1.Application of impulse oscillometry in the measurement for childhood bronchial hyperresponsiveness

Objective To explore the effectiveness of impulse oscillometry(IOS)in airway responsiveness measurement and to find out the positive threshold of IOS for asthma diagnosis. Methods Seventy-nine children aged 6-14 years who had suspicious asthma,were recruited into the study. The positive criteria of the methacholine bron-chial provocation test was a 20% reduction in forced expiratory volume in the first second (FEV 1 )compared to base-line. Simultaneously measured changes in various parameters of IOS,including resonant frequency(Fres),impedance at 5 Hz(Zrs),resistances at 5 and 20 Hz(R5,R20),reactance at 5 Hz(X5),and area of reactance(AX). The results of the challenge test were divided into positive and negative groups according to the pulmonary ventilation function me-thod. The differences between the 2 groups of IOS parameters before and after the challenge test,and the correlation be-tween the change rate of FEV1 and the change rate of IOS parameters were compared,and the positive judgment criteria of IOS parameters in the determination of respiratory responsiveness were determined. Results The positive group of bronchial provocation test had 37 patients and negative group had 42 patients. There was no significant difference in the basic values of parameters between the positive group and the negative group (all P > 0. 05). Changes in Zrs,R5,X5 of IOS were correlated with changes in FEV1 (r = 0. 374,0. 310,0. 449,all P < 0. 05). By single factor analysis,the area under the receiver operating characteristic(ROC)carve (AUC)showed:basic value of Zrs increased by 45. 85%,R5 increased by 45. 72%,X5 increased by 80. 74% respectively compared to the baseline showed the optimal combination of sensitivity and specificity. In multivariate Logistic regression models,when Zrs and R5 were combined to measure the airway responsiveness,the sensitivity and specificity were 73. 0% and 81. 0%,respectively. Conclusions IOS and spirometry can be used to determine airway responsiveness in children during methacholine bronchial challenge. Zrs≥45. 85%,or R5≥45. 72%,or X5≥80. 74%,or Zrs and R5 of multiple regression formula can be used as the positive criteria for young children with airway heperresponsiveness,the combination of Zrs and R5 has higher sensitivity and specificity.

3.Serum non-esterified fatty acids to albumin ratio increased significantly in children with nephrotic syndrome.

OBJECTIVETo analyze serum levels of non-esterified fatty acids (NEFA) and albumin (ALB) in children with nephrotic syndrome (NS) and investigate the clinical significance of altered serum NEFA to ALB ratio in children with NS in acute and remission phases.
METHODSSerum levels of NEFA and ALB were measured in 55 NS children in acute phase, in 33 NS children in remission and in 122 healthy control children, and the ratio of NEFA to ALB was calculated. The other lipid/lipoprotein and renal function parameters were also analyzed in these children.
RESULTSCompared with the healthy control children, children with NS had a significantly decreased serum ALB level (t=11.152, P<0.001) and a significantly increased NEFA to ALB ratio (t=4.991, P<0.001). Compared with NS children in remission, those in acute phase showed a significantly decreased ALB (Z=7.822, P<0.001) and an increased NEFA to ALB ratio (t=4.991, P<0.001). In all the NS children, NEFA to ALB ratio was positively correlated with the levels of TC (r=0.564, P<0.001), TG (r=0.444, P<0.001), LDL-C (r=0.625, P<0.001), urea (r=0.437, P<0.001), creatinine (r=0.278, P=0.013), and uric acid (r=0.397, P<0.001), while negatively correlated with the level of total protein (r=-0.461, P<0.001). Multiple linear regression analyses showed that NEFA to ALB ratio was independently associated with serum urea levels (β=0.703, P=0.001; adjusted R=0.494) after adjustment of other related factors.
CONCLUSIONSerum NEFA to ALB ratio is significantly increased in NS children in close association with impaired kidney function, and may function as a novel parameter for assessing the development of NS.

4.Application of endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis of mediastinal lesions.

BACKGROUNDMediastinal lesions are often difficult to diagnose in clinical practice because of the unique anatomical position of the mediastinum, which makes performance of biopsy difficult. The value of endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis of lung cancer and mediastinal lymph node staging has been widely accepted. However, few studies have been conducted on the value of endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis and differential diagnosis of mediastinal lesions. The current study was conducted to investigate the value of endobronchial ultrasound-guided transbronchial needle aspiration in the diagnosis and differential diagnosis of isolated mediastinal lesions without lung abnormalities.
METHODSWe retrospectively analyzed the data of patients with isolated mediastinal lesions without lung abnormalities for whom endobronchial ultrasound-guided transbronchial needle aspiration examination was performed at the Department of Thoracic Surgery of Peking University People's Hospital, between September 2009 and December 2010. For patients who could not be diagnosed with endobronchial ultrasound-guided transbronchial needle aspiration, surgical biopsy or more than 6 months of clinical and imaging follow-up was carried out.
RESULTSEndobronchial ultrasound-guided transbronchial needle aspiration was performed for 60 patients with isolated mediastinal lesions. Correct diagnosis was made in 48 cases. Nineteen cases were malignant, and 29 were benign. The rate of correct diagnosis was 80%. The sensitivity, specificity, and accuracy of endobronchial ultrasound-guided transbronchial needle aspiration in distinguishing benign from malignant mediastinal lesions were 95%, 100%, and 98%, respectively. The examination was tolerable for all patients. No associated complications were observed.
CONCLUSIONEndobronchial ultrasound-guided transbronchial needle aspiration is a safe and effective method of diagnosing mediastinal lesions.

5.Comparison of the autofluorescence bronchoscope and the white light bronchoscope in airway examination.

BACKGROUND AND OBJECTIVEThe sensitivity and accuracy of white light bronchoscopy (WLB) in airway examination are low. Autofluorescence bronchoscope (AFB) can determine early lesions in bronchial mucosa more sensitively, but it has seldom performed in China. To assess the clinical value of the AFB in airway examination, we compared the sensitivity and specificity of the AFB and WLB in detecting cancer of the airway mucosa.
METHODSBetween September 2009 and May 2010, bronchoscope examinations using both the AFB and WLB were performed on 136 patients, 95 men and 41 women with a median age of 61.5 years (ranged from 25 to 84 years). There were 46 lesions located in the central airway, 84 in the peripheral lung parenchyma, and 6 in the mediastinal region. All patients received local and general anesthesia and were subsequently examined with the WLB and AFB in tandem. All procedures were completed safely. Abnormal visual findings were recorded, and biopsies of the affected regions were collected for pathologic examination.
RESULTSOf 241 regions sampled for biopsy, 76 sites contained malignant lesions, whereas 165 sites contained benign lesions. The AFB detected 72 of the 76 malignant lesions, but the WLB detected only 50. The sensitivities of the AFB and WLB were 94.7% and 65.8%, respectively, and the specificities were 57.0% and 83.6%, respectively. The negative predictive values of the AFB and WLB were 95.9% and 84.1%, respectively.
CONCLUSIONSThe AFB is more sensitive than the WLB in detecting cancerous lesions in the mucosa, and is an effective airway examination.

6.hsa -let-7e-5p inhibits the migration of lung adenocarcinoma cells

Objective :
To screen differentially expressed miRNAs and explore its effect and mechanism on cell mi- gration in lung adenocarcinoma (LUAD) .
Methods :
Differentially expressed miRNAs in LUAD tissues and normal lung tissues were screened by miRNA microarrays,and then bioinformatics analysis was used to predict their poten- tial biological functions and signaling pathways.The cancer genome atlas (TCGA) analysis and quantitative real- time PCR (qRT-PCR) verified the expression level of hsa-let-7e-5p in LUAD tissues and cell lines.The effect of hsa-let-7e-5p cell migration in LUAD was examined by would healing experiment.After screening the underlying target genes by bioinformatics analysis ,the targeting relationship between hsa-let-7e-5p and DTX2,NME6, C8orf58,GATM and DHX57 were verified by qRT-PCR.
Results :
The miRNA microarray results showed that 347 miRNAs were down-regulated while 229 miRNAs were up-regulated in lung adenocarcinoma tissues.Compared with normal lung tissue and cells,the expression level of hsa-let-7e-5p was significantly down-regulated.Besides,over- expression of hsa-let-7e-5p inhibitedLUAD cell migration.
Conclusion
Non-coding RNA hsa-let-7e-5p is down- regulated in LUAD and inhibits the migration of lung adenocarcinoma cells.DTX2,NME6,C8orf58,GATM and DHX57 are the potential target genes of hsa-let-7e-5p.

Objective:To explore the current status of surgery for portal hypertension to grasp current status and future development of surgery in China.Methods:This study is jointly sponsored by China Hepatobiliary & Pancreatic Specialist Alliance & Portal Hypertension Alliance in China (CHESS).Comprehensive surveying is conducted for basic domestic situations of surgery for portal hypertension, including case load, surgical approaches, management of postoperative complications, primary effects, existing confusion and obstacles, liver transplantation(LT), laparoscopic procedures and transjugular intrahepatic portosystemic shunt(TIPS), etc.Results:A total of 8 512 cases of portal hypertension surgery are performed at 378 hospitals nationwide in 2021.Splenectomy plus devascularization predominated(53.0%)and laparoscopy accounted for 76.1%.Primary goal is preventing rebleeding(67.0%) and 72.8% of hospitals used preventive anticoagulants after conventional surgery.And 80.7% of teams believe that the formation of postoperative portal vein thrombosis is a surgical dilemma and 65.3% of hospitals practiced both laparoscopy and TIPS.The major reasons for patients with portal hypertension not receiving LT are due to a lack of qualifications for LT(69.3%)and economic factors(69.0%).Conclusions:Surgery is an integral part of management of portal hypertension in China.However, it is imperative to further standardize the grasp of surgical indications, the handling of surgical operation and the management of postoperative complications.Moreover, prospective, multi-center randomized controlled clinical studies should be performed.
9.Association between self-reported snoring and hypertension among Chinese Han population aged 30-79 in Chongqing, China.

BACKGROUND:
We aim to explore the association between self-reported snoring and hypertension among adults aged 30-79 in Chongqing, China.
METHODS:
A total of 23,342 individuals aged 30-79 were included at baseline from August 2018 to January 2019, and the final sample size for the analysis was 22,423. Face-to-face interviews and physical examinations were conducted by trained investigators. Logistic regression was performed to study age-specific and gender-specific associations between snoring and hypertension.
RESULTS:
Frequent snoring was associated with the risk of hypertension for each age and gender group, and the frequency of snoring was positively correlated with the risk for hypertension. For the three age groups (< 45, 45-59, ≥ 60), compared with the non-snoring group, those who snore often had a 64.5%, 53.3%, and 24.5% increased risk of hypertension (< 45: OR = 1.65, 95%CI 1.34-2.02; 45-59: OR = 1.53, 95%CI 1.37-1.72; ≥ 60: OR = 1.25, 95%CI 1.09-1.42), respectively. For men and women, those who snore often had a 46.8% and 97.2% increased risk of hypertension, respectively, than the non-snoring group (men: OR = 1.47, 95%CI 1.33-1.63; women: OR = 1.97, 95%CI 1.75-2.23).
CONCLUSIONS
People who snore frequently should pay close attention to their blood pressure levels in order to achieve early prevention of hypertension, particularly for snorers who are female and aged under 45; importance should be attached to their blood pressure control.

Objective:To explore the antidepressant mechanism of Yinxing Mihuan oral solution (YMO) by investigating its effect on depression model rats. Method:The depression rats were induced by isolation combined with chronic unpredictable mild stress (CUMS) and then randomly divided into model group, fluoxetine group (10 mg·kg-1) and high-dose (618 mg·kg-1) and low-dose (309 mg·kg-1) YMO groups. A blank control group was also set up and ten rats were included in each group. Modeling lasted for 21 consecutive days, and rats were administered the 8th day after stimulation at a dose of 10 mL·kg-1 for 14 days, except those in the blank control and model groups which were given distilled water. Afterward, the sucrose preference test, open field test, tail suspension test were carried out.
