1.Clinical Analysis of Preoperative Neoadjuvant Chemotherapy by Thalidomide Combined with Gefitinib for Non-small Cell Lung Cancer

Objective To investigate the clinical effect of preoperative neoadjuvant chemotherapy by thalidomide combined with gefitinib for non-small cell lung cancer (NSCLC) patients and provide clinical basis for NSCLC therapy.Methods From 2011/1 to 2013/1,we collected 160 NSCLC cases in our hospital and divided them into 2 groups randomly,80 cases in control group and 80 cases in observation group.The patients in the control group were treated with conventional surgical treatment and patients in the observation group were treated with preoperative neoadjuvant chemotherapy by thalidomide combined with gefitinib before surgery.The clinical effects,the 6 months and 1 year survival rates,and the toxicity effects were observed,and the clinical effects and survival rates between the 2 groups were compared.Results The observation group had a therapeutic efficiency ratio (TER) of 80.0% which was significantly higher than the control group (50.0%),and the difference was statistically significant (P<0.05).The 6 months survival rate and 1 year survival rate were 97.5% and 92.5%,which were both significantly higher than those of control group (87.5% and 77.5%),and all differences were statistically significant (P<0.05).The toxicity effects of observation group were lower.Conclusions Preoperative neoadjuvant chemotherapy by thalidomide combined with gefitinib for NSCLC therapy is effective and safe,which is worthy of study and further application in clinical treatment..
2.Significance of serum cytokines concentrations and APACHE scores in evaluating the illness state for Critical patients

Objective To investigate the clinical significance of serum cytokines concentrations and A- PACHE scores in evaluating the illness state for critical trauma patients.Methods A clinical prospective self-control trial was performed,in which 36 patients admitted to ICU by SIRS were enrolled.Objects were divided into mild and severe trauma group according to APACHE score.The TNF-?and IL-6 concentrations were determined on the 1st, 3rd and 5th day of admission,the APACHE score were assessed at the same time.Statistic analysis was performed according to this group.Results The TNF-?concentrations decreased continuously in the following days while IL-6 decreased from the 7th day in the mild trauma group.In the severe trauma group the TNF-?and IL-6,APACHE score concentrations kept increasing.There was a significant difference of TNF-?and IL-6 concentrations between severe trauma and mild trauma group.Conclusion Dynamic measurement of TNF-?and IL-6 concentrations with APACHE score provide great help to evaluate the illness state and predict the prognosis.

Host genetic factors, such as human leukocyte antigen (HLA) alleles, are important in Human immunod-eficiency virus (HIV) infection and its progression to AIDS. HLA class I genes, especially highly polymorphicHLA-B genes, are involved in the activation of HLA-restricted cytotoxic T lymphocytes (CTLs) against HIV, andthus control susceptibility to or protect against this virus. The present study was aimed to determine the distributionof HLA-B alleles in the Chinese Uygur ethnic group and its association with HIV infection. One hundred ten healthycontrol (HIV negative) and 128 HIV positive Chinese Xinjiang Uygur ethnic individuals were used in this study.HLA typing for B allele was performed by polymerase chain reaction (PCR) with sequence-specific primers (SSP).Hardy-Weinberg equilibrium was calculated using POPGENE software for the healthy control group. The HLA-Bfrequency of each allele was compared between the patients and the controls using the chi-square test. In HIV-1-pos-itive group, gene frequency of allele B * 4901 was significantly higher compared to the healthy control subjects (P=0.02, OR=3.06, 95%CI=1.16~8.10 forB*4901). In contrast, the gene frequency of B * 40 in healthy controlswas significantly higher than in the HIV-positive patients (P=0.02, OR=0.39, 95%CI=0.07~0. 92 for B* 40).In this study, HLA allele B * 4901 may be associated with increased susceptibility to HIV-1 infection, whereas the B* 40 allele may be associated with resistance to H HIV-1 infection.
4.Intra-specific genetic relationship analyses of Elaeagnus angustifolia based on RP-HPLC biochemical markers.

Elaeagnus angustifolia Linn. has various ecological, medicinal and economical uses. An approach was established using RP-HPLC (reversed-phase high-performance liquid chromatography) to classify and analyse the intra-specific genetic relationships of seventeen populations of E. angustifolia, collected from the Xinjiang areas of China. Chromatograms of alcohol-soluble proteins produced by seventeen populations of E. angustifolia, were compared. Each chromatogram of alcohol-soluble proteins came from a single seed of one wild plant only. The results showed that when using a Waters Delta Pak. C18, 5 microm particle size reversed phase column (150 mm x 3.9 mm), a linear gradient of 25%-60% solvent B with flow rate of 1 ml/min and run time of 67 min, the chromatography yielded optimum separation of E. angustifolia alcohol-soluble proteins. Representative peaks in each population were chosen according to peak area and occurrence in every seed. The converted data on the elution peaks of each population were different and could be used to represent those populations. GSC (genetic similarity coefficients) of 41% to 62% showed a medium degree of genetic diversity among the populations in these eco-areas. Cluster analysis showed that the seventeen populations of E. angustifolia could be divided into six clusters at the GSC=0.535 level and indicated the general and unique biochemical markers of these clusters. We suggest that E. angustifolia distribution in these eco-areas could be classified into six variable species. RP-HPLC was shown to be a rapid, repeatable and reliable method for E. angustifolia classification and identification and for analysis of genetic diversity.

5.Pharmacokinetics of a long-circulating PEGylated Radix Ophiopogonis polysaccharide.

The pharmacokinetics of a long-circulating PEGylated Radix Ophiopogonis polysaccharide (ROP) was investigated in rats following i.v. or s.c. administration at three dose levels (9, 20, 50 mg x kg(-1)). A moderate coupling reaction between the hydroxyl-activated ROP and the amino-terminated mPEG was chosen to produce PEGylate ROP. The grafting degree of the prepared conjugate was 1.03, and the molecular mass of mPEG used was 20 kDa. High-performance gel permeation chromatorgraphy with fluorescein isothiocyanate prelabeling was established to determine levels of the conjugate in plasma. The results showed that the elimination half-life of the conjugate following s.c. administration was basically identical to that after iv administration. An accurate linear correlation was observed between administration doses and areas under the curve of plasma conjugate level vs. time profile, regardless of the administration route. The absolute bioavailability of the conjugate following sc administration was approximately 56%, and the mean in vivo residence time was 52.1 h, increased 2.4 times compared to those of iv administration. In general, linear pharmacokinetics was observed for the conjugate within the dose range studied, and sc should be a promising administration route for the conjugate.

6.Genetic Diversity and Natural Selection in 42 kDa Region of Plasmodium vivax Merozoite Surface Protein-1 from China-Myanmar Endemic Border.

Plasmodium vivax merozoite surface protein-1 (PvMSP1) gene codes for a major malaria vaccine candidate antigen. However, its polymorphic nature represents an obstacle to the design of a protective vaccine. In this study, we analyzed the genetic polymorphism and natural selection of the C-terminal 42 kDa fragment within PvMSP1 gene (Pv MSP142) from 77 P. vivax isolates, collected from imported cases of China-Myanmar border (CMB) areas in Yunnan province and the inland cases from Anhui, Yunnan, and Zhejiang province in China during 2009–2012. Totally, 41 haplotypes were identified and 30 of them were new haplotypes. The differences between the rates of non-synonymous and synonymous mutations suggest that PvMSP142 has evolved under natural selection, and a high selective pressure preferentially acted on regions identified of PvMSP133. Our results also demonstrated that PvMSP142 of P. vivax isolates collected on China-Myanmar border areas display higher genetic polymorphisms than those collected from inland of China. Such results have significant implications for understanding the dynamic of the P. vivax population and may be useful information towards China malaria elimination campaign strategies.

7.Heat sensitive moxibustion for treatment of nerve root cervical spondylosis.

OBJECTIVETo observe the therapeutic effect of heat sensitive moxibustion treatment for nerve root cervical spondylosis.
METHODSOne hundred and sixty cases were randomly divided into a heat sensitive moxibustion group (n = 54), a traditional hanging moxibustion group (n = 53) and an acupuncture group (n = 53). In heat sensitive moxibustion group, heat sensitive points were explored among acupoints on neck and nucha, lateral part of forearm and crus, etc. In traditional hanging moxibustion group and acupuncture group, Jiaji (EX-B 2) points, Fengchi (GB 20), Jianwaishu (SI 14) etc. were used for hanging moxibustion and acupuncture, respectively. And scores of Pain Rating Index (PRI), as well as therapeutic effect were evaluated before and after treatment.
RESULTSThe effective rate was 98.0% (50/51) in the heat sensitive moxibustion group, 83.0% (39/47) in traditional hanging moxibustion group, and 89.6% (43/48) in acupuncture group. The therapeutic effect of heat sensitive moxibustion group was better than that of acupuncture group (P < 0.05), and it was better in acupuncture group than that of traditional hanging moxibustion group (P < 0.05); PRI scores were all decreased in three groups after treatment (all P < 0.001); pain alleviation in heat sensitive moxibustion group was better than that of acupuncture group (P < 0.05), and it was better in acupuncture group than that of traditional hanging moxibustion group (P < 0.05).
CONCLUSIONThe therapeutic effect of heat sensitive moxibustion treatment for nerve root cervical spondylosis is better than that of traditional hanging moxibustion and acupuncture.

9.Concealed penis affects the morphology of the corpus cavernosum in rats.

OBJECTIVETo explore the influence of concealed penis on the morphology of the corpus cavernosum in rats.
METHODSRat models of concealed penis were established by intra-pocket-suture of the root of the penis. Fifty rats were equally assigned to Groups A (2-month) and B (4-month), each further divided into a buried (n = 15) and a normal subgroup (n = 10). Changes in the morphology of the penile cavernous tissue were observed under the light microscope and transmission electron microscope.
RESULTSCompared with Group A, Group B showed significant ultra-structural pathological changes in the corpus cavernosum, including abnormal arrangement of endothelial and smooth muscle cells, massive hyperplasia of interstitial tissues, narrowed cavernous sinus, atrophic smooth muscle cells, degenerated mitochondria, dilated endoplasmic reticula, decreased dense bodies and contractile fibers, and cytoplasmic vacuolization. No significant differences were found in the appearance and weight of the corpus cavernosum between the buried and normal groups (P > 0.05).
CONCLUSIONConcealed penis does not significantly affect the appearance and weight of the corpus cavernosum, but causes ultra-structural pathological changes in it with the lengthening of time.

10.Differences of risk factors of asymptomatic lacunar infaction and symptomatic nonlacunar infaction diagnosed by CT

Objective To investigate the different pathogeneses of asymptomatic lacunar infaction and symptomatic nonlacunar infaction by investigating their risk factors.Methods A total of 1989 people more than 50 years old,performed physical examination were chosen with the results showing 48 patients with asymptomatic lacunar infaction (group A) and 51 patients with symptomatic nonlacunar infaction (group B).Another 1862 controls without cerebrovascular disease were also chosen.Single factor analysis and logistic regression analysis were used to analyze the risk factors.Each risk factor and amount of risk factors for the two diseases was compared between group A and group B.Results Age,hypertension and smoking were the independent predictors of asymptomatic lacunar infaction (P<0.05).Age,sex,diabetes mellitus,drinking,family history of stroke and stenosis of intracrianial artery were the independent predictors of the symptomatic nonlacunar infaction (P<0.05).Both rates of diabetes mellitus and stenosis of intracrianial artery in group B were higher than those in group A (diabetes mellitus:x2=17.603,P=0.008; stenosis of intracrianial artery:x2=19.319,P=0.005).The amount of risk factors in patients from group B was significantly more than that in patients from group A (Z=2598,P=0.009).Conclusion The risk factors and vascular mechanism in patients with symptomatic nonlacunar infaction and asymptomatic lacunar infaction are different.Diabetes mellitus and stenosis of intracrianial artery are more often noted in patients with symptomatic nonlacunar infarction.
