1.A investigation of pathogenic Yersinia enterocolitica infection in children with diarrhea under 5 years of age in western Yunnan

Abstract: Objective To investigate the prevalence and pathogenic characteristics of Yersinia enterocolitica infection in children with diarrhea under 5 years of age in western Yunnan, and to provide a basis for the prevention and treatment of infectious diarrhea in children. Methods Feces were collected from under five-year-old children with diarrhea in the First Affiliated Hospital of Dali University from 2020 to 2021. Clinical information of the cases was also collected. Yersinia enterocolitica was isolated from the samples after cold enrichment on selective culture plates, and the pathogenic characteristics of Yersinia enterocolitica were analyzed by biological type and serotype and virulence gene detection. Results A total of 397 feces were collected. Seven strains of Yersinia enterocolitica were isolated in three samples, and the prevalence of Yersinia enterocolitica infection was 0.76% (3/397). Among the three positive samples, two Yersinia frederiksenii or Yersinia intermedia were isolated in specimen No. 212 , and five Yersinia enterocolitica were detected in specimens No. 24 and 226. Two Yersinia enterocolitica isolated from one sample were biological type 1A, and the virulence gene test results were ail-/ystA-/ ystB+ /yadA-/virF-, which were non-pathogenic Yersinia enterocolitica. Three Yersinia enterocolitica isolated from the other sample were biological type 3, serotype O∶3 (rfbc+), and virulence gene detection results were ail+/ystA+/ystB-/yadA+ /virF+, which were pathogenic Yersinia enterocolitica. While pathogenic Yersinia enterocolitica was detected from feces of children with diarrhea at 11 months of age with a infection rate of 0.50%(2/397). Conclusion Sporadic infection of pathogenic Yersinia enterocolitica was found in under five-year-old children in western Yunnan Province. It is necessary to strengthen the monitoring and research of Yersinia enterocolitica.
2.Efficacy Observation of Yiguanjian Decoction Combined Adefovir Dipivoxil Tablet in Treating HBeAg Negative Chronic Viral Hepatitis B Active Compensated Liver Cirrhosis Patients.

OBJECTIVETo explore clinical efficacy of Yiguanjian Decoction (YD) combined Adefovir Dipivoxil Tablet (ADT) in treating HBeAg negative chronic viral hepatitis B (CVHB) active compensated liver cirrhosis (LC) patients.
METHODSTotally 68 HBeAg negative CVHB active compensated LC patients initially treated were assigned to the treatment group and the control group using random digit table, 34 in each group. Patients in the control group took ADT alone, 10 mg each time, once per day. Those in the treatment group additionally took YD, one dose per day. The therapeutic course for all was 48 weeks. Levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBil) were detected once in every two weeks. Hepatitis B virus (HBV)-DNA and four items of serum liver fibrosis [procollagen type I (PCN), hyaluronidase (HA), procollagen III peptide (PCIII), laminin (LN)] were detected once per every 4 weeks. Abdominal ultrasound B was performed before and after treatment. The inner diameter of the portal vein and the size of spleen were recorded. The fibrosis degree of liver was evaluated using Fibroscan. Efficacy of Chinese medicine (CM) was evaluated between the two groups before and after treatment using CM syndrome integrals. Efficacy of Western medicine (WM) was also evaluated between the two groups using Child-Pugh grading. Results Compared with before treatment in the same group, ALT and AST levels restored to normal levels, HBV-DNA turned negative (HBV-DNA < or = 1 x 10(2)) in the two groups after 48-week treatment. Besides, levels of TBil, ALB, PCIV, HA, PCIII, and LN obviously decreased (P < 0.05, P < 0.01). Results of ultrasound B showed the inner diameter of the portal vein and the size of spleen decreased. Fibroscan results showed that the elasticity value of the liver obviously decreased (P < 0.05). Besides, post-treatment levels of PCIV, HA, PCEJ, and LN, and the elasticity value of the liver decreased more obviously in the treatment group than in the control group (P < 0.01). There was no statistical difference in post-treatment levels of ALT, AST, TBil, ALB, inner diameter of the portal vein, or the size of spleen between the two groups (P > 0.05). Compared with before treatment in the same group, scores of Chinese medical syndrome and Child-Pugh scores decreased in the two groups after treatment (P < 0.05, P < 0.01). Besides, scores of Chinese medical syndrome decreased more obviously in the treatment group than in the control group (P < 0.05). The effective rate was 8824% (30/34) in the treatment group, higher than that of the control group [67.65% (23/34)] with statistical difference (P <0.05). Conclusion Combined treatment of YD and ADT could significantly improve symptoms of CM and fibrosis degree of liver of HBeAg negative CVHB active compensated LC patients.

3.The altration of Th17 cells and CD4+CD25+FoxP3+ regulatory T cells in patients with ankylosing spondylitis

Objective To investigate the percentages of Th17 and CD4+CD25+FoxP3+ regulatory T(Tr) cells and the levels of related cytokines IL-6,IL-23,IL-17 and TGF-β in serum of patients with anlylosing spondylitis(AS).Methods Forty patients with AS and 37 age-matched healthy donors were studied.Flow cytometry Was used to analyze the percentages of blood Th17 and CD4+CD25+FoxP3+Tr cells.The levels of serum IL-6,IL-23,IL-17 and TGF-β were assayed by enzyme-linked immunosorbent assay ( ELISA).Results The proportion of Th17 cells in AS group was significantly higher than those in normal group [ (1.02±0.34)% vs (0.68±0.29)%,P<0.05) ],and the proportion of CD4+CD25+FoxP3+ cells was lower in AS group comparing with normal group [(3.77±0.81)% vs (4.69±1.23)%,P<0.05)].Meanwhile,serum levels of IL-6,IL-23 and IL-17 were significantly higher in AS group than those in normal group [ (6,15±2.71) ng/L vs (3.31±1.65) ng/L; (9.44±3.12) ng/ml vs (5.82±2.61) ng/ml;(10.53±4.97) ng/L vs (6.78±3.26) ng/L,all P<0.01 ].In contrast,TGF-β level was decreased in AS group compares with the normal group [ ( 4.76±2.15) ng/ml vs (5.16±2.02) ng/ml,P>0.05 ],but the difference was not significant.No associations of serum eytokine levels with clinical and laboratory parameters were found in AS.Conclusion The abnormality Th17 cells and Tr cells and their related cytokines IL-6,IL-23,IL-17 and TGF-β changes in patients with AS,which may be involved in immunological pathogenesis of AS.
4.The influencing factors and prevention progress of motor complications of Parkinson's disease

Abstract
Long-term use of levodopa in the treatment of Parkinson's disease can cause motor complications, which seriously impair the patients'motor function, reduce the quality of life, and aggravate the functional disability. Since there has been no effective treatment for motor complications, clarifying the influencing factors and prevention methods are conducive to reducing the risk of incidence and improving the quality of life of the patients. This paper summarizes the types and mechanism of motor complications of Parkinson's disease, the influencing factors ( levodopa dose, onset age, Helicobacter pylori infection and high protein diet ) and preventive measures ( psychological intervention, low protein diet, rehabilitation exercise and drugs ), so as to provide reference for the prevention and control of the disease.
5. Effect of Wasp Venom Extract on Collagen-induced Arthritis in Rats

OBJECTIVE: The rat model of collagen-induced arthritis (CIA) was successfully established to investigate the therapeutic effect of wasp venom extract on rheumatoid arthritis (RA) rats. METHODS: The rats were randomly divided into normal group, model group, positive control group (bee venom lyophilized powder for injection, 1.25 mg•kg-1) and low, medium and high dose (0.125, 0.25, 0.5 mg•kg-1) of wasp venom extract groups, in addition the normal group, the other group rats were induced by chicken type Ⅱ collagen and complete Freund′s adjuvant which was multi-point injected into the left hindfoot of rats, once every 7 d for 14 d. After successful modeling, the rats were given corresponding dose of drugs continuous administration for 14 d. The diameter and perimeter of the ankle joint and AI score of the rats were measured before modeling, on the 14th day of modeling and on the 14th of administration, respectively; The changes of organ index and HE staining in ankle tissue were observed. The contents of inflammatory factor (IL-1β, IL-6, TNF-α, IL-8, PGE-2, COX-2) and rheumatoid factors (IgG, IgA, IgM) in serum of rats were detected by ELISA. The changes of T lymphocyte subsets in spleen tissue of rats were detected by flow cytometry. RESULTS: Compared with the model group, the extract of wasp venom had significant inhibitory effect on joint swelling (diameter and perimeter) of CIA rats (P<0.01), The joint index scores of CIA rats was decreased(P<0.01 or P<0.05). The organs index of rats restored to varying degrees (P<0.01 or P<0.05), to improved pathological structure of ankle joint, and decreased the expression of inflammatory and rheumatoid factors in serum of rats (P<0.01 or P<0.05), to regulate and improve the proportion of T lymphocyte subsets(P<0.01 or P<0.05). CONCLUSION: The extract of wasp venom has significant therapeutic effect on CIA rats, which is related to its regulation of inflammatory cytokine network and immunity.
7.siRNAs targeting La, hVAP-33, eIF2Bgamma, and HCV IRES inhibit the replication and expression of HCV in Huh7 cells.

OBJECTIVETo investigate the in vivo functional roles of the La autoantigen (La), the human homologue of the 33-kDa vesicle-associated membrane protein-associated protein (hVAP-33), and the subunit gamma of the human eukaryotic initiation factors 2B (eIF2Bgamma) as co-infection factors supporting chronic infection with hepatitis C virus (HCV).
METHODSSmall interfering (si)RNAs were designed against the HCV internal ribosome entry site (IRES) and transfected into Huh7 cells chronically infected with the HCV pseudovirus (designated as Huh7-HCV cells). The IRES siRNA producing the most effective silencing was selected for further analysis by fluorescence quantitative polymerase chain reaction (qPCR). siRNAs designed against La, hVAP-33, and eIF2Bgamma and the IRES-specific siRNA were then transfected, respectively or in various combinations, into the Huh7-HCV cell line for 48 h. The delta CT values were calculated and used to compare the HCV inhibitive efficacies of the siRNAs in isolation or in combination. Western blotting analysis was used to compare the quantity of core protein expression in each group.
RESULTSThe four gene-specific siRNAs, in isolation or in combination, caused inhibition of HCV replication and gene and protein expressions to varying degrees. The combination of La + IRES siRNAs produced the strongest inhibition of HCV core antigen expression. The combinations of hVAP-33 + IRES siRNAs and eIF2Bgamma + IRES siRNAs produced stronger inhibitions of HCV replication and gene and protein expressions than either hVAP-33 siRNA or eIF2Bgamma siRNA alone.
CONCLUSIONLa, hVAP-33, and eIF2Bgamma act as co-infection factors of HCV chronic infection in vivo. HCV replication and gene and protein expression can be inhibited significantly by RNA interference of these co-infection factors and/or HCV IRES.

8.Expression of JAK/STAT signal pathway in collagen-induced rat rheumatoid arthritis

Objective To explore the dynamic change and meaning of JAK/STAT signal path in the invasive course of collagen-induced arthritis(CIA).Methods Forty-eight rats were divided into 6 groups ran- domly,eight rats served as the control group,the rest were for the development of CIA models.Rats were sac- rificed at the 2nd,3rd,4th,5th and 6th week after the initial immunity respectively.Then HE staining,im- munohistochemistry and immunoblotting were used to detect the pathological changes of synovium in different st ages and the expression of p-STAT1 and p-STAT3.Results On the second week after initial immunity,the rats had arthritis,and the inflammation achieved its peak at the 4th week,then gradually relieved,and a few joints developed rigidity,synovium hyperplasia and inflammatory cell infiltration could be seen at the second week of initial immunity,and pannus could be seen at the 4th week as well as cartilage destroy at the 6th week by HE staining.In the invasive course of CIA,STAT1 and STAT3 were all in activated state detected by im- munohistochemistry and immunoblotting,which were significantly different from those of control group and they had a positive correlation with arthritis index,the Pearson's value of them were 0.798 and 0.873 respectively. However,there was some difference on time.STAT3 kept at activated state and had positive correlation with pathology score of synovium,and the Pearson's value was 0.622.On the contrary,the activation of STAT1 was obviously delayed and only confined to the middle and advanced stage of the disease.Conclusion JAK/STAT signal pathway participates the pathological course of CIA,and blocking the different point of JAK/STAT path- way'activation process may reach the goal of reversing the RA's pathological course.
9.Distribution of children cases in the Chinese National Injury Surveillance System, 2006 - 2008

Objective To understand the pattern of children injuries treated in hospitals and to provide evidence for its further prevention and control. Methods Data of children cases was descriptively analyzed from the Chinese National Injury Surveillance System (NISS), from 2006 to 2008. Results The incidence rates of boys were twice (2.22,2.15,2.15 ) higher than girls. About one half (50.74% ,49.75% ,49.75%) of them were 5-14 year of age. The main causes were falls (44.14%,45.68%, 47.15% ), RTIs (15.71%, 14.46%, 13.79% ), and blunt force injuries (13.20%, 12.92%,12.40% ). Main locations where the injuries happened were at home (34.96%, 36.86%, 38.84% ),school and public places (24.72%, 19.80%,21.19%) ,and road/street (21.21%, 19.63%, 19.33%). The major injured body parts were head (34.88%, 35.84%, 37.07% ), upper limbs (28.00%, 28.21%,27.81% ) and lower limbs (21.86%, 21.49%, 21.31% ). The majority cases were unintentional (93.01%, 92.66%, 90.58% ), minor (78.50%, 81.20%, 81.52% ) injuries, treated and discharged (82.37%, 85.19%, 84.84% ). There were considerable differences in the distributions of causes,locations, injured body parts, and other contexts depending on age. Conclusion The pattern of children' s injury would include causes, places and other characteristics treated at hospitals from NISS and was related to the age of the injured child which was quite different from the spectrum of death caused by injury. NISS acts as an important source of information on children's injury in China, and can contribute to the prevention and control program on children' s injury.
10.Analysis of poisoning cases from Chinese National Injury Surveillance System, 2006-2008

Objective To study the preventive strategies through analyzing the poisoning cases from the National Injury Surveillance System (NISS), from 2006 to 2008. Methods Data of poisoning cases was descriptively analyzed from Chinese NISS, from 2006 to 2008. Results The proportion of poisoning cases to all injuries cases from NISS were 2.57%, 2.48% and 2.52% from 2006 to 2008, which ranked sixth in all the injuries causes. Most people being poisoned had junior middle school education and most of them were agriculture/animal husbandry/fishery/water producers or commercial/service personnel. Most of the poisoning incidents were happened at home, always occurred in leisure time - around 8 PM, every day. The common types of poisoning were alcohol,clinical drugs, pesticide and carbon monoxide. Unintentional injuries were the main causes. Self-harm/suicidal cases in the rural areas were more than in the urban areas, with women more than men. The main type of self-harm/suicide related poisoning cases were through drugs or pesticide. ≥65, 15-29and 30-44 year-olds were most commonly seen. Conclusion Alcoholism was the primary type of poisoning injuries which is the highest in young adults ( 15-29 years and 30-44 years). It's important to promote civilized drinking habits and limit access to alcohol for youth. Self-harm/suicide had close relationship with clinical drugs and pesticide. The key points to prevent pesticide and clinical drugs poisoning were safe storage of pesticides, universal security of pesticide, and the supervision on drug producing and marketing. Children and the elderly were the high risk people for carbon monoxide poisoning. Monitoring and intervention must be strengthened.
