1.Percutaneous fenestration of intimal flap and endovascular stent placement for aortic dissection: 1 case report and reference review

Objective To evaluate the safety and effect of percutaneous fenestration of intimal flap(FIF) and endovascular stent(ES) placement for aortic dissection. Methods Male patient, 54 years old. DeBakey Ⅲb aortic dissection, tear of intimal flap situated at the beginning of desconding aorta, developed to abdominal aorta and right iliac artery. The true lumen was 3 mm at narrowest locatation. Through femoral artery approach, percutaneous fenestration of intimal flap and ES placement are operated and four ES were placed. Results The blood flow of aortic true lumen and branches were resumed. The true lumen raised to 12 3 mm at the narrowest locatation. The clinical symptoms vanished. Conclusion Percutaneous fenestration and ES placement for aortic dissection feature little injure, high safety and effecacy. So, It is the first choice for certain aortic dissection.
2.Effects of Pollen Typhae Total Flavone on Inflammatory Factors and Insulin Sensitivity in Type 2 Diabetic Rats

Objective To investigate the effects of Pollen Typhae total flavone ( PTF) , an extract from Pollen Typhae which has the actions of activating blood and removing stasis, on inflammatory factors and insulin sensitivity in type 2 diabetic rats. Methods SD rats were used as the experimental animal. Type 2 diabetic rats induced by high fat diet plus low dose of streptozotocin were randomly divided into model group, PTF group (in the dosage of 200 mg·kg-1·d-1) , and rosiglitazone group (in the dosage of 4 mg·kg-1·d-1) . Additionally, normal control group was set up. After treatment for 4 weeks, plasma interleukin 6 ( IL-6) and tumor necrosis factor alpha ( TNF-α) levels were detected, the insulin tolerance test ( ITT) was performed, and the protein expression of suppressor of cytokine signaling-3 ( SOCS-3) in skeletal muscle was determined. Results After treatment for 4 weeks, the plasma levels of IL-6 and TNF-α, the homeostasis model of insulin resistance ( HOMA-IR) , and expression level of SOCS-3 in skeletal muscle in the model groups were significantly increased ( P﹤0.05) as compared with those in the normal control group, and insulin tolerance was also impaired in the model group ( P﹤0.05) . Compared with the model group, IL-6 level and HOMA-IR were markedly decreased in the PTF group ( P﹤0.05) , and the impaired insulin tolerance was obviously improved (P﹤0.05) . The level of SOCS-3 in the skeletal muscle of PTF group was also much lower than that of the model group and rosiglitazone group (P﹤0.05) . Conclusion PTF has effects on decreasing the levels of plasma IL-6 and SOCS-3 in the skeletal muscle and on improving insulin sensitivity in type 2 diabetic rats.
3.Study of the efficacy and safety of diacerhein in the treatment of knee osteoarthritis

Objective To evaluate the efficacy and safety of diacerhein in the treatment of patients with knee osteoarthritis.Methods The efficacy and safety of diacerhein was randomly investigated in 42 pa- tients with knee osteoarthritis using parallel group methodology and a double-dummy technique to ensure dou- ble blind status with respect to diacerhein and control drugs diclofenac.Results Significant changes were ob- served in 20 meters walk pain,knee joint tenderness scale,WOMAC index scale,5F-36 health survey,knee joint swelling scale,compared with baseline(P>0.05)in both diacerhein and diclofenac group respectively.No difference was found between diacerhein group and diclofenae group.The patient global assessment and physi- cian's global assessment were similar in diacerhein group and diclofenac group(P>0.05).The side effect was similar in two groups.All of these side effects in gastrointestinal tract appeared to be transient.Conclusion Diacerhein can effectively relieve pain and swelling of knee osteoarthritis,and provides us a new effective and safe approach for treating knee osteoarthritis.
4.Simultaneous determination of gastrodin and eight nucleosides and nucleobases in Tibet cultured gastrodia elata by HPLC method.

This study aims to establish an HPLC method for simultaneous determination of gastrodin and eight nucleosides and nucleobases components in Gastrodia elata. The separation was carried out on an Agilent Zorbax Bonus-RP (4.6 mm x 250 mm, 5 μm) column with a methanol-(0.04% acetic acid) water solution gradient elution program at a flow rate of 1.0 mL x min(-1). The column temperature was 36 degrees C, and the detection wavelength was 254 nm. The volume of injection was 20 μL. The nine components including gastrodin, cytosine, uracil, cytosine, adenine, thymine, uridine, guanosine and adenosine were well separated. The calibration curve was well linear in the range of 2.04-262.00 mg x L(-1), 0.20-24.67 mg x L(-1), 0.18-23.75 mg x L(-1), 0.20-25.83 mg x L(-1), 0.20-26.67 mg x L(-1), 0.16-20.00 mg x L(-1), 0.22-27.71 mg x L(-1), 0.20-24.29 mg x L(-1), 0.24-30.58 mg x L(-1), respectively, and the correlation coefficient was between 0.998 9-0.999 9. The average recovery of gastrodin and eight nucleosides and nucleobases were 96.4%-99.6%, RSD less than 2.7% (n = 6). The contents of gastrodin in all the seven Tibet cultured Gastrodia elata samples were over 2 mg x g(-1). Further, all samples contain higher contents of adenosine, guanosine, uridine and cytidine compared to low contents of cytosine, uracil, adenine and thymine. The established method is accurate, reproducible and suitable for the determination of gastrodin and eight nucleosides and nucleobases comppnents in Gastrodia elata.

5.Thinking of study on secondary development of major traditional Chinese medicine varieties based on system theory.

Under the guidance of the system theory, in response to the lack of the overall guideline and practical methods for the secondary development of TCMs at present, we introduced the overall thought of the secondary development of major TCM varieties, as well as the roles and contents of clinical research, pharmacology and pharmaceutics in the process of the secondary development of major TCM varieties, so as to provide systematic strategies and methods for the development of major TCM varieties.

6.Study thought of material basis of secondary development of major traditional Chinese medicine varieties on basis of combination of in vivo and in vitro experiments.

The secondary development of major traditional Chinese medicine varieties is one of important links during the modernization, scientification and standardization of traditional Chinese medicines. How to accurately and effectively identify the pharmacodynamic material basis of original formulae becomes the primary problem in the secondary development, as well as the bottleneck in the modernization development of traditional Chinese medicines. On the basis of the existing experimental methods, and according to the study thought that the multi-component and complex effects of traditional Chinese medicine components need to combine multi-disciplinary methods and technologies, we propose the study thought of the material basis of secondary development of major traditional Chinese medicine varieties based on the combination of in vivo and in vitro experiments. It is believed that studies on material basis needs three links, namely identification, screening and verification, and in vivo and in vitro study method corresponding to each link is mutually complemented and verified. Finally, the accurate and reliable material basis is selected. This thought provides reference for the secondary development of major traditional Chinese medicine varieties and studies on compound material basis.

8.Establishment of rabbit liver cancer model by implanting VX2 tumor under ultrasound guidance and its sonographical evaluation

Objective To improve the establishment of rabbit model with VX2 liver tumor and assess sonographical value in monitoring the tumor growth.Methods Fifteen New Zealand white rabbits were implanted with VX2 tumor in two liver lobes under ultrasound guidance percutaneously.Ultrasound examinations were performed at twenty days and thirty-six days after implantation.At the same time exploratory laparotomy was performed.Results Among 30 implanted tumors of 15 rabbits,16(53%) tumors were successfully implanted with few complications.It took 15 to 40 minutes to implant tumors in one rabbit.Implantation rates of left lobe and right lobe were 53% and 50% respectively,and there was no significant difference.Ultrasound examination could monitor tumor growth well.Conclusions After the improvement,the establishment of model was easier,less time-consuming,minimally invasive and caused few complications than other implantation methods.Serial ultrasound examinations should be performed in monitoring the tumor growth.
9.Pharmacokinetics of Paeonia lacliflora and Glycyrrhiza uralensis Compound

AIM: To study compatibility rationality of combination of Paeonia lacliflora and Glycyrrhiza uralensis. METHODS: The effective combination of paeoniflorin(44% purity),glycyrrhizic acid(50% purity) and liquorice flavones(52% purity),glycyrrhizic acid(50% purity) and liquorice flavones(52% purity) were respectively administered to rats.Pharmacokinetic change of these constituents in rat blood was studied. RESULTS: The pharmacokinetic parameters of these constituents in rat blood showed that the increases in AUC and C_(max) of effective combination group were more than that of glycyrrhizic acid group or that of liquorice flavones group.T_(max) of the former was extended with respect to the latters.Clearance of effective combination markedly slowed down. CONCLUSION: The effective combination of paeonia lacliflora and Glycyrrhiza uralensis have the advantage of either Paeonia lacliflora or Glycyrrhiza uralensis.
10.Hemostatic Effect of Gongxueting Mixture on Metrorrhagia and Metrostaxis with Blood Stasis Syndrome and Its Mechanism

A prospective clinical trial was carried out to explore the hemostatic effect of Gongxueting Mixture on metrorrhagia and metrostaxis with blood stasis syndrome and its mechanism. Bleeding time and bleeding volume in 37 cases were detected before and after treatment to assay the hemostatic effect. Indexes of blood analysis, blood rheology, blood coagulation and prostaglandin in 8 cases were determined to study the mechanism. The results showed that Gongxueting Mixture shortened the menstrual period and regulated the volume of menses (P 0.05 ). It is concluded that Gongxueting Mixture is effective for metrorrhagia and metrostaxis with blood stasis syndrome (the total effective rate being 91.89% ), and enhancement of platelet aggregation, promotion of fibrinolytic activity and regulation of prostaglandin release may be its hemostatic mechanism.
