1.The efficacy of surgical resection or radiofrequency ablation in treatment of medium-sized tumor

Objective To investigate the efficacy of two different methods in treatment of liver cancer which sizes ranged from 3 to 6cm ,and provide the basis for clinical treatment methods .Methods A retrospective analysis of the curative effect of liver cancer patients whose tumor diameter were rang from 3 to 6 cm was done .All patients collected from the Forth Cadre′s Sanatorium and Health Center ,Military Region in Shandong province Taian zones at the time of December 2008 to December 2011 who carried out surgical resection or radiofrequency ablation .Results 41 cases of recurrence within a year in surgical group ,the recurrence rate was 32 .5% ,87 cases(69 .1% ) of recurrence ,103 cases(81 .7% ) of recurrence within 3 years ;41 cases of recurrence within a year in radio frequency ablation group ,the recurrence rate was 40 .8% ,51 cases(71 .8% ) of recurrence within 2 years ,60 cases(84 .5% ) of recurrence within 3 years ,a statistically significant difference were found between the two group at the same period time (P<0 .05);1 ,2 ,3 year survival rates were 84 .6% ,69 .8% ,56 .3% in surgical group ,radiofrequency 1 ,2 ,3 year survival rates were 85 .3% ,70 .9% ,53 .4 in radio frequency ablation group ,The difference was not statistically significant at the same period time (P>0 .05) .Conclusion For size between 3 to 6 cm liver cancer ,radiofrequency ablation can be used as the preferred method of treatment .
2."The value of ""streaks"" sign in diagnosis of central neurocytoma"

ObjectiveTo investigate the value of “streaks” sign in diagnosis of central neurocytoma.MethodsFifty-two cases of central neurocytoma confirmed by pathology from 2008 to 2011 were collected.In 52 cases,both MRI and CT scan were performed on 12 patients,MRI only on 36 patients and CT only on 4 patients.MRI and CT features were analyzed retrospectively.ResultsIn MRI,the “streaks” sign was found in 39 cases,accounting for 81.25%(39/48).In CT,the“streaks” sign was found in 11 patients,accounting for 68.75%(11/16).The “streaks” sign in CTwas also found in MRI.ConclusionsThere is a certain specificity of“streaks” sign in central neurocytoma imaging.In combination with other imaging findings,it can be helpful for an accurate preoperative diagnosis of the disease.
3.Isolation, culture and surface markers detection of human umbilical cord mesenchymal stem cells

Objective To investigate the methods of isolation and culture in vitro and detect the surface markers of human umbilical cord mesenchymal stem cells.Methods Human umbilical cord Wharton' s jelly was separated and cut up as small as possible,and then cultured with α-MEM.Human umbilical cord mesenchymal stem cells could be obtained by culturing the tissue block adhered the bottle wall.And the cells were passaged at a certain density.The surface markers of human umbilical cord mesenchymal stem cells were detected by FACS when the cells were in Generation Three.Results Human umbilical cord mesenchymal stem cells were obtained from Wharton' s jelly conveniently,with fibroblast shape and stable proliferation and passage.CD29,CD44,CD105 were strongly expressed on human umbilical cord mesenchymal stem cells.But CD45,CD34,HLA-DR,HLA-G,CD80,CDs6 were not expressed.Conclusion Human umbilical cord mesenchymal stem cells can be obtained effectively from the culture of the tissue block,which provides a rich source of cells for tissue engineering.
4.Fragmentation behaviors of taxanes drugs using electrospray ionization with quadrupole time-of-flight mass spectrometry.

The fragmentation pathways of two taxanes drugs have been studied in positive ion mode by Q-TOF with the advantages of high mass accuracy and high resolution analysis. The [M+H] + ions were observed by ESI-MS, from which the molecular weights were obtained. Using the protonated pseudo-molecular ions [M+H]+ as internal reference compounds, the accurate mass and element composition of the fragment ions were determined. The collision induced dissociation (CID) data of the [M+H] ions provided fragmentation pathways of related compounds. Results showed that the major cleavage pathways of paclitaxel and docetaxel were the same that the cleavage of C-O bond between the side chain and taxol skeleton easily occurred, then stripping of the functional groups on the parent ring. Some common fragments were formed, such as m/z 105.033 7, 291.137 3, 309.148 5, 327.159 7, 387.181 2 and 509.217 4, which would provide a basis for future qualitative and quantitative analysis of taxanes in vitro and in vivo.

5.Study on intra-retinal layers imaged with ultra - high resolution optical coherence tomography

To evaluate the automated segmentation algorithm for detection of intra - retinal layers to process images obtained from ultra- high resolution optical coherence tomography ( OCT ) . Graph theory and the shortest path search based on dynamic programming were applied to automatically segment the 8 intra - retinal layers. We experimentally verified the accuracy and reliability of the algorithm. The results showed that the intra-retinal layer boundaries between automated and manual segmentations matched well. The algorithm successfully segmented the intra- retinal layers in glaucoma, high myopia, and retinitis pigmentosa patients. The proposed automatic segmentation for intra-retinal layers provides a promising tool for quantitative analysis in clinical diagnosis and treatment.
6.Differentiation of Brucella abortus , Brucella melitensis , and Brucella suis by multiple primers PCR

Objective To establish a method for rapidly identifying Brucella abortus, Brucella melitensis and Brucella suis by multiple primers PCR. Methods According to Brncella abortus, Brucella melitensis and Brucella suis IS711 insertion sequences, a public primer and three specific primers(544A, 16M, 1330S) were designed to set up multiplex PCR detection method. Yersinia O : 9, Escherichia coli O157 : HT, Salmonella typhimurium 47729 were selected to undergo multiple PCR reactions to detect the specificity. The sensitivity of multiple primers PCR of Brucella abortus was detected using multiple proportion dilution method. Results The amplified fragment size of Brucella abortus was 485 bp, that of Brucella melitensis 731 bp, and that of Brucella suis 248 bp, but PCR for the DNA of Yersinia O : 9, Escherichia coli O157 : H7, Salmonella typhimurium 47729 was negative. A sensitivity of the multiple primers PCR with Brucella abortus DNA using multiple proportional dilution quantitative method was 0.0967 pg. Conclusions Multiple PCR amplification method for rapidly detecting Brucella abortus, Brucella melitensis and Brucella suis has been successfully established, resulting in good specificity and sensitivity.
7.Ultrasound-guided percutaneous nephrolithotomy for management of renal complete staghorn stones

ObjectiveTo report our experience and assess the safety and efficacy of ultrasound-guided percutaneous nephrolithotomy (PCNL) with ultrasonic and pneumatic lithotripters for complete renal staghorn stones.MethodsFrom Jan 2006 to Jan 2015, 128 patients (142 kidneys) with complete staghorn stones underwent PCNL, included 14 patients with bilateral renal complete stones,13 patients with solitary or founctional solitary kidney, 22 patients with renal insufficiency, 25 patients with renal empyema, 4 patients with spine malformation, ultrasound-guided PCNL were performed. Stones were fragmented and cleared using a combination of ultrasonic and pneumatic lithotripters.ResultsAll PCNL procedures were successful. Successful access to the collecting system was 100%. Mean operation time was 64.5±39. 1min (range 30–150). The primary stone-free rate of PCNL was 76.8%(109/142). Most of the renal stones(63.4%, 90/142)were managed satisfactorily by a single tract,a second tract was used in 25 renal stones(17.6%, 25/142). There were 36 auxiliary procedures (24 second PCNL and 3 third PCNL and 9 extracorporeal shock wave lithotripsy). One month after treatment, the overall stone-free rate was 85.2%(121/142). Six patients (4.2%) received blood transfusion,three patients(2.1%) received angiography and embolization . Three patients (2.1%) with infective shock were cured by intravenous antibiotics and antishock treatments. Three patients with temporary renal function receded were observed in solitary kidney for two tracts PCNL. No other severe complications occurred. The mean postoperative stay was 5.2 days (range 2–12). Conclusion: Ultrasound-guided Percutaneous Nephrolithotomy with ultrasonic and pneumatic lithotripters for management of renal complete staghorn stones is safe, effective, and worthy of wider use in clinical practice.
8.A pilot study on establishment of clinical evaluation system for gut barrier dysfunction

Objective To investigate the potential influencing factors which possibly effected the gut barrier function.The effort was made to establish a clinical evaluation system of gut barrier dysfunc- tion.Methods Fifty-three critically ill patients with an APACHEⅡscore of 8 or more and 27 patient which APACHEⅡscore was 6 or less were recruited.Plasma was reserved for measurement of endo- toxin,tumor necrosis factor-?,diamine oxidase,D-lactic acid and high sensitive C reactive protein,uri- nary excretion of lactulose and mannitol and the urinary content of intestinal fatty acid binding protein (IFABP) were determined as well.Analyses was achieved by univariate,multivariate analysis and receiver operating characteristic curve.Results In the logistic regression models,gut barrier was affect- ed by many factors.The ratio of lactulose and mannitol in urine,the urinary content of IFABP of 24 hours and endotoxin level of plasma were identified as the most intimate factors which could associate with gut barrier function.The optimal operating point of plasma endotoxin,ratio of urinary lactulose and mannitol and content of urinary IFABP of 24 hours were 0.145,17 ng and 0,055 EU/ml respectively based on the results of receiver operating characteristic curve,the sensitivity and specificity were 84.5% vs 88%,78% vs 88% and 78% vs 78%.The doubtable value interval of urinary ratio of lactulose and mannitol was limited as 0.178 to 0.082.Conclusion Gut barrier dysfunction should be suspected when critically ill patients presented eertains gastrointestinal symptoms and had the proofs of increasing intesti- nal permeability,hypoperfusion of gut and higher level of plasma endotoxin.
9.Correlation of coagulation function to tumor stage and metastasis status in patients with renal cell carcinoma

Objective To investigate the relationship between coagulation function and the tumor size, clinical stage and metastasis status in renal cell carcinoma (RCC) patients. Methods A total of 290 RCC patients from 2004 to 2009 were included in present study. There were 181 male patients and 109 female patients. The average ages was (56.3± 13. 5) years. There were 252 cases of clear cell carcinoma, 19 cases of papillary carcinoma, 5 cases of chromophobe cell carcinoma, 3 cases of cystic RCC, and 11 cases of other types. TNM classification: stage Ⅰ 202 patients, stage Ⅱ32 patients , stage Ⅲ 32 cases, stage Ⅳ 24 cases. There were N0 264 patients, N1 11 patients and N2 15 cases. There were M0 273 cases, M1 17 cases. One hundred and eighty-six cases of benign renal tumors were set as the control group. Fibrinogen (Fib), prothrombin time (PT), activated partial thromboplastin time (APTT) and international normalized ratio (INR) were detected. Results The preoperative serum Fib of RCC patients was (39. 6±15.6) g/L, the control group was (32. 8±8. 2)g/L. There was significant difference between them (P<0. 05). The values of preoperative APTT,INR, and PT were (31.7±5.2)s, (1.0±0. 1), (11.2±1.3)s in RCC group and (32. 4±4.2)s,(1.0±0. 1), (11.1±1.3)s in the control group. There were no significant differences between them (P<0.05). The values of Fib in stage Ⅰa, Ⅰb, Ⅱ, Ⅲ, Ⅳ groups were (32. 6±6. 6), (36. 1±8. 7),(48.8±21.6), (49.9± 17.8) and (59.7± 19.2)g/L, respectively. There was no significant difference between stage Ⅰ, and the control group. But the other stages showed significant difference with the control group (P<0.01). Hyperfibrinogenemia (Fib>44.0 g/L) in the RCC group accounted for 74 cases (25.5%). If the value of Fib ≤44. 0 g/L, 92.1% of patients can be excluded from the probability of metastasis. Conclusions Preoperative plasma Fib levels could be elevated in RCC patients with distant metastasis or lymph node metastasis. Increased preoperative plasma Fib levels, especially hyperfibrinogenemia may be a predictor of metastasis.
10.Effect and impact of holmium laser versus thulium laser enucleation of the prostate on erectile function.

OBJECTIVETo compare the effect and impact of holmium laser enucleation of the prostate (HoLEP) and 120-W thulium: YAG vapoenucleation of the prostate (ThuVEP) on erectile function in the treatment of benign prostatic hyperplasia (BPH).
METHODSWe retrospectively analyzed 93 cases of symptomatic BPH treated by HoLEP or 120 W ThuVEP. We made comparisons between the two groups of patients in the baseline and postoperative clinical and surgical indexes as well as their IPSS, quality of life (QOL), maximum flow rate (Qmax), postvoid residual urine volume (PVR), and IIEF-EF scores before surgery and during the 12-month follow-up.
RESULTSThuVEP, in comparison with HoLEP, achieved a significantly shorter operation time ([57.6 +/- 12. 8] vs. [70.4 +/- 21.8] min, P = 0.001) and a higher laser efficiency ([0.71 +/- 0.18] vs. [0.62 +/- 0.19] g/min, P = 0. 021). At 1, 6, or 12 months of follow-up, no significant differences were observed in IPSS, OOL, Omax, and PVR between the two groups (P > 0.05). Both the HoLEP and ThuVEP groups showed low incidences of complications and remarkably improved IIEF-EF scores at 12 months postoperatively, but with no significant differences (both P > 0.05). However, in those with relatively normal erectile functions before operation, the mean IIEF-EF score was reduced from 22.8 +/- 2.2 preoperatively to 21.0 +/- 2.7 after HoLEP, (P = 0.036).
CONCLUSIONBoth HoLEP and 120W ThuVEP are effective and safe in the treatment of BPH. Compared with HoLEP, 120 W ThuVEP has even a higher laser efficiency. However, neither can significantly improve erectile function, and HoLEP may have a short-term negative impact on the relatively normal erectile function of the patient.
