2.Clinical Study of Diaphragmatic Muscle Fatigue in Children with Pneumonia

Objective To explore the occurrence of diaphragmatic muscle fatigue(DMF) in children with pneumonia and its relationship with respiratory failure.Methods One hundred and twenty-six cases of pneumonia(pneumonia group) and 60 healthy children(healthy control group) were selected,and 2K-V1 type intelligence electric impedance respirograph was used to measure diaphragmatic function of the children in both groups to find if the ribcage-abdomen motion was synchronic and to calculate M levels.The abdomen impedance values as X-axis were calculated,which were regarded,and the chest impedance values as Y-axis were calcaulated,which described two-dimensional chart of ribcage-abdomen motion curve and the ? angle which was between connection of maximum X value to the smallest X value and the X-axis were calculated.According to the changes of chest-abdominal curve in one-dimensional chart and the changes of M value and ? angle,the degree of the diaphragmatic muscle fatigue was judged.Results There were 98 cases of DMF in the 126 children with pneumonia,and 68 cases of which were type Ⅰ DMF,and 30 cases were type Ⅱ DMF.One-dimensional chart showed contradictory chest-abdominal respiratory motional curve,and two-dimensional chart moved in a clockwise direction.M value was(46.1?8.4)%,? angle was(136.7?12.0) degrees in children with type Ⅰ DMF;One-dimensional chart showed that the peak value of chest-abdominal respiratory motional curve dislocates,and two-dimensional chart moved in a counter-clockwise direction.M value was(17.2 ? 3.2)%,? angle was(48.2 ? 9.5) degrees in children with type Ⅱ DMF;the control group had M value(4.3 ? 1.0)%,? angle was(31.7 ? 5.2) degree.There were significant differences in M value and ? angle between type Ⅰ DMF children,type Ⅱ DMF children and healthy children(Pa
4.Dysfunctional beliefs and attitudes on sleep and sleep disturbances pre- and post-antidepressant treatments in patients with major depression

Objective To explore the relationship between dysfunctional beliefs and attitudes on sleep and sleep disturbances before and after a short-term pharmacotherapy in patients with major depression. Methods Sixty-six (29 male, 37 female, 34.0±9.5 years old) patients with major depression were recruited before the treatment. Dysfunctional Beliefs and Attitudes about Sleep (DBAS) questionnaire, Pittsburgh Sleep Quality Index (PSQI), Hamilton Depression Rating Scale (HDRS), and Hamilton Anxiety Scale (HAMA) were assessed before and after a 4-week antidepressant treatment. Results After the 4-week antidepressant treatment, depressive and anxiety symptoms were alleviated significantly, whereas sleep disturbance still persisted. And sleep quality was closely related with problematic sleep beliefs after the treatment.Conclusion Some evidences are provided for cognitive behavioral therapy during antidepressant therapy, and the therapy should be matched with problematic beliefs and attitudes.

7.Effects of TanshinoneⅡA Sulfonic Acid Sodium on Blood Supply and Hemorheology of Patients with Central Retinal Vein Occlusion

OBJECTIVE:To observe the effects of Tanshinone ⅡA sulfonic acid sodium injection on blood supply and blood rheology of patients with central retinal vein occlusion )CRVO). METHODS:92 CRVO patients were selected and randomly divided into control group and trial group with 46 cases in each group. Control group was given routine treatment of laser photocoagulation;trial group was additionally given Tanshinone ⅡA sulfonic acid sodium injection 80 mg added into 0.9% Sodium chloride injection 150 ml,ivgtt,qd. A treatment course of 2 groups lasted for 7 days,and both received 4 courses of treatment. The hemodynamics and hemorheology of central retinal artery (CRA) were recorded in 2 groups before and after treatment. The fluorescence angiogra-phy (FFA) and optical coherence tomography (OCT) of 2 groups were also observed. RESULTS:Before treatment,there was no statistically significant difference in Vmax,Vmin and RI of CRA between 2 groups (P>0.05);after treatment,Vmax,Vmin and RI of CRA in 2 groups were improved significantly,the trial group was better than control group,with statistical significance(P<0.05). After treatment,whole blood high shear viscosity,whole blood low shear viscosity,plasma specific viscosity and fibrinogen of trial group were significantly lower than those of control group,and FFA and OCT of trial group were better than those of control group, with statistical significance(P<0.05). CONCLUSIONS:TanshinoneⅡA sulfonic acid sodium injection can obviously improve hemo-dynamics and hemorheology of CRVO and blood supply and vision of lateral eye.
8.Prognostic factors of epithelial ovarian carcinoma in patients with age under 40

Purpose:To study prognostic factors of younger women's eptihelial ovarian neoplasms.Methods: From Jan. 1980 to Dec. 1992, there were 86 cases of younger women's epithelial ovarian carcinoma in our hospital. We studied serveral prognostic factors retrospectively.Results:In this setting, 2 year survival rate were 79.07%, 5 year survival rate were 54.65%. There were 49 cases with stage Ⅰ and 41 cases with grade Ⅰ. No recurrence was found in 4 patients who preserved ovarian function. Cox model multifactor results showed that grade, residual tumor size and the method of surgery were prognostic factors( P 0 05). 9.Inhibitory effect of Meloxicam on the cultured fibroblasts from the excised pterygium

AIM: To investigate the association between cycloxygenase-2 (COX-2) expression and VEGF intervention as well as the inhibitory effect of Meloxicam on the cultured human pterygium fibroblasts (HPF).METHODS: Expression of COX-2 was measured by immunohistochemistry in the cultured HPF from twenty excised pterygium cases. Expression of COX-2 in HPF was measured by Western blot following the treatment of vascular endothelial growth factor (VEGF) at the different concentrations. In addition, the effect of Meloxicam on proliferation of HPF was studied by adding the different concentrations into the cultured HPF plates by Mono-nuclear cell direct cytotoxicity (MTT) reduction assay.RESULTS: COX-2 expression was present in the cultured HPF. The level of the expression increased following VEGF treatment. The proliferation of the cultured HPF decreased following addition of the different concentrations of Meloxicam (from 75μ mol/L to 300μ mol/L) and the magnitude of the inhibition was dose-time dependent.CONCLUSION: COX-2 levels in the cultured HPF werepositively associated with VEGF stimulation and Meloxicam was inhibitory to HPF proliferation.
10.Clinical Observation of Wenjing Shexue Recipe in Treating Polycystic Ovarian Syndrome

Objective To observe the clinical efficacy of Wenjing Shexue Recipe ( WSR) in treating polycystic ovarian syndrome ( PCOS) with liver-kidney deficiency, and to explore the possible therapeutic mechanism. Methods Seventy-four PCOS patients were randomized into observation group ( N=38) and control group ( N=36). The observation group was given oral use of WSR and the control group was treated with Ethinylestradiol and Cyproterone Acetate Tablets orally. Both groups received treatment for 6 months, and were followed up for 3 moths after drug withdrawal. After treatment, the therapeulic effect was evaluated , and the changes of body mass index ( BMI) , hairiness. acne, and sexual hormone levels were monitored before and after treatment. Results ( 1) The total effective rate of the observation gorup was 92.11% and that of the control group was 75.00%, the difference being significant ( P<0.05). ( 2) After treatment for 6 months, body mass index (BMI), the scores of hairiness and acne, and serum levels of testosterone (T) and luteotrophic hormone (LH) were decreased in the observation group (P<0.05 compared with those before treatment). The control group had lower scores of hairiness and acne and serum levels of T and LH than those before treatment ( P<0.05) , but BMI showed no obvious decrease (P>0.05) . The results of follow-up showed that BMI, the scores of hairiness and acne, and serum levels of T and LH were still lower in the observation group than those before treatment (P<0.05), but the above indexes except acne scores were increased in the control group (P>0.05 compared with those before treatment) and were higher than those of the observation group (P<0.05). Conclusion WSR exerts better effect for the treatment of PCOS with liver-kidney deficiency than Ethinylestradiol and Cyproterone Acetate Tablets, and the possible mechanism is probably related with the regulation of endocrine disturbance by decreasing serum T and LH levels.
