2.Clinical study of Basen's effect on insulin resistance in type 2 diabetic patients

Objectives:To study the effect of Basen on the sensitivity of insulin in diabetes mellitus.Methods:Blood glucose,lipoprotein,insulin,C-peptide were assayed after and before the administration of Basen.Results:Levels of insulin,blood glucose,and C-peptide were lowered after four weeks administration of Basen,which changed contrarily to ISI.Conclusions:Basen has effects not only on lowering blood glucose but also on improving the insulin resistance.
3. Investigation and analysis of clinical application of rivaroxaban

OBJECTIVE: To investigate the clinical application of rivaroxaban and analyze the rationality, effectiveness, safety and drug interactions of its clinical application, so as to provide a basis for regulating its clinical application. METHODS: A summary of guidelines or consensus of clinical application experts at home and abroad was made to form a rationality evaluation standard for rivaroxaban. A retrospective survey was conducted on the use of rivaroxaban in all inpatient departments in a hospital in the first quarter of 2018, and the utilization rate of each department was calculated. Through the inclusion criteria, the cases that meet the requirements are analyzed for rationality, efficacy, adverse reactions and drug interactions. RESULTS: In the first quarter of 2018, there were 20 wards and 1 117 cases use of rivaroxaban. The highest utilization rate was the osteological surgery ward, which was mainly used for the prevention of postoperative thrombosis in orthopedics. In the clinical application of rivaroxaban, the prophylaxis or treatment of thromboembolism is mainly performed according to the instructions. The case of irrational rate was 27.14% all 1 028 cases of patients, with insufficient dose, lack of treatment, long course of treatment as the main reason for unreasonable, need to take active intervention measures. The clinical treatment and safety of rivaroxaban were better, with an ineffective rate of 1.17% (12/1 028) and an adverse reaction rate of 9.63% (99/1 028), of which 74.58% (88/118) belonged to hemorrhagic adverse reactions. CONCLUSION: Rivaroxaban is widely used for clinical anticoagulant therapy or prevention, and has good curative effects, but there are irrational usage, especially in terms of dose. During using, it is necessary to pay close attention to hemorrhagic-related adverse reactions. Clinical pharmacists should pay active attention to the update of the clinical application guidelines for drugs and actively communicate with doctors to jointly promote the rational use of rivaroxaban.
4. Warfarin application research and analysis in patients with non-valvular atrial fibrillation

OBJECTIVE: To understand the application of warfarin hospitalized patients with non-valvular atrial fibrillation, summarize the clinical experience and provide reference for clinical medication, and explore the significance of clinical pharmaceutical care for warfarin. METHODS: The information of 726 cases with non-valvular atrial fibrillation hospitalized in the first cardiovascular ward from January 2012 to December 2013 were retrospectively collected and analyzed in the aspect of types of anticoagulant solution, warfarin dosage and INR monitoring. RESULTS: The anticoagulation solution commonly used in our patients with atrial fibrillation included dual antiplatelet therapy, aspirin antithrombotic therapy and warfarin anticoagulant therapy. 13.8% of the cases had initial INR of 2.0-3.0, and 43.4% of the cases attained INR target after dosage adjustment. CONCLUSION: At present the clinical application of warfarin still exists deficiencies. It is necessary to continue to raise people's awareness of warfarin and atrial fibrillation anticoagulant. Active clinical pharmaceutical care for warfarin is of great significance.
5.Comparison between Truview~(TM)EVO_2 optic laryngoscope and Macintosh laryngoscope in patients receiving cervical vertebral surgery

Objective:To evaluate the clinical value of Truview~(TM)EVO_2 optic laryngoscope by comparing it with the Macintosh laryngoscope in patients receiving cervical vertebral surgery.Methods:One hundred patients scheduled for elective cervical vertebral surgery were enrolled in this randomized crossover study.After induction,the patients'glottis in group A (n=50)was displayed by Macintosh laryngoscope and the Cormack-Lehane(C/L)grade was recorded,and then optic laryngoscope was employed to display the laryngeal structure.The order of laryngoscopy attempts was reversed in group B(n= 50).Parameters recorded included demographics,airway assessment feat ures(BMI,thyromental distance,mandibular size,mouth opening,mallampati oropharyngeal scale,and neck movement),C/L grade,laryngoscopic force applied,duration of intubation, difficulties of laryngeal view and injury of upper airway.Results:There were no significant difference in demographics,airway assessment features,C/L grade and duration of intubation between the 2 groups,whereas the laryngoscopic force in group A was significantly lower than that in group B(P

7.EXTRACTION OF DNA FROM SOIL FOR PCR AMPLIFICATION

In this paper, five different methods were carried out for DNA extraction directly from soil. The result shows that all five methods could generate DNA with more than 15 kb in size. They were subsequently used as templates for PCR amplification with success, using primers of the bacterial 16S rRNA gene and Shiva-1 gene encoding an antibacterial peptide. However, method 5 is more suitable for DNA extraction directly from a small amount of soil sample as it produced a good yield of DNA in high integrity with reliable reproducibility.

The type of basic media and the contents of plant growth substances were investigated by orthogonal design experiment,and also the effects of different culture conditions on the growth of suspension cells and the accumulation of total flavonoids in Eucommia ulmoides were studied.The results showed that B5 medium supplemented with 0.5mg/L NAA,0.6mg/L 6-BA and 30g/L sucrose,at initial pH 5.0~5.5,20g(FW)/L inoculation quantity and 110 r/min of rotation speed was a preferable culture conditions for E.ulmoides suspension cells growth and flavonoids synthesis.The results of metabolic kinetics analysis for E.ulmoides cell suspension culture showed that the logistic and Luedeking-Piret equations can be used for describing the kinetics of cell growth,sucrose consumption and flavonoids production during the process.The maximum specific growth rate(?m),the actual growth yield based on sucrose(YG) and maintenance coefficient(m) were 0.417/d,0.619g/g and 0.0206g/(g?d-1) respectively.All these outcomes could give a basis for establishing the suspension cell culture of E.ulmoides and production of the natural active components in large-scale.

Objective To study the mechanisms of extrarenal arterial blood supply of renal malignancy for its interventional therapy.Methods Routine abdominal aortography and selective questionable feeding arteriography were performed in 141 patients with renal malignancy.The characteristics and formation mechanisms of extrarenal arterial blood supply for renal malignancy were analyzed.Results Of the 141 patients,extrarenal arterial blood supply of renal malignancy were found in 51 patients and there were 87 branchs.The breakthrough of renal capsule with malignancy were found in those 51 patients.No extrarena]arterial blood supply of renal malignancy was found in 90 patients,including 50 patients with and 40 patients without the renal capsule breakthrough with malignancy.The emerge of extrarenal arterial blood supply of renal malignancy were significantly different(x~2 = 31.64,P
10.Detecting Hepatitis B Virus Deoxyribonucleic Acid Gene in Breast Milk by Nested Polymerase Chain Reaction

Objective With the method of detecting hepatitis B virus deoxyribonucleic acid(HBV-DNA) gene in breast milk,in ordered to diagnose whether there is HBV in breast milk in those mothers whose HBsAg are positive.Methods Collected breast milk from 187 mothers whose HBsAg were positive,and complified HBV-N gene in breast milk by Nested-Polymerase chain reaction (Nested-PCR).Results Nested-PCR could detect HBV-DNA in breast milk,and the positive rate was 3.2%.Conclusion The method of detecting HBV-N gene in breast milk by Nested-PCR can detect HBV-DNA in breast milk,and it can be an laboratory evidence for whether breast feeding or not.
