Objective To explore the efficacy and safety of transjugular intrahepatic portosystemic shunt (TIPS) in the treatment of hepatorenal syndrome (HRS).Methods From January 2009 to December 2012,the clinical data of three type 1 HRS cases and five type 2 HRS cases received TIPS operation were collected.The volume of 24 h urine,liver and renal function,and the occurrence of complications of portal hypertension were recorded and followed-up.Wilcoxon rank sum test was performed for non-normal distributed measurement data.Results TIPS of all the patients was successful and combined with devascularization in four patients.Before operation,the serum creatinine and volume of urine was 204.0 (180.8,229.8) μmol/L and 810 (600,925) mL,seven days after operation was 160.7 (120.2,184.0) μmol/L and 1 300 (975,1 500) mL,respectively,and the differences were statistically significant (Z=-2.1 and-2.2,both P＜0.05).One month after operation,renal function of five patients recovered to normal.At the 7th day after operation,total bilirubin level of three patients increased and that of two patients decreased to the preoperative base line value at one month and three month.Mild or moderate hepatic encephalopathy occurred in three patients after operation and relieved after medicine treatment.At 6th month after operation,two of three type 1 HRS patients survived.Of five type 2 HRS patients,one received liver transplantation at 6th week after operation,three of the left four cases survived at the 6th month after operation.Conclusion After safety and efficacy were evaluated,TIPS may be one of the methods used for the treatment of HRS.

Objective To investigate the plasma PAF-AH level in type 1 diabetes patients and the association between the PAF-AH gene polymorphism and type 1 diabetes mellitus. Methods Plasma PAF-AH in115 type 1 diabetes patients and 106 controls were determined by PAF-AH assay kit, and PAF-AH gene G275A genotypes were detected by PCR-RLFP. Results The activity of plasma PAF-AH in the patients with type 1 diabetes was significantly lower than that in the controls. Compared with control group, the frequencies of the GA genotype on G275A polymorphism were significantly higher than that in T1DM group. Conclusion The activity of plasma PAF-AH in the patients with type 1 diabetes was significantly lower than that in normal controls. The G275A polymorphism of PAF-AH gene might be related to type 1 diabetes mellitus.

OBJECTIVETo preliminarily study the effective dosage range and mechanism of the abirritation of volatile oil of Evodia Fructus on the stomach cold syndrome model in mice, and discuss the correlation between its accompanying toxicity and oxidative damage mechanism, in order to provide the experimental basis for explaining the efficacy-syndrome-toxicity correlation.

METHODThe stomach cold-syndrome model in mice was induced by the classic hot plate test by orally administrating with different doses of volatile oil of Evodia Fructus, in order to observe its abirritation and companying toxic and side effects and detect serum ALT, AST, PGE2, NO, NOS, MDA, SOD, GSH, GSH-Px, BUN, CR and hepatic ALT, AST. The companying toxic symptoms in mice were recorded in toxic reaction integral table.

RESULTVolatile oil of Evodia Fructus had an obvious analgesic effect at 30 min after the oral administration and reached the peak effect at 60 min, with certain "dose-effect" and "time-effect" relations, rises in serum and hepatic ALT and AST levels, serum PGE2, MDA, NO and NOS and hepatic indexes, decreases in SOD, GSH and GSH-Px and no notable change in BUN, CR levels and kidney weight/body ratio. Conclusion: The abirritation mechanism of volatile oil of Evodia Fructus was related to the inhibition of pain transmitter release, peroxidative damage and NO damage, which is accompanied by certain hepatotoxicity, mainly mainly oxidative damage, with a concurrent "dose-time-toxicity" relationship.

The range of effective dose and mechanism of abirritation about water extraction components of Evodiae Fructus on the stomach cold syndrome model in mice were preliminary studied. The method of stomach cold-syndrome model in mice was built, which were administrated with different doses water extraction components of Evodiae Fructus, observing abirritation and toxicity by the classical hot plate method, detecting the level of ALT, AST, PGE2, NO, NOS, MDA, SOD, GSH, GSH-Px, BUN, CR in serum and ALT, AST in hepatic tissue, and recording toxicity symptoms in mice according to the list of relevant toxicity reaction. The water extraction component of Evodiae Fructus has obvious analgesic action after administration 30 min, arriving peak effect after administration 60 min, showing certain "dose-time-toxicity" relationship. ALT and AST levels in mice serum and liver tissue enhanced; PGE2, MDA, NO, NOS enhanced in mice serum; SOD, GSH, GSH-Px reduced; the BUN, CR levels was no significant alteration; liver weight/ body weight enhanced; kidney weight/body weight was no significant alteration. The a irritation mechanism of volatile oil of Evodiae Fructus was connected with suppressing pain transmitters release, per oxidative damage mechanism and NO damage, which also induced hepatotoxicity and the mechanism of hepatotoxicity is main lyoxidative damage, showing certain "dose-time-toxicity" relationship in accordance to hepato-toxicity injury.

Femoral head necrosis and nonunion is a common complication after surgery of femoral neck fracture.Recently,femoral neck shortening have caused attentions which can impact the function of hip.Incidence,mearsurement,influence factor,prevention and treatment of femoral neck shortening have been retrospective reviewed.Amount of femoral neck shortening with sliding compression system is significantly more than length of stability system,which is result from its own mechanism.However,effcacy of length of stability system still need a large scale of sample.It uses radiography to diagnose shortening.The present measurement methods include displacement of femoral head,xyz-axis measurement,the length of exposed screw,the collapse of femoral head.However,position,the angle of screen and the defect of the method would increase deviation.A measurement with simple,wide application and accuracy need to continue to explore.Apart from patients' condition,shortening also affected by the pattern and quality of surgery.Overall,older women,displacement fracture,unsatisfactory reduction will increase the risk and shrinkage of shortening.In addition,the pattern of the internal fixation becomes the research focus in the field of orthopedic clinical research.Including the transformation of sliding compression system device and design of novel device.How to combine them together organically could be a future development direction.Meanwhile,there are still in exploratory stage to decide whether biological combined with internal fixation can prevent shortening.Intertrochanteric osteotomy can achieve union and correct the limblength discrepancy,However,it did not restore the length of femoral neck.So,it is unknown that whether the method can solve the slight shortening which have impacted the hip function.

Objective To investigate the species distribution and the difference of virulence gene spectra of Aeromonas isolated from intestinal tract and extraintestinal body fluid,and the correlation of their pathogenicity with infection sites.Methods A total of 156 Aeromonas strains isolated from the fecal specimens of patients with acute diarrhea and extraintestinal specimens were collected during May 2013 and September 2015.Eighteen virulence genes in these strains,including hlyA,aerA,act,alt,ast,aexT,ascV,aopP,ascF-G,gcat,tapA,fla,Ser,exu,ahyB,eprCAl,lip and laf,were detected by polymerase chain reaction(PCR).Last,the differences of virulence gene spectra between intestinal and extraintestinal Aeromonas were analyzed.Results Among 156 Aeromonas strains,79 were from fecal specimens,and 77 from extraintestinal specimens.Aeromonas caviae(A.caviae,51.9％) was the most common species in the intestinal strains,while Aeromonas hydrophila(A.hydrophila,48.1％) and A.caviae(39.0％) were the main pathogens in extraintestinal infections.The most prevalent virulence genes in intestinal and extraintestinal Aeromonas were gcat,act,fla,ahyB,exu and lip (＞ 45.57 ％),while aexT,aopP,ascF-G and ascV were less frequently detected (＜ 20.78％).The detection rates of gcat,ahyB,laf,ast,exu,lip,hlyA and aerA genes in intestinal Aeromonas were significantly lower than those in extraintestinal isolates (P ＜ 0.05).The detection rates of gcat,ahyB,exu,lip,eprCAl and hlyA genes in extraintestinal A.hydrophila were significantly higher than those in intestinal A.hydrophila (P ＜ 0.05).The detection rates of lip and hlyA genes in extraintestinal A.caviae were significantly higher than those in intestinal A.caviae (P ＜ 0.05),while that of aopP gene was just the reverse.There was no significant difference in the detection rates of virulence genes between intestinal and extraintestinal Aeromonas veronii.Conclusion There are significant differences in the species distribution and virulence genes of Aeromonas isolated from intestinal and extraintestinal specimens,indicating that clinicians should treat them differentially.

Age-related macular degeneration (AMD) has become a leading cause of irreversible visual loss in senior population and a serious issue of public health as the aging of society worldwide,especially wet AMD.Wet AMD is characterized by choroidal neovascularization(CNV)with a rapidly developing process andsevere impairment of visual acuity.The effective therapy that can destroy choroidal neovascularization and improve visual acuity for very long time is lack.In the recent 20 years,more attentions focuse on the treatment of choroidal neovascularization.From bench to bedside,the research on targeted treatment of angiogenesis brings the bloom of wet AMD medical therapy.This paper will summarize the update information of wet AMD.
