1.Relation of visual field defect with pupil in patients with primary open angle glaucoma

Abstract?AIM:To evaluate the pupil light reflex in patients with primary open angle glaucoma, and to investigate the relation between pupil and visual field defect.?METHODS:From July 2014 to October 2015, 115 eyes in 86 patients with primary open angle glaucoma and 23 eyes in 16 healthy individuals were continuously enrolled in this study.All the subjects received comprehensive eye examination, visual field examination ( Humphrey, SITA Standard 24-2 ) and dynamic pupil measurement ( MonCV3 Metrovision) .According to the visual field and the Glaucoma Staging System, the patients with glaucoma were divided into 5 subgroups: stage 1, stage 2, stage 3, stage 4 and stage 5. The parameters of pupillary light reflex were as follows: pupil diameter ( minimum, maximum ) , latency and duration of contraction, latency and duration of dilatation, contraction amplitude, contraction and dilatation speed, and percent of pupil contraction ( PPC ). SPSS 19.0 statistical software was used to analyze the measurement results.?RESULTS:The control group significantly differed from the stage 4 subgroup ( P=0.032 ) and stage 5 subgroup (P=0.014) in terms of minimum pupil diameter; there was significant difference in the pupil contraction speed between groups ( F =648.675, P <0.01 ), and the contraction speed in stage 5 subgroup was significantly lower than those in the other subgroups and control group (P<0.05); the control group significantly differed from the stage 3, stage 4, and stage 5 subgroup in terms of PPC ( P<0.05 ).Pupil contraction speed, PPC and minimum diameter showed correlation with the stages of glaucoma.?CONCLUSION:Pupil contraction ability in patients with primary open angle glaucoma was impaired, and the degree of impairment is related with the degree of visual field defect.

Objective To explore the polysomnography(PSG) characteristice of obstructive sleep apnea hypopnea syndrome(OS-AHS) and primary snoring(PS) in children and clinical value of PSG in children with sleep disorders. Methods We analyzed 74 children with OSAHS and 62 with PS, every patient being monitored with PSG for 7 hours at night for 16 parameters, including apnea hypopnea index(AHI), periodic leg movement index(PLMI),and the lowest oxygen saturation(LSaQ2) etc. The parameters of the 2 groups were comparaed. There are many progresses in the therapy of MM with ASCT. ASCT improves the ratio of CR,EFS and OS,compared with conventional chemotherapy.Myeloma patients with renal failure or old age also can be candidate for ASCT.Tandem autologous transplantation improves the ratio of CR. Its impact to long-term EFS and OS still need to be judged.To improve conditional project,to choose suitable time,and- toselect consolidation schedule can boost therapeutic effect.
4.Clinical Observation of Puncturing Sifeng Points (EX-UE10) plus Pricking Back-Shu Points for Dyspepsia Due to Chemotherapy for Triple-negative Breast Cancer

Objective To assess the effectiveness of puncturing Sifeng points (EX-UE10) and pricking Back-Shu points in treating dyspepsia due to chemotherapy for triple-negative breast cancer (TNBC). Method Sixty patients were randomized into an observation group (30 cases) and a control group (30 cases). The observation group was intervened by puncturing Sifeng points and pricking Back-Shu points, once a week. The selected Back-Shu points included bilateral Pishu (BL20), Weishu (BL21) and Geshu (BL17). The control group was treated by promoting gastrointestinal motility (itopride hydrochloride 50 mg) and supplementing digestive enzymes (compound azintamide tablets). The two groups were observed before and after treatment in terms of traditional Chinese medicine (TCM) symptom score, nutritional status score and Karnofsky Performance Score (KPS). The therapeutic efficacies were also assessed. Result The total effective rate was 93.3% (28/30) in the observation group versus 70.0% (21/30) in the control group, and the between-group difference was statistically significant (P<0.05). The TCM symptom score showed significant improvement in both groups after treatment (P<0.01), and the improvement in the observation group was more significant than that in the control group (P<0.01). After treatment, the score of Patient Generated-Subjective Global Assessment (PG-SGA) decreased significantly in both groups (P<0.01), while there was no significant difference in the score between the two groups (P>0.05). The KPS score increased significantly in both groups after treatment (P<0.01), and there was a significant difference between the two groups (P<0.05), indicating a more significant improvement of KPS score in the observation group. Conclusion Puncturing Sifeng plus pricking Back-Shu points is effective in treating dyspepsia due to chemotherapy for TNBC. It can improve patient's appetite and quality of life.

7.Treating chronic persistent bronchial asthma children with abnormal myocardial enzyme spectrum by Yupingfeng powder: an efficacy observation.

OBJECTIVETo observe the clinical efficacy of treating chronic persistent bronchial asthma (CPBA) children with abnormal myocardial enzyme spectrum (AMES) by Yupingfeng Powder (YP) combined routine therapy.
METHODSFrom January 2010 to December 2012, 156 CPBA children patients with AMES were randomly assigned to the treatment group (80 cases) and the control group (76 cases). All patients received routine treatment (inhaled corticosteroids and/or leukotriene regulator). Besides, those in the treatment group took YP. The treatment duration was 3 months. The scores of children asthma control test (C-ACT), pulmonary function (FEV,% and PEF%), myocardial enzyme spectrum were observed before and after treatment, and 3 months before and after treatment. The myocardial enzyme spectrum of 40 healthy children at the baby clinics during the same period were recruited as the control.
RESULTSCompared with the control group, creatine kinase isoenzyme (CK-MB), creatine kinase(CK), and lactate dehydrogenase (LDH) increased in the two treatment groups (P <0.01), but there was no statistical difference in AST (P >0.05). Compared with before treatment in the same group, CK-MB, CK, LDH, and AST decreased in the treatment group after treatment and 3 months after treatment (P <0.01). CK-MB, CK, LDH, and AST decreased in the control group 3 months after treatment (P <0.01, P <0.05).Compared with after treatment, CK decreased in the control group 3 months after treatment (P <0.01). C-ACT score, FEV(1),%, and PEF% all increased in the two groups after treatment and 3 months after treatment (P <0.01, P <0.05). Compared with after treatment in the same group, CK decreased in the control group 3 months after treatment (P <0. 01). Compared with the control group in the same period, post-treatment CK-MB and CK decreased (P <0. 01, P <0. 05), while post-treatment C-ACT score, FEV, %, and PEF% increased (P <0.05) in the treatment group (P <0.05).
CONCLUSIONYP could strengthen specific and non-specific immunity of the organism, and improve clinical symptoms and the level of myocardial enzyme spectrum.

9.The effect of different intraocular irrigating solutions on lens opacity after pars plana vitrectomy for proliferative diabetic retinopathy

AIM: To explore the effect of different intraocular irrigating solutions on lens opacity after pars plana vitrectomy(PPV)for proliferative diabetic retinopathy(PDR).
METHODS: Totally 48 subjects(48 eyes)patients with proliferative diabetic retinopathy from March 2018 to February 2019 were enrolled in this study, randomly divided into BSS intraocular irrigating solution group(23 cases and 23 eyes)and Shike intraocular irrigating solution group(25 cases and 25 eyes). LOCS grading system Ⅲ was applied to grade lens opacity. Pentacam occipital analyzer was applied to analyze the density and thickness of lens, and the results were compared and discussed.
RESULTS:Three months after PPV,NC, P score and the density of lens in LOCSIII grade in the two groups were significantly higher than those before surgery(P<0.05), but the C score was not different significantly(P>0.05). In addition, the thickness of the lens in BSS intraocular irrigating solution group was significantly higher than that before PPV(P<0.05), but there was no significant difference in the Shike intraocular irrigating solution group(P>0.05). NC, P score, the density and thickness of lens were significantly lower in Shike intraocular irrigating solution group than those in the BSS intraocular irrigating solution group. But the C score was not different significantly between the two groups(P>0.05).
CONCLUSION: Shike intraocular irrigating solution can delay the development of postoperative complicated cataract formation.
10.Clinical and renal pathologic analysis of acute interstitial nephritis in 51 children

Objective:To discuss the etiology, clinical manifestations and renal pathological features of acute interstitial nephritis (AIN) in children.Methods:The etiology, clinical manifestations, pathological characteristics, clinical effects and outcome of the children with AIN diagnosed by renal biopsy from January 2010 to December 2019 in Nanjing Jinling Hospital were analyzed retrospectively. The Kaplan-Meier method was used to evaluate the kidney survival rate. Cox regression model was built to analyze the risk factors for developing end-stage renal disease (ESRD) at baseline in AIN children.Results:A total of 51 children with AIN were diagnosed by renal biopsy, including 36 males and 15 females. The age was (12.94±2.55) years old (2-17 years old). The clinical manifestations of AIN in children were various and lack of specificity. Only 2 cases (3.92%) had triad. All of the 51 children with AIN showed acute renal injury (AKI), accompanied by increased serum creatinine and decreased estimated glomerular filtration rate. The stage of AKI was mainly stageⅢ(33 cases, 64.71%). Infection was the main cause (38 cases, 74.51%) and drug factor was the second cause (27 cases, 52.94%) in children with AIN. Nonsteroidal antiinflammatory drugs (NSAIDs) were the main inducers of drug-induced AIN (18 cases, 35.29%). The interstitial inflammatory cell infiltration or interstitial edema was found in 51 children. The infiltration of inflammatory cells was mainly mononuclear cells (46 cases, 90.20%). After 4 weeks of treatment, 32 cases (62.75%), 11 cases (21.57%) and 8 cases (15.69%) showed complete, partial and no recovery of renal function, respectively. After 12 weeks of treatment, 49 cases (96.08%), 0 cases (0) and 2 cases (3.92%) showed complete, partial and no recovery of renal function, respectively. After an average follow-up of 4.0(2.0-15.0) months, 2 case (3.92%) patients developed ESRD. The cumulative survival rates of ESRD at 1 year and 2 years after renal biopsy both were 100%, and renal survival rates at 5 years and 10 years were 96.55% and 72.41%, respectively. Multivariate Cox regression analysis results showed that N-acetyl-β-D-glucosidase (NAG) enzyme level>17.6 U/g·cr ( HR=15.729, 95% CI 1.045-15.977, P=0.042) and IgM deposition in renal tissue ( HR=7.523, 95% CI 1.142-9.541, P=0.033) were independent risk factors for developing ESRD in AIN children. Conclusions:AKI is the main clinical manifestation of AIN in children. The characteritic of renal pathology in AIN is tubulointerstitial lesions. After active treatment, most of the patients have a good prognosis. Prevention of infection and rational use of drugs are the key to reduce the incidence rate of AIN in children. The N-acetyl-β-D-glucosidase enzyme level>17.6 U/g·cr and IgM deposition in renal tissue are independent risk factors for developing ESRD in AIN children.
