1.Prognostic Analysis of High Intensity Focused Ultrasound Application in Primary Hepatocellular Carcinoma

Objective To assess the survival of patients receiving high intensity focused ultrasound(HIFU) and investigate the prognostic factors for primary hepatocellular carcinoma(PHCC) victims with HIFU application.Methods One hundred and eighty-seven patients with PHCC undergoing HIFU treatment in our department were enrolled into this study from June 2004 to June 2007.Among them,101 patients were males and 86 were females(mean age: 47.7 years old,range: 19-79 years old).The average tumor size was 5.7 cm(range: 0.5-18.0 cm).Of these 187 patients,numbers according to Child-Pugh grade of A,B and C were 104,52 and 31,respectively.According to TNM system,45,111 and 31 patients were in stage Ⅱb,Ⅲa and Ⅲb respectively.Kaplan-Meier model and log-rank test were used in univariate analysis and Cox regression model was used in multivariate analysis to identify prognostic factors for survival.Results Survival period was(17.3?2.5) months after HIFU treatment of PHCC.The overall survival rate of 3-month,6-month,1-year and 2-year were 79.1%,60.1%,35.7%,and 29.3%,respectively.It was significant that tumor number(P=0.02),size(P=0.04),AFP(P=0.04),Child-Pugh grade(P=0.00),TNM stage(P=0.01),tumor metastasis(P=0.03) before HIFU,and tumor recurrence after HIFU(P=0.02) and standard treatment(P=0.02) were prognostic factors by single factor analysis.The following factors were identified as independent prognostic factors for overall survival by multivariate model: standard treatment protocol(P=0.000),and TNM stage(P=0.004) and Child-Pugh grade(P=0.009) before HIFU.Conclusion It is used for improving overall survival rate to found PHCC early,protect liver function,examine comprehensively before HIFU treatment,focus on standard treatment and auxiliary treatment.
2.The practice and thinking of curriculum construction on patient safety in undergraduate medical education

To strengthen the medical students in patient safety education is the first step to effectively prevent medical accidents.But at present patient safety education has not been given enough attention by domestic mnedical colleges.Research group consulted literature at home and abroad related to patient safety courses, according to the Medical colleges patient safety curriculum guide proposed by The World Health Organization (WHO) Alliance for Patient Safety, opened the patient safety course in undergraduate medical education with the goal to improve the medical students to maintain patient safety consciousness, ability and methods and implemented scene simulation teaching and formative assessment.We achieved good teaching effect and appraisal by students.
3.Reoperation of Laparoscopic Common Bile Duct Exploration in the Management of Choledocholithiasis

Objective To investigate the feasibility and safety of reoperation of laparoscopic common bile duct exploration in the management of choledocholithiasis in patients with biliary surgical history. Methods Thirty-one patients with choledocholithiasis and biliary surgical history were performed by reoperation of laparoscopic common bile duct exploration between January 2012 and January 2014 in our hospital.The adhesion was disconnected under the laparoscopy, and then the common bile duct was opened to introduce an endoscope for stone removal.A T-tube was placed for drainage. Results The operations were successfully completed in all the 31 cases, without conversion to open surgery.The vital signs of those patients were smooth during the operation.The operation time was 65-125 min, with a mean of (118 ±21) min.The volume of blood loss was 10-75 ml during operations, with a mean of (39 ±19) ml.The patients could take food normally 24-48 h after operation, with a mean of (27 ±13) h.The drainage tubes were pulled out 48 -72 hours after operation.No severe complications, such as residual stones, bleeding, gastrointestinal leakage, or pancreatitis happened during hospitalization, except for 1 case of biliary leakage and 1 case of incision infection.The patients stayed in hospital for 6-9 days after operation, with a mean of (7.0 ±1.5) days.No biliary stricture or recurrence was found during a 3-24 months of follow-up (mean, 15.6 months) in 31 cases. Conclusion Reoperation of laparoscopic common bile duct exploration by experienced laparoscopic specialists for choledocholithiasis in patients with biliary surgical history is relatively safe and effective.
4.The Expression of MMP-2 in EAM and the Effects of Methylprednisolone

Objective To investigate the effect of matrix metalloproteinase(MMP-2) in the attack of exprimental autoimmune myositis(EAM) rats, and the effects of methylprednisolone on expressions of gene mRNA and protein of MMP-2. Methods The expression levels of MMP-2 in the peripheral blood, spleen lymphocytes and muscles were detected by using RT-PCR and immunohistochemistry techniques and was compared among EAM group,EAM with methylprednisolone treatment group (EAMM) and control group. Results ⑴ The degree of episode in EAMM group was lower than that of EAM group , and the infiltration of the inflammatory cells and the necrosis of the muscles were also mild in EAMM group. ⑵There was significant upregulation of the expression of the MMP-2 gene mRNA in the lymphocytes of peripheral bood and spleen of EAM rats as compared with that of the control,Elevation of the expression of MMP-2 protein in rats muscles tissues of EAMM group was observed obviously. However, the expression of the mRNA and the protein of MMP-2 was suppressed significantly in EAM group compared with control group. Conclusions The upregulation of the expression of MMP-2 may be correlated with the onset of EAM. Methylprednisolone may relieve the degree of the pathology of EAM by the downregulation of MMP-2 .
5. Electroacupuncture Improves Limb Locomotor Function Possibly by Suppressing Rho-ROCK Ⅱ Pathway Related Factors in Anterior Horns of Spinal Cord in Rats with Acute Spinal Cord Injury

OBJECTIVE: To observe the effect of electroacupuncture (EA) of "Jiaji" (EX-B 2) on limb locomotor function and expression of Ras homolog gene family member A (RhoA), Rho-associated kinase Ⅱ (ROCK Ⅱ) and myosin light chain (MLC) proteins in the anterior horn of spinal cord in acute spinal cord injury (ASCI) rats, so as to explore its mechanisms under-lying improvement of SCI-induced limb locomotor dysfunction. METHODS: Forty-eight female Wistar rats were randomly divided into sham operation (sham), ASCI model (model), EA EX-B 2 (EA) and ROCK inhibitor (Fasudil) groups which were further divided into 14 d and 28 d subgroups (n=6 in each). The ASCI model was made by using weight drop striking method. Three hours after modeling, EA (100 Hz, 0.4, 0.6 mA) was applied to EX-B 2 (T 9, T 11) for 30 min, once daily for 14 d and 28 d, respectively. The ROCK inhibitor (hydrochloride Fasudil, 10 mg/kg) was administrated by intraperitoneal injection immediately after modeling, once a day, continuously for 14 d or 28 d. The expression of RhoA, ROCK Ⅱ and MLC proteins in the spinal cord anterior horn tissue (T 10) was detected by immunohistochemistry. The rats' hindlimb locomotor function was assessed according to Basso, Beattie and Bresnahan (BBB) locomotor rating scale (21-points). RESULTS: After ASCI, the BBB scores were significantly lower in the model group than in the sham group on day 14 and 28 (P<0.05), and obviously higher in the EA and inhibitor groups than in the model group (P<0.05), suggesting an improvement of the hindlimb locomotor function after EA intervention or suppression of ROCK. Immunohistochemical results indicated that the numbers of RhoA, ROCK Ⅱ and MLC immune-reaction positive cells in the anterior horn of spinal cord were significantly more in the model group than in the sham group (P<0.05), and remarkably decreased in both EA and inhibitor groups on day 14 and 28 relevant to the model group (P<0.05). The therapeutic effects of EA were markedly weaker than those of inhibitor Fasudil in up-regulating BBB score and down-regulating the number of RhoA, ROCK Ⅱ and MLC positive cells (P<0.05). CONCLUSION: EA of EX-B 2 can improve the hindlimb locomotor function in ASCI rats, which may be associated with its effect in down-regulating the expression of RhoA, ROCK Ⅱ and MLC proteins (i.e., inhibiting the RhoA/ROCK signaling pathway) in the anterior horn of spinal cord.

8.Surveillance analysis of drinking water-born endemic fluorosis in 2009 in Xuchang city, Henan province

ObjectiveTo investigate the status of an illness of endemic fluorosis in Xuchang city,and to evaluate the effect of implementation of control measures.MethodsIn 2009,Changge city and Xuchang county were chosen as survey counties(city).We investigated the surveillance counties (city) about the running state of water-inproving projects,and fluoride content of drinking water in the villages,and ten villages were chosen as survey villages,dental fluorosis and fluoride content of children aged 8 to 12,clinical skeletal fluorosis and urinary fluoride content of 16-year-old adult were investigated.ResultsIn all the villages with projects of defluoridation for drinking water of the two monitoring counties,that the projects was being used accounted for 61.39%(97/158),and the projects discarded accounted for 38.61%(61/158).In the 9 non-water-improving villages,the average water fluoride ranged 1.32 - 2.25 mg/L; in the 1 water-improving village,the mean water fluoride was 0.44 mg/L.The total rate of dental fluorosis of children aged 8 to 12 was 38.65% (80/207),and the geometric mean of urinary fluoride was 1.65 mg/L.The rate of adult skeletal fluorosis by clinical detection was 0.30% (9/2982),and the geometric mean of urinary fluoride was 2.09 mg/L.ConclusionsXuchang city is a mild to moderate fluorosis epidemic areas of endemic fluorosis,and condition of the disease has not yet been effectively controlled,and there is need to further intensify the prevention and control efforts.
9.The Screen of Plant Endophytic Actinomycetes Producing ?-1,3-Glucanase and Antifungal Activity of ?-1,3-Glucanase

Using of clearing zones on pachyman agar medium, there are 217 plant endophytic actinomycetes, producing ?-1,3-glucanase were screened. 45.6% of the strains produced ?-1,3-glucanase, in which the strains from cucumber are up to 38. The percentages of endophytic actinomycetes from different hosts produceing ?-1,3-glucanases were different. The percentage of the strains in Rhizoma Polygonatum produced ?-1,3-glucanases is the highest, up to 88.9%. The Inhibited effects of plant endophytic actinomycetes which produced extracellular ?-1,3-glucanases on mycelium growth of Sclerotinia sclerotiorum were detected in vitro. Cucumber endophytic actinomycete gCLA4 strain was screened out from 99 isolates, which can strongly inhibit the growth of S. sclerotiorum. The optimal ?-1,3-glucanases fermentation conditions of strain gCLA4 were investigated, they were pachyman 0.2%, peptone as nitrogen, pH 7~8 for 5 days. The ?-1,3-glucanases of strain gCLA4 had some inhibiting efficiency on 13 plant pathogens, in which inhibiting efficiency to Botryosphaeria dothidea was the strongest.
