1.Effect of light therapy combined with fluoxetine on winter attack depression

ObjectiveTo observe the effect of light therapy combined with fluoxetine on winter attack depression.Methods68 patients of winter attack depression were divided randomly into the treatment group and control group with 34 cases in each group. Patients of the treatment group were treated by bright white light combined with fluoxetine (20 mg/d), but cases of the control group were treated only with fluoxetine. Therapeutic effects of two groups were evaluated with BDI, HAMD, and SCL-90 scales and traditional evaluation.ResultsPatients of the treatment group had a better effect than that of the control group (P<0.05).ConclusionLight therapy combined with fluoxetine has a good short and long term effect on winter attack depression, and can prevent from recurrence.

3.Determination of trace elements in whole blood of patients with chronic Keshan disease and dilated cardiomyopathy

Objective To detect the levels of five trace elements in whole blood of patients with Keshan disease(KSD) and dilated cardiomyopathy(DCM) and explore their role in the pathogenesis of KSD.Methods One hundred and four patients with chronic KSD were selected from Keshan diseased areas in Shandong,Sichuan and Inner Mongolia.Thirty patients with DCM were selected from Qilu Hospital of Shandong University,Jinan Central Hospital,The First People's Hospital.Ninety-one healthy people from KSD endemic areas and 39 healthy people from Jinan were selected as endemic healthy controls and non-endemic healthy controls,respectively.Blood samples were collected to determinate the level of selenium (Se),copper (Cu),zinc (Zn),chromium (Cr) and manganese (Mn) with fluorescence method and atomic absorption spectrometry,according to the principle of informed consent.Results The level of Se,Zn and Cr of KSD group[(36.0 + 4.9)μg/L,(22.73 + 4.62)mg/L,(0.56 + 0.17)mg/L] was significantly lower than that of non-endemic healthy controls [(56.4 ± 6.8)lμg/L,(25.35 ± 4.44)mg/L,(0.71 ± 0.17)mg/L,all P < 0.05],but the level of Cu of KSD group[(0.95 ± 0.24)mg/L] was significantly higher than that of non-endemic healthy controls[(0.73 ± 0.13) mg/L,all P < 0.05].The level of Se and Cr of KSD was significantly lower than that of endemic healthy controls[(54.5 ± 5.4)μg/L,(0.87 ± 0.02)mg/L,P < 0.05],and Cu was significantly higher than that of endemic healthy controls[(0.66 ± 0.02)mg/L,P < 0.05].The level of Cu and Zn of KSD was significantly lower than that of DCM [(1.21 ± 0.23)mg/L,(27.09 ± 7.10)mg/L,all P < 0.01].The level of Se and Cr of DCM group[(39.6 ± 3.5)μg/L,(0.58 ± 0.14)mg/L] was significantly lower than that of non-endemic healthy controls(all P < 0.01),but Cu[(1.21 + 0.23)mg/L] was significantly increased (P < 0.01).Compared with non-endemic healthy controls,the level of Se of endemic healthy control group was significantly decreased (P < 0.01),while Cu was significantly increased (P < 0.01).Se,Zn and Cr level of KSD decreased gradually following elevated heart function level,but the level of Cu gradually increased.Conclusions The metabolism of Se,Cr,Cu and Zn is unbalanced in KSD patients,whose Se level is still lower than that of people in non-endemic areas.The change of Se,Cr,Cu and Mn level between KSD and DCM is consistent.
4.Experimental observation of articular impairment of New Zealand rabbits by deoxynivalenol

Objective To observe toxic effect of deoxynivalenol(DON)on articular cartilage and synovium of New Zealand rabbits's knee ioints.Methods Fifteen male rabbits were divided randomly into 3 groups:control, high-dosage,and low-dosage group.In high-dosage and low-dosage group,saline solution of DON was injected with a dose of 0.10 and 0.05 ms/kg every 48 h into ear vein of rabbits.Specimen of articular cartilage and synovium were through pathologY methods,and IL-1β,TNF-α,NO levels were assayed in joint liquid,after 20 days. Results Morphological changes were observed, such as synovium inflammative infiltration, chondrocytes deformation and necrosis under light microscope.The levels of IL-1β,TNF-α and NO had statistical significance in comDarison between 3 grouPs(F=19.396,18.195,22.136,P<0.05).The levels of IL-1β,TNF-α and NO were significantly higher(all P<0.05),high-dosage[(0.451±0.091),(0.575±0.122)μg/L;(70.27±11.53)μmol/L] and low-dosage group[(0.295±0.107),(0.387±0.131)μg/L;(45.32±12.24)μmol/L]compared with control ((0.1 13±0.049),(0.138±0.087)μg/L;(23.56±9.35)μmoL/L],and high-dosage compared with low-dosage group Conclusions DON results in articular and synovial impairment,which has the symptom similar to osteoarthritis. DON probably causes osteoarthritis.
5.Possibility of psychotic patients treated in open wards in general hospital

ObjectiveTo explore the possibility and significance that psychotic patients treated in open wards in general hospital,a new managing model for psychosis cases.Methods1 200 psychotic cases were treated in the open ward,that was a new managing model for psychotic patients.ResultsNew managing model had following advantages: a. free for patients to communicate with outsides and keep their secret and human rights; b. having better effective,shorter housing days,and more economy compared with controlled cases.ConclusionSetting up open managing psychosis wards in general hospital can help treatment of psychotic patients,and it is the tendency of modern mental health.
6.Influence of integrated education of hospitalization and follow-up on change of life by this treatment among patients with metabolic syndrome

Objective To explore the influence of integrated education of hospitalization and follow-up that impacts on the mode change of life and compliance to the treatment among the patients with metabolic syndrome.Methods 56 inpatients with metabolic syndrome (MS) were selected as the education objects.The pattents were given the systematic class education and bed-side education in hospital,and were follewed up and instructed the diet and exercise behavior after discharge.Results The satisfaction rate of total calories control per day achieved 71.4%in different stages after discharge.Compared to those before being hospitalized,there were statistical meanings in the percents of earbohydrate,fat and protein in total calories.the scores of exercise knowledge,exercise attitude and exercise compliance (P<0.05).Conclusions The integrated education of hospitalization and fellow-up makes the MS patients'total calories per day controlled into an ideal state, optimize the diet structure,and improve the exercise knowledge,exercisc attitude and exercise compliance.
7.Comparison of characterization of hemagglutination and HA1 sequence of currently circulating strains of influenza A (H3N2) virus in Zhejiang province and in Japan.

OBJECTIVETo compare the characterization of hemagglutination and the sequence of HA1 region of the influenza A (H3N2) virus in Zhejiang province and Japan in recent years.
METHODSHemagglutinin antigenic analysis of influenza A (H3N2) viruses which were isolated in Zhejiang province was carried out and to compare their characterization of hemagglutination and the sequence of amino acids in HA1 region with the virus strains from Japan during the same period.
RESULTSThe antigenicity and the amino acid sequence in HA1 region of the virus strains prevalent in Zhejiang in recent years have changed to some extent. Most of the virus strains were lack of hemagglutination reaction to chicken red blood cell and became "O" phase virus strains. The amino acid sequence of HA1 region of the virus strains exhibited only 94.82% of its homology with that of A/Wuhan/359/95 strain. The virus strains of Japan in the same period showed similar characteristics.
CONCLUSIONHA1 gene variation of influenza A (H3N2) viruses was the main cause leading to the spread of influenza in the last years.

8.Gene expression profiles of peri-implantation endometrium of high responders during controlled ovarian hyperstimulation

Objective To investigate the gene expression profiles of peri-implantation endometrium of high responders during controlled ovarian hyperstimulation. Methods High responders with cancelled embryo-transfer during controlled ovarian hyperstimulation (high responder group, n=4) and healthy fertile volunteers (control group, n=3) were performed endometrial biopsies during peri-implantation. Histologic changes of endometrium were observed by HE staining, genes of differential expression were screened with microarrays Affymetrix U133A 2.0 and identified by Real-time PCR. The biological process analysis was performed by online biological information analysis tool PANTHER. Results The ehdometrium was in mid-secretory phase in control group, while development delay was found in some glandular organs in endometrium of high responder group. Three hundred and sixty-four genes of differential expression were screened, among which 233 were up-regulated genes and 131 were down-regulated genes. OPN, PLA2G2, DPPIV, IGFBP5 and SSAT were identified as endometrial function-related genes, whose Real-time PCR findings were positively correlated to gene signal values detected by microarray(r=0.44, P<0.01). PANTHER analysis indicated that genes of differential expression participated in the biological processes of cytokine signal transduction and immunological regulation. Conclusion Ovarian high response affects the gene expression profiles of peri-implantation endometrium, which may be one of the causes of sub-optimal endometrial receptivity.
9.Clinicopathological implications of positive CK7 expression in colorectal carcinoma.

OBJECTIVETo investigate the clinicopathological significance of positive CK7 expression in human colorectal carcinoma (HCC).
METHODSImmunohistochemistry was used to detect CK7 and CK20 protein expressions in 68 cases of HCC, 20 cases of canalicular adenoma (CA), 5 cases of serrated adenoma (SA) and 20 cases of hyperplastic polyps (HP).
RESULTSThe positivity rate of CK20 expression was 89.7% in HCC, and 100% in CA, SA and HP. In HCC, the expression rate of CK7 (39.7%) was not correlated with Dukes' classification, differentiation and tumor location. CK7 positivity rate in colon cancer was 35.7% (15/52) and 42.3% (11/26) in rectal cancer. CK7 expression was negative in CA. CK7 positivity rate in SA was 49% and 30% in HP.
CONCLUSIONCK7 is a possible marker for colorectal carcinogenesis from HP to SA, and ultimately to HCC, and examination of the colorectal polypoid lesions for CK7 expression can be significant for estimating the colorectal polypous cancerization.

10.Prospective cohort study on the relationship between pathogenic bacteria in the nasal middle meatus and acute bacterial respiratory infection in children.

OBJECTIVETo study the relationship between pathogenic bacteria in the nasal middle meatus and acute bacterial respiratory infection in children.
METHODSThree hundred and twenty eight children with respiratory infection (mean age 8 years) were included into the prospective cohort study. The mucosal fluid specimens from the nasal middle meatus were collected under an endoscope for bacterial culture. The patients with bacterial culture positive were defined as the Exposed group and those with bacterial culture negative as the Non-exposed group. The grouping of the patients was blinded to the patients, patients' parents and physicians. Both groups received anti-virus and symptomatic treatments, without antibiotic administration. Five days later, the patients were evaluated as to whether they had bacterial infection based on the leucocyte count and CRP results.
RESULTSOf the 328 patients, 168 had a positive nasal bacterial culture. The incidence of bacterial respiratory infection in the Exposed group [51.2% (86/168)] was significantly higher than in the Non-exposed group [13.1% (21/160)] (P < 0.01). The relative risk of bacterial respiratory infection occurrence in patients with nasal bacterial culture positive was 3.9002.
CONCLUSIONSThe children with respiratory infection who had potential pathogenic bacteria in the nasal middle meatus were more prone to develop bacterial respiratory infection.
