1.Diagnosis and Operative Treatment of Spinal Tumor

Objective To explore the diagnosis and operative treatment of spinal tumor. Methods Clinical symptom, diagnosis and operative treatment of 22 patients with spinal tumor in our institution from August 1987 to August 2002 were analysed retrospectively. Results Combining with patients' clinical symptoms and results of corresponding examination, all cases were diagnosed before operation. After operation, all the patients recovered with different outcome except 3 patients died. There were 18 cases nearly cured. Conclusion The clinical symptoms of the patients with spinal tumor have some specialty. The preoperative diagnosis mainly depends on MRI. The various operative modes should be taken according to tumor localization.
2.Upregulated expression of plasticity-related gene 1 protein in cerebral cortex in a rat model of recurrent neonatal seizures

Objective To explore the dynamic expression of a new phospholipid phosphatase, plasticity related gene 1 ( PRG-1 ) in cerebral cortex following recurrent neonatal seizures.Methods A seizure was induced by inhalant flurothyl daily in neonatal Sprague-Dawley rats from postnatal day 6 (P6).Rats were assigned into the recurrent-seizure group (seizures induced in six consecutive days) and the control group.At 3 h,12 h,48 h,14d after the last convulsion,PRG-1 protein level in cerebral cortex was detected by western blot method.Results At 3 h, 12 h,48 h, 14d after the last convulsion in control group, the expression of PRG-1 in cerebral cortex respectively was 1.363 ± 0.742,1.278 ± 0.687 ,0.763 ± 0.374,1.004 ± 0.113, in experimental group , the expression of PRG-1 in cerebral cortex respectively was 1.818 ± 1.093,1.562 ± 0.782,1.024 ± 0.510,1.378 ± 0.279.At 3 h, 12 h,48 h, the expression of PRG-1 in cerebral cortex was not significantly different between the experimental and control group( t = 0.843,0.668,1.011 ,all P > 0.05 ).However, at 14 d, the level of PRG-1 protein in cerebral cortex of experimental group was significantly higher than that of control ( t = 3.041, P < 0.05 ).Conclusion The up-regulated expression of PRG-1 in cerebral cortex may be associated with the recurrent neonatal seizure-induced brain damage.
3.Radiographic evaluation of the effect that local injection of platelet-rich plasma enhances bone consolidation

Objective In this study, we tested the ability of platelet-rich plasma to enhance bone consolidation during distraction osteogenesis in a rabbit model of leg lengthening. Methods Twenty New Zealand white rabbits, which were randomly divided into two groups, the PRP treated group and the control group (n=10). Osteotomy will be made under the tibiofibular junction of left tibia, and 1cm tibia shaft will be removed through a second osteotomy. The tibia will be shorted for 1cm and then fixed by Orthfix unilateral lengthener with 4 pins. Lengthening will start 7 days after the osteotomy surgery, at a rate of 1.0 mm/day, in two steps, for 10 days. 500ul PRP were respectively injected into the distraction gaps at the end of the 10-day lengthening and at 10 days before termination. No management in the control group. The animals were terminated at day 20 following final lengthening. Serial radiographs will be taken at the day of surgery, day 12, day 17, day 27, day 37, using a C-arm radiography system. The excised bone specimens were scanned by X-ray and Micro-CT, and the bone mineral content(BMC) and the bone mineral density(BMD) of the regenerates were calculated and compared. Results The regenerate of PRP treated group was significantly better than the control group. The X-ray figures showed that the regenerate was significantly more than the control group. The figures of micro-CT 3D reconstruction showed that the growth and remodel of the regenerate in PRP treated group was better than the control group. Micro-CT examination of regenerates demonstrated a significantly greater bone mineral content(BMC) and the bone mineral density(BMD) in the PRP treated group compared to the control group(P
4.Efficacy analysis of four antiviral solutions for chronic hepatitis B treatment

Objective To investigate the efficacy of four antiviral solutions for chronic hepatitis B (CHB) treatment .Methods 240 cases of patients with CHB were selected ,divided into four groups (group A ,group B ,group C and group D) ,and treated with lamivudine ,adefovir dipivoxil ,adefovir dipivoxil/lamivudine ,and entecavir ,respectively .The recovery rates of ALT and AST ,HBV DNA and its negative conversion rates ,and HBeAg/HBeAb conversion rates of the four groups were compared respectively after 12 ,24 and 48 weeks treatment .And the YMDD mutation rates were compared after 48 weeks treatment .Results The recovery rates of ALT and AST ,HBV DNA and its negative conversion rates ,HBeAg/HBeAb conversion rates ,HBeAg/HBeAb conversion rates of group C and group D were significantly better than those of group A and group B (P<0 .05) .Conclusion The antiviral so‐lutions of adefovir dipivoxil/lamivudine and entecavir are two kinds of rational treatment .
5.The in vitro anti-atherosclerotic activity of compound E0869.

ATP-binding cassette transporter A1 (ABCA1) and scavenger receptor class B type I (SR-BI/CLA-1) are the key proteins in reverse cholesterol transport (RCT). The high expression of ABCA1 and SR-BI/CLA-1 can decrease the danger of atherosclerosis. The purpose of the study is to find ABCA1 and CLA-1up-regulators for treating atherosclerosis by using cell-based high throughput screening models. Among 20 000 compounds screened, E0869 [1-(3, 4-dimethylphenyl)-1-oxopropan-2-yll4-((methylsulfonyl)methyl)benzoate] was found as the positive hit. The up-regulated activities of E0869 in ABCAl1-LUC and bCA-l1-LUC HepG2 cell were 160% and 175%, respectively. The EC50 values of E0869 in ABCAl1-LUC and CLA-l1-LUC HepG2 cell were 3.79 and 1.42 pμol- x ,(-1) respectively. E0869 could upregulate the mRNA and protein levels of ABCA1, SR-BI/CLA-1 and ABCGJ1genes in HepG2 and RAW264.7 cells by Real-Time Quantitative PCR and Western blotting analysis, but could not influence the expression of FAS, SREBP-l1 and CD36. Foam cell assay showed that E0869 could inhibit lipids accumulation in mouse peritoneal macrophages RAW264.7. Cholesterol efflux assay showed that E0869 could induce HDL-mediated cholesterol efflux in mouse peritoneal macrophages RAW264.7. In conclusion, E0869 could up-regulate ABCA1 and CLA-1 activity, and had good anti-atherosclerotic activity in vitro.

7.The in vitro anti-atherosclerotic activity of compound IMB-1680.

In the previous study, a high-throughput screening method was established to find the antagonists of CD36. In the present study, a new compound named IMB-1680 was found using this method. The anti-atherosclerotic activities of IMB-1680 were then evaluated. Dose-dependent activities of IMB-1680 were detected by using Sf9 [hCD36] and CHO [hCD36] models. Fluorescence microscopic photography and flow cytometry were used to analyze uptake of mLDL. Foam cell test with RAW264.7 macrophages was used to examine lipid accumulation. The results showed that IMB-1680 inhibited CD36 activity with IC50 of 2.80 and 8.79 micromol x L(-1) in Sf9[hCD36] and CHO [hCD36] cells, respectively. Fluorescence microscopic photography and flow cytometry revealed that IMB-1680 could significantly reduce DiI-AcLDL uptake. Meanwhile, IMB-1680 also could reduce lipids accumulation in RAW264.7 macrophages. In all, the data indicated that IMB-1680 might be a potent effective anti-atherosclerotic leading compound.

8.A case report of adult onset of primary hemophagocytic syndrome with literature review.

OBJECTIVETo introduce the clinical manifestations and laboratory tests of adult onset of primary hemophagocytic syndrome (HPS), and to investigate the essentials of diagnosis and the genotype characteristics in adult onset patient.
METHODSThe definite diagnosis of HPS was made according to HLH-2004. Exons of PRF1, STX11, UNC13D, SH2D1A and RAB27A genes coding region were amplified using polymerase chain reaction.
RESULTSA 48-year-old man was admitted to our hospital because of recurrent fever, pancytopenia and lymph node enlargement. His laboratory test revealed bone marrow hemophagocytosis, elevated ferritin level (2000 µg/L), reduced level of NK cell activity (20.13%) and elevated soluble CD25 level (12277 U/ml). Based on the HLH-2004 diagnostic criteria, the patient was diagnosed as HPS. The patient had viral infection, and no other primary disease was identified that would cause HPS. The patient responded poorly to anti-viral therapy. DNA sequencing was used to confirm that perforin gene mutations might be one of the causes of the patient suffered from primary HPS.
CONCLUSIONSAlthough primary HPS usually affects infants and young children, it also occurred in teens and adults. It is essential to perform genetic screenings to patient whose illnesses recur with unknown causes. In addition, detection of molecular genetic alterations can be used to distinguish primary HPS from acquired HPS.

9.Correlation of Serum Insulin and Chronic Kidney Disease and Its Application Value

Objective To study serum insulin level change and its clinical significance in the patients with chronic kidney dis-ease (CKD).Methods 800 cases of patients with chronic kidney disease were observed with 400 cases of healthy people as control group at the same time.According to whether there was a complication of diabetic,chronic nephropathy was divided into two groups:diabetic nephropathy chronic renal failure group (425 cases)and non-diabetic nephropathy chronic renal failure (375 cases).According to the classification standard,chronic renal failure and the diabetic nephropathy group was di-vided into four groups:type 1 diabetes (105 cases),type 2 diabetes (135 cases),gestational diabetes (95 cases),diabetes with other reasons(90 cases).Relevant clinical information was collected,including age,gender,height,weight,and the cor-responding body mass index (BMI)was calculated based on height and weight.After some laboratory examinations,related parameters of the research were collected,including object of urea(Urea),serum creatinine (SCr),fasting plasma glucose (FPG),fasting insulin (FIns).According to the corresponding formula,insulin sensitive index (ISI)and glomerular filtration rate (GFR)were calculated based on the above parameters.Statistical comparison was made after collection.Results Urea, Scr,FPG and FIns of the patients with chronic kidney disease were significantly higher than those of normal control group (t=36.788,35.612,137.216,9.294;all P=0.000 respectively).Age,gender and BMI calculatedfrom height and weight in three groups had differences,but they were not statistically significant (F=1,363,P=0.256;F=1.577,P=0.454;F=1.641,P=0.194),and had no effect on the occurrence of chronic kidney disease development.Comparison of patients with different types of diabetes and chronic kidney disease,when a group had higher FIns and lower ISI,it also had a smaller GFR,and the differences between groups were statistically significant (F=12.01,P=0.000;F=3.891,P=0.009;F=3.774,P=0.011).Conclusion The serum insulin were closely related to CKD.Observing the serum insulin can have a dee-per understanding of the disease development,make a more effective treatment and predict the probability to be recover.
10.Temperature variation during percutaneous radiofrequency ablation treatment for vertebral tumor:an in vivo study in experimental rabbits

Objective To investigate the temperature variation within intra-spinal canal and intra-spinal tumor during percutaneous radiofrequency ablation (RFA) procedure for vertebral tumor in experimental rabbits. Methods Eight New Zealand white rabbits were transplanted with VX2 carcinoma in the lumbar vertebral body by percutaneous puncture inoculation technique under CT guidance in order to set up vertebral tumor models. The eight vertebral tumor models were treated with RFA under CT guidance. The temperature within the spinal canal and vertebral tumor of rabbits was measured and recorded every 30 seconds during the RFA treatment. The results were statistically analyzed by paired sampled t text. Results The intra-tumor temperature rose to 90℃ rapidly and remained stable during the whole RFA procedure, whereas the temperature in the spinal canal exceeded 42℃ when treatment time was over three minutes during the procedure. Statistically significant difference in the temperature level during RFA existed between the spinal canal and the vertebral tumor (P < 0.05). Conclusion The temperature in the vertebral tumor of rabbit can quickly reach to the therapeutic level during RFA. Prolonging operative time of RFA may hurt the nerve due to high temperature.
