1.The impact of burden, positive feelings, coping style and social support on caregivers' fatigue of dementia patients

Objective To explore the impact of burden,positive feelings,coping style and social support on caregivers' fatigue of dementia patients.Methods 190 caregivers of dementia patients were investigated by using the questionnaires of Caregivers Burden Inventory(CBI),Positive Aspects of Caregiving(PAC),Simplified Coping Style Questionnaire (SCSQ),Social Support Rating Scale (SSRS) and Fatigue Scale-14 (FS-14).Results The caregivers' fatigue of dementia patients was positively correlated with burden(r=0.56,P<0.01),negative coping style(r=0.31,P <0.01),and negatively associated with positive feelings(r=-0.33,P <0.01),social support (r =-0.36,P < 0.01) and positive coping style (r---0.55,P < 0.01),respectively.Path analysis showed coping style and social support had direct impacts on fatigue; burden had a direct impact on fatigue,and also had an indirect influence on it through coping style or social support; fatigue might be decreased by positive feelings and increased by negative coping style.Conclusion To prevent and relieve the caregivers'fatigue of dementia patients,caregivers should be encouraged to take positive coping style and increase social support so as to reduce burden and increase positive feelings.
2.The physical states of HPV DNA and expression of human papillomavirus late capsid protein 1 in HepG2 cells

Objective To find out the physical state of the human papillomavirus ( HPV) genome in hepatoma cell line HepG2 cells and the regulation of HPV late capsid protein 1 ( L1) expression and to explore the nature of the cytoryctes in HepG2 cells.Methods E2 and E6 in HPV18 were detected by PCR to evaluate the physical state of HPV18 genome .HepG2 L1 expression was detected by ELISA , light microscropy and electron microscrope immu-nohistochemistry assays , Western blot assay using HPV L 1 mice monoclonal antibody .L1 mRNA in HepG2 cells was detected by reverse transcriptional PCR ( RT-PCR) .Results PCR assay displayed that HPV DNA was inte-grated with HepG2 genome.ELISA assay showed that HPV L1 was present in lysate of HepG2 cells.Light micros-cropy demonstrated strong positive reaction in HepG2 cells.In microscopy, in the cytoplasm of partial HepG2 cells, there were lumpish cytorrhyctes materials which consists of very small and uniform particles and these parti -cles were marked by HPV L1 antibody labeled by colloidal gold .Western blot analysis showed a band at 56 ku dis-trict and it was L1 specific strap which demonstrated HPV 18 L1 was present in HepG2 cells.RT-PCR assay demon-strated the presence of L1 mRNA in HepG2 cells.Conclusions HepG2 cells are HPV18-positive HPV DNA ge-nome is integrated with HepG2 cells.HepG2 cells can express L1.The cytorrhyctes in HepG2 cells are composed of HPV18 L1 indicating that L1 can be expressed in HepG2.

5.Multiple Organic Damages in 202 Cases with Kawasaki Disease

Objective To analyze the clinical features of organic damages in Kawasaki disease(KD),and to improve the diagnosis and treatment level.Methods Two hundred and two cases with KD admitted in Children's Hospital Affiliated to Capital Institute of Pediatrics from Jan.2000 to Oct.2007 were reviewed,and their blood routine,urine,stool routine,liver function,kidney function,chest X-ray,and so on,were investigated.The incidence of organic damages in KD was evaluated.Results Among the 202 KD children,137 were male and 65 were female,the male-female ratio was 2.11.0.Most of them were infants,and 71.3% of them were younger than 3 years old.The damages of hematological system were detected in 78/202 cases(38.6%),respiratory system 76/202 cases(37.6%),cardiovascalar system 68/202 cases(33.7%),urinary system 52/202 cases(25.6%),digestive system 23/202 cases(11.4%),and central nervous system 3/202 cases(1.5%).On the other hand,3 cases complicated with septemia,1 case with juvenile rheumatoid arthritis and 1 case with Henoch-Schonlein purpura.Conclusions The data indicate that the incidence of hematological system involvement in the sick-children with KD were highest,with more than 2 organs involved simaltaneously.Many organic damages were involved in KD and diverse clinical manifestation could be found.Physicians should understand not only the typical features but also the rare symptoms and organic damage so as to prevent misdiagnosis and improper treatment.J Appl Clin Pediatr,2009,24(1):31-32
6.Therapeutic strategy of persistent idiopathic atrial fibrillation

Objective The aim of this study is to evaluate the effects of drug,electric cardiovertion,radiofrepuency ablation and implantable anti-atrial-fibrillation pacemaker on patients with persistent idiopathic atrial fibrillation.Methods 58 patients with persistent idiopathic atrial fibrillation were treated with drug or electric cardjovertion,radiofrequence ablation and anti-atrial-fibrillation pacemaker.Results There were 30 patients successfully converted to sinus rhythm by drug.The mean conversion time of drug was 8?5 days.22 patients were converted by electric cardioversion.10% of cases(6 of 58)failed to convert by both methods.In follow-up period,12 cases of patients were healed,19 cases got significant improvement,14 cases got moderate improvement,8 cases had no improvement.The total efficiency rate was 76%.18 cases in successful cardioversion patients were healed,5 cases got significant improvement,5 cases got moderate improvement,4 cases had no improvement.The total efficiency rate was 75%(14 of 18).2 cases got improvement by anti-atrial-fibrillation pacemaker.Conclusion Combined treatment of drug,electric cardioversion,radiofreqency ablation and anti-atrial-fibrillation pacemaker can imrove cure rate of persistent idiopathic atrial fibrillation.
8.AFLP fingerprinting of Pinellia ternata and its application.

OBJECTIVETo obtain more information on DNA fingerprintings and to confirm the genetic relationships of ten wide populations of Pinellia tornata.
METHODCollect wide-type or cultivated P. tornata from ten major production areas nation-wide as material. Twelve combinations of EcoR I and Mse I primers were used to asses divergence among ten populations. The data were analyzed using unweighted pair-group method, based on arithmetic averages (UPGMA) bootstrap analysis. Cluster analyses were performed by using NTSYSpc software version 2.1. The alkaloid was extracted from P. ternate with chlorolform and determined in UV absorption spectroscop.
RESULT1,673 bands were applied with 12 combinations. These results of cluster were congruent with differences in total alkloids.
CONCLUSIONThe results of the study showed a potential application of AFLP fingerprinting for identification of P. ternata.

9.An anatomical study and clinical application of free transfer of foot tissues pedicled with plantar metatarsal artery

Objective To explore a new method of functional reconstruction of hand digits and joints with free transfer of foot tissues so as to increase the success rate of the operation.Methods After micro-anatomic study of the plantar and dorsal metatarsal arteries,retrograde and free grafts of foot tissues pedicled with plantar metatarsal arteries were designed and applied in transplantation to treat 76 cases of hand digital or joint defects.The surgeries included 58 cases of transfer of the second toe,four cases of transfer of composite tissues of the second toe, eight cases of transfer of proximal interphalangeal joint,and six cases of nail flap transfer.Results The mi- cro-anatomic study found that the first plantar metatarsal artery was anatomically constant and the diameter of its branch to the second toe was larger than that of the first dorsal metatarsal artery.Flaps survived in 75 of the 76 patients(98.7%),with fine appearance and significantly improved function.One patient who had received free transfer of the second toe to reconstruct the thumb function had to undergo a second repair with infraclavicula skin tube because of refractory arteriospasm of anastomosed vessels.Conclusion Transfer with free retrograde grafts of foot tissues pedicled with plantar metatarsal artery to reconstruct hand functions can effectively improve the success rate of the operation,because it is free of the shortcomings of great anatomic variation of blood vessels and time-consuming and complex procedures in conventional transfer.
10.Impact of endogenous hydrogen sulfide on the content of pulmonary artery collagen in rats with high pulmonary blood flow.

AIMTo explore the possible impact of endogenous hydrogen sulfide (H2S) on the content and metabolism of collagen in rats with high pulmonary blood flow.
METHODSThirty-two male SD rats, weighing 120-140 g, were randomly divided into 4 groups (n = 8), shunt group, shunt + PPG (propargylglycine, an antagonist of endogenous H2S producing enzyme) group, sham group and sham + PPG group. Rats in shunt group and shunt + PPG group were subjected to an abdominal aorta-inferior vena cava shunt to create an animal model of high pulmonary flow. In the sham group and sham + PPG group, rats experienced the same experimental processes except the shunting procedure. After 4 weeks of experiment, lung tissue H2S content of rat was determined by a modified sulfide electrode method. Pulmonary artery collagen I, collagen III, MMP-13 and TIMP-1 protein expressions of rat were investigated by immunohistochemistry.
RESULTSAfter 4 weeks of experiment, lung tissue H2S content increased significantly in rats of shunt group as compared with that of sham group (P < 0.05). Pulmonary artery collagen I and collagen III protein expression increased obviously in rats of shunt group as compared with that of sham group (P < 0.01). After administration of PPG for 4 weeks, lung tissue H2S content decreased significantly in rats of shunt + PPG group as compared with that of shunt group (P < 0.05). In contrast to rats in shunt group, collagen I and collagen III protein expression in pulmonary arteries of shunt + PPG group increased significantly, respectively (P < 0.05). Compared with rats of shunt group, pulmonary artery MMP-13, TIMP-1 and the ratio of MMP-13/TIMP-1 in shunt + PPG group down-regulated significantly (P < 0.05).
CONCLUSIONEndogenous H2S might play a protective regulatory role in the development of pulmonary hypertension and pulmonary vascular structural remodelling in rats by decreasing the content of pulmonary artery collagen resulting from catabolism of collagen.
