1.Professor ZHAN Wen-tao’s academic experience of prevention and early treatment of diseases on critical illness
China Journal of Traditional Chinese Medicine and Pharmacy 2005;0(11):-
The‘prevention and early treatment of diseases’conception as‘Huangdi Neijing’suggested,is the highest academic realm among the Chinese medical theory,it flows through the entire process of Chinese medical theory and diseases prevention.Four aspects of the prevention conception manifested in the process of Chinese medical prevention of diseases are: Taking preventive measures before confirmed diagnosis(health cultivation),impeding diffusion after confirmed diagnosis, deterring diffusion from deterioration, and restraining deterioration from being malignant. The malignant causal chain conversion of the coexistence of asthenia and sthenia is considered as the inducing links of critical illness’occurrence, evolution and transformation. Therefore,to intercept the malignant causal chain conversion comes to the key technique of Chinese medical remedy for critical illness. The‘prevention and early treatment of diseases’conception must impenetrate the versal process of treating critical illness. The overall clue and method of Chinese medical remedy for critical illness is to identify and grasp the vital contradiction and each respect of the malignant causal chain conversion which concurs with the evolution of the critical illness, simultaneously utilize the method which combined the prevention in etiology and pathogenesis with the remedy in etiology and pathogenesis, then actuate the pathologically changed organism to extricate from critical conditions at the first opportunity, finalizing with convalescence and recuperation thereafter.

BACKGROUND: Results from clinical trials suggested that clopidogrel and ticlopidine had side effects of granulopenia, and aspirin could inhibit endothelial progenitor cell proliferation. There is no report of effects of these drugs on human bone marrow mesenchymal stem cells (hBMSCs) in stem cell transplantation. OBJECTIVE: To investigate the effects of antiplatelet drugs including clopidogrel, ticlopidine and aspirin on hBMSC proliferation and secretion. DESIGN, TIME AND SETTING: The cytology in vitro observation was performed at the Laboratory of Toxicology, Shanghai Municipal Center for Disease Control and Prevention from March to December 2006.MATERIALS: The second passage of hBMSCs was kindly donated from Shanghai Tissue Engineering Research & Development Center, Shanghai Ninth People's Hospital. Clopidogrel (Lot number J20040006) and ticlopidine (Lot number H19980186) were obtained from Hangzhou Sanofi-Synthelabo Minsheng Pharmaceutical CO., Ltd. Aspirin (Lot number 20050059) was obtained from Bayer Vital GmbH. METHODS: The standard culture medium consisted of DMEM-LG, 10% heat-inactivated FBS, 100 U/mL penicillin and 100 μg/mL streptomycin. After being cultured in vitro expanded out to passage 6, hBMSCs were treated with antiplatelet drugs of different concentrations and compared with control group. MAIN OUTCOME MEASURES: Cell proliferation was assessed by 3- (4, 5-dimethylthiazol -2-yl)-2,5-diphenyltetrazolium bromide (MTT) colorimetric assay, level of vascular endothelial growth factor (VEGF) of culture medium was detected by enzyme-linked immunoadsordent assay (ELISA), and surface antigens of hBMSCs were analyzed by the flow cytometry. RESULTS: A570 values of hBMSCs treated by clopidogrel or ticlopidine of 0.02,0.1,0.4,2,10,40 μmol/L were higher than control group (P < 0.01), while A570 values of aspirin group of 60, 600, 2 000 μmol/L were lower than control group(P < 0.05). Antiplatelet drugs had no obvious effect on cell surface antigens(CD34, CD105, CD106)expressed by hBMSCs. Treated by high dose clopidogrel or ticlopidine (40 μmol/L), VEGF level from hMSCs was lower than that of control group(P < 0.01), but VEGF level of low dose (0.02 μmol/L) ticlopidine group was higher than control group(P < 0.01), and there was no significantly difference of VEGF level among low dose clopidogrel group (0.02 μmol/L), aspirin group (5, 2 000 μmol/L), and control group(P > 0.05). CONCLUSION: Clopidogrel and ticlopidine improve proliferation of hBMSCs, but aspirin inhibits proliferation of hBMSCs. High dose of clopidogrel and ticlopidine suppress VEGF secretion of hBMSCs, while low dose of ticlopidine promote it. Antiplatelet drugs have no obvious effect on hBMSCs differentiation.

Objective To investigate the inhibition effect in vitro of mucin gene MUC2 antisense oligodeoxynucleotide (ASODN) on its gene expression and proteolytic enzyme on gastric cancer cell line. Methods Phosphorothioated MUC2 ASODN were synthesized and transfected to SGC7901 cells mediated by lipofectin. MUC2 mRNA and MUC2 protein expressed on SGC7901 cells was detected by RT-PCR and immunohistochemical method. The inhibitory effects of ASODN on proteolytic enzyme, such as cathepsin D, MMP-2 and MMP-9 protein were detected by immunohistochemical staining. The cell cycle and apoptosis were determined by flow cytometry (FCM). Results The inhibition effects peaked at 48th hour after transfection, and the inhibition rate reached 55% when the concentration of ASODN was 0.5 ?mol/L. The number of the cells treated with MUC2 ASODN was decreased as compared to the control cells. MUC2 ASODN transfection could inhibit the transition period of S phase to G2/M phase, and G2/M did not change much. The apoptosis rate was about 4.38%. As compared with blank control group, MUC2 ASODN could significantly inhibit MUC2 mRNA expression of SGC7901 cells. ASODN could downregulate the expression levels of MUC2 protein, MMP-2 and cathepsin D protein. Conclusion MUC2 ASODN transfection could specifically inhibit SGC7901 cells by downregulating the expression levels of proteolytic enzyme.
