Authors described the patterns, core measures and mechanisms of coordinated care delivery in the localities and analyzed initial outcomes of this practice. It is found that such practice has optimized distribution of regional healthcare resources, implemented first visit to primary institutions, and the care delivery to separate emergency from outpatient service. In addition, the paper analyzed problems found in localities in terms of policy, public hospitals and primary institutions, and proposed elements required for coordinated care delivery system to succeed, such as enhanced policy support, enhanced multi-departmental coordination, and public hospital reform to explore feasible approaches.

· AIM: To investigate the expression and the possible implication of CD40/CD40L costimulatory molecules in erythema nodosum of patients with Beh(c)et's disease.· METHODS: Sampling was done from erythema nodosum of 5 patients with Beh(c)et's disease and normal skin of 2 healthy individuals. Immunohistochemical staining was performed to examine the expression of CD4, CD8, CD19, CD68, HLA-DR,CD40 and CD40L molecules in the obtained tissues.· RESULTS: Approximately 90% of epidermic cells in erythema nodosum expressed CD40 molecule. In the dermis and subcutaneous tissue, a significantly increased number of CD4+Tcells, CD8+Tcells, CD19+cells, CD68+cells, HLA-DR+cells,CD40L+cells, and CD40+cells were observed in the erythema nodosum as compared with that in normal skin. Double staining showed that CD40L molecules were expressed on 45% of CD4+T cells. CD40 molecules were expressed on 100% CD68+ cells and 59.2% of HLA-DR+cells respectively.· CONCLUSION: A number of CD40/CD40L costimulatory molecules are upreguiated in the erythema nodosum of patients with Behcet's disease.

This study was aimed to reveal current achievements and the future trend about Chinese materia medica (CMM) processing, which pointed future research direction for scholars.“CMM processing” was used as the subject term in document retrieval to the Web of Science (WoS) and CNKI. The CitespaceII and Ucinet & Netdraw were used to build mapping knowledge domains to acceptable both English and Chinese literature data. The results showed that the mapping knowledge for“CMM processing” for co-citation, source journal, author, key words and so on were established. It was concluded that the rapid present period of Chinese literature on CMM processing was earlier for around 15 years than English literature. And the total amount of English documents was much less than Chinese documents. Combing the developing process of CMM, it revealed its current achievements and future trend, which pointed future research direction for scholars, and moreover it put forward new thinking and method to summarize or expand traditional medicine and related research fields.

To study the effects of ouabain on the inner ear glial cells, and to lay the foundation for the study of stem cell transplantation in the treatment of sensorineural hearing loss. Methods Sixty adult female SPF grade CBA / J mice were randomly divided into experimental group and control group, with 30 mice in each group.Animals in the experimental group received 3mM ouabain via the round window membrane, while mice in control group received normal saline. The mice were sacrificed at 7 days, 14 days and 30 days after the administration, respectively.Immunofluorescence histochemical staining was used to detect the inner ear glial cells in spiral ganglion. Results Some inner ear glial cells survived in the spiral ganglion of the experimental group, while with decreased numbers and disorganized structure compared to those of in the control group.Comparing to those of in the control group, the number and density of inner ear glial cells in the experimental group were significantly decreased from 7 days afterouabain administration, further decreased at 14 days and reduced to the lowest at 30 days after ouabain administration, the differences between the 2 groups were statistically significant (P < 0.05).Among the experimental group, the number of inner ear glial cells at 30 days was significantly decreased when compared to those of at 7 days and 14 days, respectively. Conclusion Application of ouabain to mouse inner ear via the round window membrane leads to an acute and progressive direct damage to the inner ear glial cells in the spiral ganglion.

Objective To study the estradiolNGF regulatory cascade in retinal endothelial cells. Methods Retinachoroids vascular endothelial cell line RF/6A from rhesus was cultured in M199 medium contain 20% FBS.mRNA of ER,NGF and its receptors,TrkA and P75 NTR were detected with RTPCR.The cells were incubated with NGF,VEGF,antibodies against NGF or VEGF,and K252a(100nmol/L),the specific inhibitor of TrkA,separately or in different combination.MTT based cell counting assay was used to study the viability of the cells. The apoptosis was evaluated by FACS,mass migration by wound healing assay, and tubogenesis by AngioMatrix assay. Results We amplified the specific fragments of cDNA of ER,NGF and NGF receptor TrkA using RTPCR.10?nmol/L1??mol/L estradiol augments the proliferation and increases the viability of RF/6A in a dosage dependent manner. In the wound healing based migration assay,we found the similar alteration. This effects of estradiol was partially blocked by NGF neutralized antibody and K252a.Apoptosis rates were at the similar level among the groups. For the tubogenesis of RF/6A,we found no augmentation by NGF,and no blocked augmentation by estradiol. Conclusion NGF,first identified as the survival factor of nerve system, now also seemed to be an activator of retinal endothelial cell,is under the regulation of estrogen. The proliferation and migration, but not the tubogenesis of retinal endothelial cells are regulated by this regulatory cascade.

Objective The purpose of this experiment is to study the in vivo differentiation and myelin formation of rat striatal neural precursor cells after transplantation into homogeneous retina, observe the order of myelination and its influence on the structure of retina, establish an animal model of CNS myelin formation in vivo. Methods Passage cultured striatal neural precursor cells from embryonic Sprague-Dawley rats were transplanted into the vitreous cavity of neonatal rats.In different stages after transplantion, myelin formation in retina was observed under light and electron microscope and analysed with different stained methods. Results Bundles of myelin appeared in parts of retina 4 weeks later. The distribution and morphology of myelined area expanded with prolonged survival time after cell transplantation.Oligodendrocyte wrapped the naked axons and formed normal myelin limited in the nerve fiber layer.Myelination influenced the distribution of local retinal ganglion cells. Conclusion Striatal neural precursor cells could differentiate into oligodendrocytes and formed myelin after transplanted into retina and the naked axons in retina promoted the myelin formation. This model provides a new method to study the myelin formation and myelin-axon interaction in vivo.

The specification of decoction pieces and quality uniformity are the important factors to influence the efficacy of clinical medicine. Considering the deficiency of diversity, poor quality uniformity and confusion of decoction pieces specifications, we first propose a new idea of precision decoction pieces (PDP) based on clinical demands and fresh-processed technology. In order to explain the idea, a study case of aconite SUP is provided, including the optimized specification design, processing technology, extraction effects, quality uniformity, and toxic and efficacy variation and so on. The results showed that preparing 5 mm PDP by fresh-cutting is rather simple and practicable, with high efficiency and large yield; then, this technology could significantly decrease the ingredients loss and increase the efficacy components; moreover, it was helpful for achieving the quality uniformity and best extraction effects. This work revealed the quality superiority of PDP, and provided a good strategy and example for the standard of decoction pieces specification and modernization of processing technology.

The contents of schisandrol A, schisandrol B, schisantherin A, schisandrin A , schisandrin B, schisandrin C in Schisandrae Chinensis Fructus (SCF) were determined simultaneously by HPLC. Collect 100-seed weight, color, pulp content, longitude and latitude of SCF of different batches were collected. SIMCA-P and SPSS were applied to make PLS-DA analysis of 24 batches of SCF and correlation analysis of relevant parameters. According to the 13 parameters, SCF from three different places of origin could be distinguished effectively. It was found that the content of chemical component of SCF increased with latitude and longitude first, and then decrease. The results provide some theoretical basis for study of SCF genuineness and traditional method of identifying just from experience.
