endothelial progenitor cell mobilization in rat ischemic myocardium.

OBJECTIVETo investigate the effect of COX1 and COX2 on angiogenesis and endothelial progenitor cell mobilization in rats with experimental myocardial infarction (MI).
METHODSThe rats were randomly divided into 3 groups: MI group, MI plus rofecoxib group and MI plus valeryl salicylate group. At the 7th day after operation, circulating EPCs, plasma VEGF and HIF-1alpha mRNA of ischemic myocardium were measured. At the 28th day post operation, capillary densities were also measured in ischemic myocardium.
RESULTCompared with the MI group and the MI plus valeryl salicylate group, circulating EPCs, plasma VEGF, HIF-1alpha mRNA and capillary densities of ischemic myocardium were all decreased in MI plus rofecoxib group.
CONCLUSIONThe present study revealed that COX2 play an important role with angiogenesis and endothelial progenitor cell mobilization in rat with experimental MI by modulating expression of VEGF and HIF-1alpha.

9.Isolation of cytotoxic compounds from the seeds of Crataegus pinnatifida.

AIM:
To study the chemical constituents and bioactivity of the seeds of Crataegus pinnatifida.
METHODS:
The chemical constituents were isolated and purified by macroporous adsorptive resin D101, silica gel, and ODS column chromatography, and preparative HPLC. Their structures were elucidated on the basis of spectroscopic methods. In addition, the cytotoxic activities of compounds 1-4 were investigated on OPM2 and RPMI-8226 cells.
RESULTS:
Four compounds were obtained and their structures were identified as (7S, 8S)-4-[2-hydroxy-2-(4-hydroxy-3-methoxyphenyl)-1-(hydroxymethyl)ethoxy]-3, 5-dimethoxybenzaldehyde (1), (+)-balanophonin (2), erythro-guaiacylglycerol-β-coniferyl aldehyde ether (3), buddlenol A (4).
CONCLUSION
Compound 1 is a novel norlignan, while compounds 1-4 exhibited marginal inhibition on the proliferation of OPM2 and RPMI-8226 cells.

10.Association among circulating endothelial progenitor cells, insulin resistance and severity of coronary lesions in patients with coronary artery disease

Objective To investigate the correlation between the number and activity of circulating endothelial progenitor cells (EPCs), insulin resistance and severity of coronary lesions in patients with coronary artery disease (CAD). Methods Patients with coronary angiography evidenced CAD were divided in insulin resistance group ( IR, n = 25 ) and insulin sensitive group ( IS, n = 44) according to insulin level, 25 health volunteers served as control. Circulating EPCs were marked as KDR/CD133<'+ cells via fluorescence- activated cell sorter analysis. EPCs were also isolated from peripheral blood and cultured in vitro for 7 days, identified by DiI-acLDL uptake and lectin staining methods. EPCs migration activities were determined by modified Boyden chamber assay, EPCs proliferation activities were determined by MTT assay. Result Circulating EPCs number was significantly lower in IR group compared with IS group [ (0. 34±0. 08 ) ‰ vs. (0. 47±0. 09 )‰, P < 0. 01 ] and control group ( P < 0. 05 ). Both insulin resistence index (r = - 0. 291, P = 0. 01) and Gensini score ( r = - 0. 3984, P = 0. 006) were negatively correlated with number of circulating EPCs. Proliferation and migration capacities of EPCs were also significantly lower in IR group compared to those in IS group ( all P < 0. 05 ) and control group ( all P < 0. 05). Conclusions Insulin resistence/hyperinsulinemia could aggravate severity of coronary artery lesions via reducing the number and activities of circulating EPCs in patients with CAD.
