1.Therapeutic effect observation of selegiline hydrochloride in early stage of Parkinson's disease

Objective To study the efficacy and safety of selegiline in the treatment of patients with Parkinson's disease (PD) at early stage. Methods The randomized and blank controlled study was performed. Sixty PD patients at early stage who had been treated with trihexyphenidyl, amantadine and vitaminE were divided into two groups. One was added to selegiline hydrochloride (5 mg/d) for eight weeks, the other was blank controlled group. Efficacy of selegiline in each patient was evaluated by modified Webster scale. The nuclear factor erythroid 2 related factor 2 (Nrf2) is a key protein of endogenous antioxidant defense systems in the body. In response to oxidative stress, Nrf2 translocates to nucleus and binds to antioxidant response elements (ARE), regulating the expression of a large amounts of antioxidant genes and maintaining a proper redox balance. The pathological processes of neurodegenerative diseases are associated with generation of reactive oxygen species, which cause oxidative stress. Oxidative stress plays a cardinal role in the onset and progression of neurodegenerative diseases. Nrf2-inducer compounds can reduce oxidant stress and have shown therapeutic efficacy in many neurodegenerative disease models. How to activate the Nrf2 signaling pathway effectively has been received much attention. Here we provided an overview of specific mechanism of Nrf2-ARE pathway and the protective effects of Nrf2 in different neurodegenerative diseases, and summarized the Nrf2 activators recently in preclinical study.

4.Optimization for Extracting Polysaccharide from Paris polyphylla Smith var. chinensis (Franch.) Hara in Shaan'xi by Response Surface Method

Objective To select the optimum extracting method of polysaccharide from Paris polyphylla Smith var. chinensis(Franch. )Hara in Shaan'xi. Methods The optimum extraction technology and condition were determined by using response surface methodology and single factor experiment,with the extraction ratio of polysaccharides from Paris polyphylla Smith var. chinensis( Franch. ) Hara in Shaan'xi as indicator. Results The optimum extraction condition for the polysaccharides was as follows:extracting at 92 ℃ for 2. 6 h and with the liquid to solid ratio of 15 ( mLg). The average extraction yield of polysaccharide was 3. 22%under the optimum extraction condition. Conclusion The method is suitable for the extraction and determination of polysaccharides from Paris polyphylla Smith var. chinensis( Franch. ) Hara in Shaan'xi.
5.Clinical characteristics and genetic diagnosis of dopa-rcsponsive dystonia

Objective To investigate the clinical characteristics and mutations of guanosine triphosphate eyclohydrolase (GCH) Ⅰ gene in patients with dopa-responsive dystonia (DRD). Methods Five families with 18 affected family members and 17 patients with sporadic DRD were examined. Patients were allocated into 3 groups according to onset time, either in childhood, or in adolescence or adult. Interview, physical examination, psychologic testings and CT/MR scan were performed. Mutation screening was performed on 26 patients and 1 normal family nember. Thirty-five healthy control subjects were matched for age and sex. Statistical analysis were conducted with the use of SPSS 13.0 computer software. Results(1)Most of patients started with dystonia. The main clinical manifestation was dystonia too. There was no difference among 3 groups.(2) There were significant differences in diurnal fluctuation among 3 groups(15/15,6/6,7/14, χ2=13.125,P=0.001). Diurnal fluctuation negatively correlated with age (r=-0.720, P<0.01).(3)The differences in postural tremor were also found among 3 groups (7/15,5/6,1/14, χ2=8.073, P=0.018). Postural tremor positively correlated with age (r=0.399, P=0.018).(4)There were differences in exaggeration of tendon among three groups(11/15,1/6,4/14, χ2=8.309, P=0.016). Exaggeration of tendon reflexes negatively correlated with age (r=-0.429, P=0.010).(5)The scores of Hamilton Depression Scale and Hamilton Anxiety Scale in patients were higher than those in controls.(6)DNA sequencing revealed a heterozygous A224G missense mutation(Tyr75Cys)located within exon 1 in one autosomal dominant inheritance family. Conclusions The manifestations of DRD varies. The clinical course is closely correlated with age. A missense mutation(A224G)in coding region of the GCH 1 gene probablyleads to the occurrence of DRD.
6.Assessment on changes of anterior chamber parameters after ICL surgery with Allegro Oculyzer system and their correlation with IOP

Abstract? AIM: To investigate changes of anterior chamber parameters after posterior chamber phakic implantable collamer lens ( ICL ) surgery and its correlation with intraocular pressure ( IOP) .?METHODS: This was a retrospective case series study. Seventy four eyes in 43 myopia patients were examined by Allegro Oculyzer anterior segment tomography to obtain the changes of anterior chamber volume ( ACV ) , anterior chamber angle ( ACA) , central anterior chamber depth ( ACD) and vault, meanwhile, to measure the IOP to analyze the correlation with anterior chamber parameters.?RESULTS: Compared with preoperative, ACV, ACA, ACD all decreased apparently ( P <0.01 ) in the postoperative period of 1, 3, and 6mo.ACV, ACA and ACD showed their stability in each period after the surgery.There were changes of vault in 1 and 6mo ( t=27.66, P=0.01).IOP had not altered when compared with the preoperative.This research revealed that IOP had no relation with ACV, ACA, ACD and vault in every phase( all P>0.05).?CONCLUSION:For patients underwent ICL, the anterior chamber parameters all decreased which included ACV, ACA, ACD, and had stabilized since early postoperative period. Correspondingly, IOP was stable and had not correlate with ACV, ACA, ACD and vault, however the long-term observation is still necessary.
7.Glycosynthases: a Novel Efficient Synthetic Tool for Oligosaccharides

Oligosaccharides are one of the essential physiological constituents of glycoproteins and glycolipids on mammalian cell surfaces and microbial metabolites. They have considerable potential as therapeutics but are only now slowly assuming this important role. One of the reasons for their slow development has been the considerable difficulty in synthesizing oligosaccharides on the scale necessary for their clinical evaluation. Classical chemical and enzymatic methods both have limitations in synthesizing large-scale oligosaccharides. In recent years, the rapid progress on molecular biotechnology has promoted the development of retaining glycosidases in oligosaccharides synthesis, which led to the production of a novel class of enzymatic activities termed the glycosynthases. These new enzymes are retaining glycosidase mutants in which the catalytic nucleophile has been converted to a non-nucleophilic residue,synthesizing oligosaccharides in high yields ( the highest yields reach 99%) without any hydrolysis. Furthermore thioglycoligases and thioglycosynthases have been developed subsequently in the past three years. Glycosynthases can be screened in high-throughput assay by the two-plasmid system and the yeast three-hybid system respectively. Their activity can be significantly enhanced by substituting alternative residues for nucleophile, additional random mutations and optimizing reaction conditions. Their regioselectivity can be modified through changes in receptors.
8.The clinical characters of tear film instability in diabetes patients

Background The current epidemiology study had shown the prevalence of age and sex adjusted dry eye was higher in patients with diabetes than population without diabetes.Further researches demonstrated that the tear film disturbance is common after the phacoemulsification or photocoagulation in the eyes of diabetic patients.Objective The aim of this study was to characterize the clinical features of tear film instability in diabetes patients.Methods One hundred and sixty-two patients with tear-film abnormality referred to Tongren Eye Center from January 1,2010 to September 1,2010 underwent questionnaire about diabetes and other diseases,BUT,Schirmer test.Tear film instability was diagnosed as abnormality of either Schirmer test or BUT and showed as M ( Q25,Q75 ).The right eyes of 162 dry eye patients meeting with the including criteria were enrolled.The patients were assigned to two groups according to with ( 80 patients) or without ( 82 patients) diabetes mellitum.DEQ questionnaire were scored.The percentage of cases with meibomain gland abnormal score > 1 was calculated.Mann-Whitney U analysis and Chisquare analysis were used to compare the difference between the two groups.Results The Schirmer test in diabetic group was 8 ( qualities:5,9 )mm and was longer than 6 ( qualities:5,7 ) mm in non-diabetic patients ( U =2452,P =0.00).The result of BUT test was 3 ( qualities:2,4 ) seconds in diabetic patients and was shorter than 4 (qualities:3,5) seconds in non-diabetic patients( U=2104,P<0.01 ).The DEQ score of diabetic patients was 15 ( qualities:1 0,19,which was less than21 ( qualities:19,23.25 ) in non-diabetic patients.51.2 % ( 41/80 ) diabetic participants and 32.9% (27/82) nondiabetic participants appeared meibography ( grade larger than 1 ) (x2 =16.07,P=0.00).The percentages of dry eyes were 51.2% (41/80) and 93.9% (77/82) respectively in diabetes and nondiabetes groups(x2 =37.24,P<0.01 ).No significant correlation was found between the diabetes course and DEQ score or meibography( r =0.16,P =0.16 ; r =0.10,P =0.36 ).Conclusions Diabetes patients with tear film instability have longer Schirmer test results,shorter BUT,more severe meibomain glands damage and lower DEQ scores.The dry eye symptom is lack in the diabetic patients though appearing the tear film and meibomain glands damage.Therefore,more attention should be given to ocular surface health in diabetes patients.

10.Protective Effect of Progesterone on Brain Edema in Newborn Rats with Hypoxic-Is chemic Encephalopathy

Objective To observe the protective effect of progesterone on brain edema in newborn rats with hypoxic-ischemic encephalopathy(HIE).Methods Fifty-two 7-day-old neonatal rats were randomly divided into 5 groups:normol control group(group A),hypoxic-ischemic group (group B) and pretreatment groups(group C,D and E).Rats were subjected to left common carotid artery ligation,then were exposed to 8% oxygen and 92% nitrogen for 2.5 hours to establish HIE model.Progesterone was injected intraperitoneally into the groups C,D and E rats respectively at 30 minutes before hypoxia.All rats were killed at the 24 hours after operations,then the contents of water,sodium,potassium were mensurated. Results The contentation of water in group B was higher than that in group A,the contentation of water,sodium in groups C,D and E were significantly lower compared with group B, but the contents potassium were no difference between group C,E and B. Conclusion Progesterone exerts a prominent preventive and protetive effects on the hypoxic ischemic brain edema in newborn rats.
