1.Diagnostic significance of the difference values between Mini-Mental State Examination and Montreal Cognitive Assessment in elderly patients with dementia

Objective To investigate the diagnostic significance of the difference values between Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA)in elderly patients with dementia.Methods 331 elderly patients with dementia were collected from outpatients in our hospital.There were 148 people with Alzheimer's disease (AD),87 cases with vascular dementia (VaD),44 cases with mixed dementia (MD),41 cases with frontotemporal dementia (FTD) and 11 cases with dementia with Lewy bodies (DLB).MMSE and MoCA were applied to test the cognitive impairment separately.Results The difference values between MMSE and MoCA was (3.3±1.7) points,(6.6±2.1) points,(6.6±2.1) points,(5.4±2.3) points,(6.1 ± 1.9) points in AD,VaD,MD,FTD and DLB group respectively,and there were statistical differences among the five groups (F=46.420,P=0.000).Statistical differences were found in the difference values between MMSE and MoCA between dementia patients with AD and non-AD (t=-13.429,P=0.000).According to receiver operating characteristic curve (ROC curve),the optimal cut off point of the difference values between MMSE and MoCA for differential diagnosis between AD and non-AD dementia was 5 points,with 79.8% sensitivity and 78.4% specificity,and area under the curve was 0.848 (95%CI:0.807-0.890).Conclusions The difference values between MMSE and MoCA may be one of parameters for differential diagnosis between AD and non-AD dementia.

4.Effects 'of β3 adrenoceptors on the contractility of rat thoracic aorta smooth muscle and the mechanism.

OBJECTIVETo observe the effect of β₃adrenoceptors (β₃-AR) activation on rat thoracic aorta smooth muscle contractility and the possible related mechanism.
METHODSThe endothelium removed thoracic aorta was pre-contracted with 30 mmol/L KCl physiological saline solution (PSS). Then the tension of the thoracic aorta was recorded in presence of BRL37344 (BRL) to determine the action of β₃-AR. The tension of the thoracic aorta was also recorded in the presence of Propranolol (PRA), SR59230A (SR), L-NNA, H-89 and Iberiotoxin (IBTX) respectively to reveal the underling mechanism of β₃-AR activation on rat vascular smooth muscle. Immunohistochemistry was adopted to confirm the existence and the distribution of β₃-AR in rat thoracic aorta.
RESULTSThe results showed that: (1) The thoracic aorta was relaxed by β₃-AR activation, with a relaxation percentage of (10.59 ± 0.79). (2) β₃-AR was expressed in both endothelial and smooth muscle layer in thoracic aorta sections of rats. (3) PRA did not block the effect of BRL on the thoracic aorta. The relaxation actions of BRL could be antagonized by pre-incubating the thoracic aorta with SR. (4) L-NNA (a NOS inhibitor) and H-89 (a PKA inhibitor) reversed the relaxation effect of BRL on vascular smooth muscle. (5) The effect of BRL was decreased after application of Ibriotoxin (IBTX), a large conductance calcium dependent potassium channel blocker.
CONCLUSIONThe results confirmed that activation of β₃-AR led to relaxation of thoracic aorta smooth muscle. The relaxation action of β₃-AR on smooth muscle of rat thoracic aorta was related to activation of NOS and PKA signaling pathway. Large conductance Ca²⁺-K⁺ channels were involved in the relaxation action of β₃-AR activation on rat thoracic aorta smooth muscle.

5.Characteristics of achalasia:a high-resolution manometry study

Objective To evaluate the clinical and manometric characteristics of achalasia.Meth-ods Patients diagnosed as having achalasia from July 2010 to July 2014 at our hospital were enrolled.High resolution manometry(HRM)results were analyzed.Data of Eckardt scale,MDADI and SF-36 were ana-lyzed.Results All subjects had dysphagia,35.6% (37 /104)accompanied with regurgitation,26.9%(28 /104)with heartburn and 17.3%(18 /104)with weight loss.According to HRM results and Chicago classification criteria,16.35%(17 /104)of the subjects were classified as type Ⅰ,76.92%(80 /104)as type Ⅱ and 6.73%(7 /104)as type Ⅲ.27.9%(29 /104),19.2%(20 /104)and 24.0%(25 /104)of the subjects finished Eckardt scale,MDADI and SF-36,respectively.Eckardt score was positively correlated with integrated relaxation pressure(IRP)(r =0.421,P <0.05)and MDADI physical score was negatively with IRP(r =-0.530,P <0.05).Conclusion Dysphagia often occurs as the chief complaint among acha-lasia patients.And type Ⅱ is the most common.IRP is an indicator of the severity of clinical symptoms and impairment of quality of life.
6.Efficacy of tolvaptan for delusional hyponatremia in decompensated liver cirrhosis

Objective To investigate the efficacy and safety of vasopressin receptor antagonist tolvaptan for treating dilutional hyponatremia casused by decompensated liver cirrhosis.Methods Ninety-six subjects with decompensated liver cirrhosis complicated by dilutional hyponatremia were divided into test group (n =56) and control group (n =40) by double blind method.Test group were treated with a single dose of tolvaptan 15 mg orally in addition to routine therapy,while control group were treated with routine therapy plus placebo.The changes in serum sodium concentration,liver functions,ascites,and edema of lower extremities between the two groups were observed.Measurement data were compared by t test,and categorical data were compared by chisquare test.Results On day 7,36 (64.3%) patients in test group and 9 (22.5%) patients in control group reached normal serum sodium concentrations (x2 =5.241,P<0.01).All the patients in test group and only 3 patients in control group had 24 hours' total urine volume above 3500 mL.Difference of 24 hours' total urine volume during 7-day treatment between the two groups was statistically significant (t=20.899,P<0.01).Thirty-nine patients in test group and 15 patients in control group had reduced ascites (x2 =4.260,P=0.039),but improvement of edema in lower extremities of both groups was comparable (12 cases in test group and 7 cases in control group; x2 =0.227,P=0.634).Serum alanine aminotransferase (ALT),total bilirubin (TBil),potassium and creatinine levels of test group were improved after treatment,but were not significantly different from either baseline levels (t=1.509,0.783,1.107,1.237; both P>0.05) or those of the control group (t=1.712,1.635,1.121,0.873; both P>0.05).Conclusions Single dose of tolvaptan (15 mg) exerts a strong diuretic effect and is able to greatly increase serum sodium concentration,thus has distinct therapeutic effect for patients with decompensated liver cirrhosis complicated by dilutional hyponatremia.
7.Effects of recombined rat insulin-like growth factor-1 gene with or without transforming growth factor beta-1 gene on osteoarthritis of rabbit knee in vivo

Objective To determine the therapeutic effect of recombined rat insulin-like growth factory 1 gene and transforming growth factor beta-1(TGF-?_1)gene on anterior cruciate ligament transection(ACLT)- induced osteoarthritis-like changes in NZW rabbit knee joints.Methods Eighteen NZW rabbits were divided into 3 groups randomly after osteoanhritis was established by ACLT and another six rabbits were used as normal control group(group 1).Chondrocytes which had been transfected with IGF-1 gene,co-transfected with TGF-?_1 and IGF-1 gene(group 3,4)were injected into the rabbits knee joints.Experimental control group(group 2)only had ACLT bul was not transfected.After 4,8 weeks,rabbits were sacrificed and their joints were evaluated by morphological grades,histological examination,in situ hybridization examination,immunohistochemistry exami- nation,and transmission electron microscopy examination(TEM).Results The morphological grades showed that the normal control group had a very significant difference with the experimental control group(P
8.Studies on the Bacterial Diversities of the Hot Springs at Tengchong Rehai in Yunnan Province China

Bacterial biodiversities of microbial mat and sediments, which were sampled from thermal springs of Tengchong Rehai in Yunnan, were preliminarily studied with PCR-denaturing gradient gel electrophoresis (PCR-DGGE). Directly extracted total DNA from environmental samples amplified by PCR with two sets of bacteria-specific primers. The PCR products, which include the V 8 and V 9 high-variable regions respectively, were analyzed by using DGGE. The DGGE profiles not only indicated the existence of higher levels of bacterial diversity, but also showed that the microbial mat and sediments have different dominant bacteria. Furthermore, the bacterial PCR-DGGE displayed clear profiles of bacterial structure selected by the key abiotic factors of the extreme environments, such as temperature and concentration of oxygen.

10.Rehabilitation of tongue cancer patients with dysphagia path action research

Objective To evaluate the action research method in the effect of swallowing disorder in patients with tongue cancer recovery path. Methods Based on the recovery path construction, according to the questions, plan, action, observation and reflection, improvement of summarizing the research process, through two stages of the research, assessment, diagnosis, planning, implementation, evaluation and comparison of stage 1 and stage 2 swallowing disorder in patients with rehabilitation evaluation, quality of life score, spirit to adapt to the score. Results Nearly 79.69% (51/64) of first phase swallowing rehabilitation effectively, and 93.75% (60/64) effectively in the second stage. Compared to the first stage,the second stage had an obvious increase. Two stages at the university of Washington Quality of Life Score, the first phase of (770.400 ±87.299) points, (1117.100 ± 43.153) points in the second stage, two stages of life quality score comparison, the difference was statistically significant (t=-19.500, P=0.012). The comparison of two stage patients mental adjustment scale scores, the first phase of (15.933±1.285) points, (31.733±2.083) points in the second stage, two stages score spirit to adapt to the comparison, the difference was statistically significant (t=-35.357, P=0.003). Conclusions Tongue cancer patients with swallowing disorder treatment on the basis of action study method to build and implement path specification, can improve the quality of care and quality of life of patients.
