1.The relationship of visual prognosis and G11778A point mutation in Leber hereditary optic neuropathy

Background Leber hereditary optic neuropathy (LHON) is a disease characterized by maternal inheritance.A number of mitochondrial DNA (mtDNA) mutation has been thought to be associated with this disease.Objective This study was to investigate the clinical and molecular genetic properties of LHON in two Chinese families.Methods Forty subjects from two Chinese families with LHON were enrolled in Affiliated First Hospital of Zhengzhou University,including 28 maternal members (10 of these members are LHON and 12 controls from two families.All the participants had a complete ophthalmic examination including visual acuity,direct ophthalmoscopy,color sensation and visual evoked potentials.MtDNA was extract from the whole blood sample of all participants.PCR-DNA sequencing was performed to detect the point mutation of the G11778A,T14484C,and G3462A for each subject.Written informed consent was obtained from each subject prior to this study.Results Only G11778A point mutation was identified in all 28 maternal members from the two families.No point mutation of G11778A was identified in non-maternal members,and no point mutation of the T14484C and G3462A were found in the two families.Conclusions The inherited pattern of these two families shows typical clinical and genetic features of LHON.LHON patients with G1 1778A mutation have a poor prognosis of visual acuity.
2.Influence of Nimodipine on [Ca~(2+)]i Levels in Red Blood Cell in Newborn Infants with Hypoxic-Ischemic Encephalopathy and Its Clinical Significance

Objective To explore change of RBC[Ca2+]i levels in neonates with hypoxic-ischemic encephalopathy(HIE)and the influence of nimodipine on RBC[Ca2+]i and its clinical significance.Methods Fifty-eight neonates with moderate and severe HIE were randomly divided into 2 groups including routine treatment group(n=28)and nimodipine group(n=30),and 20 healthy full-term neonates were selected as healthy control group.Based on the routine treatment,nimodipine[2 mg,0.5-1.0 ?g/(kg?min)] was given intravenously in the nimodipine group for 7-10 days.Blood samples were collected before and after treatment for 72 hours and 10-14 days,respectively.The levels of RBC[Ca2+]i were measured by Fura-2 pentakis(acetoxymethyl)ester[Fura-2/AM].The results were analyzed by SPSS 10.0 software.Results 1.The levels of RBC[Ca2+]i in neonates with HIE were significantly higher than those in healthy control group[(2.83?0.36)mmol/L vs(2.15?0.18)mmol/L,P
3.Changes of Plasma Pulmonary Surfactant Protein-A and Serum Ferritin in Neonates with Mechanical Ventilation

Objective To observe the changes of plasma pulmonary surfactant protein-A (SP-A) and serum ferritin (SF) in neonates with mechanical ventilation and its clinical significance in ventilation-induced lung injury (VILI).Methods The study was carried out in 36 neonates with mechanical ventilation and 31 neonates without mechanical ventilation .Plasma SP-A and SF levels in venous blood were mea- sured on 1,24,48,72 hours after mechanical ventilation and 24 hours after mechanical ventilation removal by Western-dot blot and radioimmunoassay (RIA).Plasma SP-A and SF levels of non-mechanical ventilation group were determined at the same time.Results Plasma SP-A levels in neonates on 24,48,72 hours after mechanical ventilation were significantly higher than those of non-mechanical ventilation group[( 2.20? 0.22)vs(1.97?0.29) ?g/L,(2.43?0.28)vs(1.94?0.33) ?g/L,(2.61?0.38)vs(1.80?0.34) ?g/L,respectively P
4.Simultaneous screening for 45 poisonous alkaloids in blood by LC-MS/MS.

OBJECTIVE:
To develop a liquid chromatography-tandem mass spectrometry (LC-MS/MS) screening method for 45 poisonous alkaloids in blood.
METHODS:
Identification was based on the compound's retention time and two precursor-to-production transitions. The method involved a liquid-liquid extraction (LLE) followed by LC-MS/MS with multiple-reaction monitoring (MRM). When 1 mL of blood was extracted with diethyl ether at pH = 9.2 with SKF525A as the internal standard, the target compounds were analyzed with LC-MS/MS in the positive ionization mode.
RESULTS:
The target alkaloids had good linearity (r>0.995 1), both the intra-day precision and inter-day precision being less than 14.77%. The limits of detection ranged from 0.05 to 25 ng/mL in blood.
CONCLUSION
The method is selective and sensitive in detecting poisonous alkaloids with a total running time of 12 minutes; therefore it was successfully applied to some actual cases of suspected alkaloids poisoning.

6.Clinical Analysis of Complications of 115 Children with Serious Pneumonia with Ventilatory Support

Objective To explore the clinical characteristics of children with serious pneumonia with ventilatory support,and combined with heart failure,encephalopathy,hyponatramia,acute respiratory distress syndrome(ARDS) and multiple organ dysfunction syndrome(MODS).Methods One hundred and fifteen children(81 males,34 females) with serious pneumonia in ICU from Jul.2005 to Jul.2007 were reviewed,The duration of ventilation support was over 48 h.And blood electrolyte,blood gas,cerebrospinal fluid and CT were detected.Results Ninety-three cases had a good outcome,7 cases were given up,15 cases were dead.The complications included heart failure in 60 cases(52.17%);encephalopathy in 40 cases(34.78%),in which tics type in 38 cases(95%),apnea type in 11 cases(27.5%);hyponatremia in 42 cases(36.52%),of which tics type in 31 cases(73.81%),syndrome of inappropriate secretion of antidiuetic hormone in 27 cases(72.97%)resulted in hyponatremia;ARDS in 8 cases(6.96%),pa(O2)/FiO2
7.Respiratory Failure Caused by Laryngotracheal Disorders in 15 Children

Objective To explore the clinical modalities and diagnosis of respiratory failure due to laryngotracheal disorders in children,in order to reduce the misdiagnosis.Methods Fifteen patients(11 cases were male,4 cases were female) with respiratory failure in Intensive Care Unit, Tianjin Children′s Hospital from Jul.2006 to Jul.2009 were selected.Data including history,characteristics and results of spiral computerized tomography(CT) and reconstruction and magnetic resonance imaging(MRI) were reviewed.Results The major clinical manifestations indicated that 7 cases had laryngeal stridor,6 cases had recurrent pneumonia,5 cases had cyanosis after crying,and 12 patients were complicated with respiratory failure in 12 hours.Nine cases were recovered,3 cases were given up,and 3 cases died.All patients were performed with CT about laryngohypopharynx,and the results were cyst of root of tongue in 3 cases,trachea pressed by cyst of parapharyngeal space in 1 case.There were 11 cases examined by spiral CT and reconstruction,in which 6 cases were confirmed as tracheal stenosis,2 cases were left bifid tracheostoma,2 cases were pulmonary artery sling,and 1 case was diagnosed as tracheomalacia.Chest X-ray radiograph indicated that bronchopneumonia in 8 cases,bronchitis in 3 cases and negative in 4 cases.Echocardiograph examination was performed among 11 cases,in which 2 cases were pulmonary artery sling,2 cases were atrial septal defect,1 case was patent ductus arteriosus, and 6 cases were negative.Conclusions There is high morbidity of respiratory failure caused by laryngotracheal disorders in infants,but misdiagnosis ccures usually because of non-specific symptoms.If the infant must depend on mechanic ventilation for a long time and has airway obstruction,he should be examined early by using spiral CT.
8.Attentional bias of low trait anxiety college students with or without fear-induced mood to emotional faces

Objective To investigate the attentional bias for emotionally faces in subjects with low-trait anxiety when they are under normal or induce-fear emotion.Methods Trait anxiety inquiry(T-AI) were used to evaluate the mental health status of 480 freshman in a university,and 450 valid questionnaires were obtained.Among them,90 students were identified as low trait anxiety.Sixty students from them were randomized into 2 group(n=30) to receive a dot-probe task with emotional face as stimulus.They were asked to judge the target.The attentional bias under normal or fear-induced emotion to the emotionally faces was examined.Results Low trait anxious college students showed attentional avoidance to the negative faces when they were under normal emotion [D=(-11.09?25.40),t=-2.391,P=0.024],and the orientation was significant [OI=(-20.28?22.90),t=-4.850,P0.05].Conclusion Low trait anxious college students have attentional avoidance to negative faces because of orientation,but the avoidance will disappear when they are under fear emotion.
9.Effect of Ginkgo biloba extract on the function of alveolar polymorphonuclear neutrophils in severe acute pancreatitis rats complicated with lung injury.

OBJECTIVETo explore the effect of Ginkgo biloba extract (GBE) on the function of alveolar polymorphonuclear neutrophils (PMN) in severe acute pancreatitis (SAP) rats complicated with lung injury (LI).
METHODSForty-eight adult SD rats were randomly divided into three groups, i.e., the sham-operation group, the SAP group, and the GBE treatment group, 16 in each group. The SAP model was successfully induced by retrograde injection of 5% sodium taurocholate solution into the biliopancreatic duct. Rats in the sham-operation group only received flipping of the duodenum. Those in the GBE treatment group received GBE intervention based on SAP model. Equal volume of normal saline was given to rats in the sham-operation group and the SAP group. Rats were sacrificed at 6 and 12 h after operation respectively. The lung tissue was sampled to evaluate the LI score. The wet/dry ratio (W/D) of lung tissues was detected. The activity of myeloperoxidase (MPO) was measured. Alveolar PMN was harvested by bronchoalveolar lavage. The content of neutrophil elastase (NE) in bronchoalveolar lavage fluid (BALF) was measured by enzyme-linked immunoabsorbent assay (ELISA). The percentage of CD11b/CD18 double positive PMN was detected using flow cytometry. The expression of intercellular adhesion molecule-1 (ICAM-1) and NE protein in the lung tissue was detected by Western blot.
RESULTSCompared with the sham-operation group, significant pathologic lesion occurred in the lung tissue of rats in the SAP group; the pathologic LI score, lung tissue W/D ratio, MPO, and NE content in BALF significantly increased, the expression of ICAM-1 and NE in the lung tissue was obviously up-regulated, and the percentage of CD11b/CD18 double positive PMN significantly increased (P < 0.01). Compared with the SAP group, pathological lesion of the lung tissue was obviously attenuated, and the above indices were all significantly declined in the GBE treatment group (P < 0.01).
CONCLUSIONSExpression of ICAM-1 in the lung tissue and the percentage of D11b/ CD18 double positive PMN were up-regulated in SAP rats complicated with LI, resulting in the adherence of PMN to pulmonary vascular endothelial cells, and then activating PMN to release NE and aggravate LI. GBE could alleviate LI through down-regulating the expression ICAM-1 and CD11b/CD18, and hindering the adherence and activation of PMN to pulmonary vascular endothelial cells.
